Gastric malignancies in Northern Jordan with special emphasis on descriptive epidemiology

AiM:To study the epidemiology of gastric malignancies in Jordan as a model for Middle East countries where such data is scarce. METHODS:Pertinent epidemiological and clinicopathological data for 201 patients with gastric malignancy in north of Jordan between 1991 and 2001 were analyzed. RESULTS:Male:female ratio was 1.8:*** mean age was 61.2 years,and 8.5% of the patients were younger than 40 years of *** overall age-adjusted incidence was 5.82/100 000 population/*** age specific incidence for males raised from 1.48 in those aged 30-39 years to 72.4 in those aged 70-79 ***, gastric lymphomas,malignant stromal tumors,and carcinoids were found in 87.5%,8%,2.5%,and 2% *** was an average of 10.1-month delay between the initial symptoms and the *** 82 patients underwent“curative”*** adenocarcinoma groups,Lauren intestinal type was the commonest(72.2%)and the distal third was the most common localization(48.9%).The mean follow up for patients with gastric adenocarcinoma was 25.1 mo(range 1-132mo).The 5-year survival rates for stages Ⅰ(n=15), Ⅱ(n=41),Ⅲ(n=59),and Ⅳ(n=53) were 67.3%,41.3%, 5.7%,and 0% respectively(P=0.0001).The overall 5 year survival was 21.1%. CONCLUSiON:Despite low inddence,some epidemiological features of gastric cancer in Jordan mimic those of high- risk *** are detected and treated after a relatively long *** justification in favor of a possible gastric cancer screening effort in Jordan is supported by our study;rather,the need of an earlier diagnosis and subsequent better care.

Ultrasound-guided transrectal extended prostate biopsy:a prospective study

Aim:To evaluate the diagnostic value of the 10 systematic transrectal ultrasound-guided (TRUS) prostate biopsy compared with the sextant biopsy technique for patients with suspected prostate ***:One hundred and fifty-two patients with suspected prostate cancer were included in the *** were entered in the study because they presented with high levels of prostate specific antigen (PSA) (over 4 ng/mL) and/or had undergone an abnormal digital rectal examination (DRE).in addition to sextant prostate biopsy cores,four more biopsies were obtained from the lateral peripheral zone with additional cores from each suspicious area revealed by transrectal ***,lateral peripheral zone and suspicious area biopsy cores were submitted separately to the patho- logical ***:Cancer detection rates were 27.6 % (42/152) and 19.7 % (30/152) for the 10-core and sextant core biopsy protocols,*** the lateral peripheral zone (PZ) to the sextant prostate biopsy showed a 28.6 % (12/42) increase in the cancer detection rate in patients with positive prostate cancer (Pion rate in patients who presented with elevated PSA was 29.3 % (34/116).When serum PSA was 4-10 ng/mL TRUS-guided biopsy detected cancer in 20.6 %,while the detection rate was 32.4 % and 47.0 % when serum PSA was 10-20 ng/mL and above 20 ng/mL,***:The 10 systematic TRUS-guided prostate biopsy improves the detection rate of prostate cancer by 28.6 % when compared with the sextant biopsy technique alone,without increase in the *** therefore recommend the 10-core biopsy protocol to be the preferred method for early detection of prostate cancer.
