Analysis of Patient Evacuations at the Centre Hospitalier Universitaire Communautaire (CHUC), Central African Republic

Obstetric complications are the main cause of mortality and morbidity. Many factors limit women’s access to the quality care they need to reduce this mortality. We proposed to analyse our referral system to identify certain factors that may limit its proper functioning and contribute to maternal deaths. This was a 12-month descriptive and analytical cross-sectional study of 259 evacuated women. The average age was 28.8 ± 7 with extremes ranging from 15 to 44 years. Most of the women were single (81.5%), accompanied by medical staff (78.8%), and had a venous approach (81.9%). The means of transfer used were ambulance (78.8%) and motorbike (11.6%). The transfer time was less than one hour (40.9%). The reasons for referral were consistent with the definitive diagnoses (64.5%) and diagnostic errors were noted (35.5%). The majority of patients were treated surgically (77.6%). We recorded maternal death (7.7%), due to unqualified personnel, transfer time, and poor general and haemodynamic condition (P < 0.05). The causes of death were abortion, ectopic pregnancy, arterial hypertension and post-partum haemorrhage (P = 0.014). Fresh stillbirths (14.9%), newborns transferred to neonatology (57.7%). Counter-referrals were assured (46.7%). Reducing diagnostic errors and late evacuations, as well as ongoing training for providers in peripheral health facilities on the signs of serious obstetric and neonatal emergencies will help to reduce the frequency of evacuations.

Morphology of the Left Atrial Appendage in the Senegalese: About 36 Anatomic Parts

Introduction: the left atrial appendage, a dormant embryonic vestige, would play a major role in cardiac hemodynamic changes, volume homeostasis and thrombi formation. It, therefore constitutes a therapeutic target. Its morphology is extremely variable. Objective: it consisted on determining the morphological variations of the left auricle as well as their interest in the prevention of thrombi in the Senegalese. Material and Method: This study was conducted by dissecting 36 fresh hearts from Senegalese anatomical subjects with a sampling of the left atrial appendage, including specimens with normal morphological appearance. Their morphology was studied using the fresh and frozen plaster molding method. The data obtained were analyzed statistically. Results: the average age of the subjects was 33 years, with a sex ratio of 1.06, the cauliflower shape was dominant with 57% of cases and the cactus shape (4%) was less frequent. The cauliflower shape presented a clear angulation compared to the others without statistically significant differences according to age. Conclusion: This work showed a great morphological variability of the left atrial appendage. It can provide an update on the specificity of Senegalese. Taking these variations into account is important in the safe management of hemodynamic conditions and the improvement of the management of recurrent strokes.

Self-sealing of fractures in indurated claystones measured by water and gas flow

Self-sealing of fractures in the indurated Callovo-Oxfordian(COX)and Opalinus(OPA)claystones,which are considered as host rocks for disposal of radioactive waste,was investigated on artificially fractured *** samples were extracted from four lithological facies relatively rich in clay mineral,carbonate and quartz,*** self-sealing of fractures was measured by fracture closure,water permeability variation,gas penetration,and recovery of gas-induced *** of the fractured samples exhibited a dramatic reduction inwater permeability to low levels that is close to that of intact rock,depending on their mineralogical composition,fracture intensity,confining stress,and load *** self-sealing capacity of the clay-rich samples is higher than that of the carbonate-rich and sandy *** effects of sample size and fracture intensity were *** sealed fractures become gas-tight for certain in-jection ***,the measured gas breakthrough pressures are still lower than the confining *** gas-induced pathways can recover when contacting *** important findings imply that fractures in such indurated claystones can effectively recover to hinder water transport but allow gas release under relatively low pressures without compromising the rock integrity.

The constant increase in power and heat flux densities encountered in electronic devices fuels a rising demand for lightweight heat sink materials with suitable thermal *** this study,discontinuous pitch-based carbon fiber reinforced aluminum matrix（Al-CF） composites with aluminum–silicon alloy（Al–Si） were fabricated through hot *** small amount of Al–Si contributed to enhance the sintering process in order to achieve fully dense Al–CF composites.A thermal conductivity and CTE of 258 W/（m K） and 7.0 9 10-6/K in the in-plane direction of the carbon fibers were obtained for a（Al95 vol%＋ Al–Si5 vol%）-CF50 vol%*** fiber provides the reducing of CTE while the conservation of thermal conductivity and weight of *** achieved CTEs satisfy the standard requirements for a heat sink material,which furthermore possess a specific thermal conductivity of 109 W cm3/（m K g）.This simple process allows the low-cost fabrication of Al–CF composite,which is applicable for a lightweight heat sink material.

First Total Posterior Craniopagus Twins in Central African Republic: Two Years Challenge of Management

Introduction: Countries with limited resources have real difficulties in separating craniopagus twins. Separation surgery cannot be considered there due to lack of technological conditions and appropriate means. Objective: To show the benefits of an Italian-Central African humanitarian collaboration, after two years follow-up, aiming to improve the first childhood years of craniopagus twins, born in the Central African Republic and separated in Rome. Case Report: The two female conjoined craniopagus twin, born at term on June 29, 2018 in Mbata located about 96 km from Bangui (CAR), were referred to the neonatology unit of the CHUPB on their 3rd day of life. The mother was 31-year-old, gravida 4 and para 5. Prenatal cares were quantitatively and qualitatively poorly followed. The diagnosis of posterior conjoined craniopagus twins associated with early neonatal infection was retained. After seven days of antibiotics and regression of infectious symptoms, the newborns were kept in the neonatology unit for a closer follow-up, pending the completion of administrative procedures for a medical evacuation as part of the international humanitarian activities of the Pediatric Hospital of the Holy Seat in collaboration with the Pediatric Teaching Hospital in Bangui. Finally, the twins were transferred to the Bambino Gesu Hospital in Rome on September 10, 2018 where the 3D images confirmed the total posterior fusion. Nonlinear dynamic behavior of inhomogeneous functional plates composed of sigmoid graded metal-ceramic materials

This paper presents a study on nonlinear vibration of inhomogeneous functional plates composed of sigmoid graded metalceramic materials. The material properties vary continuously along the thickness direction according to a sigmoid distribution rule, which is defined by piecewise functions to ensure smooth distribution of stress among all the interfaces. The geometric nonlinearity is considered by adopting the von Kármán geometrical relations. Based on the d'Alembert's principle, the nonlinear out-of-plane equation of motion of the plates is developed. The Galerkin method is employed to discretize the motion equation to a series of ordinary differential ones, which are subsequently analyzed via the use of the method of harmonic balance. Then, the analytical results are validated by the comparison to numerical solutions, which are obtained by using the adaptive step-size fourth-order Runge-Kutta method. The stability of the steady-state response is examined by the perturbation technique. Results show the first and third modes are both activated while the second mode is not activated for the plates under harmonic point excitation. The frequency response relationships of activated modes exhibit very complicated curves due to the nonlinear modal interaction. In addition, influences of key system parameters on nonlinear vibrational characteristics of the present inhomogeneous plates are illustrated.
