Pyunkang-hwan (Pyunkang-Tang) ameliorates air pollutant-induced inflammatory hypersecretion of airway mucus and bleomycin-induced pulmonary fibrosis in rats

OBJECTIVE: To investigate the effect of Pyunkanghwan(Pyunkang-tang) extract(PGT) on secretion of airway mucin in an experimental animal model involving hyperplasia of goblet cells and mucus hypersecretion, and to test its effects on bleomycin(BLM)-induced pulmonary fibrosis in ***: The protective activity of orally administered PGT was assessed in two rat pulmonary disease models. Effects on hypersecretion of pulmonary mucin in sulfur dioxide(SO_2)-induced bronchitis in rats were assessed by quantifying the amount of mucus secreted and examining histopathology in the tracheal epithelium. In a rat model for BLMinduced pulmonary fibrosis, toxicity to the pulmonary system was examined by measuring levels of malondialdehyde and hydroxyproline,indicators of lipid peroxides and collagen,respectively, in lung tissue 28 days post-BLM treatment. Serial sections of lung tissue were stained with Masson trichrome to visualize collagen deposition. Effects of PGT on collagen synthesis were also assessed in vitro, in a cell culture ***: PGT inhibited mucin secretion and normalized SO_2-induced increased muco- substances in goblet cells. In the BLM- induced model,PGT decreased the characteristic histopathological features of lung fibrosis and inhibited fibrotic lesions, as indicated by decreased hydroxyproline content. PGT also inhibited the BLM-induced increase in malondialdehyde levels, demonstrating its protective effect against lipid peroxidation in cell membranes of the lung. In MLg 2908 mouse lung fibroblast cells, PGT decreased transforming growth factor(TGF)-β-stimulated type I collagen ***: PGT can inhibit both hypersecretion of airway mucins and pulmonary fibrosis.

Exercise improves the effects of testosterone replacement therapy and the durability of response after cessation of treatment： a pilot randomized controlled trial

The effects of the combination of exercise and TRT on symptoms of late-onset hypogonadism （Loh） and the durability of response after cessation of TRT were investigated. A total of fifty patients with erectile dysfunction （ED） who had a sedentary lifestyle and low serum total testosterone （T） levels were enrolled and followed for 20 weeks. Patients were randomly divided into two groups; all of them received T gel for 12 weeks and it was discontinued for 8 weeks. Patients assigned to Group II were offered a supervised exercise program for 20 weeks. Measurement of serological testing was performed and self-assessment questionnaires and Global Assessment Question （GAQ） were asked. Baseline characteristics and the initial symptom scores showed no significant difference between the two groups. Serum total T levels and the symptom scores were increased at 12 weeks in both groups, and Group II showed better results with statistical significance. There was a decrease in T levels and worsening of symptom scores at week 20 compared to week 12 in both groups, and Group II showed better results with statistical significance. On the GAQ, Group II showed higher ratio of ＂yes＂ at week 12 and the same tendency was sustained at week 20 with significant difference between two groups. The combination of exercise and TRT showed significant improvements in serum T levels and Loh symptoms compared to TRT alone. In addition, these improvements were maintained in the combination group with continuous exercise, even after cessation of TRT.

BACKGROUND Even at present,we are in the middle of the novel coronavirus disease 2019(COVID-19)pandemic and are facing challenges in trial and ***,emergency surgery for patients with suspected COVID-19 is burdensome not only for patients but also for healthcare ***,we established a surveillance system in the emergency room and established principles for managing patients suspected of COVID-19 who require emergency *** SUMMARY A 67-year-old man was diagnosed with appendicitis in March *** wife was diagnosed with COVID-1910 d earlier,and the patient was in close contact with *** patient tested negative twice on an upper respiratory COVID-19 reverse transcription–polymerase chain reaction screening test,but chest X-ray and chest computed tomography revealed patchy ground-glass opacity in both upper lobes of the patient’s *** same emergency surgery procedure for patients with confirmed COVID-19 was applied to this patient suspected of having the disease to ensure that surgery was not delayed while waiting for the reverse transcription–polymerase chain reaction results.A few hours after surgery,the upper respiratory tract specimen taken in the emergency room was negative for COVID-19 but the lower respiratory tract specimen was found to be positive for the *** When COVID-19 is suspected,emergency surgery should be performed as for confirmed COVID-19 without delay.

Clinicopathological features and prognosis of combined hepatocellular carcinoma and cholangiocarcinoma after surgery

BACKGROUND： Combined hepatocellular carcinoma and cholangiocarcinoma （cHCC-CC） is a rare subtype of primary liver cancer consisting of both hepatocellular carcinoma （HCC） and cholangiocarcinoma （CC）. Because of the rarity of this tumor, its feature is poorly understood. The present study aimed to evaluate the clinicopathological features and long-term prognosis of patients with cHCC-CC after surgery and to compare with those of the patients with stage-matched HCC and CC. METHODS： The dinicopathological features of the patients who underwent surgery for cHCC-CC at our center during the period of 2001-2010 were retrospectively analyzed and compared with those of stage-matched HCC and CC patients. Cancer staging was performed according to the AJCC Cancer Staging Manual （6th ed.）. Overall survival and disease-free survival were compared among the groups and prognostic factors of cHCC-CC were evaluated. RESULTS： Significant differences were observed in clinico- pathological features among 42 patients with cHCC-CC, 90 patients with HCC and 45 patients with CC. Similar to HCC patients, cHCC-CC patients had frequent hepatitis B virus antigen positivity, microscopic vessel invasion, cirrhosis and high level of serum alpha-fetoprotein. Similar to CC patients, cHCC-CC patients showed increased bile duct invasion and decreased capsule. The 1-, 3-, and 5-year overall survival and disease- free survival of patients with cHCC-CC were not significantly different from those with stage-matched patients with CC;but significantly poorer than those with HCC. In subanalysis of patients with stage Ⅱ, the overall survival in patients with cHCC-CC or CC was significantly poorer than that in patients with HCC. We did not find the difference in patients with other stages. Univariate analysis of overall and disease-free survival of patients with cHCC-CC showed that the vascular invasion and intrahepatic metastasis were the significant predictive factors. Role of oxygen free radicals in patients with acute pancreatitis

AIM:The generation of oxygen free radicals has been implicated in the pathogenesis of experimental pancreatitis. The aim of this study was to determine the role of oxygen free radicals in patients with acute pancreatitis. METHODS:The plasma levels of C-reactive protein(CRP), lipid peroxide(LPO),myeloperoxidase(MPO)and superoxide dismutase(SOD)were measured in 13 patients with acute pancreatitis and 14 healthy volunteers. RESULTS:Among the patients with acute pancreatitis,there were higher plasma levels of LPO and MPO and lower SOD activity in patients with severe pancreatitis than in those with mild ***,there was no significant difference in the serum marker of oxidative stress no matter what the etiology *** LPO level was especially correlated with the concentration of serum CRP and CT severity index. CONCLUSION:The oxygen free radicals may be closely associated with inflammatory process and the severity of acute ***,the concentration of plasma LPO is a meaningful index for determining the severity of the disease.

Abstract This study was conducted to clarify the characteristics high-pressure water sprayer in the period from June 2010 to April 2011 of algal succession following rock scraping using hoe or We divided the research area off the eastern coast of Korean near Imwon into 3 categories depending upon the severity of the barren ground, i.e., the urchin barren-affected, urchin bar- ren-ongoing and urchin barren-free areas. In April 2011, in the urchin barren-affected area with 25 seaweed species, the cover per- centage and importance value （IV） of erustose coralline algae were higher than those of other species. In the urchin barren-ongoing area with 33 seaweed species, crustose coralline algae （mean IV=62%） as well as Sargassum sp. （mean IV=28%）, and Gelidium amansii （mean IV= 19%） were observed following rock scraping. In the urchin barren-free area where seaweed communities were relatively abundant with 42 species, a variety of algal species including G. amansii （mean IV = 32%） underwent algal succession. Overall, it was observed that, as an aspect of algal succession, the weaker the barren ground severity was, the more frequent and di- verse the seaweeds were, and the more complex the succession pattern was in the study. As an aspect of recovering algal community, rock scraping using hoe was shown to be superior to the method using high-pressure water spraying. Therefore, we conclude that rock scraping using hoe is a very effective strategy for recovering the algal community in urchin barren-ongoing area.

We developed a hybrid scaffold and a bioreactor for cartilage regeneration. The hybrid scaffold was developed as combination of two components: a biodegradable framework and hydrogel-containing chondrocytes. We performed the MTT cell proliferation assay to compare the proliferation and viability of chondrocytes on three types of scaffolds: an alginate gel, the hybrid scaffold, and an alginate sponge. Cells were encapsulated in 2% agarose gel. The bioreactor consisted of a circulation system and a compression system. We performed dynamic cell culture on these agarose gels in the bioreactor for 3 days.

Few long-term follow-up studies have compared the changes in renal function according to the type of statin used in *** compared the long-term effects of statin intensity and type on the changes in the glomerular filtration rate(GFR).We extracted data of patients who took statin for the first *** analyzed whether or not different statins affect the changes in GFR at 3 months after baseline and 4 years *** included 3678 patients and analyzed the changes in *** GFR decreased by 3.2%±0.4%on average 4 years after the first statin prescription,indicating statistically significant deterioration(from 83.5±0.4 mL/min/1.73 m2 to 79.9±0.4 mL/min/1.73 m2,P<0.001).When comparing the GFR among different statins,significant differences were observed between atorvastatin and fluvastatin(−5.3%±0.7%vs.1.2%±2.2%,P<0.05)and between atorvastatin and simvastatin(−5.3%±0.7%vs.−0.7%±0.8%,P<0.05).In pitavastatin(odds ratio[OR]=0.64,95%confidence interval[CI]=0.46-0.87,P<0.005)and simvastatin(OR=0.69,95%CI=0.53-0.91,P<0.008),the GFR rate that decreased by<60 mL/min/1.73 m2 was significantly lower than that of *** long-term statin intake,GFR changed with the type of *** work is the first in Korea to compare each statin in terms of changes in the GFR after the statin prescription.
