AIM:To conduct a meta-analysis comparing outcomes after pancreaticoduodenectomy(PD)with or without prophylactic ***:Relevant comparative randomized and nonrandomized studies were systemically searched based on specific inclusion and exclusion *** outcomes were compared between patients with and those without routine *** odds ratios(OR)with 95%CI were calculated using either fixed effects or random effects ***:One randomized controlled trial and four non-randomized comparative studies recruiting 1728patients were *** without prophylactic drainage after PD had significantly higher mortality(OR=2.32,95%CI:1.11-4.85;P=0.02),despite the fact that they were associated with fewer overall complications(OR=0.62,95%CI:0.48-0.82;P=0.00),major complications(OR=0.75,95%CI:0.60-0.93;P=0.01)and readmissions(OR=0.77,95%CI:0.60-0.98;P=0.04).There were no significant differences in the rates of pancreatic fistula,intraabdominal abscesses,postpancreatectomy hemorrhage,biliary fistula,delayed gastric emptying,reoperation or radiologic-guided drains between the two ***:Indiscriminate abandonment of intraabdominal drainage following PD is associated with greater mortality,but lower complication *** randomized trials should compare routine vs selective drainage.

We have previously shown the neuroprotective and pro-neurogenic activity of microneurotrophin BNN-20 in the substantia nigra of the“weaver”mouse,a model of progressive nigrostriatal ***,we extended our investigation in two clinically-relevant ***,we assessed the effects of BNN-20 on human induced pluripotent stem cell-derived neural progenitor cells and neurons derived from healthy and parkinsonian ***,we assessed if BNN-20 can boost the outcome of mouse neural progenitor cell intranigral transplantations in weaver mice,at late stages of *** found that BNN-20 has limited direct effects on cultured human induced pluripotent stem cell-derived neural progenitor cells,marginally enhancing their differentiation towards neurons and partially reversing the pathological phenotype of dopaminergic neurons generated from parkinsonian *** agreement,we found no effects of BNN-20 on the mouse neural progenitor cells grafted in the substantia nigra of weaver ***,the graft strongly induced an endogenous neurogenic response throughout the midbrain,which was significantly enhanced by the administration of microneurotrophin *** results provide straightforward evidence of the existence of an endogenous midbrain neurogenic system that can be specifically strengthened by ***,the lack of major similar activity on cultured human induced pluripotent stem cell-derived neural progenitors and their progeny reveals the in vivo specificity of the aforementioned pro-neurogenic effect.

Advances in the neurobiology of growth factors, neural development, and prevention of cell death have resulted in a heightened clinical interest for the development of protective and regenerative neuromodulatory strategies for the cavernous nerves （CNs）, as therapies for prostate cancer and other pelvic malignancies often result in neuronal damage and debilitating loss of sexual function. Nitric oxide released from the axonal end plates of these nerves within the corpora cavernosa causes relaxation of smooth muscle, initiating the haemodynamic changes of penile erection as well as contributing to maintained tumescence; the loss of CN function is primarily responsible for the development of erectile dysfunction （ED） after pelvic surgery and serves as the primary target for potential neuroprotective or regenerative strategies. Evidence from pre-clinical studies for select neuromodulatory approaches is reviewed, including neurotrophins, glial cell line-derived neurotrophic factors （GDNF）, bone morphogenic proteins, immunophilin ligands, erythropoetin （EPO）, and stem cells.

A 2-year field study was conducted in northern Greece to investigate the effect of nitrogen fertilization and irrigation on productivity of three Greek chickpea varieties (“Amorgos” “Serifos”, “Andros”). Chickpea, grown under irrigation regime (30 + 30 mm of water) and fertilized with 50 kg·N·ha-1 before planting and with 40 kg·N·ha-1 at blossom growth stage, produced more total dry biomass and seed yield as compared with that grown under non-irrigated conditions and fertilized with 50 kg·N·ha-1 before planting only. In particular, irrigation and nitrogen fertilization at blossom growth stage increased total dry weight of chickpea by 18.3% and 18.5%, respectively, as compared with that of non-irrigated and fertilized with N before planting. The corresponding increase of seed yield was 30.5% and 20%, respectively. The total dry biomass of “Amorgos” was 10% and 13% greater than that of “Serifos” and “Andros”, while its respective seed yield increase was 5% and 16%. Finally, the quantum yield of photosystem II of chickpea was not affected by irrigation or fertilization. These results indicated that nitrogen fertilization at blossom growth stage combined with irrigation increased seed yield of all chickpea varieties, whereas the same treatments did not have any effect on plant quantum yield of photosystem II.

Many in vitro studies focus on effects of wall shear stress (WSS) and wall shear stress gradient (WSSG) on endothelial cells, which are linked to the initiation and progression of atherosclerosis in the arterial system. Limitation in available flow chambers with a constant WSSG in the testing region makes it difficult to quantify cellular responses to WSSG. The current study proposes and characterizes a type of converging parallel plate flow chamber (PPFC) featuring a constant gradient of WSS. A simple formula was derived for the curvature of side walls, which relates WSSG to flow rate (Q), height of the PPFC (h), length of the convergent section (L), its widths at the entrance (w0) and exit (w1). CFD simulation of flow in the chamber is carried out. Constant WSSG is observed in most regions of the top and bottom plates except those in close proximity of side walls. A change in Q or h induces equally proportional changes in WSS and WSSG whereas an alteration in the ratio between w0 and w1 results in a more significant change in WSSG than that in WSS. The current design makes possible an easy quantification of WSSG on endothelial cells in the flow chamber.

Fat Embolism Syndrome in a Patient with Bilateral Tibial Fractures: Report of the Case and Review of the Literature

Background: Although the original clinical description of fat embolism syndrome (FES) dates from 1873, the condition remains a diagnostic challenge for modern clinicians. The syndrome is described as a serious consequence of fat emboli producing a distinct pattern of clinical symptoms and signs. It is mainly associated with fractures of the long-bones and the pelvis. The present paper describes the case of a trauma patient with bilateral tibial fractures that present the syndrome and highlights the importance of early identification and aggressive treatment of FES which is obligatory for the patient’s survival. Case Description: A 32 year-old man reached the hospital as a trauma call after a road traffic accident. Clinical examination was unremarkable with the only finding that the patient was unable to straight leg raise and weight bear. Both shins were painful and swollen at their middle third, while the left one was in varus and posterior displacement. Neurovascular status of both lower extremities was normal. Plain radiographic control (X-Rays) revealed an oblique right tibial fracture of the mid-shaft with a distal third fibular fracture and a comminuted fracture of the left tibia with a fracture of the fibula at the same level. The legs were immobilised on a splint and the decision was to treat the patient surgically. Within a few post admission hours the haemoglobin dropped by 2.9 mg/dL, however a source of active bleeding could not be allocated. The abdomen was soft and not tender in palpation and the central nervous system did not present any abnormalities. The following day the haemoglobin dropped but the new ultrasonography control did not reveal any free abdominal fluid. Both lower limbs were not compromised. The second post-injury day the patient went to theatre and the right tibia was stabilised with an antegrade nail (T2-Stryker) while the left one with an external fixation. The olive fruit fly Bactrocera oleae is a major olive pest in Algeriaand other Mediterranean countries. Its attacks, in some cases, seriously compromised production. Bacillus thuringiensis is a spore-forming soil bacterium which produces a protein crystal toxic to some insects. The objective of this study was to search for isolates toxic to larvae and adults of B. oleae. Four doses test were performed on neonate larvae, second instar larvae and third instar larvae. Seven Bt strains examined, showed highest toxicity levels against adults. 86% mortality was observed against neonate larvae after 7 days of application by Bt toxin isolate. The mortality of the second instar larvae was 65% mortality at dose of 108 UFC/mL. The third instar larvae was very susceptible to Bt strain with 70% of mortality. This study demonstrated that some local Algerian B. thuringiensis isolates exhibit toxic potential that could be used to control B. oleae.
