The first search for the doubly heavyΩ^(0)_(bc)baryon and a search for the E^(0)_(bc)baryon are performed using pp collision data collected via the lHCb experiment from 2016 to 2018 at a centre-of-mass energy of 13 TeV,corresponding to an integrated luminosity of 5.2 fb^(-1).The baryons are reconstructed via their decays to Λ^(+)_(c)π^(-)and E^(+)_(c)π^(-).No significant excess is fbund for invariant masses between 6700 and 7300 MeV/c^(2),in a rapidity range from 2.0 to 4.5 and a transverse momentum range from 2 to 20 MeV/*** limits are set on the ratio of the Ω^(0)_(bc)and E^(0)_(bc)production cross-section times the branching fraction to Λ^(+)_(c)π^(-)(E^(+)_(c)π^(-))relative to that of the Λ^(0)_(b)(E^(0)_(b))baryon,for different lifetime hypotheses,at 95%confidence *** upper limits range from 0.5 x 10^(-4)to 2.5 x 10^(-4)for theΩ^(0)_(bc)→Λ^(+)_(c)π^(-)(E^(0)_(bc)→Λ^(+)_(c)π^(-))decay,and from 1.4x 10^(-3)to 6.9 x 10^(-3)for theΩ^(0)_(bc)→E^(+)_(c)π^(-)(E^(0)_(bc)→E^(+)_(c)π^(-))decay,depending on the considered mass and lifetime of theΩ^(0)_(bc)(E^(0)_(bc))baryon.

Background-We hypothesized that normal and pathological vessel walls display a differential pattern of secreted proteins. We have recently set up the conditions for comparing secretomes from carotid atherosclerotic plaques and control arteries using a proteomic approach to assess whether differentially secreted proteins could represent markers for atherosclerosis. Methods andResults-Normal endartery segments and different regions of endarterectomy pieces (noncomplicated/ complicated plaques) were incubated in protein-free medium, and the released proteinswere analyzed by 2D electrophoresis(2-DE). Among the differently secreted proteins, we have identified heat shock protein-27(HSP27). Surprisingly, compared with control arteries, HSP27 release was drastically decreased in atherosclerotic plaques and barely detectable in complicated plaque supernatants. HSP27 was expressed primarily by intact vascular cells of normal arteries and carotid plaques (immunohistochemistry). Plasma detection of soluble HSP27 showed that circulating HSP27 levels are significantly decreased in the blood of patients with carotid stenosis relative to healthy subjects(0.19 [0.1 to 1.95] versus 83[71.8 to 87.8]) ng/ml, Plusions-HSP27 secretion is decreased in complicated atherosclerotic plaques, and sHSP27 plasma levels are decreased in atherosclerotic patients compared with healthy subjects. Plasma sHSP27 levels could be a potential index of atherosclerosis, although further validation is needed in large patient cohorts.

A first search for the Ξ_(bc)^(+)j/ψΞ_(c)^(+) decay is performed by the lHCb experiment with a data sample of proton-proton collisions, corresponding to an integrated luminosity of 9 fb−1 recorded at centre-of-mass energies of 7, 8, and 13 TeV. Two peaking structures are seen with a local (global) significance of 4.3(2.8) and 4.1(2.4) standard deviations at masses of 6571 and 6694 MeV/c2, respectively. Upper limits are set on the Ξ+bc baryon production cross-section times the branching fraction relative to that of the B+c→j/ψD+s decay at centre-of-mass energies of 8 and 13 TeV, in the Ξ+bc and in the B+c rapidity and transverse-momentum ranges from 2.0 to 4.5 and 0 to 20GeV/c, respectively. Upper limits are presented as a function of the Ξ+bc mass and lifetime.

Objective: To determine whether fluid hysteroscopic directed biopsies , in pati ents with endometrial cancer upstages the tumor and worsens the prognosis. Study design: Between january 1996 and September 2001, a total of 62 consecutive pati ents with endometrial cancer, treated at our institution, were randomized 3 ∶2 to have or not to have a fluid hysteroscopic biopsy just prior to surgery. A tot al of 38 patients underwent a hysteroscopy after the induction of anesthesia. Al l patients had pelvic washings performed, followed by a hysterectomy, bilateral salpingooforectomy and pelvic +/-para-aortic lymph node dissections. Only sta ges I and II endometrioid type tumors or stage IIIa, secondary to positive pelvi c washings, were included in the study. Eight patients in the hysteroscopy group and four patients in the control group were excluded for various reasons. Patie nts received post-operative radiation therapy depending on the surgical-pathol ogical risk factors. The median follow up was 34 months. Fishers Exact Test wa s performed to compare differences between the hysteroscopic (n=30) and the cont rol (n=20) groups. Results: We found three patients (10%) with positive washing s in the hysteroscopic group compared to one (5%) among the controls (P=0.64), with a statistical power of ld persist, we would n eed 588 patients in each arm to obtain a power of 80%, and reach definitive con clusions. The Odds Ratio (OR) of performing a hysteroscopy and upstaging the tum or in this study was: 2.1 95%CI (0.20-21.09). Prognostic variables were compar ed between both groups and no differences were observed. All patients but one (d ead due to intercurrent disease), were alive and with no evidence of disease at the completion of the study. Conclusions: Fluid hysteroscopy and directed biopsi es may have a small risk of upstaging early endometrial cancers, but does not se em to influence prognosis.

背景在KEYNOTE-189此项Ⅲ期研究(NCT02578680)中,与安慰剂联合培美曲塞加铂类相比,帕博利珠单抗联合培美曲塞加铂类化疗(培美曲塞-铂类)显著改善了初治转移性非鳞状非小细胞肺癌(non-small cell lung cancer,NSClC)患者的总生存期(overall survival,OS)和无进展生存期(progression-free survival,PFS)。本研究报告了研究方案指定的最终分析的疗效结局,包括从培美曲塞-铂类交叉至帕博利珠单抗治疗的患者,以及完成35个周期(约2年)帕博利珠单抗治疗组的疗效。方法符合条件的患者以2:1随机分配至帕博利珠单抗,每3周一次,200 mg (n=410)或安慰剂(n=206)(共35周期,约2年),加上4个周期的培美曲塞(500 mg/m~2)和由研究者选择的每3周顺铂(75 mg/m~2)或卡铂(曲线下面积5 mg/min/ml)治疗,随后使用培美曲塞直至疾病进展。若符合纳入标准,分配至安慰剂联合培美曲塞加铂类治疗的患者可在疾病进展时转向帕博利珠单抗治疗。主要研究终点为OS和PFS。结果中位随访31.0个月,无论PD-l1的表达水平如何,与安慰剂联合培美曲塞-铂类治疗相比,帕博利珠单抗联合培美曲塞-铂类治疗可使患者的OS[风险比(hazard ratio,HR)为0.56;95%可信区间(confidence interval,CI) 0.46-0.69]和PFS (HR=0.49;95%CI:0.41-0.59)显著获益。接受帕博利珠单抗联合培美曲塞-铂类治疗的患者客观缓解率(objective response rate,ORR)(48.3%vs 19.9%)和第二次/后续无进展生存期(PFS2;HR=0.50;95%CI:0.41-0.61)均得到改善。安慰剂联合培美曲塞-铂类组的84例患者(40.8%)交叉至帕博利珠单抗治疗组。72.1%的接受帕博利珠单抗联合培美曲塞-铂类的患者和66.8%的接受安慰剂联合培美曲塞-铂类的患者发生了3级-5级不良事件。56例患者完成了35个周期(约2年)的帕博利珠单抗治疗;ORR为85.7%,53例(94.6%)患者存活至数据截止时间。结论与安慰剂联合培美曲塞-铂类治疗相比,帕博利珠单抗联合培美曲塞-铂类可持续提高疗效,且安全性可控。该研究结果支持将帕博利珠单抗联合培美曲塞-铂类作为初治转移性非鳞状NSClC患者的一线治疗方案。

背景与目的我们开展了一项帕博利珠单抗用于伴未治疗脑转移的非小细胞肺癌(non-small cell lung cancer,NSClC)或黑色素瘤患者的疗效和安全性的II期试验,旨在评估程序性死亡受体1(programmed cell death 1,PD-1)抑制剂在中枢神经系统(central nervous system,CNS)中的疗效。中期结果已发表,现报道对NSClC队列的更新分析结果。方法这是一项开放性、单中心、II期试验。纳入标准:年龄≥18岁,诊断为晚期NSClC并伴有≥1个5 mm-20 mm脑转移病灶,既往从未治疗或之前放疗后进展,无神经系统症状,不需要激素治疗且美国东部肿瘤协作组(Eastern Cooperative Oncology Group,ECOG)ll death ligand 1,PD-l1)≥1%的患者,队列2为PD-l1l1≥1%的NSClC伴脑转移患者有效,且对所有纳入的未经治疗的脑转移患者安全。需要进一步探索免疫治疗用于NSClC合并CNS转移。
