Estimation of visceral fat is useful for the diagnosis of significant fibrosis in patients with non-alcoholic fatty liver disease

BACKGROUND Obesity is a risk factor for non-alcoholic fatty liver disease(NAFLD),although obese patients with NAFLD do not always develop significant *** distribution of body fat could predict the risk of NAFLD *** To investigate the role of bioelectrical impedance-estimated visceral fat(VF)in assessing NAFLD *** In this cross-sectional study,patients with biopsy-proven NAFLD were prospectively *** patients underwent anthropometric evaluation,blood tests and bioelectrical impedance *** Between 2017 and 2020,119 patients were included[66.4%male,56 years(SD 10.7),62.2%obese,61.3%with metabolic syndrome].Sixty of them(50.4%)showed significant fibrosis(≥F2)in liver ***,VF and metabolic syndrome were associated with significant fibrosis(61 years vs 52 years,16.4 vs 13.1,73.3%vs 49.2%,respectively;P<0.001 for all).In the multivariate analysis,VF and age were independently associated with significant fibrosis(VF,OR:1.11,95%CI:1.02-1.22,P=0.02;age,OR:1.08,95%CI:1.03-1.12,P<0.01).A model including these variables showed and area under the receiver operating characteristic curve(AUROC)of 0.75,which was not inferior to transient elastography or NAFLD fibrosis score *** developed a nomogram including age and VF for assessing significant fibrosis in routine *** VF is a surrogate marker of liver fibrosis in patients with *** impedance analysis is an inexpensive and simple method that can be combined with age to guide patient referral when other resources may be unavailable.

Acetic acid can be directly produced from glucose in one-step fermentation by using yeasts of the genus Brettanomyces bruxellensis, hence increasing the industrial application to manufacture products with simplified bioprocesses. Thereby, this work evaluates the influence of initial glucose concentration on the growth and acetic acid production by B. bruxellensis. The results obtained confirmed the presence of Crabtree effect on B. bruxellensis under low glucose concentrations. The maximum acetic acid concentration reached was 15.4 g·L-1 starting with 100 g·L-1 leading to a product yield of 0.154 g·g-1 and a specific acetic acid production rate of 0.05 g·g-1·h-1. The results also indicate that after reaching the acetic acid critic threshold of 4 g·L-1 the metabolism can induce the growth second phase even residual glucose was present on the culture media at high starting glucose concentrations. Additionally, it was observed a lineal relationship between cell viability and acetic acid production.

Background: Long-term survival in acute coronary syndrome has increased steadily in the last decades. Follow-up studies developed in this patient clearly reveal that they are at risk of suffering a new event, placing them in a new stage, secondary prevention. Assuming this increased risk, the control target of their cardiovascular risk factors become more ambitious. In this field, control of Cholesterol levels, particularly LDL-C, has arisen as a priority objective in patients with coronary arterial disease. AIM:To evaluates the effectiveness and safety of the first generation,NS3/4A protease inhibitors(PIs) in clinical practice against chronic C virus,especially in patients with advanced fibrosis. METHODS:Prospective study and non-experimental analysis of a multicentre cohort of 38 Spanish hospitals that includes patients with chronic hepatitis C genotype 1,treatment-na?ve(TN) or treatment-experienced(TE),who underwent triple therapy with the first generation NS3/4A protease inhibitors,boceprevir(BOC) and telaprevir(TVR),in combination with pegylated interferon and ribavirin. The patients were treatment in routine practice settings. Data on the study population and on adverse clinical and virologic effects were compiled during the treatment period and during follow ***:One thousand and fifty seven patients were included,405(38%) were treated with BOC and 652(62%) with TVR. Of this total,30%(n = 319) were TN and the remaining were TE:28%(n = 298) relapsers,12%(n = 123) partial responders(PR),25%(n = 260) null-responders(NR) and for 5%(n = 57) with prior response unknown. The rate of sustained virologic response(SVR) by intention-to-treatment(ITT) was greater in those treated with TVR(65%) than in those treated with BOC(52%)(P < 0.0001),whereas by modified intention-to-treatment(m ITT) no were found significant differences. By degree of fibrosis,56% of patients were F4 and the highest SVR rates were recorded in the non-F4 patients,both TN and TE. In the analysis by groups,the TN patients treated with TVR by ITT showed a higher SVR(P = 0.005). However,by m ITT there were no significant differences between BOC and TVR. In the multivariate analysis by m ITT,the significant SVR factors were relapsers,IL28 B CC and non-F4; the type of treatment(BOC or TVR) was not significant. This manuscript collects in a joint and orderly manner the existing evidence at the present time about postoperative treatment with radiotherapy in non-small cell lung *** also systematically reviews the current evidence,the international recommendations in the most relevant guidelines,the most controversial aspects in clinical and pathological staging,the specific technical aspects of radiotherapy treatment,and also collects all the potential risk factors that have been postulated as significant in the prognosis of these patients,evaluating the possibility of segmenting a particularly sensitive subpopulation with a high risk of relapse on which an adjuvant treatment with radiotherapy could have an impact on their clinical ***,currently active trials that aspire to provide more evidence on this topic are reviewed.

＜正＞ Multiple alphal-adrenoceptors seem to contract rat tail artery. AlphalA- predominates and either alpha1B-,alpha1D-,or alpha1L-is the second population. We characterized alphal-adrenoceptors with selective agonists/antagonists in tail artery of young Wistar rat. Tail artery was exposed to A61603 （N-[5-（4.5- dihy-dro- 1H- imidazol - 2 - yl） - 2 - hydroxy - 5. 6. 7. 8 - tetrahydronaphthalen- 1 - yl] methanesulfonamide） or to phenylephrine （alphalA- and alphal- agonists ）, and to prazosin （alphal- antagonist）, the alphal A-antagonists 5 - methy-lurapidil,RS 100329 （5-methyl - 3 - [3 - [4 - [2 - （2,2,2,-trifluoroethoxy） phenyl] - 1-piperazinyl] propyl] - 2,4-（1H）-pyrimidinedione）,RS 17053 （N- [2（2-cyclopropylmethoxy） ethyl] - 5-chloro-a, a-dimethyl - 1H-indole - 3-ethylamine）, and the alpha1D-antagonist BMY 7378 （8-[2-[4-（2-methoxyphenyl） - 1-piperazinyl] ethyl] - 8 - azaspiro[4.5] decane - 7,9 -dione） . A61603 showed 100 fold higher affinity over PHE. Prazosin,RS100329,5-MU,and RS17053 displaced agonists with high affinity indicating alphal A-adrenoceptors while PHE activated alphal B-adrenoceptors since BMY 7378 had low affinity.

Update on the treatment of type 2 diabetes mellitus

To achieve good metabolic control in diabetes and keep long term, a combination of changes in lifestyle and pharmacological treatment is necessary. Achieving near-normal glycated hemoglobin significantly, decreases risk of macrovascular and microvascular complications. At present there are different treatments, both oral and injectable, available for the treatment of type 2 diabetes mellitus(T2DM). Treatment algorithms designed to reduce the development or progression of the complications of diabetes emphasizes the need for good glycaemic control. The aim of this review is to perform an update on the benefits and limitations of different drugs, both current and future, for the treatment of T2 DM. Initial intervention should focus on lifestyle changes. Moreover, changes in lifestyle have proven to be beneficial, but for many patients is a complication keep long term. Physicians should be familiar with the different types of existing drugs for the treatment of diabetes and select the most effective, safe and better tolerated by patients. Metformin remains the first choice of treatment for most patients. Other alternative or second-line treatment options should be individualized depending on the characteristics of each patient. This article reviews the treatments available for patients with T2 DM, with an emphasis on agents introduced within the last decade.

Recirculating chemohyperthermia as a treatment for non-muscle invasive bladder cancer:Current and future perspectives

About 75% of all bladder cancer diagnosed are non-muscle invasive bladder cancer(NMIBC), recurring over 50% of them after transurethral resection of the bladder tumor. In order to prevent recurrences, adjuvant intravesical chemotherapy with mitomycin C and immunotherapy with bacillus Calmette-Gu-érin(BCG) is traditionally used. Unfortunately, many patients relapse after receiving these treatments and a significant proportion of them require surgery. After a one-to-three years BCG maintenance, the risk for progression at 5 years was 19.3% for T1G3 tumors. Many new treatment approaches are being investigated to increase the effectiveness of adjuvant intravesical therapy. One of the developing treatments for intermediate and high-risk NMIBC is the combination of intravesical chemotherapy and hyperthermia, called chemohyperthermia. This article provides a review of the mechanism of action, current status and indications, results and future perspectives.

摘要在阿尔茨海默病(AD)中,在β-淀粉样蛋白斑块周围发现了反应性星形胶质细胞和营养不良的神经突(异常的突触前元件)。笔者的前期研究已经表明,反应性星形胶质细胞会包裹、吞噬和降解APP小鼠以及AD患者海马中营养不良的突触,但仅波及不到7%的营养不良神经突,这表明星形胶质细胞在AD中的吞噬能力降低。本研究旨在通过分析原代星形胶质细胞培养物对分离突触(突触神经小体,SNs)吞噬和降解的能力来深入了解其潜在机制。SNs分别来自APP(包含营养不良性突触和β淀粉样肽)小鼠、Tau(包含AT8-和AT100-阳性磷酸化Tau)小鼠以及WT(对照)小鼠。我们发现与SNs-WT相比,星形胶质细胞的吞噬和降解能力在SNs-APP大大降低,但在AT8/AT100阳性的SNs-Tau却没有。在12月大的APP小鼠的海马中证实了星形胶质细胞吞噬能力的降低,因为只有(1.60±3.81)%的斑块状星形胶质细胞具有吞噬结构。
