Background: Reviewing and analyzing the Clinico-pathologic aspects of non-melanoma skin cancer of the head and neck (NMSCHN), type of management, prognostic factors, and disease-free survival (DFS) in a period of 5 years at the National Cancer Institute—Cairo University—Egypt. Materials and Methods: A retrospective study of two hundred patients with NMSCHN was treated at the National Cancer Institute—Cairo University—Egypt from January 2008 to December 2012. The mean follow-up was 6 months (1 - 84 months). Results: 117 males and 83 females with 90% ≥ 50 years old. The scalp (27.5%), the periorbital region (13%), the cheek (12.5%) and the nose (12.5%) are the main anatomical sites affected. BCC represented 71.5% with nodular type (79%) predominance;SCC represented 21% with GII (61.1%) the commonest grade. Surgery was the main modality of treatment (93%) with local flaps only (63.9%) and primary closure (14.7%) were the main surgical options following wide local excision. Positive and close margins were detected in 23.5% of excised specimens. No significant association was found between disease-free survival (DFS) and pathology, treatment modality, the occurrence of complications or safety margin status. Conclusion: NMSCHN lesions should be surgically excised in specialized high volume centers with readily available peripheral margin control and should be operated by senior experienced surgeons.

Desert truffle is popular nutritious food that shows high content of protein, fibers, phenolic compounds and health promoting bioactive compounds. The aim of present study is to produce fortified functional biscuits by incorporated Dessert Truffle Powder (DTP). The effect of replacing 0 (control), 5%, 10% and 15% of wheat flour with DTP on chemical composition, antioxidant activity, physical properties and sensory characteristics of biscuits was investigated. The results revealed that the crude protein, ash and crude fiber content of prepared biscuits improved with the incorporation of DTP. Phenolic contents of fortified biscuits were increased gradually with increasing the level of DTP addition. Furthermore, all the selected levels of DTP incorporated in prepared biscuits showed a good ability in radical scavenging activity which ranged from 21.52% to 42.11%, compared to 2.08% in case of control biscuit. Data also indicated that replacing 5% and 10% of wheat flour with DTP in biscuit preparation resulted in high significance of spread ratio 8.78 and 8.59, respectively when compared to control biscuit which given 7.53. The lightness (L) and total intensity of biscuits were decreased from 71.19 to 60.31 and from 76.09 to 66.87 with increasing the substitution level of DTP in biscuits from 5% to 15%, respectively. Biscuits incorporated with DTP at up to 10% had the acceptable mean scores by panelists for all sensory attributes. There was no significant difference in overall acceptability of biscuits incorporated with 5% and 10% of DTP which recorded 7.4 and 7.5 and control biscuit 8.2, respectively. Thus, the present study concluded that replacing up to 10% of wheat flour with DTP enhanced the nutritional quality and antioxidant activity with acceptable sensory characteristics of biscuits. So, our study recommends encouraging the bakery products producers to incorporated DTP at 10% in biscuits production.

To evaluate factors that influence the diagnostic accuracy of endoscopic ultrasound (EUS)-guided tissue acquisition for lymph node enlargement in the absence of an on-site pathologist. METHODSA retrospective analysis of patients who underwent EUS-guided tissue acquisition for the pathological diagnosis of lymph node enlargement between April 2012 and June 2015 is reported. Tissue acquisition was performed with both cytology and biopsy needles of different calibers. The variables evaluated were lymph node location and size, number of passes and type of needle used. Final diagnosis was based on surgical histopathology or, in non-operated cases, on EUS-guided tissue acquisition and imaging assessment with a minimum clinical follow-up of 6 mo. RESULTSDuring the study period, 168 lymph nodes with a median size of 20.3 mm (range 12.5-27) were sampled from 152 patients. Ninety lymph nodes (53.6%) were located at mediastinum, and 105 (62.5%) were acquired with biopsy needles. The final diagnosis was benign/reactive origin in 87 cases (51.8%), malignant in 65 cases (38.7%), and lymphoma in 16 cases (9.5%). The sensitivity, specificity, positive predictive value and negative predictive value for the detection of malignancy were 74.1%, 100%, 100% and 80.6%, respectively. The overall accuracy was 87.5% (95%CI: 81.7-91.7). No variables were independently associated with a correct final diagnosis according to the multivariate analysis. CONCLUSIONEUS-guided tissue acquisition is a highly accurate technique for assessing lymph node enlargement. None of the variables evaluated were associated with diagnostic accuracy.

Background: HIV/AIDS is a pandemic disease and its scourge has had a devastating impact on health, nutrition, food security and overall socioeconomic development in affected countries. Moreover, intervention programmes, which simply employ antiretroviral drugs, have been found to lack effectiveness particularly when the patient is under-nourished. Aim and Purpose: This presented pilot intervention provides evidence that suggests use of local resources as therapeutic nutrition. This can act as a fundamental part of the comprehensive package of care at the country level. Methodology: Local ingredients, which were known for their availability, accessibility, micro and macro-nutrient strengths were selected and optimised into a nutritional functional meal (Amtewa). Daily consumption was ascertained to assess its effects on nutritional status and biomedical indices of the study participants (n = 100) who were/were not taking Highly Active Anti-retroviral Therapy (HAART). Findings: Mean CD4 count for ART-Test group at baseline and sixth months increased by 40.8 cells/mm3 while the ART-Control group decreased 18.12 cells/mm3. This positive outcome qualified Amtewa meal to the next phase of intervention (400 participants) to ascertain its effectiveness on health status of HIV infected subjects and appraise its position within the National Health Services framework as innovative approach to attenuate the progression of HIV to AIDS in Nigeria. Conclusion: Amtewa-based approach in HIV management is innovative, culturally relevant, reliable and requiring low technology in order to assure compliance, sustainability and cost effectiveness. Although the achieved results take the form of specific technology, it suggests that a prolong consumption of the intervention meal (Amtewa) will be suitable to sustain the gained improvements in MUAC and CD4 cell counts thereby slowing the progression of the disease.

Several studies are conducted to improve nutritional enrichment of ice cream. The aim of the present study was to produce healthy ice cream by partial replacement of fat with whey protein (WP), dietary fibers (oat (OT) and wheat germs (WG)) and modified starch (MS). Reduced fat ice cream (8% fat) was prepared as control. Healthy reduced fat ice cream was prepared by replacing 1% and 2% of fat with WP, OT, WG and MS. The fat replacement with OT and WG increased the fiber content of ice cream. In addition, ice cream from all treatments characterized by a significant great free radical scavenging activity when compared with control, especially when fat was replaced with WG, WP and OT, respectively. The sensory evaluation of ice cream revealed an enhancement of flavor when fat was replaced with 1% WP and OT and 2% WP (P < 0.05 vs. control). The modified starch treatments (1% and 2%) showed the highest viscosity followed by OT (2%) treatment (P < 0.05). In conclusion, these results showed that dietary fibers (OT & WG), WP and MS could be used to replace fat in ice cream and to give a healthy reduced fat ice cream with high fiber content and with high antioxidant activity.

Inflammatory myoblastic tumour (IMT) is a rare benign neoplasm that can affect any part of the body including the head and neck. When it is affecting the head and neck region, the clinical presentations can mimic other common head and neck pathology. If the tumour involves the upper airway it can cause stridor and hoarseness. Other presenting features are neck swelling and dysphagia. This tumour may mimic other chronic granulomatous diseases. This case report described a rare IMT arising from the vocal cord and presents with hoarseness and stridor for six months duration. The patient has no known risk factors. Despite surgical resection, the tumour recurred within six month and further surgical resection was performed, but yet the tumour recurred after one year following second surgery. She was then further treated with mass excision by Endolaryngeal Microsurgery (ELMS) with steroid laryngeal injection. Currently she is still being followed up and remains asymptomatic.

Tonsillitis is one of the most common presentations to the primary care clinic. Majority of the tonsillitis is viral etiology and occasionally is caused by bacteria such as streptococcal pharyngo-tonsillitis. However, if tonsillitis lasts longer than a week and associated with cervical lymph nodes, other differential diagnoses should be considered. This case report illustrated a patient who presented with fever, sore throat and odynophagia which lasted for one week then developed trismus. Examination revealed a left tonsillar hypertrophy with soft palate swelling and pus aspirated from the tonsil revealed positive smear to Acid-fast Bacilli and he responded to anti-tuberculosis treatment.
