Ecthyma Gangrenosum about a Case at the Centre Hospitalier Roi Baudoin de Guédiawaye in Senegal

Background and Aim: Ecthyma gangrenosum (EG) is a rare necrotic skin infection caused by Pseudomonas aeruginosa occurring on healthy skin mainly, but not exclusively, in immunocompromised patients. We report the case of EG of unusual presentation in a 12-month-old infant. Case Presentation: This was a 12-month-old male infant who was referred from a peripheral structure for the treatment of facial swelling associated with fever. In his antenatal and intra nasal history, there was a notion of hospitalization for 15 days for early neonatal infection and a delay in psychomotor development. The admission examination showed the fair general condition, hyperthermia at 39°C, tachycardia at 136 beats/minute, and polypnea at 48 cycles/minute. The weight-for-height index with a z-score of 3DS was also found. The dermatological examination at the entrance found an extensive brownish inflammatory tense infiltrated plaque taking the mandibular, submental, retro-auricular occipital regions rapidly evolving towards necrosis, a significant swelling predominant on the upper right eyelid preventing the opening of the associated eyes to mouth ulceration. Biological abnormalities were predominantly neutrophilic leukocytosis, elevated CRP, and elevated muscle enzymes. Microbiological examination of skin samples had isolated Pseudomonas aeruginosa. He had received antibiotic therapy, and a necrosectomy under general anesthesia. The evolution was favorable and he was discharged from the hospital on D26 of hospitalization. Conclusion: This observation is rare due to its location, its early onset, and non-neutropenic background.

Morbidity and Mortality Related to Infectious Diseases in the Roi Boudouin Paediatric Ward in Senegal

Introduction: Infant and child morbidity and mortality linked to infections remain a major concern in both developed and developing countries. The general objective was to determine the characteristics of hospital morbidity and mortality linked to infectious pathologies in the pediatric department of the King Baudouin hospital center in Dakar. Methods: This was a retrospective, descriptive and analytical study focusing on children hospitalized from January 1, 2018 to December 31, 2021 in the pediatric department of the King Baudouin hospital center in Dakar. Results: During the study period, 1474 children were hospitalized for an infectious pathology in the pediatric department. The sex ratio was 1.48. Children aged between one and twelve months represented 34.12%. Morbidity due to respiratory and digestive infections was 42.33% and 33.45%, respectively. Pediatric hospital mortality was 0.75% with a predominance noted in infants between 1 and 12 months (55.86%). Respiratory, digestive infections, sepsis and neurological infections were the main causes of death. Factors associated with mortality were age (1 to 12 months) with p Conclusion: Reducing infant and child mortality linked to infectious diseases remains a crucial challenge to address for improving children’s health. Early detection of related signs, prevention of infections, and adequate care and monitoring of infants and children can contribute to this reduction.

Introduction: HIV infection in children is a major public health problem. The objective of this study carried out in the paediatric department of the Centre Hospitalier Roi Baudouin de Guédiawaye, was to evaluate the epidemiological, clinical, therapeutic and evolutionary aspects of a cohort of children with HIV over a 15-year period and to determine the factors associated with discontinuation of treatment and transition to second-line treatment. Patients and Methods: This was a descriptive and analytical cross-sectional study from August 2004 to December 2019 at the Centre Hospitalier Roi Baudouin de Guédiawaye. Through this study, age at diagnosis, current age, gender, orphan status, therapeutic aspects, status announcement and evolution were evaluated. A total of 129 patient records were studied over a 15-year period (2004-2019). HIV-positive children with profile I represented 93.8% and there was a male predominance with a sex ratio of 1.43. The current mean age of the children was 12.2 ± 4.5 and the median was 14 years. Fatherless and motherless children represented 24.0% and 20.2%, respectively, while both fatherless and motherless children represented 11.6%. The mean age at the start of ARV treatment was 7.6 ± 4.3 years and the median was 7.5 years. The mean age at the start of second-line treatment was 13.7 years with a standard deviation of 2.6 while the median was 13 years. AZT + 3TC + NVP was the most commonly used combination (29.5%) followed by TDF + FTC + NVP (26.2%). The mean age at treatment discontinuation was 13.3 ± 4.3 years while the median was 15 years. As a vital energy resource and raw material for many industrial products,syngas(CO and H_(2))is of great *** reforming of methane(DRM)is an important approach to producing syngas(with a hydrogen-to-carbon-monoxide ratio of 1:1 in principle)from methane and carbon dioxide,with a lower operational cost as compared to other reforming ***,many pure metallic catalysts used in DRM face deactivation issues due to coke formation or sintering of the metal particles.A systematic search for highly efficient metallic catalysts,which reduce the reaction barriers for the rate-determining steps and resist carbon deposition,is urgently *** is a typical low-cost transition metal for activating the C–H bond in *** this work,we applied a two-step workflow to search for nickel-based bimetallic catalysts with doping metals M(M-Ni)by combining density functional theory(DFT)calculations and machine learning(ML).We focus on the two critical steps in DRM—CH_(4) and CO_(2) direct *** used DFT and slab models for the Ni(111)facet to explore the relevant reaction pathways and constructed a data set containing structural and energetic information for representative M-Ni *** used this dataset to train ML models with chemical-knowledge-based features and predicted CH_(4) and CO_(2) dissociation energies and barriers,which revealed the composition–activity relationships of the bimetallic *** also used these models to rank the predicted catalytic performance of candidate systems to demonstrate the applicability of ML for catalyst *** emphasized that ML ranking models would be more valuable than regression models in high-throughput ***,we used our trained model to screen 12 unexplored M-Ni systems and showed that the DFT-computed energies and barriers are very close to the ML-predicted values for top candidates,validating the robustness of the trained model.

Inflammatory bowel diseases （IBDs） are complex and chronic disabling conditions resulting from a dysregulated dialogue between intestinal microbiota and components of both the innate and adaptive immune systems. Cytokines are essential mediators between activated immune and non-immune cells, including epithelial and mes- enchymal cells. They are immunomodulatory peptides released by numerous cells and these have significant effects on immune function leading to the differentiation and survival of T cells. The physiology of IBD is becom- ing a very attractive field of research for development of new therapeutic agents. These include cytokines involved in intestinal immune inflammation. This review will focus on mechanisms of action of oytokines involved in IBD and new therapeutic opportunities for these diseases.

Formation mechanisms of sub-micron pharmaceutical composite particles derived from far-and near-field Raman microscopy

Surface enhanced Raman spectroscopy(SERS) and confocal Raman microscopy are applied to investigate the structure and the molecular arrangement of sub-micron furosemide and polyvinylpyrrolidone(furosemide/PVP) particles produced by spray flash evaporation(SFE). Morphology, size and crystallinity of furosemide/PVP particles are analyzed by scanning electron microscopy(SEM) and X-ray powder diffraction(XRPD). Far-field Raman spectra and confocal far-field Raman maps of furosemide/PVP particles are interpreted based on the far-field Raman spectra of pure furosemide and PVP *** far-field Raman microscopy shows that furosemide/PVP particles feature an intermixture of furosemide and PVP molecules at the sub-micron scale. SERS and surface-enhanced confocal Raman microscopy(SECo RM) are performed on furosemide, PVP and furosemide/PVP composite particles sputtered with silver(40 nm). SERS and SECo RM maps reveal that furosemide/PVP particle surfaces mainly consist of PVP molecules. The combination of surface and bulk sensitive analyses reveal that furosemide/PVP sub-micron particles are formed by the agglomeration of primary furosemide nanocrystals embedded in a thin PVP matrix. Interestingly, both far-field Raman microscopy and SECo RM provide molecular information on a statistically-relevant amount of sub-micron particles in a single microscopic map;this combination is thus an effective and time-saving tool for investigating organic sub-micron composites.

The Anaerobic digestion in Senegal is of particular interest to the scientific community given the availability of substrates and their distribution throughout the country. However, from a technological point of view, the existing installations seem to be obsolete, which does not allow to reproduce the results of the laboratory tests. Thus, the present study aims to take stock of the situation in relation to the studies carried out in laboratories and those concerning the actual monitoring of the bio-digesters in situ. In fact, most experimental bio-digesters operate under optimal implementation conditions with strict control of input and output parameters. However, this is not the case for reactors installed in the field, as these so-called bio-digesters are exposed to real environmental conditions with a periodic variation of the physic-chemical parameters in the reactors throughout the day. This leads to a differential behavior of the micro-organisms, thus affecting their performance. This results in lower yields for those digesters operating under real environmental conditions.

Diallel analyses of soluble sugar content in cowpea (Vigna unguiculata ***.)

The content of soluble sugar in cowpea(Vigna unguiculata L. Walp.) seeds is important for their acceptability to consumers. The objective of this study was to evaluate the genetic control of soluble sugar content in cowpea using diallel analysis. For this purpose,half-diallel crosses were made among seven diverse parents. The amount of total soluble sugar was estimated by the phenol sulfuric acid reagent method and reducing sugar were measured by the DNS(3,5-dinitrosalicylic acid) reagent method. The total soluble sugar ranged from 11.12(VYA) to 40.79 mg g^(-1)(IT93 K-693-2) with a mean of 22.31 mg g^(-1).Reducing soluble sugar showed a mean value of 9.11 mg g^(-1) and ranged from 13.61(TVx3236) to 4.98 mg g^(-1)(Lori niebe). Diallel analysis showed that both additive and non-additive gene effects were significant in the genetic control of these traits. However,dominance variance was greater than additive variance for soluble sugar. The traits were controlled mainly by an overdominance model, suggesting the advantage of delaying selection to later generations. Soluble sugar content was found to be highly heritable, with broad-sense heritability ranging from 0.987 to 0.976. Narrow-sense heritability ranged from0.154 to 0.422 and revealed the lower importance of additive variance. Parents and F_1 hybrids differed significantly on the basis of their general and specific combining abilities,respectively. Recessive alleles had positive effects on total soluble sugar and reducing soluble sugar. These results could help cowpea breeders improve the acceptability of cowpea seeds and cowpea-based foods by appropriate selection.
