MELD or MELD-Na as a predictive Model for Mortality Following Transjugular Intrahepatic portosystemic Shunt placement

Background and Aim:The model for end-stage liver disease(MELD)was originally developed to predict survival after transjugular intrahepatic portosystemic shunt(TIpS).The MELD-sodium(MELD-Na)score has replaced MELD for organ allocation for liver ***,there are limited studies to compare the MELD with MELD-Na to predict mortality after ***:We performed a retrospective chart review of patients who underwent TIpS placement between 2006 and 2016 at our *** primary outcome was mortality,and the secondary outcomes sought to assess which variables could provide prognostic information for mortality after TIpS *** performed receiver operating characteristic(ROC)curve analysis to assess the performance of MELD and ***:There were 186 eligible patients in the *** mean pre-TIpS MELD and MELD-Na were 13 and 15,***,mortality after TIpS was 15%at 30 days and 16.7%at 90 *** a comparison of the areas under the ROCs for MELD and MELD-Na,MELD was superior to MELD-Na for 30-day(0.762 vs.0.709)and 90-day(0.780 vs.0.730)mortality after *** optimal cutoff score for 30-day mortality was 15(0.676–0.848)for MELD and 17(0.610–0.808)for MELD-Na,whereas the optimal cutoff score for 90-day mortality was 16(95%CI:0.705–0.855)for MELD and 17(95%CI:0.643–0.817)for *** were 24 patients with high MELD-Na≥17,but with low MELDp was 8.3%.Conclusions:Although MELD-Na is a superior prognostic tool to MELD for predicting overall mortality in cirrhotic patients,MELD tended to outperform MELD-Na to predict mortality after TIpS.

Many animals use color to signal their quality and/or behavioral *** signals have been well studied in the contexts of competi-tion and mate choice;however,the role of these signals in nonsexual,affiliative relationships is not as well ***,we used wild social groups of the cichlid fish Neolamprologus pulcher to investigate whether the size of a brightly colored facial patch was related to 1)individual quality,2)social dominance,and/or 3)affiliative *** with larger patches spent more time foraging and tended to perform more aggressive acts against conspecific territory *** did not find any evidence that the size of these yellow patches was related to social rank or body size,but dominant males tended to have larger patches than dominant ***,patch size had a rank-specific relationship with the number of affiliative interactions that individuals engaged *** males with large patches received fewer affiliative acts from their groupmates compared to dominant males with small ***,subordinates with large patches tended to receive more affiliative acts from their groupmates while performing fewer affiliative acts *** together,our results suggest that patch size reflects interindividual variation in foraging effort in this cichlid fish and offer some of the first evidence that colorful signals may shape affiliative relationships withinwildsocialgroups.

Modeling a business to match its customer perceived （and customer desired） outcomes, remains an on-going task. This paper considers shows how a service value networks （SVNs） approach may be engaged to model, and deliver, understanding of the front-end business and its direct engagement with its immediate （or front-end） customers. These front-end customers may be either：（1） its off-line, in-store customers - engaging directly with the business sales staff （or representative）, or-（2） its on-line, virtual customers - engaging with the business via internet or remote access. In this situation, both the business, and the customer draw upon their respective viewpoints, and both sides influence the interaction. External to these business and customer influences are additional factors that capture the immediate and broader global effects - termed environmental effects. These three business-customer engagement areas of influence are captured by a SVN SEM approach. Without a business-customer encounter of some kind the likelihood of a services business, and a prospecting customer, successfully engaging in an exchange process is reduced. This exchange may be a physical, and/or services exchange, and/or an information and/or ideas exchange. To the business, this encounter is, in effect, a trade, and as a result, the business targets acquiring an economic exchange that will ultimately deliver a net positive economic outcome. To the customer, external and internal information feeds, servicing, perceived value, and satisfaction, are key encounter drivers. SVNs offers a new way to understand the business-customer encounter, and to then utilize this acquired knowledge to either：（1） build a strategic management model, or （2） reengineer its business networks, and to then create a re-positioned, more customer-aligned business operation - one that is capable of delivering win - win, competitive business-customer solutions.

BENCHMARKING ON-LINE SERVICES INDUSTRIES

The Web Quality Analyser (WQA) is a new benchmarking tool for industry. It has been extensively tested across services industries. Forty five critical success features are presented as measures that capture the user’s perception of services industry websites. This tool differs to previous tools, in that it captures the information technology (IT) related driver sectors of website performance, along with the marketing-services related driver sectors. These driver sectors capture relevant structure, function and performance components. An ‘on-off’ switch measurement approach determines each component. Relevant component measures scale into a relative presence of the applicable feature, with a feature block delivering one of the sector drivers. Although it houses both measurable and a few subjective components, the WQA offers a proven and useful means to compare relevant websites. The WQA defines website strengths and weaknesses, thereby allowing for corrections to the website structure of the specific business. WQA benchmarking against services related business competitors delivers a position on the WQA index, facilitates specific website driver rating comparisons, and demonstrates where key competitive advantage may reside. This paper reports on the marketing-services driver sectors of this new benchmarking WQA tool.

Abnormal liver chemistries as a predictor of COVID-19 severity and clinical outcomes in hospitalized patients

BACKGROUND Abnormal liver chemistries are common findings in patients with Coronavirus Disease 2019(COVID-19).However,the association of these abnormalities with the severity of COVID-19 and clinical outcomes is poorly understood AIM We aimed to assess the prevalence of elevated liver chemistries in hospitalized patients with COVID-19 and compare the serum liver chemistries to predict the severity and in-hospital *** This retrospective,observational study included 3380 patients with COVID-19 who were hospitalized in the Johns Hopkins Health System(Baltimore,MD,United States).Demographic data,clinical characteristics,laboratory findings,treatment measures,and outcome data were *** regression modeling was used to explore variables associated with abnormal liver chemistries on admission with disease severity and prognosis RESULTS A total of 2698(70.4%)had abnormal alanine aminotransferase(ALT)at the time of *** more prevalent abnormal liver chemistries were aspartate aminotransferase(AST)(44.4%),alkaline phosphatase(ALp)(16.1%),and total bilirubin(T-Bil)(5.9%).Factors associated with liver injury were older age,Asian ethnicity,other race,being overweight,and *** ALT,AST,T-Bil,and ALp levels were more commonly associated with disease *** adjusted Cox regression analysis revealed that abnormal AST and T-Bil were associated with the highest mortality risk than other liver injury indicators during *** AST,T-Bil,and ALp were associated with a need for vasopressor drugs,whereas higher levels of AST,T-Bil,and a decreased albumin levels were associated with mechanical ventilation CONCLUSION Abnormal liver chemistries are common at the time of hospital admission in COVID-19 patients and can be closely related to the patient’s severity and *** liver chemistries,specifically ALT,AST,ALp,and T-Bil levels,can be used to stratify risk and predict the need for advanced therapies in t

Background: While agriculture has taken much environmental water in Australia's Murray-Darling Basin, agricultural expansion has resulted in a vast number of farm dams, almost three-quarters of a million in the Murray-Darling Basin ***: Over a summer we studied(1) waterbird abundance and species richness and(2) the influence of biophysical and landscape characteristics across 49 farm dams at a large mixed-enterprise farm in northern Victoria on the southern reach of the Murray-Darling ***: On average, dams were found to host 27.1 ± 71.1 individuals/ha and 1.8 ± 2.9 species per pond. Such densities are comparable to those on natural wetlands. Dam surface area and perimeter and amount of vegetation were positively and strongly correlated with the Rallidae density(birds/ha), but no other parameters were strongly correlated with any other functional group. The landscape in which the dams were embedded had a highly significant effect(p plemented with further studies in other parts of the Basin and on other taxa, but given at our site they supported similar densities of individuals and species to natural wetlands, and given the fact that there are 710,539 farm dams in the Murray-Darling Basin, which hosts much of Australia's waterbird fauna, it is reasonable to suggest that farm dams are overlooked, and possibly very important, avian biodiversity hotspots. It also highlights the importance of a landscape setting, in which dams are situated, on the number of birds using the dams.

Live cell imaging methods provide important insights into the dynamics of cellular processes that cannot be derived easily from population-averaged *** the bioenergy field,much research is focused on fermentation of cellulosic biomass by thermophilic microbes to produce biofuels;however,little effort is dedicated to the development of imaging tools to monitor this dynamic biological *** is,in part,due to the experimental challenges of imaging cells under both anaerobic and thermophilic *** an imaging system is described that integrates confocal microscopy,a flow cell device,and a lipophilic dye to visualize *** to technical obstacles regarding suitable fluorescent markers,photodamage during imaging,and maintenance of environmental conditions during imaging are *** system was utilized to observe cellulose colonization by Clostridium thermocellum under anaerobic conditions at 60℃.This method enables live cell imaging of bacterial growth under anaerobic and thermophilic conditions and should be widely applicable to visualizing different cell types or processes in real time.

Evaluation of atherosclerotic lesions in cholesterol-fed mice during treatment with paclitaxel in lipid nanoparticles： a magnetic resonance imaging study

Cholesterol-core nanoparticles （LDE） have been shown to be recognized by low-density lipoprotein receptors （LDLR） after administration; therefore, LDE is an ideal vehicle to deliver drug with targeting property. paclitaxel, when incorporated into LDE, promotes atherosclerosis regression with reduced drug toxicity in rabbits through LDLR. Here, we tested whether LDE-paclitaxel could still be effective in reducing diet-induced atherosclerosis in a mouse model without LDLR. Nineteen LDLR knockout male mice were fed 1% cholesterol for 12 weeks. Then, 12 animals received 4-weekly intraperitoneal LDE-paclitaxel （4 mg/kg） while 7 controls received saline solution. On week 12 and 16, in vivo MR/of the aortic roots was performed. Aorta macroscopy was made after euthanasia. Reduction ofatherosclerotic lesions was observed. LDE-paclitaxel treatment resulted in reduction of wall area （14%） and stenosis （22%） by MR/and 33% by macroscopy. Thus, LDE-paclitaxel may produce pharmacological effects through LDE uptake by mechanisms other than LDLR.

Radical Addition of perfluorinated Alkyl Iodides to Multi- Layered Graphene and Single-Walled Carbon Nanotubes

A simple one-pot reaction that serves to functionalize graphite nanosheets(graphene)and single-walled carbon nanotubes(SWNTs)with perfluorinated alkyl groups is *** radical addition of 1-iodo-1H,1H,2H,2H-perfluorododecane to ortho-dichlorobenzene suspensions of the carbon nanomaterial is initiated by thermal decomposition of benzoyl ***,UV photolysis of 1-iodo-perfluorodecane serves to functionalize the carbon ***-SWNTs,perfluorododecyl-graphene,and perfluorodecyl-graphene are characterized by infrared(IR)and Raman spectroscopy,X-ray photoelectron spectroscopy(XpS),thermogravimetric analysis(TGA),and atomic force microscopy(AFM).The products show enhanced dispersability in CHCl3 as compared to unfunctionalized starting *** advantage of this one-pot functionalization procedure lies in the use of pristine graphite as starting material thereby avoiding the use of harsh oxidizing conditions.
