BACKGROUND pancreaticoduodenectomy(pD)is a technically complex operation,with a re-latively high risk for *** ability to rescue patients from post-pD complications is as a recognized quality *** protocols were instituted at our low volume facility in the year *** To document the rate of rescue from post-pD complications with tailored protocols in place as a measure of *** A retrospective audit was performed to collect data from patients who experienced major post-pD complications at a low volume pancreatic surgery unit in Trinidad and Tobago between january 1,2013 and june 30,***-dardized definitions from the International Study Group of pancreatic Surgery were used to define post-pD complications,and the modified Clavien-Dindo classification was used to classify post-pD *** Over the study period,113 patients at a mean age of 57.5 years(standard deviation[SD]±9.23;range:30-90;median:56)underwent pDs at this *** complications were recorded in 33(29.2%)patients at a mean age of 53.8 years(SD:±7.9).Twenty-nine(87.9%)patients who experienced major morbidity were salvaged after aggre-ssive treatment of their ***(3.5%)died from bleeding pseudoaneurysm(1),septic shock secondary to a bile leak(1),anastomotic leak(1),and myocardial infarction(1).There was a significantly greater salvage rate in patients with American Society of Anesthesiologists scores≤2(93.3%vs 25%;p=0.0024).CONCLUSION This paper adds to the growing body of evidence that volume alone should not be used as a marker of quality for patients requiring *** low volumes at our facility,we demonstrated that 87.9%of patients were rescued from major *** attributed this to several factors including development of rescue protocols,the competence of the pancreatic surgery teams and continuous,and adaptive learning by the entire institution,cul-minating in the development of tailored peri-pancreatectomy protoc

The Soft X-ray Imager(SXI)is part of the scientific payload of the Solar wind Magnetosphere Ionosphere Link Explorer(SMILE)*** is a joint science mission between the European Space Agency(ESA)and the Chinese Academy of Sciences(CAS)and is due for launch in *** is a compact X-ray telescope with a wide field-of-view(FOV)capable of encompassing large portions of Earth’s magnetosphere from the vantage point of the SMILE *** is sensitive to the soft X-rays produced by the Solar wind Charge eXchange(SwCX)process produced when heavy ions of solar wind origin interact with neutral particles in Earth’s *** provides a mechanism for boundary detection within the magnetosphere,such as the position of Earth’s magnetopause,because the solar wind heavy ions have a very low density in regions of closed magnetic field *** sensitivity of the SXI is such that it can potentially track movements of the magnetopause on timescales of a few minutes and the orbit of SMILE will enable such movements to be tracked for segments lasting many *** is led by the University of Leicester in the United Kingdom(UK)with collaborating organisations on hardware,software and science support within the UK,Europe,China and the United States.

The joint European Space Agency and Chinese Academy of Sciences Solar wind Magnetosphere Ionosphere Link Explorer(SMILE)mission will explore global dynamics of the magnetosphere under varying solar wind and interplanetary magnetic field conditions,and simultaneously monitor the auroral response of the Northern Hemisphere *** these large-scale responses with medium and fine-scale measurements at a variety of cadences by additional ground-based and space-based instruments will enable a much greater scientific impact beyond the original goals of the SMILE ***,we describe current community efforts to prepare for SMILE,and the benefits and context various experiments that have explicitly expressed support for SMILE can offer.A dedicated group of international scientists representing many different experiment types and geographical locations,the Ground-based and Additional Science working Group,is facilitating these *** include constructing an online SMILE Data Fusion Facility,the discussion of particular or special modes for experiments such as coherent and incoherent scatter radar,and the consideration of particular observing strategies and spacecraft *** anticipate growing interest and community engagement with the SMILE mission,and we welcome novel ideas and insights from the solar-terrestrial community.

Enhancing Ulcerative Colitis Diagnosis:A Multi-Level Classification Approach with Deep Learning

The evaluation of disease severity through endoscopy is pivotal in managing patients with ulcerative colitis,a condition with significant clinical ***,endoscopic assessment is susceptible to inherent variations,both within and between observers,compromising the reliability of individual *** study addresses this challenge by harnessing deep learning to develop a robust model capable of discerning discrete levels of endoscopic disease *** initiate this endeavor,a multi-faceted approach is embarked *** dataset is meticulously preprocessed,enhancing the quality and discriminative features of the images through contrast limited adaptive histogram equalization(CLAHE).A diverse array of data augmentation techniques,encompassing various geometric transformations,is leveraged to fortify the dataset’s diversity and facilitate effective feature extraction.A fundamental aspect of the approach involves the strategic incorporation of transfer learning principles,harnessing a modified ResNet-50 *** augmentation,informed by domain expertise,contributed significantly to enhancing the model’s classification *** outcome of this research endeavor yielded a highly promising model,demonstrating an accuracy rate of 86.85%,coupled with a recall rate of 82.11%and a precision rate of 89.23%.

Intelligent Machine Learning Based Brain Tumor Segmentation through Multi-Layer Hybrid U-Net with CNN Feature Integration

Brain tumors are a pressing public health concern, characterized by their high mortality and morbidity ***, the manual segmentation of brain tumors remains a laborious and error-prone task, necessitatingthe development of more precise and efficient methodologies. To address this formidable challenge, we proposean advanced approach for segmenting brain tumorMagnetic Resonance Imaging (MRI) images that harnesses theformidable capabilities of deep learning and convolutional neural networks (CNNs). while CNN-based methodshave displayed promise in the realm of brain tumor segmentation, the intricate nature of these tumors, markedby irregular shapes, varying sizes, uneven distribution, and limited available data, poses substantial obstacles toachieving accurate semantic segmentation. In our study, we introduce a pioneering Hybrid U-Net framework thatseamlessly integrates the U-Net and CNN architectures to surmount these challenges. Our proposed approachencompasses preprocessing steps that enhance image visualization, a customized layered U-Net model tailoredfor precise segmentation, and the inclusion of dropout layers to mitigate overfitting during the training ***, we leverage the CNN mechanism to exploit contextual information within brain tumorMRI images,resulting in a substantial enhancement in segmentation *** experimental results attest to the exceptionalperformance of our framework, with accuracy rates surpassing 97% across diverse datasets, showcasing therobustness and effectiveness of our approach. Furthermore, we conduct a comprehensive assessment of ourmethod’s capabilities by evaluating various performance measures, including the sensitivity, jaccard-index, andspecificity. Our proposed model achieved 99% accuracy. The implications of our findings are profound. we have recently proposed a new technique of plasma tailoring by laser-driven hydrodynamic shockwaves generated on both sides of a gas jet[Marquès et al.,*** 28,023103(2021)].In a continuation of this numerical work,we study experimentally the influence of the tailoring on proton acceleration driven by a high-intensity picosecond laser in three cases:without tailoring,by tailoring only the entrance side of the picosecond laser,and by tailoring both sides of the gas *** tailoring,the acceleration is transverse to the laser axis,with a low-energy exponential spectrum,produced by Coulomb *** the front side of the gas jet is tailored,a forward acceleration appears,which is significantly enhanced when both the front and back sides of the plasma are *** forward acceleration produces higher-energy protons,with a peaked spectrum,and is in good agreement with the mechanism of collisionless shock acceleration(CSA).The spatiotemporal evolution of the plasma profile is characterized by optical shadowgraphy of a probe *** refraction and absorption of this beam are simulated by post-processing 3D hydrodynamic simulations of the plasma *** with the experimental results allows estimation of the thickness and near-critical density of the plasma slab produced by tailoring both sides of the gas *** parameters are in good agreement with those required for CSA.
