Celiac disease in South-West of Iran

AIM： Celiac disease is characterized by life-long gluten intolerance. Clinical features of patients with celiac disease are variable. Studies about the prevalence of celiac disease in our country are scarce and there is no study on the prevalence of celiac disease in southern Iran. In the current study, clinical, laboratory and histological features of 52 patients with celiac disease were evaluated. METHODS： In a cross sectional study we retrospectively studied the characteristics of 52 celiac patients at Ahwaz JundiShapour University Hospitals （AJSUH） from November 1, 1999 to 1st Sep 2004. Intestinal biopsy and serum antigliadin and anti-endomysium antibodies were used for the diagnosis of patients. Mucosal lesions were classified according to the criteria of Marsh. Antigliadin antibodies were measured with a commercial enzyme-linked immunosorbent assay. Anti-endomysium antibodies were analyzed by indirect immunofluorescence with the use of a section of monkey esophagus. Routine hematological and biochemical analyses and measurement of immunoglobulin levels were undertaken.RESULTS： Male： female ratio was 1.08. The mean ＋ SD patient age was 21 ＋ 4.5 years （range 10-70 years） and the most common symptoms were diarrhea and weight loss （78.8%） followed by fatigue （73.1%）, pallor （65.4%）, anorexia （40.4%）, abdominal distention （32.7%）, and failure to thrive （23.1%）. Diarrhea and weight loss and fatigue were the most common findings. Iron deficiency anemia was found in 63.2% of patients and this became normal after adoption of a gluten-free diet in all patients. Immunoglobulin A, IgG antigliadin antibodies and IgA anti-endomysium antibodies were found in 33 and 48 cases, 78.8% and 85.4% of patients, respectively. Biopsy of the small intestine revealed that 90.4% of patients had typical lesions according to the Marsh classification. This work presents,design and specific absorption rate(SAR)analysis of a 37GHz antenna,for 5th Generation(5G)*** proposed antenna comprises of 4-elements of rectangular patch and an even *** radiating element is composed of copper material supported by Rogers RT5880 substrate of thickness,0.254 mm,dielectric constant(εr),2.2,and loss tangent,*** 4-elements array antenna is compact in size with a dimension of 8mm×20mm in length and *** radiating patch is excited with a 50 ohms connector i.e.,*** antenna resonates in the frequency band of 37 GHz,that covers the 5G *** antenna behavior is studied both in free space and in the proximity of the human *** models of the human body,i.e.,belly,hand,and head(contain skin,fat,muscles,and bone)are considered for on-body *** resonant frequency,the antenna gives a boresight gain of 11.6 *** antenna radiates efficiently with a radiated efficiency of more than 90%.Also,it is observed that the antenna detunes to the lowest in the proximity of the human body,but still a good impedance matching is achieved considering the−10 dB ***,SAR is also being *** safe limit of 2 W/kg for any 10 g of biological tissue,specified by the European International Electro Technical Commission(IEC)has been *** calculated values of SAR for human body models,i.e.,belly,hand and head are 1.82,1.81 and 1.09 W/kg,*** SAR values are less than the international recommendations for the three ***,the simulated and measured results of the antenna are in close agreement,which makes it,a potential candidate for the fifth-generation smart phones and other handheld devices.

Virtual reality is an emerging field in the whole *** problem faced by people today is that they are more indulged in indoor technology rather than outdoor ***,the proposed system introduces a fitness solution connecting virtual reality with a gaming interface so that an individual can play first-person *** system proposed in this paper is an efficient and cost-effective solution that can entertain people along with playing outdoor games such as badminton and cricket while sitting in the *** track the human movement,sensors Micro Processor Unit(MPU6050)are used that are connected with Bluetoothmodules andArduino responsible for sending the sensor data to the ***,the sensor data is sent to a machine learning model,which detects the game played by the *** detected game will be operated on human gestures.A publicly available dataset named IM-Sporting Behaviors is initially used,which utilizes triaxial accelerometers attached to the subject’s wrist,knee,and below neck regions to capture important aspects of human *** main objective is that the person is enjoying while playing the game and simultaneously is engaged in some kind of sporting *** proposed system uses artificial neural networks classifier giving an accuracy of 88.9%.The proposed system should apply to many systems such as construction,education,offices and the educational *** experimentation proved the validity of the proposed system.

Identifying human actions and interactions finds its use in manyareas, such as security, surveillance, assisted living, patient monitoring, rehabilitation,sports, and e-learning. This wide range of applications has attractedmany researchers to this field. Inspired by the existing recognition systems,this paper proposes a new and efficient human-object interaction recognition(HOIR) model which is based on modeling human pose and scene featureinformation. There are different aspects involved in an interaction, includingthe humans, the objects, the various body parts of the human, and the backgroundscene. Themain objectives of this research include critically examiningthe importance of all these elements in determining the interaction, estimatinghuman pose through image foresting transform (IFT), and detecting the performedinteractions based on an optimizedmulti-feature vector. The proposedmethodology has six main phases. The first phase involves preprocessing theimages. During preprocessing stages, the videos are converted into imageframes. Then their contrast is adjusted, and noise is removed. In the secondphase, the human-object pair is detected and extracted from each image *** third phase involves the identification of key body parts of the detectedhumans using IFT. The fourth phase relates to three different kinds of featureextraction techniques. Then these features are combined and optimized duringthe fifth phase. The optimized vector is used to classify the interactions in thelast phase. TheMSRDaily Activity 3D dataset has been used to test this modeland to prove its efficiency. The proposed system obtains an average accuracyof 91.7% on this dataset.

Survival rate and the determinants of progression from HIV to AIDS and from AIDS to the death in Iran: 1987 to 2016

Objective: To examine the prognostic factors of progression from HIV to AIDS and AIDS to the death in people living with HIV/AIDS in ***: In this registry-based retrospective cohort study, we recruited 28 873 HIV-infected people from 158 Behavioral Diseases Counseling Centers of *** outcomes of interest included survival rates from HIV diagnosis to AIDS and from AIDS to the *** used Kaplan-Meier and Cox regression model to investigate survival rate and factors affecting on survival controlling effect of confounding ***: The one, three, five, and ten-year survival rate from HIV to AIDS were 85%, 73%, 61% and 32%, and for AIDS to death were 90%, 81%, 74% and 55%, *** Cox regression analysis indicated that the risk of progression from AIDS phase toward death in individuals with CD4 less than 200/mm^3, infected with tuberculosis(TB) and not treated by antiretroviral therapy(ART) was 2.17(95% CI: 1.62-2.90), 1.49(95% CI: 1.01-2.20) and 4.88(95% CI: 3.42-6.96) times higher *** risk of progression to AIDS in patients with baseline CD4 less than 200/mm^3 was 2.32(95% CI: 2.14, 2.51) times higher than patients with CD4 > 200/mm^3(P=0.001).On the other hand, tuberculosis increases the risk of death by 49.0%(P=0.04).The hazard ratio of death in patients who did not receive ART was 4.88(95% CI: 3.42, 6.96) times higher than patients who received ART(P<0.001).Conclusion: The early detection of HIV, the screening and treatment of TB and receiving the ART improve the survival of HIV/AIDS patients significantly, and prevent the transmission of HIV to other people.

Yield of testing for micronutrient deficiencies associated with pancreatic exocrine insufficiency in a clinical setting: An observational study

BACKGROUND Pancreatic exocrine insufficiency(PEI)can be difficult to diagnose and causes maldigestion symptoms and *** has been a number of studies that have identified PEI associated micronutrient deficiencies(PEI-MD),however there is variation in both the frequency and type of PEI-MD reported,with the majority of studies including patients with PEI due to chronic pancreatitis(CP)or CP without *** is a paucity of information regarding the prevalence of PEIMD in patients with PEI without CP and the yield of testing for PEI-MD in a clinical setting in patients with suspected benign pancreatic *** To prospectively assess the yield and type of PEI–MD in patients with and without PEI secondary to benign pancreatic *** Patients investigated for maldigestion symptoms with Faecal Elastase-1(FEL-1)and suspected or proven benign pancreatic disease were prospectively *** the time of FEL-1 testing,serum samples were taken for micronutrients identified by previous studies as PEI-MD:prealbumin,retinol binding protein,copper,zinc,selenium,magnesium and later in the study lipid adjusted vitamin ***-1 was recorded,with a result<200μg/g considered diagnostic of *** underwent computed tomography(CT)imaging when there was a clinical suspicion of CP,a new diagnosis of PEI recurrent,pancreatic type pain(epigastric abdominal pain radiating to back with or without previous acute pancreatitis attacks)or weight *** After exclusions,112 patients were recruited that underwent testing for FEL-1 and *** was identified in 41/112(36.6%)patients and a pancreatic CT was performed in 82 *** a PEI-MD was identified in 21/112(18.8%)*** yield of PEI-MD was 17/41(41.5%)if PEI was present which was significantly higher than those without 4/71(5.6%)(P=0.0001).The yield of PEI–MD was significantly higher when PEI and CP were seen together 13/22(59.1%)compared to CP without PEI and PEI without CP(P<0.03).In

Clinical profile of gastric cancer in Khuzestan, southwest of Iran

AIM： To analyze the characteristics of epidemiological, clinical and survival patterns among patients with carcinoma of the stomach. METHODS： We retrospectively studied the characteristics of 186 gastric adenocarcinoma patients at Ahwaz Jundishapur University Hospitals （AJSUH） from September 1, 1996 to September 1, 2002. All the patients had histopathologically-confirmed malignancy. Demographic variables, family history of gastric cancer （GC）, clinicopathologic characteristics and treatment-related variables were analyzed. Univariate analysis was performed with the log-rank test and multivariate analysis with Cox regression. P 〈 0.05 was considered statistically significant. RESULTS： Male to female ratio was 2.6：1. The mean age was 60.6 years and 14% of the patients were younger than 40 years. Adenocarcinoma, gastric lymphoma, and gastric metastasis were found in 94.5%, 2.3%, and 3% patients, respectively. There was an average of 6-mo delay between the initial symptoms and the diagnosis. Among adenocarcinoma groups, intestinal type was the commonest （55.9%） and the distal third was the most common localization （88.4%）. One hundred and thirty-four patients （72.1%） were males. Thirty-one patients （17%） had a family history of GC. Surgery was performed in 90% of patients （non-curative）. CONCLUSION： The epidemiological features of GC in south Iran mimic those in high-risk areas. There is a higher frequency of GC in young patients at our institution. Patients are detected and treated after a relatively long delay. Most patients present in advanced stages, which favors a poor overall survival, Family history of GC has a significant problem in our area, Studying the etiology of this cancer in south Iran and earlier diagnosis and subsequent better cares are recommended.

Nowadays,activities of daily living(ADL)recognition system has been considered an important field of computer *** and optical sensors are widely used to assess the daily living activities in healthy people and people with certain *** conventional ADL utilizes RGB optical sensors but an RGB-D camera with features of identifying depth(distance information)and visual cues has greatly enhanced the performance of activity *** this paper,an RGB-D-based ADL recognition system has been ***,human silhouette has been extracted from the noisy background of RGB and depth images to track human movement in a *** on these silhouettes,full body features and point based features have been extracted which are further optimized with probability based incremental learning(PBIL)***,random forest classifier has been used to classify activities into different *** n-fold crossvalidation scheme has been used to measure the viability of the proposed model on the RGBD-AC benchmark dataset and has achieved an accuracy of 92.71%over other state-of-the-art methodologies.
