Coupling virio-and bacterioplankton populations with environmental variable changes in the Bohai sea

Uncovering the role of environmental factors and finding critical factors which harbor significant fractions in governing microbial communities remain key questions in coastal marine *** detect the interactions between environmental factors and distributions of virio-and bacterioplankton in trophic coastal areas,we used flow cytometry to investigate the abundance of virio-and bacterioplankton covering 31 stations in the Bohai sea of *** results suggested that the average abundance of total virus(TV)in winter(~2.29×108 particles/mL)was slightly lower than in summer(~3.83×108 particles/mL).The mean total bacterial abundance(TB)was much lower in winter(~2.54×107 particles/mL)than in summer(~5.43×107 particles/mL).Correlation analysis via redundancy analysis(RDA)and network analysis among virioplankton,bacterioplankton and environmental factors revealed that the abundances of viral and bacterial subpopulations depend on environmental *** winter,only temperature significantly influenced the abundances of virio-and *** summer,in addition to temperature,both salinity and nutrient(siO2)had a remarkable impact on the distribution of virioand *** results showed a clear seasonal and trophic pattern throughout the whole water system,which revealed that temperature and eutrophication may play crucial roles in microbial distribution pattern.

Total Arterial Revascularization with Internal Mammary Artery or Radial Artery π Graft Configuration

summary： To investigate the clinical use of π graft in total arterial revascularization and its outcomes, a retrospective analysis of 23 patients out of 1000 patients undergoing total arterial coronary bypass surgery with a π graft between september 1994 and December 2004 was performed. In the selected patients for the management of triple vessel disease with middle diagonal/intermediate ramus disease such that a skip with the left internal mammary artery （LIMA） or radial artery （RA）, the main stem of π graft, to the left anterior descending coronary artery （LAD） will not work and the right internal mammary artery （RIMA） or right gastroepiploic artery （RGEA） cannot pick up the diagonal/intermediate ramus, hence the LAD and diagonal/intermediate ramus were grafted with a mini Y graft using the distal segment of LIMA, RIMA, RA or RGEA, together with the bilateral internal mammary artery （BIMA） or LIMA-RA T graft to compose π graft. Twenty-three patients （18 males, 5 females） underwent the π graft procedure. There were no deaths or episodes of myocardial infarction, stroke, and deep sternal wound infection. One patient required reopening for controlling bleeding. Until the end of 2004, during a mean follow-up of 81.0±28.4 months, no angina needing re intervention or operative therapy or coronary related death occurred. In conclusion, in patients with specific coronary artery anatomy/stenosis, the BIMA （sometimes LIMA with RA or RGEA） π graft can be successfully performed for total arterial revascularization with good midterm outcomes.

Total Arterial Revascularisation in Left Ventricular Dysfunction

The feasibility and safety of total arterial coronary revascularization with 2 arterial conduits in patients with impaired left ventricular function was evaluated. Data were prospectively collected on all patients with multiple vessel disease and moderately or severely impaired left ventricular function, who underwent coronary surgery with the intention of total arterial revascularization with 2 conduits between March 1995 and August 2002. One hundred and seventy-nine patients were included in the study. Acute coronary insufficiency was present in 3 patients and 43 had unstable angina. severe left ventricular impairment was present in 29 patients. There were 17 redo operations including 3 redo-redo procedures. Eighty-two percent of patients had a Y graft configuration from the left internal mammary artery （right internal mammary artery 40. 8 %, radial artery 33. 5 %, other 7.8 % ）. The perioperative mortality was 2. 2 %, myocardial infarction 1.7 % and stroke 0. 6 %. Total arterial revascularization in patients with ischaemic left ventricular dysfunction can be safely performed with 2 arterial conduits. The radial artery provides conduit length greater than the right internal mammary artery and allows full revascularization despite left ventricular dilatation.

summary： Phrenic nerve injury after cardiac surgery increases postoperative pulmonary complications. The purpose of this study was to analyze the causes and effects of phrenic nerve injury after cardiac surgery. Prospectively collected data on 2084 consecutive patients who underwent cardiac surgery from Jan. 1995 to Feb. 2002 were analyzed. Twenty-eight preoperative and operation related variables were subjected to logistic analysis with the end point being phrenic nerve injury. Then phrenic nerve injury and 6 perioperative morbidities were included in the analysis as variables to determine their independent predictive value for perioperative pulmonary morbidity. An identical approach was used to identify the independent risk factors for perioperative mortality. There were 53 phrenic nerve injuries （2.5 %）. There was no phrenic nerve injury in non-coronary surgery or coronary surgery using conduits other than the internal mammary artery. The independent risk factors for phrenic nerve injury were the use of internal mammary artery （Odds ratio （OR）=14.5） and the presence of chronic obstructive pulmonary disease （OR=2.9）. Phrenic nerve injury was an independent risk factor （OR=8.1） for perioperative pulmonary morbidities but not for perioperative mortality. Use of semi-skeletonized internal mammary artery harvesting technique and drawing attention to possible vascular or mechanical causes of phrenic nerve injury may reduce its occurrence. Unilateral phrenic nerve injury, although rarely life-threatening, is an independent risk factor for postoperative respiratory complications. When harvesting internal mammary arteries, it should be kept in mind avoiding stretching, compromising, or inadvertently dissecting phrenic nerve is as important as avoiding damage of internal mammary artery itself.

This paper reviews the progress on nano-aperture vertical-cavity surface-emitting lasers （VCsELs）. The design, fabrication, and polarization control of nano-aperture VCsELs are reviewed. With the nanoaperture evolving from conventional circular and square aperture to unique C-shaped, H-shaped, I-shaped, and bowtie-shaped aperture, both the near-field intensity and near-field beam confinement from nanoaperture VCsELs are significantly improved. As a high-intensity compact light source with sub-100- nm spot size, nano-aperture VCsELs are promising to realize many new near-field optical systems and applications.

Current recommendations of managing HBV infection in preconception or pregnancy

Hepatitis B remains a leading cause of cirrhosis, hepatocellular carcinoma and liver transplantation worldwide. Management of chronic hepatitis B during pregnancy is challenging. Transmission of hepatitis B to infants still occurs perinataily although immunoprophylaxis is widely available for infants born to mothers with chronic hepatitis B infection. The emerging data suggest that initiation of antiviral therapy in the beginning of the third trimester in highly viremic mothers can prevent immunoprophylaxis failure in their infants. The available drug safety data show that lamivudine, telbivudine and tenofovir are generally safe to be used during the pregnancy. In order to minimize the fetal exposure to the antiviral medication, antiviral therapy during the pregnancy should be limited to a selected group of patients with cirrhosis, high hepatitis B viral load, or prior history immunoprophylaxis failure. An elective Caesarean section may reduce the risk of perinatal transmission. For those females planning for pregnancy or in early stage of pregnancy, communication and follow-up among obstetrician, gastroenterologist, and primary care physician are important. In this article, we will review the features of hepatitis B infection before, during and after the pregnancy; the risk factors that increase mother-tochild transmission; safety data on antiviral drug use during pregnancy; and the potential role of Caesarean section in selected cases.

Exploring the impact of COVID-19 on the movement behaviors of children and youth:A scoping review of evidence after the first year

Purpose:The objective of this scoping review was to summarize systematically the available literature investigating the relationships between the coronavirus disease 2019(COVID-19)pandemic and movement behaviors(physical activity,sedentary behavior,and sleep)of school-aged children(aged 511 years)and youth(aged 1217 years)in the first year of the COVID-19 ***:searches for published literature were conducted across 6 databases on 2 separate search dates(November 25,2020,and January 27,2021).Results were screened and extracted by 2 reviewers(DCP and KR)independently,using *** numeric analysis and content analysis were undertaken to present thematically the findings of included studies according to the associated impact on each movement ***:A total of 1486 records were extracted from database searches;of those,150 met inclusion criteria and were included for *** 150 articles,110 were empirical studies examining physical activity(n=77),sedentary behavior/screen time(n=58),and sleep(n=55).Results consistently reported declines in physical-activity time,increases in screen time and total sedentary behavior,shifts to later bed and wake times,and increases in sleep *** reported impacts on movement behaviors were greater for youth than for ***:The COVID-19 pandemic is related to changes in the quantity and nature of physical activity,sedentary behavior,and sleep among children and *** is an urgent need for policy makers,practitioners,and researchers to develop solutions for attenuating adverse changes in physical activity and screen time among children and youth.
