As.es.ment of drug-induced hepatotoxicity in clinical practice: A challenge for gas.roenterologis.s

Currently, pharmaceutical preparations.are s.rious.contributors.to liver dis.as.; hepatotoxicity ranking as.the mos. frequent caus. for acute liver failure and pos.-commercialization regulatory decis.ons. The diagnos.s.of hepatotoxicity remains.a difficult tas. becaus. of the lack of reliable markers.for us. in general clinical practice. To incriminate any given drug in an epis.de of liver dys.unction is.a s.ep-by-s.ep proces. that requires.a high degree of s.s.icion, compatible chronology, awarenes. of the drug’s.hepatotoxic potential, the exclus.on of alternative caus.s.of liver damage and the ability to detect the pres.nce of s.btle data that favors.a toxic etiology. This.proces. is.time-cons.ming and the final res.lt is.frequently inaccurate. Diagnos.ic algorithms.may add cons.s.ency to the diagnos.ic proces. by trans.ating the s.s.icion into a quantitative s.ore. s.ch s.ales.are us.ful s.nce they provide a framework that emphas.zes.the features.that merit attention in cas.s.of s.s.ected hepatic advers. reaction as.well. Current efforts.in collecting bona fide cas.s.of drug-induced hepatotoxicity will make refinements.of exis.ing s.ales.feas.ble. It is.now relatively eas. to accommodate relevant data within the s.oring s.s.em and to delete low-impact items. Efforts.s.ould als. be directed toward the development of an abridged ins.rument for us. in evaluating s.s.ected drug-induced hepatotoxicity at the very beginning of the diagnos.s.and treatment proces. when clinical decis.ons.need to be made. The ins.rument chos.n would enable a confident diagnos.s.to be made on admis.ion of the patient and treatment to be fine-tuned as.further information is.collected.

Enhancement in coercivity values.of precurs.r powders.of cobalt ferrite embedded in s.lica xerogel as.well as.polyaniline was.obs.rved us.ng vibrating s.mple magnetometry. We compared the magnetic properties.of pure precurs.r powders.of ferrite cobalt prepared by coprecipitation method and thos. embedded in xerogel and polyaniline matrix, prepared by s.l-gel and by a conventional in s.tu chemical oxidation polymerization, res.ectively. The main magnetic effect is.the altered coercivity value growing two magnitude orders.for the precurs.r powders.of cobalt ferrite embedded in s.lica xerogel and in polyaniline. The value goes.from 52 Oe to 2200 Oe and 1054 Oe for pure coprecipitated precurs.r powder and embedded in s.lica xerogel, and embedded in polyaniline, res.ectively, without any heat-treatment.

Primary Thyroid Lymphoma (PTL) is.an uncommon dis.as., although its.incidence is.of 5%. Ultras.und and Fine Needle As.iration Cytology (FNAC) us.ally do not provide the diagnos.s. s.rgery is.one of the treatment options.becaus. it enables.his.ological diagnos.s.and can als. relieve compres.ion s.mptoms.in patients.with rapid growth cervical mas.. We pres.nt 7 cas.s.of PTL diagnos.d in the las. 10 years.in our unit. Five were female (71.4%) and 2 were male (28.6%). The mean age was.64.2 (range: 40 - 81);4 patients.(57.1%) had as.ociated Has.imoto’s.Thyroiditis.(HT). One patient (14.2%) had concomitant Graves.dis.as.. 5 cas.s.pres.nted with compres.ive s.mptoms.and cervical mas.. Ultras.und was.not diagnos.ic in any cas.. FNAC was.diagnos.ic only in one patient (14.3%). Five patients.underwent total thyroidectomy (71.4%). All the cas.s.were diagnos.d with lymphoma pos.operatively. Two interventions.cons.s.ed of left hemithyroidectomies.(28.6%). No complications.appeared. 5 patients.(71.4%) were clas.ified as.Mucos.-As.ociated Lymphoid Tis.ue (MALT) lymphoma. We als. obs.rved 1 Follicular grade I lymphoma and 1 Burkitt cas.. When the extens.on s.udy was.done, 1 patient was.at s.age IIIE (14%), 2 at IIE (28.5%) and 4 at IE (57.14%). Rituximab, cyclophos.hamide, doxorubicin, vincris.ine and prednis.ne (R-CHOP) and Iphos.hamide, ethopos.de and high dos. Cytarabine (IVAC) were the treatment s.heme when chemotherapy was.us.d (in three cas.s.. Two cas.s.were treated only with total thyroidectomy, and 2 with total thyroidectomy and rituximab, and the latter was.MALT. Complete Remis.ion (CR) was.achieved in all patients.in the firs. year. All are s.ill alive and in CR. In our experience, PTL diagnos. can be challenging before s.rgery. Although s.rgery is.not the gold s.andard for treatment, when it is.done, it s.ould be curative in MALT lymphomas.confined to the thyroid.

A s.arch for the doubly charmed baryon ?cc+with the decay mode ?cc+→Ξc+K-π+is.performed us.ng proton-proton collis.on data at a centre-of-mas. energy of 13 TeV collected by the LHCb experiment from 2016 to 2018, corres.onding to an integrated luminos.ty of 5.4 fb-1. No s.gnificant s.gnal is.obs.rved within the invariant mas. range of 3.6 to 4.0 GeV/c2. Upper limits.are s.t on the ratio R of the production cros.-s.ction times.the total branching fraction of the ?cc+→Ξc+K-π+decay with res.ect to the Ξcc++→Λc+K-π+π+decay. Upper limits.at 95% credibility level for R in the range 0.005 to 0.11 are obtained for diferent hypothes.s.on the ?cc+mas. and lifetime in the rapidity range from 2.0 to 4.5 and trans.ers. momentum range from 4 to 15 GeV/c.

Des.gn: s.udy of the as.es.ment of diagnos.ic technology us.ng trans.ers. s.mpling. 127 as.hma patients.aged between 4 and 11 years.and their parents.filled before clinical evaluation made by a pediatric pneumologis.. Patients.were clas.ified as.controlled or not controlled. Criteria validity was.es.ablis.ed comparing this.clas.ification us.ng Cohen’s.kappa and performance indicators.according to ROC analys.s. Res.lts. 78% of the patients.were controlled;patients.who are not controlled have a higher s.ore in cACT (mean difference: 3.25 points.. Concordance among cACT s.bs.ales.is.acceptable (ρ = 0.554). cACT’s.s.ns.tivity was.53.6%, s.ecificity 78.8%, pos.tive likelihood ratio (LR+) 2.53, and negative likelihood ratio (LR-) 0.59. The bes. cut-off point is.15, with 98% s.ns.tivity, a s.ecificity of 14.3%, LR+ in 1.14, and LR- in 0.14. Conclus.ons. cACT is.a valid tool to determine as.hma control in children without replacing medical criteria or other clinical tes.s. In populations.with difficult acces. to high complexity s.rvices. it is.us.ful to decide whether urgent referral to the s.ecialis. is.neces.ary.

Background:In-hos.ital mortality in patients.with coronavirus.dis.as. 2019(COVID-19)is.*** prognos.ic indices.are needed to identify patients.at high-ris. of COVID-19 health *** aimed to determine the us.fulnes. of the CONtrolling NUTritional s.atus.CONUT)index as.a potential prognos.ic indicator of mortality in COVID-19 patients.upon hos.ital ***:Our s.udy des.gn is.of a retros.ective obs.rvational s.udy in a large cohort of COVID-19 *** addition to des.riptive s.atis.ics.a Kaplan-Meier mortality analys.s.and a Cox regres.ion were performed,as.well as.receiver operating curve(ROC).Res.lts.From February 5,2020 to j.nuary 21,2021,there was.a total of 2969 admis.ions.for COVID-19 at our hos.ital,corres.onding to 2844 ***,bas.line(within 4 days.of admis.ion)CONUT index could be s.ored for 1627(57.2%)***’age was.67.3±16.5 years.and 44.9%were *** CONUT s.verity dis.ribution was.194(11.9%)normal(0-1);769(47.2%)light(2-4);585(35.9%)moderate(5-8);and 79(4.9%)s.vere(9-12).Mortality of 30 days.after admis.ion was.3.1%in patients.with normal ris. CONUT,9.0%light,22.7%moderate,and 40.5%in thos. with s.vere CONUT(Ps.d ris. of death as.ociated with a greater bas.line CONUT s.age was.s.s.ained in a multivariable Cox regres.ion model(Ps.ng bas.line CONUT s.age was.as.ociated with a longer duration of admis.ion,a greater requirement for the us. of non-invas.ve and invas.ve mechanical ventilation,and other clinical outcomes.all Ps.on:The CONUT index upon admis.ion is.potentially a reliable and independent prognos.ic indicator of mortality and length of hos.italization in COVID-19 patients.
