Prediction of early-stage hepatocellular carcinoma using Onco Scan chromosomal copy number aberration data

AIM To identify chromosomal copy number aberrations(CNAs) in early-stage hepatocellular carcinoma(HCC) and analyze whether they are correlated with patient *** One hundred and twenty patients with early-stage HCC were enrolled in our study, with the collection of formalin fixed, paraffin-embedded(FFPE) specimens and clinicopathological data. Tumor areas were marked by certified pathologists on a hematoxylin and eosinstained slide, and cancer and adjacent non-cancerous tissues underwent extraction of DNA, which was analyzed with the Affymetrix Onco Scan platform to assess CNAs and loss of heterozygosity(LOH). Ten individuals with nonmalignant disease were used as the control group. Another cohort consisting of 40 patients with stage Ⅰ/Ⅱ HCC were enrolled to analyze gene expression and to correlate findings with the Onco Scan *** Copy number amplifications occurred at chromosomes 1 q21.1-q44 and 8 q12.3-24.3 and deletions were found at 4 q13.1-q35.2, 8 p 23.2-21.1, 16 q23.3-24.3, and 17 p13.3-12, while LOH commonly occurred at 1 p32.3, 3 p21.31, 8 p23.2-21.1, 16 q22.1-24.3, and 17 p 13.3-11 in early-stage HCC. Using Cox regression analysis, we also found that a higher percentage of genome change(≥ 60%) was an independent factor for worse prognosis in early-stage HCC(P = 0.031). Among the 875 genes in the Onco Scan Gene Chip, six were independent predictors of worse disease-free survival, of which three were amplified(MYC, ELAC2, and SYK) and three were deleted(gak, MECOM, and WRN). Further, patients with HCC who exhibited ≥ 3 CNAs involving these six genes have worse outcomes compared to those who had gak, MECOM, or WRN are at risk for poorer outcome after resection.

＜正＞ CKD-501 is a novel peroxisome proliferator-activated receptor α/γ dual agonist for the treatment of diabetes mellitus. This study aimed to investigate safety, tolerability, and pharmacokinetics of CKD-501 in healthy volunteers. A randomized, placebo-controlled, double-blind, parallel-group, dose-rising study was performed. Thirty-six healthy male subjects received single oral doses ranging from 0.5-8 mg CKD- 501 or placebo. In the multipledose study, 24 subjects received 1-4 mg once daily for 7 days. Serial blood and urine samples were collected. No serious adverse events （AEs） were observed and AEs reported were all of mild severity. In the single-dose study, mean Cmax and AUCinf increased linearly up to the 8 mg dose *** and t1/2 ranged from 0.5-4 h and 7.8-9.8 h. respectively. Less than 1 % CKD-501 was excreted in urine. After multiple dosing, accumulation index was around 1.2. Mean apparent clearance, Tmax and t1/2 in steady state were independent of dose and time. Single oral doses up to 8 mg CKD- 501 and multiple doses up to 4 mg were safe and well tolerated. Mean Cmax and AUCinf were dose proportional and there was no remarkable accumulation after multiple dosing.

AIM To assess the performance of proposed scores specific for acute-on-chronic liver failure in predicting shortterm mortality among patients with alcoholic *** We retrospectively collected data from 264 patients with clinically diagnosed alcoholic hepatitis from January to December 2013 at 21 academic hospitals in Korea. The performance for predicting short-term mortality was calculated for Chronic Liver FailureSequential Organ Failure Assessment(CLIF-SOFA), CLIF Consortium Organ Failure score(CLIF-C OFs), Maddrey'sdiscriminant function(DF), age, bilirubin, international normalized ratio and creatinine score(ABIC), Glasgow Alcoholic Hepatitis Score(GAHS), model for end-stage liver disease(MELD), and *** Of 264 patients, 32(12%) patients died within 28 d. The area under receiver operating characteristic curve of CLIF-SOFA, CLIF-C OFs, DF, ABIC, GAHS, MELD, and MELD-Na was 0.86(0.81-0.90), 0.89(0.84-0.92), 0.79(0.74-0.84), 0.78(0.72-0.83), 0.81(0.76-0.86), 0.83(0.78-0.88), and 0.83(0.78-0.88), respectively, for 28-d mortality. The performance of CLIF-SOFA had no statistically significant differences for 28-d mortality. The performance of CLIF-C OFs was superior to that of DF, ABIC, and GAHS, while comparable to that of MELD and MELD-Na in predicting 28-d mortality. A CLIF-SOFA score of 8 had 78.1% sensitivity and 79.7% specificity, and CLIF-C OFs of 10 had 68.8% sensitivity and 91.4% specificity for predicting 28-d *** CLIF-SOFA and CLIF-C OF scores performed well, with comparable predictive ability for short-term mortality compared to the commonly used scoring systems in patients with alcoholic hepatitis.

Outcome of transarterial chemoembolization-based multi-modal treatment in patients with unresectable hepatocellular carcinoma

AIM:To investigate the efficacy and safety of transarterial chemoembolization(TACE)-based multimodal treatment in patients with large hepatocellular carcinoma(HCC).METHODS:A total of 146 consecutive patients were included in the analysis,and their medical records and radiological data were reviewed ***:In total,119 patients received TACE-based multi-modal treatments,and the remaining 27 received conservative *** survival(P<0.001)and objective tumor response(P=0.003)were significantly better in the treatment group than in the conservative *** subgroup analysis,survival benefits were observed not only in the multi-modal treatment group compared with the TACE-only group(P=0.002)but also in the surgical treatment group compared with the loco-regional treatment-only group(P<0.001).Multivariate analysis identified tumor stage(P<0.001)and tumor type(P=0.009)as two independent pre-treatment factors for *** adjusting for significant pre-treatment prognostic factors,objective response(P<0.001),surgical treatment(P=0.009),and multi-modal treatment(P=0.002)were identified as independent post-treatment prognostic ***:TACE-based multi-modal treatments were safe and more beneficial than conservative *** surgery after successful downstaging resultedin long-term survival in patients with large,unresectable HCC.

Cimicifuga heracleifolia Komarov(CH) is used traditionally as a herbal remedy for many ***,published literature suggests that large doses of cimicifuga racemosa might be associated with ***,its long-term toxicity was not evaluated *** study was performed to evaluate 14-day repeated dose toxicity(RDT) and genotoxicity of CH extract according to the USNTP protocol and GLP guideline. 背景与目的奥沙利铂、伊立替康、氟尿嘧啶和亚叶酸的联合化疗方案(FOLFIRINOX)极大提高了晚期胰腺癌患者的生存期。然而,FOLFIRINOX方案作为吉西他滨失败后二线治疗的疗效尚未进行前瞻性的检测。我们研究了减量FOLFIRINOX方案治疗吉西他滨抵抗的晚期胰腺癌患者的可行性和安全性。方法在14家医院进行了这一多中心II期前瞻、开放标签的单臂研究。经组织学确诊为胰腺浸润性导管腺癌、有可测量或可评价的病灶、东部肿瘤协作组体能状况评分0或1分、器官功能良好、年龄19岁或以上的患者符合入组条件。减量FOLFIRINOX方案的组成:奥沙利铂65 mg/m2、伊立替康135 mg/m2和亚叶酸400 mg/m2,第1天静脉注射;5?氟尿嘧啶2000 mg/m2,第1–2天持续静脉输注46 h;每2周重复。主要终点为FOLIFLIOX治疗起始后的无进展生存期。次要终点为客观缓解率、疾病控制率、总生存期、安全性和耐受性。采用Kaplan?Meier法评估了总生存期和无进展生存期。结果我们招募了来自14个中心的39例患者。客观缓解率为10.3%,疾病控制率为64.1%。6个月和1年总生存率分别为59.0%和15.4%。中位无进展生存期和总生存期分别为3.8个月[95%置信区间(confidence interval,CI)=1.5–6.0个月]和8.5个月(95%CI=5.6–11.4个月)。3或4级不良事件为中性粒细胞减少(41%)、恶心(10.3%)、食欲减退(10.3%)、贫血(7.7%)、黏膜炎(7.7%)、肺炎/胸膜融合(5.1%)和乏力(5.1%)。发生1例由感染性休克引起的治疗相关死亡。结论减量FOLFIRINOX方案有望作为二线方案治疗吉西他滨抵抗的胰腺癌。

Comparison of Protein Profiles in Sputum between COPD and Acute Exacerbation of COPD

Background and objective: Chronic obstructive pulmonary disease (COPD) is characterized by progressive airflow limitation that is associated with an abnormal inflammatory response of the lung to noxious particles or gases. Cigarette smoking is the major risk factor for the development of COPD. This study evaluated the levels of cyclophilin B in sputa from patients with COPD and COPD with acute exacerbation (AECOPD). Materials and Methods: Two-dimensional electrophoresis was used for differential display proteomics. Western blotting was used to identify and quantify cyclophilin B in sputum from subjects with AECOPD and COPD. Results: Forty-nine protein spots differed in relative intensity between the AECOPD (n = 6) and COPD (n = 6) subjects. Twenty proteins showed increased expression in the sputum of AECOPD subjects, and 29 proteins were present at lower levels in AECOPD sputum compared with COPD sputum. One of these proteins was associated with cyclophilin B. Cyclophilin B concentrations were lower in sputum from subjects with COPD (n = 4) versus AECOPD (n = 4). Conclusion: The sputum proteomic analysis suggests that changes in various proteins are associated with the development of AECOPD.

Background:Recent studies have shown that MMPs driven from tumors influence the host immune system to alleviate anti-tumor activity。The aim of this study is to examine the influence of MMP-2 and MMP-9 on the natural killer (NK) cell。Methods: NK cells were pretreated with either MMP-2 or MMP-9 in the experimental group but not in the control group。NK cell cytotoxicity against oral squamous cell carcinoma cells were examined using the [Cr5 l] release assay and the expression levels of surface receptors on NK cells were measured by the flow cytometry respectively。Results: NK cell cytotoxicity was significantly reduced in the experimental group compared to the control group, Significant increases in KIR expression and decreases in NKG2D expression on the NK cells were observed in the experimental group。Conclusions: This study suggests an additional role of MMP-2 and MMP-9 in an immune escape mechanism of OSCC。

目的探讨外伤性脾破裂合并其他严重损伤的诊断与治疗。方法分析32例外伤性脾破裂合并其他部位严重损伤的临床资料,其中行脾脏手术后再行其它合并伤的治疗17例;先行合并伤的手术治疗,再行脾脏手术11例;脾脏手术与合并伤的手术同时进行2例;合并伤手术治疗,脾脏损伤行保守治疗2例。结果治愈28例,好转转院2例,死亡2例。平均随访2.32年,术后恢复生活自理能力,无脾切除术后凶险感染发生。结论对外伤性脾破裂合并其他严重损伤患者,准确辨别危害生命的主要因素,抓住主要矛盾,制定合理的抢救治疗措施及程序,可显著提高患者救治成功率,降低病残、死亡率,改善患者的生存质量。
