This article delves into the intricate relationship between big data, cloud com.uting, and artificial intelligence, shedding light on their fundam.ntal attributes and interdependence. Effectiveness of the Suprainguinal Ultrasound-Guided Block for the m.nagem.nt of Postoperative Pain after Application of a Total Hip Prosthesis

Objective: To assess the effectiveness of ultrasound-guided supraginguinal block (SIB) in the m.nagem.nt of pain after total hip replacem.nt. m.terial and m.thods: This was a prospective, random.zed, single-blind controlled study carried out in the anesthesia-intensive care unit of the Donka National Hospital in Conakry, over a period of 06 m.nths (01/01/2020 to 30/06/2020). It concerned 32 patients: 16 patients in the “ultrasound-guided SIB” group and 16 patients in the “standard analgesia” group. Results: The pain scores assessed by the sim.le verbal scale and collected at the different tim. intervals (6H, 12H, 24H, 36H, 48H) showed m.an scores m.an scores were ≤3 in the standard analgesia group (P m.vem.nt, the m.an pain scores were ≤1 for the ultrasound-guided SIB group versus m.an scores > 3 in the standard analgesia group (P m. to m.bilization was greater than 48 hours in all patients in the standard analgesia group while it was less than 48 hours in the m.jority of patients (75%) in the ultrasound-guided SIB group. Nausea and vom.ting were the m.st observed side effects. We did not observe any respiratory distress. The length of day hospitalization of patients in the ultrasound-guided SIB group was on average 5.50 ± 0.52 com.ared with 13.44 ± 1.55 in the group of standard analgesia patients (P = 0.001). The vast m.jority of patients in the ultrasound-guided SIB group were satisfied and unhappy in the standard analgesia group. Conclusion: Our study dem.nstrated that echo-guided SIB provided better analgesia com.ared to standard analgesia for the m.nagem.nt of postoperative pain after total hip replacem.nt.

The Reasons for Adm.ssion of Elderly Subjects in Intensive Care at the CHU Ignace Deen

Objective: To determ.ne the reasons for adm.ssion of elderly subjects and the prognosis in general intensive care. Patients and m.thods: Observational descriptive and analytical study with prospective collection of data over a period of one year from.january 1 to Decem.er 31, 2021. Patients aged 65 or over were included. Abstract: During the study period, 223 cases were collected out of 587 patients adm.tted, giving a prevalence of 37.9%. The average age was 74.127.39 ± years with extrem.0} of 65 and 96 years and a m.le predom.nance (58.7%). The com.rbidities were dom.nated by arterial hypertension (71.3%). The patients were: transferred from.m.dical and surgical em.rgencies (75.8%). The average adm.ssion tim. was 48.8 ± 29.8 hours. One hundred and eight patients had a Glasgow score between 3 and 7. The reasons for adm.ssion were dom.nated by vascular causes (51.6%). Strokes of any type accounted for 43.9% of these reasons for adm.ssion. The average tim. for carrying out the biological assessm.nts and im.ging was 41.8 ± 27.3 hours with the extrem.0} of 3 and 89 hours, 37.2% had a com.lete assessm.nt within 24 hours. The average duration of hospitalization was 7.10 ± 8.87 days with extrem.0} of 1 and 72 days. The m.rtality rate was 71.7%. Conclusion: This study has m.de it possible to take stock of the reasons for the adm.ssion of elderly subjects to intensive care. It appears that vascular causes are the m.in reasons for adm.ssion with heavy com.rbidities which results in high m.rtality.

目的探讨山楂叶总黄酮对血管紧张素Ⅱ(AngⅡ)诱导的心肌肥大的影响,及其可能作用机制。方法将H9c2细胞分为对照组(正常培养),模型组(100 nm.l·L^(-1)AngⅡ处理24 h),低、中、高剂量实验组(先给予100 nm.l·L^(-1)AngⅡ处理24 h后,再分别给予25、50、100 m.·L^(-1)的山楂叶总黄酮处理24 h),anti-m.R-NC、anti-m.R-21a-5p组(分别转染anti-m.R-NC、anti-m.R-21a-5p后,给予100 nm.l·L^(-1)AngⅡ处理24 h),m.R-NC+高剂量组和m.R-21a-5p+高剂量组(分别转染m.R-NC、m.R-21a-5p m.m.cs,再给予100 nm.l·L^(-1)AngⅡ+100 m.·L^(-1)山楂叶总黄酮各处理24 h)。用CCK-8法和流式细胞术检测细胞活力和凋亡情况,用实时荧光定量聚合酶链反应法检测m.R-21a-5p的表达水平,用蛋白质印迹法检测环氧化酶-2(COX2)/前列腺素E2(PGE2)的表达水平。结果对照组、模型组和高剂量实验组的细胞活力分别为1.03±0.09、0.51±0.05和0.93±0.08,凋亡率分别为(7.69±0.61)%、(23.04±1.82)%和(9.43±0.71)%,m.R-21a-5p表达水平分别为1.00±0.09、2.43±0.18和1.09±0.08,COX2蛋白相对表达水平分别为0.42±0.03、0.85±0.08和0.40±0.04,PGE2蛋白相对表达水平分别为0.34±0.03、0.75±0.07和0.35±0.03,模型组的上述指标与高剂量实验组和对照组相比,在统计学上差异均有统计学意义(均Pm.R-NC组、anti-m.R-21a-5p组、m.R-NC+高剂量组和m.R-21a-5p+高剂量组的细胞活力分别为0.52±0.04、1.12±0.08、0.94±0.09和0.57±0.04,凋亡率分别为(23.04±1.82)%、(9.86±0.73)%、(9.47±0.64)%和(24.96±1.94)%,m.R-21a-5p表达水平分别为1.00±0.10、0.43±0.04、1.00±0.09和2.12±0.18,COX2蛋白相对表达水平分别为0.86±0.05、0.39±0.04、0.41±0.03、0.78±0.07,PGE2蛋白相对表达水平分别为0.74±0.06、0.38±0.07、0.36±0.02、0.71±0.05,anti-m.R-21a-5p组的上述指标与anti-m.R-NC组比较,m.R-21a-5p+高剂量组的上述指标与m.R-NC+高剂量组比较,在统计学上差异均有统计学意义(均Pm.R-21a-5p和COX2/PGE2通路的表达,从而抑制AngⅡ诱导的心肌肥大。

Objectives: To describe the practice of ultrasound-guided TAP in the m.nagem.nt of postoperative pain after gyneco-obstetric surgery. m.thods: This was a descriptive prospective study carried out at the Gynecology-Obstetrics departm.nt of the Ignace Deen National Hospital over a period of three (03) m.nths from.February 01, 2020 to April 31, 2020. Results: In total, we collected 95 patients. These patients had a m.an age of 30 ± 9.5 years. The ASA I class was the m.st represented with 76% of the cases and the cesarean was the m.st perform.d intervention. Regarding the assessm.nt of the pain score by the sim.le verbal scale (SVE) postoperatively at rest, the m.an SLE scores at H6 were 0.17 ± 0.38;at H12 of 1.15 ± 0.62;at H24 of 0.84 ± 0.51;at H36 0.45 ± 0.52 and at H48 0.09 ± 0.29. On m.bilization, the m.an pain scores were 0.77 ± 0.51 at H6, at H12 1.89 ± 0.61;at H24 of 1.53 ± 0.56;at H36 of 1 ± 0.29 and at H48 of 0.82 ± 0.44. The m.jority of our patients (66.3%) had a m.bilization tim. of less than 24 hours. The m.an length of stay was 3.1 ± 1.3 days and m.st patients (82%) were satisfied with the m.nagem.nt of their pain by ultrasound-guided TAP block. Conclusion: Ultrasound-guided TAP is an effective technique for the m.nagem.nt of postoperative pain in gyneco-obstetrics surgery. Its integration in a context of m.ltim.dal analgesia could im.rove the m.nagem.nt of postoperative pain in gynecological obstetrics.

Objective: Was to present the m.nagem.nt of a pregnant wom.n at 39 SA received for severe head traum. in the em.rgency room. Severe traum. in pregnant wom.n can have serious m.ternal and especially fetal consequences in the absence of a diagnostic and therapeutic strategy. We report the case of serious traum. in a 39 WA pregnant wom.n, nulliparous. Adm.tted to the em.rgency room.for serious head traum. following a public road accident. The clinical picture was dom.nated by im.aired consciousness with a Glasgow score of 8/15, right otorrhagia, desaturation at 89% AA and respiratory rate at 36 cycles/m.n. The obstetrical exam.nation noted a fundal height of 32 cm. a fetal heart sound (BCF) of 167 beats/m.n, a cephalic presentation, a cervical opening of 3 cm.and uterine contractures. Im.ging exam.nations outside the hospital because not available. The abdom.nopelvic ultrasound shows a m.nofetal pregnancy without placental abruption. The cerebral scanner shows an edem.to-haem.rrhagic contusion intra parenchym.tous frontotem.oral left. Respiratory support and an indication for caesarean section allowed the extraction of a live m.le child, Apgar at one m.nute 10/10 to five m.nutes 10/10, weighing 3000 g without abnorm.lity on clinical exam.nation. The evolution was favorable after 27 days of resuscitation. Conclusion: The care of a severely traum.tized pregnant wom.n relies above all on good m.ltidisciplinary coordination, based on close collaboration between em.rgency physicians, anesthetists-resuscitators, obstetricians, pediatricians, neonatologists and biologists.

Effect of ultrasound on reverse leakage current of silicon Schottky barrier structure

The influence of ultrasonic loading on reverse current-voltage characteristics of m./n-n＋-Si structures has been investigated. The research of leakage current variation has been carried out for various ultrasonic wave frequencies （4.1 and 8.4 m.z）, intensities （up to 0.8 W/cm.） and loading tem.eratures （130-330 K）. The observed reversible acoustically induced increase in reverse currents was as large as 60%. It has been found that dom.nant charge transfer m.chanism. are the therm.onic em.ssion （at high tem.erature） and the phonon-assisted tunneling （at low tem.erature）. The ultrasound loading affects both processes due to the decrease of Schottky barrier height and binding energy of the electron on the trap.
