Background To determine whether the introduction of pasteurized donor human milk and probiotics for infants bornweeks gestational age orweight is associated with a reduction in mortality and the incidence of necrotising enterocolitis (NEc) and sepsis. Methods we performed a retrospective analysis of two cohorts: before and after the introduction of probiotics and pasteurised donor human milk. Univariate analysis of primary and secondary outcomes was performed;variables impacting outcomes were assessed using multivariate logistic regression. Results There were 1791 infants: 1334 in the pre-donor milk/probiotic cohort and 457 in the post-donor milk/probiotic cohort. On univariate analysis, mortality (7.6 vs. 2.4%, Pcidence of sepsis (6.2 vs. 3.5%, P=0.028) were statistically significantly lower in the post-donor milk/probiotic group. NEc (2.8 vs. 1.5%, P=0.14) and non-NEc associ-ated gastrointestinal perforation (1.6 vs. 0.4%, P=0.052) were lower in the post-donor milk/probiotics cohort, but these were not statistically significant. The difference in mortality remained statistically significant on multivariate analysis in the post-donor milk/probiotic cohort compared to those in the pre-donor milk/probiotic cohort (odds ratio 0.31, 95% confidence interval 0.16–0.61). The decrease in the incidence of NEc was consistent with previous observational studies but the differ-ence was not statistically significant. conclusion The availability of probiotics and pasteurised donor human milk is associated with a reduction in mortality in very preterm infants.

Tertiary lymphoid structures(TLS)are ectopic lymphoid formations that form within nonlymphoid *** share structural and functional characteristics with secondary lymphoid structures such as lymph nodes and can contain B-cell follicles and germinal centers surrounded by a T-cell *** have been described in several types of cancers and are usually associated with positive patient ***,TLS differ vastly in cellular composition and location within tissue *** this review,we discuss factors confounding the interpretation of the evidence for a prognostic role for TLS in cancer and frame these factors in the context of translation to regular clinical use.

The dimensional and temporal distribution of Antarctic Intermediate water(AAIw)and North Pacifi c Intermediate water(NPIw)in the Philippine Sea were explored using Argo profi les and gridded Argo *** the salinity minimum of intermediate water from mid-high latitude of the southern and northern hemisphere of the Pacifi c Ocean,the properties of AAIw and NPIw merge at about 10°N with diff erent properties in the Philippine *** core of AAIw is located below 600 dbar with potential density of 27≤σθ≤27.3 kg/m 3 and salinity of 34.5≤S≤*** core of NPIw is located between 300–700 dbar with potential density of 26.2≤σθ≤27 kg/m 3 and salinity of 34≤S≤*** volume of AAIw and NPIw during January 2004 to December 2017 is negatively *** time series of AAIw and NPIw is dominated by signifi cant periods of 6 and 8 months,*** variations of AAIw and NPIw are mainly aff ected by volume transport through a 130°E section by the North Equatorial current(NEc)and North Equatorial Undercurrent(NEUc).

A functional microvascular system is imperative to build and maintain healthy *** microvasculature results in ischemia,thereby limiting the tissue’s intrinsic regeneration ***,the ability to regenerate microvascular networks is key to the development of effective cardiovascular *** stimulate the formation of new microvasculature,researchers have focused on fabricating materials that mimic the angiogenic properties of the native extracellular matrix(EcM).Here,we will review biomaterials that seek to imitate the physical cues that are natively provided by the EcM to encourage the formation of microvasculature in engineered constructs and ischemic tissue in the body.

Objective:Traditionally,gallbladder is routinely removed during left hepatectomy even if there is no gallbladder ***,adverse consequence after cholecystectomy,though rare,still occa-sionally *** study aims to evaluate the feasibility of gallbladder preservation during robotic left ***:All consecutive robotic left hepatectomy cases between December 2010 and January 2022 in Prince of wales Hospital,the chinese University of Hong Kong were retrieved from a prospectively collected *** gallbladder was preserved by moving the liver transection line just away from the gallbladder *** were divided into two groups:gallbladder preservation(GBP)and non-gallbladder preservation(NGBP).Operative results and long-term outcomes were compared between these two ***:There were 11 cases in the GBP group and 25 cases in the NGBP *** two groups were comparable in terms of the patient demographics and disease *** was no operative *** was no difference between the two groups in operative time(GBP 270 min *** 332 min,p=0.132),blood loss(GBP 50 mL *** 150 mL,p=0.115)or complication rate(GBP 27.3%*** 24.0%,p>0.999).There was also no difference in 5-year overall *** the GBP group,no patient developed specific symptoms or complications related to the preserved ***-up ultrasound or computed tomography revealed a normal appearance of the preserved gallbladders except in one patient who developed a 3-mm gallbladder *** the other hand,one(4%)patient in the NGBP group developed troublesome diarrhoea after ***:Gallbladder preservation is safe and feasible during robotic left *** preserved gallbladder does not lead to any symptoms,while postcholecystectomy diarrhoea can be avoided.

Background： The purpose of this study was to investigate how inclusion of chicory forage or ribwort forage in a cereal-based diet influenced the fecal microbial community（microbiome） in newly weaned（35 days of age） *** piglets were fed a cereal-based diet without（B） and with inclusion（80 and 160 g/kg air-dry forage） of vegetative shoots of chicory（c） and leaves of ribwort（R） forage in a 35-day growth trial. Fecal samples were collected at the start（D0）, 17（D17） and 35（D35） days after weaning and profiles of the microbial consortia were generated using terminal restriction fragment length polymorphism（T-RFLP）. 454-FLX pyrosequencing of 16 S r RNA gene amplicons was used to analyze the microbial composition in a subset of the samples already analyzed with ***： The microbial clustering pattern was primarily dependent on age of the pigs, but diet effects could also be observed. Lactobacilli and enterobacteria were more abundant at D0, whereas the genera Streptococcus, Treponema,clostridium, clostridiaceae1 and coprococcus were present in higher abundances at D35. Pigs fed ribwort had an increased abundance of sequences classified as Treponema and a reduction in lactobacilli. However, the abundance of Prevotellaceae increased with age in on both the chicory and the ribwort diet. Moreover, there were significant correlations between the abundance of Bacteroides and the digested amount of galactose, uronic acids and total non-starch polysaccharides, and between the abundance of Bacteroidales and the digested amount of ***： This study demonstrated that both chicory and ribwort inclusion in the diet of newly weaned pigs influenced the composition of the fecal microbiota and that digestion of specific dietary components was correlated with species composition of the microbiota. Moreover, this study showed that the gut will be exposed to a dramatic shift in the microbial community structure several weeks after weaning.

Tight focusing properties of an azimuthally polarized Gaussian beam with a pair of vortices through a dielectric interface is theoretically investigated by vector diffraction theory. For the incident beam with a pair of vortices of opposite topological charges, the vortices move toward each other, annihilate and revive in the vicinity of focal plane, which results in the generation of many novel focal patterns. The usable focal structures generated through the tight focusing of the double-vortex beams may find applications in micro-particle trapping, manipulation, and material processing, etc.
