Information on the dynamics of savannah is important to a country's plan to overcome the problems of uncontrolled development and environmental hazards. Taking the reserve partielle de Dosso, Niger as the case study area, this paper analyzed the long-term land use land cover change from 2002 to 2022. Satellite images were processed by using Google Earth Engine (GEE). Therefore, four major land cover classes were identified based on spectral characteristics of Land sat, namely, built-up, vegetation, cropland, bare land and water. The result revealed that barren and built-up areas increased at the expense of vegetation and water. From the four major land use land cover the large area is covered by vegetation which comprises about 192963.5 hectares followed by cropland and water consisting of 32506.43 and 1596.4 hectares respectively. The built-up area gained substantial area (most) during the study period. The reduction in some of the land cover/uses underlines the dangerous trend of the pressure poised by population growth and the changing functionality. Land cover change is influenced by a variety of societal factors operating on several spatial and temporal levels. The area estimates and spatial distributions of the LULC classes produced from the current study will assist local authorities, managers, and other stakeholders in decision-making and planning regarding forest land cover and uses.

The aim of this study was to present the epidemiological, clinical and therapeutic aspects of adult urological cancers in the department of urology at Zinder National Hospital. Patients and Methods: This was a cross-sectional study of adult primary urologic cancers during the period of January 2019 to December 2023. Data were obtained from the record of patients admitted to urology, and the medical oncology department. The parameters studied were;frequency, age, sex, urogenital distribution, anatomopathological type, and therapeutic aspects. Data were recorded on Excel 2013 and analyzed with Epi-info version 7.2.5. Results: Epidemiology: About 289 cancers were diagnosed through 6017 consultations during the period, corresponding to 4.80% of urological pathologies: prostate cancer (n = 221;76.47%), bladder cancer (n = 46;15.92%), kidney cancer (n = 13;04.5%), testis cancer (n = 09;03.11%). Median age of patients was 50.42 years. Prostate cancer: was Adenocarcinoma in 100% (n = 221) with Gleason scores > 7 (77.83%) and prostatique specific antigen (PSA) > 20 ng/ml in 90.04%. Extension evaluation 134 cases (60.63%). Treatment was chirurgical castration in (62.44%) and resistance to castration appeared between 8 and 11 months. Hospital mortality for prostate cancer was 16.29%. Bladder cancer was found in 46 cases with Sex ratio 7/1. Cystoscopy was performed for all. Histology revealed squamous cell carcinoma (n = 41;89.13%), and (n = 5;10.87%) urothelial. Endoscopic resection performed (n = 14;30.43%). Kidney cancers were 13 cases (10 men, 03 women). Nephrectomy was performed in (n = 8;61.54%) cases and Histology revealed renal cell cancer in 76.92%. Testicular cancers were diag 09 cases. History of cryptorchidism was found in 4 cases, 4-year infertility in 3 cases. Orchidectomy was done in all cases. Chemotherapy in 3 cases. Conclusion: urological cancers are frequent, dominated by prostate cancer. The endemicity of bilharziasis has increased squamous cell carcinoma. T

Introduction: Obstetric fistula (OF) is an abnormal communication between the genital and urinary tracts and/or associated with a recto-vaginal communication resulting from prolonged dystocic delivery. It is a frequent pathology in underdeveloped countries. In Zinder no study has been carried out on obstetric fistula. The aim of this study was to evaluate the epidemiological, social impact and therapeutic aspects of obstetric fistula at the CSME of Zinder. Patient and Method: This was a descriptive cross-sectional study of obstetric fistula care at CSME during the period of (January 2018 to June 2020). Results: A total of 196 cases were collected over 30 months. This represents an annual incidence of 78.4 fistulas. The median age of the patients was 18.63 years, and more than 65% were over 20 years old. One hundred and ninety-one patients (97.45%) did not attend school, and 56.12% (n = 110) lived in polygamous households. One hundred and fifty-one women (n = 151), 77.04% had full assistance from their husband during the first four (4) months of the pathology. Twenty-four (12.24%) had been notified of repudiation. Obstetrical risk of dystocia was found in 39.79% (n = 78) of the patients, of whom 24.49% (n = 48) had a focused antenatal consultation (CPNR). The labour lasted more than 24 hours in 100%. The majority of women are multiparous and 60% had at least 2 pregnancies. vesicovaginal fistula is the main pathological type with 86.23%. Fistulas were closed in 83.16% and 68.87% exited without urine loss. Conclusion: Obstetric fistula is a devastating disease affecting girls and young women in Niger, as in other countries in Africa and Asia. It is a major concern worldwide and is a social tragedy because it is disabling and depressing.

Cerebral venous thrombosis (CVT) in sickle cell disease has been rarely described in the literature. Some authors consider sickle cell disease as a risk factor for CVT. We report the case of a 20-year-old boy, known as sickle cell, followed at the National Reference Center for Sickle Cell Disease in Niger. Admitted in consultation for a decrease of acuity with the right eye with perception of a black spot evolving since about three (3) days, ophthalmological examination revealed sickle cell retinopathy associated with cerebral venous thrombosis on MRI. The rest of the balance showed normochromic anemia, ionic disturbance and a D-dimer at 1500 μg/l. Rehydration, strict rest, antibiotic therapy and analgesics were started with an internal medicine opinion for further treatment. There was an improvement in visual acuity of 6/10 in 48 hours with disappearance of black spot perception, after three weeks, an ad integrum recovery of retinal lesions to control imaging. Despite the ocular complications of sickle cell disease involving visual prognosis, cerebral venous thrombosis is another complication to be investigated in any sickle cell retinopathy.

Introduction: Adult bone tumors may be primary or secondary. Their diagnosis is often based on the results of imaging tests but can also be completed by biopsy. Case Presentation: We report the observation of a clinical case of fortuitous discovery in the orthopaedic-traumatology department of the national hospital Amirou Boubacar Diallo of Niamey, pathological fracture of the right humeral shaft secondary to metastasis of clear-cell adenocarcinoma of renal origin, a 57-year-old man, a refrigeration engineer by profession, from Niger living in the Middle East. This patient has no known pathological history. He consulted us for pain in his right arm that was more severe at night. All imaging and pathology examinations carried out both in Niger and in the Middle East confirmed the diagnosis of humeral metastasis of clear-cell renal cell carcinoma. Surgical management consisted of tumor resection and Kuntscher-type centromedullary alignment nailing of the fracture site. The loss of bone substance in the fracture is filled with acrylic cement. The treatment was completed by chemotherapy. Conclusion: Bone tumours are most often secondary in location. However, the diagnosis must be sought in the presence of any clinical sign.

Equalisation of Lower Limb Length in Young Adults Using Internal Osteosynthesis: A Case Report of 22 Patients

Introduction: Inequality in the length of the lower limbs has an impact on the stability and function of the musculoskeletal system because of the imbalance it causes in our bodies. Several treatment modalities were used by practitioners. The aim of this study was to evaluate the anatomical and functional results of length equalisation of the lower limbs using internal osteosynthesis in the Orthopaedic and Traumatological Surgery Department of the Amirou Boubacar Diallo National Hospital in Niamey. Patients and method: This was a retrospective, prospective study of 22 patients from January 2010 to November 2015, a period of 5 years. The patients concerned were aged at least 18 years and had undergone a femoral shortening osteotomy not exceeding 5 cm on the contralateral healthy limb with a screw plate as the internal osteosynthesis device. Results: The average age of our patients was 24.18 years (18 to 48 years). The etiologies of these length inequalities were: vascular 54.55% (n = 12), post-traumatic 27.27% (n = 6), post-infectious 13.64% (n = 3) and neuromuscular 4.54% (n = 1). The mean length inequality of our patients was 4.93 cm, with extremes ranging from 4 to 8 cm. At an average follow-up of 6.5 months (6 to 24 months), our functional results were satisfactory overall. Discussion: Despite the significant nature of our initial inequalities, which were moderate and severe, we ended up with slight inequalities or even total correction for some of our patients. Conclusion: Inequality in the length of the lower limbs is a handicap that requires appropriate management.
