Objectives:In patients with head and neck carcinoma,“treatment package time”(TPT)was proven to impact outcomes in cases receiving adjuvant radiotherapy *** impact in patients receiving radiotherapy with concurrent systemic therapy has not been studied *** TPT influence on survival endpoints for patients treated with surgery followed by radiation and concurrent systemic therapy was ***:Institutional database to identify head and neck carcinoma cases treated with definitive surgery followed by concomitant chemo(bio)radiotherapy(CRT)was *** was the number of days elapsed between surgery and the last day of radiation.%FINDCUT SAS macro tool was used to search for the cutoff TPT that was associated with significant survival ***-Meier curves,log-rank tests as well as univariate and multivariate analyses were used to assess overall survival(OS)and recurrence free survival(RFS).Results:One hundred and three cases with a median follow up of 37 months were included in the *** tumors were 43%,oral cavity 40%and laryngeal 17%of *** systemic therapy included platinum and cetuximab in 72%and 28%,*** TPT was found to be<100 days with significantly better OS(P=0.002)and RFS(P=0.043)compared to TPT≥100 *** multivariate analysis;TPT<100 days,extracapsular nodal extension,high-risk score,iymphovascular space and perineural invasion were independent predictors for worse OS(P<0.05).T4,extracapsular nodal extension and high-risk score were all significantly detrimental to RFS(P<0.05).Conclusions:Addition of concomitant systemic therapy to adjuvant radiotherapy did not compensate for longer TPT in head and neck squamous cell *** coordinated care must be provided to ensure the early start of CRT with minimal treatment breaks.

Investigation on Fire Protection System of Highrise Buildings in the Context of Bir Uttam Aminul Haque Avenue in Dhaka, Bangladesh

Rapid high-rise building construction of different occupancies is becoming popular in Dhaka due to its economic gain and functional flexibility. Fire prevention poses a significant difficulty to this type of construction due to its complexity and economic worth as well. Therefore, construction of high-rise building without following the proper fire safety measures, is a common practice at present in the city and it poses a greater threat to urban life considering its associated loss. Even though, most of the owners or authorities do not respect the construction code and the majority of them lack sufficient awareness and basic fire control knowledge and practice. More often, they are unable to comprehend the magnitude and severity of severe fire hazards, and recognize the causes and implementing effective mitigation measures are rare. As a result, the number of fire hazard in high-rise construction is increasing day by day. Hence, investigating the present condition of high-rise buildings (already built) in terms of Fire Protection System is imperative to prevent the upcoming fire hazard. An urban chunk of 33 plots along with Bir Uttam Aminul Haque Avenue, located in Banani, has been chosen for the study area considering its vulnerability to fire hazard. Among these buildings, STAR Tower and HBR Tower, these two high-rise buildings are assessed thoroughly using FRI (Fire Risk Index) Method. The method is semi-quantitative in nature with seventeen parameters associated with fire protection. This assessment represents a scenario in what extent fire codes are being followed in the selected area. It concludes that most of the buildings lack in providing protection against fire hazards for not following the code strictly. This research provides some recommendations which can be followed to improve the fire safety measures in this existing context. Understanding the relation between spatial heterogeneity and structural rejuvenation is one of the hottest topics in the field of metallic glasses(MGs).In this work,molecular dynamics(MD)simulation is implemented to discover the effects of initial spatial heterogeneity on the level of rejuvenation in the Ni_(80)P_(20)*** this purpose,the samples are prepared with cooling rates of 10^(10) K/s-10^(12) K/s to make glassy alloys with different atomic ***,it is found that the increase in the cooling rate leads the Gaussian-type shear modulus distribution to widen,indicating the aggregations in both elastically soft and hard *** the primary evaluations,the elastostatic loading is also used to transform structural rejuvenation into the atomic *** results indicate that the sample with intermediate structural heterogeneity prepared with 10^(11) K/s exhibits the maximum structural rejuvenation which is due to the fact that the atomic configuration in an intermediate structure contains more potential sites for generating the maximum atomic rearrangement and loosely packed regions under an external *** features of atomic rearrangement and structural changes under the rejuvenation process are discussed in detail.

Serum Ferritin and D-Dimer as Possible Risk Factors in Ischaemic Stroke in Cancer Patients

Background: Ischemic stroke is frequently encountered in patients with malignant disease. The pathophysiology of stroke in such cases differs from other subjects with no malignant disease. This study was conducted to compare serum levels of ferritin and d-dimer in cases with ischemic stroke in cancer versus non-cancer patients. Patients and methods: The data of consecutive 264 patients presented with ischemic stroke, confirmed by clinical examination and radiological investigations, were retrospectively reviewed. The included cases were divided into two groups: Group A (non-cancer with stroke, 210 cases) and Group B (cancer with stroke, 54 cases). The collected data included patient demographics, systemic comorbidities, disease and tumor characteristics, in addition to platelet count, serum ferritin and d-dimer. Results: Age, gender, and systemic comorbidities were statistically comparable between the two groups. Additionally, the etiology of stroke and its disability were not statistically different between the two groups. However, the incidence of mortality significantly increased in Group B (25.93% vs. 7.14% of Group A, p = 0.005). Both serum ferritin and d-dimer showed a significant increase in association with cancer (Group B). The former had mean values of 294.54 and 867.87 ng/ml, while the latter had mean values of 463.83 and 888.13 ng/ml in the same two groups, respectively. Conclusion: Serum ferritin and d

A Finite Element Approximation of the Stokes Equations

In this work, a numerical solution of the incompressible Stokes equations is proposed. The method suggested is based on an algorithm of discretization by the unstable of Q1 – P0 velocity/pressure ?nite element approximation. It is shown that the inf-sup stability constant is O(h) in two dimensions and O( h2) in three dimensions. The basic tool in the analysis is the method of modi?ed equations which is applied to ?nite difference representations of the underlying ?nite element equations. In order to evaluate the performance of the method, the numerical results are compared with some previously published works or with others coming from commercial code like Adina system.

Use of Cladophora glomerata Extract against Multidrug Resistant Bacterial Pathogens

The antibacterial effect of the crude methanol extracts and purified fractions of Ctadophora glomerata （Linnaeus） Kutzing （Cladophoraceae） against multidrug resistant human pathogen were investigated. The test bacterial strains were Staphylococcus aureus, Pseudomonas aeruginosa, Streptococcus faecalis, Escherichia coli and S. typhimurium. Ethanol extract （0.5 mg/mL） of Cladophora glomerata inhibited growth of all the test organisms. Primary detection of active compounds showed that maeroalgae （Cladophora glomerata） containing flavonoids, alkaloids, phenols and tannins. Gas Chromatography-Mass Spectrometry was used to know the compounds which responsible of antibacterial activity and they were g-Pentadecanone that was found to be a major compound （42.67%） followed by 8-Octadecanone （22.43%） and Salicylic acid （20.04%） in addition to 10-hexadecaden （8.01%）. These findings suggest the possibility of using the Cladophora glomerata as a novel source of natural antimicrobial agents in pharmaceutical industries.

Angiogenesis Factors Associated with New Breast Cancer Cell Line AMJ13 Cultured <i>in Vitro</i>

Background: AMJ13 is a new breast cancer cell line that has been established from a 70-year-old Iraqi woman with a histological diagnosis of infiltrating ductal carcinoma. It is the first for an Iraqi population. In breast cancer, angiogenesis provides the tumor tissue, which is rapidly proliferated with oxygen and nutrients, removes wastes and increases the opportunity of cancer cells to invade other organs. Methods: The AMJ13 breast cancer cell line was represented at three different passages and incubated for interval times. Microarray panel of 43 different angiogenesis markers was used to scan the supernatant for the factors. ELISA was used to quantify some of the important angiogenesis factors released in the culture medium and to confirm absence of those who was not detected by the antibody array. RT-PCR was used to confirm the gene expression (mRNA) of studied factors. Results: Microarray analysis showed that TIMP1 and two secreted at highest levels compared to the rest of the factors with low presence of endostatin. Other non-detectable factors by microarray examined by ELISA assay that showed highest expression level of VEGF-A were obtained at earliest passage, while the highest levels of FGF-b were obtained at late passage. The VEGF-D secretion was shown low concentrations at all studied passages. There is no detectable level of EGF protein in different passages and times interval tested. There are no significant differences in secretion of sICAM between different passages and incubation periods. Conclusion is that AMJ13 cell line depends on VEGF-A as main angiogenesis factor to induce micro-vessels supported by low levels of VEGF-D for lymphatic vessels formation. AMJ13 cell line depends on FGF as growth factors as in late passages it was shifted to depend mainly on FGF completely. All of this process may be regulated by TGF-β. A Hybrid Method for Edge Continuity Based on Pixel Neighbors Pattern Analysis (PNPA) for Remote Sensing Satellite Images

Edge enhancement is derived from lack of accurate result from edge detection techniques. The image which is captured from long distances carries a lot of noise and blur which causes edge discontinuity. Although some novel algorithms which are based on cellular neural network, fuzzy enhancement and binary morphology have shown accuracy in order to obtain refined edge but still the problem of edge discontinuity arises. Eliminating discontinuity of edge a hybrid technique is proposed based on pixel neighbors pattern analysis PNPA. In the technique Canny operator for initial edge detection, PNPA operation for edge enhancement are performed for remote sensing satellite image successively. The visual and subjective evaluation shows that the proposed PNPA operation can effectively eliminate the influence of edge discontinuity which occurred due to noise and blurr in original captured image, as comparing to existing edge segmenting processes.
