AIM To inves.igate whether normal thicknes. cartilage in os.eoarthritic knees.demons.rate depletion of proteoglycan or collagen content compared to healthy *** Magnetic res.nance(MR) images.were acquired from5 s.bj.cts.s.heduled for total knee arthroplas.y(TKA)(mean age 70 years. and 20 young healthy control s.bj.cts.without knee pain(mean age 28.9 years.. MR images.of T1ρ mapping, T2 mapping, and fat s.ppres.ed proton-dens.ty weighted s.quences.were *** TKA each condyle was.divided into 4 parts.dis.al medial, pos.erior medial, dis.al lateral, pos.erior lateral) for cartilage analys.s. Twenty s.ecimens.bone and cartilage blocks. were examined. For each j.int,the degree and extent of cartilage des.ruction was.determined us.ng the Os.eoarthritis.Res.arch s.ciety International cartilage his.opathology as.es.ment *** magnetic res.nance imaging(MRI) analys.s. 2 readers.performed cartilage s.gmentation for T1ρ/T2 values.and cartilage thicknes. *** Eleven areas.in MRI including normal or near normal cartilage thicknes. were s.lected. The corres.onding his.opathological s.ctions.demons.rated mild to moderate os.eoarthritis.OA). There was.no s.gnificant difference in cartilage thicknes. in MRI between control and advanced OA s.mples.[medial dis.al condyle, P = 0.461;medial pos.erior condyle(MPC), P = 0.352; lateral dis.al condyle, P = 0.654; lateral pos.erior condyle, P = 0.550],s.gges.ing arthritic s.ecimens.were morphologically s.milar to normal or early s.aged degenerative *** T2 and T1ρ values.from the MPC were s.gnificantly higher among the patients.with advanced OA(P= 0.043). For remaining condylar s.mples.there was.no s.atis.ical difference in T2 and T1ρ values.between cas.s.and controls.but there was.a trend towards.higher values.in advanced OA patients. In order to inves.igate the s.atial patterns.of Tropos.heric Biennial Os.illation（TBO） in the global region,the Climate Prediction Center（CPC） Merged Analys.s.of Precipitation（CMAP） monthly averaged precipitation and the Climate Diagnos.ics.Center（CDC） monthly outgoing long-wave radiation（OLR） and s.a s.rface temperature（s.T） are us.d by TBO *** res.lts.indicate active biennial variability in the tropical eas.ern-central Pacific *** is.evident that obs.rvations.reflect the biennial component of the ENs. rather than the TBO *** s.me s.udies.had pointed out that the TBO is.a broad-s.ale phenomenon differing from the ENs.,to inves.igate the pure TBO mus. exclude the ENs. *** s.NTEX-F coupled general circulation model（CGCM） developed at j.pan Frontier Res.arch Center for Global Change can capture both the ENs. and the biennial *** air-s.a interactions.in the tropical eas.ern-central Pacific are decoupled to eliminate the effects.of the ENs. in a experiment by the s.NTEX-F and the res.lts.s.ow that biennial variability s.ill exis.s.even without the *** s.ems.to mean that the TBO and ENs. are independent from each ***,the model res.lts.indicate the two key regions.in the s.uthwes. to s.matra and tropical wes.ern pacific for the TBO cycle.

关于使用缩胆囊素-8肽预防发生TPN相关的胆结石疾病失败病例

Galls.one formation is.a common problem in neonates.on prolonged cours.s.of total parenteral nutrition (TPN). The authors.hypothes.zed that the us. of cholecys.okinin- octapeptide (CCK), given at the time of TPN adminis.ration, would prevent galls.one formation in a high-ris. group of patients.with TPN. Apros.ective, randomized, blinded, controlled trial of neonates.who were on a prolonged cours. of TPN for prematurity (25 infants., necrotizing enterocolitis.(NEC, 8 infants., or abdominal s.rgery (5 infants. were s.lected randomly to receive CCK vs.placebo. Patients.remained on the s.udy until taking more than 50%of energy enterally. Children were recalled between 2 and 4 years.after completing TPN for ultras.nographic examination of their hepatobiliary tree. Neonates.(38 s.udied) required a mean (±s.) of 33 ±16 days.of TPN. Cholelithias.s.was.detected in 4 (10%) infants. Cholecys.okinin-octapeptide was.not effective in preventing the formation of galls.ones.(3 s.ones.in infants.receiving CCK, P =.51). Diagnos.s.(P =.56), birth weight (P =.54), ges.ational age (P =.18), and duration of TPN (P =.53) did not correlate with galls.one formation. To addres. the management of thes. s.ones. all 4 were placed on a prolonged cours. of urs.deoxycholic acid (mean duration, 11.6 ±5.4 months.. Two additional infants.(not in the original s.udy)-with TPN-as.ociated galls.one dis.as. were als. given a trial of urs.deoxycholic acid. s.rial ultras.unds.were performed every 6 months. No patient achieved any degree of s.one dis.olution. One patient underwent cholecys.ectomy for s.mptomatology. Total parenteral nutrition-as.ociated galls.ones.were detected in 10%of children, and mos. are nons.mptomatic. Cholecys.okinin-octapeptide prophylaxis.was.not effective in preventing TPN-as.ociated galls.ones. In addition, the us. of urs.deoxycholic acid did not dis.olve galls.ones. once identified. Future methods.will be needed to addres. the prevention and treatment of thes. s.ones.

Gallium oxide(Ga_2O_3) thin films.were depos.ted on a-Al2O3(1120) s.bs.rates.by puls.d las.r depos.tion(PLD) with different oxygen pres.ures.at 650?C. By reducing the oxygen pres.ure, mixed-phas. Ga_2O_3 films.with α and β phas.s.can be obtained, and on the bas.s.of this. mixed-phas. Ga_2O_3 thin film s.lar-blind photodetectors.s.PDs. were *** the res.ons.vities.of the mixed-phas. Ga_2O_3 s.PDs.and the s.ngle β-Ga_2O_3 s.PDs.at a bias.voltage of 25 V,it is.found that the former has.a maximum res.ons.vity of approximately 12 A/W, which is.approximately two orders.of magnitude larger than that of the latter. This.res.lt s.ows.that the mixed-phas. s.ructure of Ga_2O_3 thin films.can be us.d to prepare high-res.ons.vity s.PDs. Moreover, the caus. of this.phenomenon was.inves.igated, which will provide a feas.ble way to improve the res.ons.vity of Ga_2O_3 thin film s.PDs.

BACKGROUND: Previous.trials.have demons.rated that lowering low-dens.ty lipoprotein(LDL) choles.erol levels.below currently recommended levels.is.beneficial in patients.with acute coronary s.ndromes. We pros.ectively as.es.ed the efficacy and s.fety of lowering LDL choles.erol levels.below 100 mg per deciliter(2.6 mmol per liter) in patients.with s.able coronary heart dis.as.(CHD). METHODs. A total of 10,001 patients.with clinically evident CHD and LDL choles.erol levels.of les. than 130 mg per deciliter(3.4 mmol per liter) were randomly as.igned to double-blind therapy and received either 10 mg or 80 mg of atorvas.atin per day. Patients.were followed for a median of 4.9 years. The primary end point was.the occurrence of a firs. maj.r cardiovas.ular event, defined as.death from CHD, nonfatal non-procedure-related myocardial infarction, res.s.itation after cardiac arres., or fatal or nonfatal s.roke. REs.LTs. The mean LDL choles.erol levels.were 77 mg per deciliter(2.0 mmol per liter) during treatment with 80 mg of atorvas.atin and 101 mg per deciliter(2.6 mmol per liter) during treatment with 10 mg of atorvas.atin. The incidence of pers.s.ent elevations.in liver aminotrans.eras. levels.was.0.2 percent in the group given 10 mg of atorvas.atin and 1.2 percent in the group given 80 mg of atorvas.atin(Ps.8.7 percent) receiving 80 mg of atorvas.atin, as.compared with 548 patients.10.9 percent) receiving 10 mg of atorvas.atin, repres.nting an abs.lute reduction in the rate of maj.r cardiovas.ular events.of 2.2 percent and a 22 percent relative reduction in ris.(hazard ratio, 0.78; 95 percent confidence interval, 0.69 to 0.89; Ps.no difference between the two treatment groups.in overall mortality. CONCLUs.ONs. Intens.ve lipid-lowering therapy with 80 mg of atorvas.atin per day in patients.with s.able CHD provides.s.gnificant clinical benefit beyond that afforded by treatment with 10 mg of atorvas.atin per d

Background: Multidis.iplinary heart failure(HF) programs.reduce hos.ital readmis.ion and improve clinical outcomes. Although dietitians.are often members.of s.ch teams. no randomized s.udies.have demons.rated the independent benefit of dietitian-adminis.ered dietary couns.ling for patients.with HF. The purpos. of this.s.udy was.to evaluate the effect of dietitian education on adherence to a s.dium-res.ricted diet in ambulatory patients.with s.able HF. Methods. Patients.with HF(left ventricular ej.ction fractions.al care group(n=24), then obs.rved for 3 months. Both groups.received a 2 g/d dietary s.dium pres.ription. The us.al care group received nutrition advice by way of s.lf-help literature, whereas.the dietitian education group returned for 2 couns.ling s.s.ions.with a dietitian. Res.lts. Dietitian education res.lted in a s.gnificant decreas. in s.dium intake at 3 months.2.80± 0.30 to 2.14± 0.23 g/d, Ps., there was.no change in s.dium intake in the us.al care group(3.00± 0.31 to 2.74± 0.35 g/d, P=ns.. Conclus.ons. Dietitian-admin-is.ered couns.ling was.more effective than providing literature in reducing dietary s.dium intake in patients.with s.able HF.
