Background: Interpretation of parathyroid hormone (PTH) measurements requires an understanding of its structural conformation. PTH reflects calcium intake and vitamin D status but is commonly measured in the fasted state using 2nd generation assays, known to cross-react with PTH 7-84 fragments, which is not observed with 3rd generation assays. The objective was to determine if plasma PTH in the fed and non-fed state differ while controlling for diurnal rhythm and the generation of PTH assay. Methods: Blood was sampled, 2 d apart, from Sprague-Dawley rats (30 males and 30 females) in both fed and non-fed states at 20 wk of age (weight: 470.2 ± 23.2 g) for measurement of ionized calcium (iCa), total intact (INT) and bioactive 1-84 (BIO) PTH. Differences between groups were tested using a GLIMMIX model with sex and feeding state as fixed effects and individual rats as a random effect. results: Females had a lower iCa than males (F: 1.43 ± 0.01 vs M: 1.46 ± 0.01 mmol/L, P = 0.03). In males and females, there was no difference between fed and non-fed groups when PTH was assessed using the INT PTH assay (M Fed: 21.6 ± 1.3 vs M Non-fed: 22.6 ± 2.3 pmol/L, P = 0.59;F Fed: 19.3 ± 1.5 vs F Non-fed: 22.8 ± 2.9 pmol/L, P = 0.39). However, in females only, PTH measured using the BIO PTH, was significantly lower in the fed group versus the non-fed (Fed: 8.4 ± 0.8 vs Non-fed: 16.7 ± 3.4 pmol/L, P = 0.05). Conclusions:

The Internet ofMedical Things(IoMT)is mainly concernedwith the efficient utilisation of wearable devices in the healthcare domain to manage various processes automatically,whereas machine learning approaches enable these smart systems to make informed ***,broadcasting is used for the transmission of frames,whereas congestion,energy efficiency,and excessive load are among the common issues associated with existing *** this paper,a machine learning-enabled shortest path identification scheme is presented to ensure reliable transmission of frames,especially with the minimum possible communication overheads in the IoMT *** this purpose,the proposed scheme utilises a well-known technique,i.e.,Kruskal’s algorithm,to find an optimal path from source to destination wearable ***,other evaluation metrics are used to find a reliable and shortest possible communication path between the two interested *** from that,every device is bound to hold a supplementary path,preferably a second optimised path,for situations where the current communication path is no longer available,either due to device failure or heavy ***,the machine learning approach helps enable these devices to update their routing tables simultaneously,and an optimal path could be replaced if a better one is *** proposed mechanism has been tested using a smart environment developed for the healthcare domain using IoMT *** results show that the proposed machine learning-oriented approach performs better than existing approaches where the proposed scheme has achieved the minimum possible ratios,i.e.,17%and 23%,in terms of end to end delay and packet losses,***,the proposed scheme has achieved an approximately 21%improvement in the average throughput compared to the existing schemes.

BACKGrOUND radiocarpal dislocations are rare but potentially devastating *** outcomes are associated with inadequate or lost reduction,such as ulnar translocation,but no consensus exists on the ideal fixation *** bridge plate fixation has been described for various settings in the treatment of complex distal radius fractures and can be fixed distally to the second or third metacarpal,but its application for radiocarpal dislocations has not been *** To determine whether distal fixation to the second or third metacarpal *** Using a cadaveric radiocarpal dislocation model,the effect of distal fixation was studied in two stages:(1)A pilot study that investigated the effect of distal fixation alone;and(2)a more refined study that investigated the effect of described techniques for distal and proximal *** were measured in various parameters to determine the quality of the reduction *** The pilot study found that focusing on distal fixation alone without changing proximal fixation results in ulnar translocation and volar subluxation when fixing distally to the second metacarpal compared with the *** second iteration demonstrated that anatomic alignment in coronal and sagittal planes could be achieved with each *** In a cadaveric radiocarpal dislocation model,anatomic alignment can be maintained with bridge plate fixation to the second metacarpal or the third metacarpal if the described technique is *** considering dorsal bridge plate fixation for radiocarpal dislocations,the surgeon is encouraged to understand the nuances of different fixation techniques and how implant design features may influence proximal placement.

here has been a strong need for simulation environments that are capable of modeling deep interdependencies between complex systems encountered during natural hazards,such as the interactions and coupled effects between civil infrastructure systems response,human behavior,and social policies,for improved community *** such complex components with an integrated simulation requires continuous data exchange between different simulators simulating separate models during the entire simulation *** can be implemented by means of distributed simulation platforms or data passing *** order to provide a systematic reference for simulation tool choice and facilitating the development of compatible distributed simulators for deep interdependent study in the context of natural hazards,this article focuses on generic tools suitable for integration of simulators from different fields but not the platforms that are mainly used in some specific *** this aim,the article provides a comprehensive review of the most commonly used generic distributed simulation platforms(Distributed Interactive Simulation(DIS),High Level Architecture(HLA),Test and Training Enabling Architecture(TENA),and Distributed Data Services(DDS))and data passing tools(robot Operation System(rOS)and Lightweight Communication and Marshalling(LCM))and compares their advantages and *** specific limitations in existing platforms are identified from the perspective of natural hazard *** mitigating the identified limitations,two platform design recommendations are provided,namely message exchange wrappers and hybrid communication,to help improve data passing capabilities in existing solutions and provide some guidance for the design of a new domain-specific distributed simulation framework.

Specialized therapeutic delivery, or use of pharmaceuticals and other biomaterials to target specific parts of the body or diseased tissue, has long been sought as an ideal way of treating human diseases. A recent article published in Nature Biomedical Engineering revealed an innovative strategy to engineer nucleus-free human mesenchymal stem cells (MSCs) for targeted delivery of therapeutics to disease site.1 MSCs have emerged as promising vehicles of therapeutic delivery.2,3 MSCs are undifferentiated pluripotent stem cells derived from areas such as bone marrow and adipose tissue.4,5 MSCs are sought after for their chemotaxis, or ability to home towards a chemical stimulus, and capacity for modification with elements such as chemoattractant receptors and adhesion molecules.1 These properties allow for site-specific and minimally-invasive therapeutic administration and treatment.

Infertility affects roughly 15% of couples; in nearly half of these cases, there is a male contributing factor.1 Along with advances in modern drugs and assisted reproductive technologies, some infertile patients choose to use complementary and alternative medical therapies （CAM）. These are either administered as adjuncts or, in some cases, as the sole mode of medical therapy. reasons for using CAM vary and may include faith, beliefs, economic factors or simply fear of a surgical procedure and apprehension about using hormone-modifying medications.

There are multiple factors influencing this change including advanced marital age, higher levels of postgraduate education, and the changing role for women in society. Oceanic transform faults(TFs)are commonly viewed as single,narrow strikeslip seismic faults that offset two mid-ocean ridge ***,broad zones of complex deformation are ubiquitous at ***,we propose a new conceptual model for the progressive deformation within broad zones at oceanic TFs through detailed morphological,seismic,and stress *** argue that,under across-transform extension due to a change in plate motion,plate deformation occurs first along high-angle transtensional faults(TTFs)within the transform ***-transform normal faults(ONFs)form when across-transform deviatoric extensional stresses exceed the yield strength of the adjacent oceanic *** further extension,these normal faults can develop into off-transform rift zones(OrZs),some of which can further develop into transform plate *** illustrate that such progressive complex deformation is an inherent feature of oceanic *** new conceptual model provides a unifying theory to explain the observed broad deformation at global transform systems.
