AIM To evaluate the joint geometry and the clinical outcome of stemless, anatomical shoulder arthroplasty with the TESS *** Twenty-one shoulders with a mean follow-up 18 of months were included. On scaled digital radiographs the premorbid center of rotation(CoR) was assessed and compared to the CoR of the prosthesis by using the MediC AD~? software. Additionally, the pre-and post-operative geometry of the CoR was assessed in relation to the glenoid, the acromion as well as to the proximal humerus. Radiological changes, such as radiolucencies, were also assessed. Clinical outcome was assessed with the Constant and DASH score. RESULTS Both, the Constant and DASH scores improved signifi-cantly from 11% to 75% and from 70 to 30 points, P gnificant differences regarding age, etiology, cemented or metal-backed glenoids, etc.(P > 0.05). The pre-and postoperative humeral offset, the lateral glenohumeral offset, the height of the CoR, the acromiohumeral distance as well as neckshaft angle showed no significant changes(P > 0.05). The mean deviation of the CoR of the prosthesis from the anatomic center was 1.0 ± 2.8 mm. Three cases showed a medial deviation of more than 3 mm. These deviations of 5.1, 5.7 and 7.6 mm and were caused by an inaccurate humeral neck cut. These 3 patients showed a relatively poor outcome *** TESS arthroplasty allows an anatomical joint reconstruction with a very good outcome. Outliers described in this study sensitize the surgeon for an accurate humeral neck cut.

AIM: To characterize management of telaprevir(TVR)-based triple therapy of hepatitis C virus(HCV) reinfection after liver transplantation(LT).METHODS: We retrospectively analyzed safety and efficacy of telaprevir- based triple therapy in a single center cohort of 19 patients with HCV genotype(gT) 1 recurrence after LT, with respect to factors possibly predicting sustained viral response(SVR) or non-SVR. All patients were treated with TVR, pegylated(PEg) and ribavirine(RBV) for 12 wk followed by a dual phase with PEg/RBV for 12 wk in 7 patients and for 36 wk in 5 patients. RESULTS: In total 11/19(58%) of patients achieved a sustained response. All(11/11) SVR patients showed a rapid viral response at treatment weeks 4 and 11/14 rapid virological response(RVR) patients achieved SVR. Notably, all(7/7) patients who completed 48 wk of therapy and 80%(4/5) patients who completed 24 wk of therapy achieved SVR24. Treatment failure was significantly(P > 0.049) more frequent in gT1 a infection(5/7) compared to gT1b(3/12) infection and was associated with emergence of resistance-associated mutations in the NS3 protease domain. Bilirubin level at baseline is also related to SVR(P > 0.030). None of the patients had to discontinue treatment due to side effects. CONCLUSION: RVR, gT and bilirubin are clearly related to achievement of SVR. Providing a thorough patient selection and monitoring, a full course of TVR-based triple therapy in LT patients is feasible and achieves high SVR rates.

The photorespiratory pathway or photorespiration is an essential process in oxygenic photosynthetic or- ganisms, which can reduce the efficiency of photosynthetic carbon assimilation and is hence frequently considered as a wasteful process. By comparing the response of the wild-type plants and mutants impaired in photorespiration to a shift in ambient C02 concentrations, we demonstrate that photorespiration also plays a beneficial role during short-term acclimation to reduced C02 availability. The wild-type plants re- sponded with few differentially expressed genes, mostly involved in drought stress, which is likely a conse- quence of enhanced opening of stomata and concomitant water loss upon a shift toward low C02. In contrast, mutants with impaired activity of photorespiratory enzymes were highly stressed and not able to adjust stomatal conductance to reduced external C02 availability. The transcriptional response of mutant plants was congruent, indicating a general reprogramming to deal with the consequences of reduced C02 availability, signaled by enhanced oxygenation of ribulose-l,5-bisphosphate and amplified by the artificially impaired photorespiratory metabolism. Central in this reprogramming was the pro- nounced reallocation of resources from growth processes to stress responses. Taken together, our results indicate that unrestricted photorespiratory metabolism is a prerequisite for rapid physiological acclimation to a reduction in C02 availability.

AIM:Pelvic magnetic resonance imaging(MRI)and endoanal ultrasound which are established imaging methods for perianal inflammatory lesions in patients with Crohn's disease require expensive specialized equipments and *** investigated the feasibility and sensitivity of transcutaneous perianal ultrasound(PAUS)using regular ultrasound probes in the imaging of perianal inflammatory *** sonographic findings were correlated to pelvic MR]-scans. METHODS:We performed PAUS in 25 patients with Crohn's disease and clinical signs of perianal inflammatory disease. Wibhin a median of 10 d(range 0-75)these patients underwent MRI of the *** convex and linear high resolution probes were used for *** sonographic findings were correlated to the MRI findings by blinded investigators. RESULTS:The sonographic investigations were well tolerated by all *** typically presented as hypoechoic ***-nine fistulae were detected in 22 *** were detected in 7 patients and presented as hypo-or anechoic ***-six of 29 fistulae and 6 of 7 abscesses could be confirmed by *** statistics showed an excellent agreement (kappa>0.83)between the two imaging methods. CONCLUSION:PAUS is a simple,painless,feasible,real- time method that can be performed without specific patient preparation which is comparable in its sensitivity to pelvic MRI in the detection of perianal fistulae and/or abscesses. PAUS can especially be recommended as a screening tool in acute perianal disorders such as perianal abscess and for follow-up studies of perianal inflammatory disease.

摄影师timm Seuss从2009年开始每一年都重返切尔诺贝利,记录这个城市无可挽回的从一个人口密集的中心变为死城的宿命。尽管独联体的三个国家俄罗斯、白俄罗斯和乌克兰以及联合国倾尽全力希望复原这个地区的环境,社会和经济。但是这个地区已经和人类社会告别了,某种意义上它变成了野生动物的天堂,猫头鹰、天鹅、棕熊、欧洲野牛、蒙古野马、獾、河狸、野猪、鹿、狐狸、狼、水鸟和松鸡在往日喧3闹的城市与乡间出没,栖息。摄影师在拍摄切尔诺贝利这个"后人类世界"时,把镜头聚焦在两种空寂的空间上,一个享乐的日常生活场景;另一个是体积巨大的工业和交通设施。这两个符号曾强烈地代表了昔日苏联向世界夸耀的强大国力和幸福制度。如今核工业废墟与杳无人迹的公园依然并肩而立,尖刻的摄影师好像是有意夸张这两重符号在色彩与形式感上的对比,甚至带点邪恶的嘲讽感。但在骨子里却渗出怀念与记忆深处无法排遣的愤怒、哀伤、恐惧与绝望。
