目的探讨瑞格列奈治疗2型糖尿病的临床疗效。方法选取2012年10月—2014年10月医院收治的206例2型糖尿病患者,按照住院时间分为两组,对照组103例给予二甲双胍和格列本脲治疗,观察组103例给予瑞格列奈治疗,分析总结两组患者的治疗效果和不良反应。结果观察组治疗有效率为97.1%,有2例出现轻微低血糖反应,不良反应发生率为1.9%,对照组治疗有效率为76.7%,有5例出现轻度腹泻,4例出现胃痛或不适,不良反应发生率为8.7%,两组比较差异有统计学意义(P<0.05)。结论瑞格列奈治疗2型糖尿病的临床疗效满意,不良反应少,安全性高,可以明显提高患者的生活质量,值得在临床上大力推广应用。

Objective: To investigate the clinical features of the multiple cutaneous and uterine leiomyomatosis (MCUL) syndrome, including the hereditary leiomyomatosis and renal cell cancer syndrome. Design: A case series of patients with multiple skin leiomyomas solicited via a circular letter to dermatologists. Setting: Research institute. Patients: A total of 108 affected individuals, including 46 probands and 62 affected relatives. Main Outcome Measures: The proportion of probands with underlying fumarate hydratase (FH) mutations, the penetrance of FH mutations, and clinicopathologic features of MCUL. Results: Forty-one (89% ) of 46 probands with multiple skin leiomyomas had evidence of germline FH mutations, which were highly penetrant. All 26 male mutation carriers had skin leiomyomas. Of 67 women with FH mutations, 46 (69% ) had both skin and uterine leiomyomas; 10 (15% ) had only skin leiomyomas;5(7% )had only uterine leiomyomas;and 6(9% )were clinically unaffected. Patients presented with skin leiomyomas at a mean age of 24 years and had a mean of 25 lesions. Forty-one individuals (89% ) reported painful lesions, particularly in response to cold or trauma. Fibroids were histologically unremarkable, highly symptomatic, and associated with a high risk of early hysterectomy. One individual had a very aggressive collecting duct renal cancer. The G354R FH mutation predisposed patients to uterine fibroids without skin leiomyomas (P=.03). Many patients with skin leiomyomas had not previously presented for medical attention. Fibroids were rarely recognized as cases of MCUL. Conclusions: Highly penetrant FH mutations underlie MCUL. increased clinical awareness is important because of the associated risk of severe uterine fibroids and, in some cases, aggressive renal cancer.

Objectives: To investigate the occurrence of rape/attempted rape and verbal sexual coercion among an ethnically diverse adolescent population and to evaluate whether unique risk factors existed for victims of either type of forced sexual experience. Design: Cross-sectional survey assessing sexual violence within the past 12 months, behaviors that occurred on each subject’s most recent date, and the occurrence of unwanted sexual experiences on any dates in the past year and details about that date. Subjects identified as experiencing either rape/attempted rape or verbal sexual coercion were compared with nonvictims using bivariate and multivariate analyses. Setting: Urban adolescent health care facility. Participant: Female adolescents and young adults (n = 689) between the ages of 14 and 23 years who presented for care and met the eligibility criteria. Results: Approximately 30%of youngwomen reported having an unwanted sexual experience in the past year. The risk of rape/attempted rape was increased by past mild to moderate (adjusted odds ratio [AOR] = 4.27) or severe (AOR = 15.24) physical aggression by the dating partner; date-specific behaviors, including decreased level of romantic involvement [AOR] = 0.63 per point decline)-and going to the perpetrator’s house to be alone (AOR = 3.01); past sexual victimization as an adolescent (AOR = 4.70); and lower levels of self-reported ethnic identity (AOR = 1.07 per point decline). More important, alcohol use by the victim or perpetrator was unrelated to an increased risk of rape/attempted rape. The risk of verbal sexual coercion was increased by past mild to moderate (AOR = 4.38) or severe (AOR = 13.79) verbal aggression from the dating partner; date-specific behaviors, including decreased level of romantic involvement (AOR = 0.70 per point decline) and greater number of past dates (AOR = 7.53); going to the perpetrator’s house to be alone (AOR = 3.52); past sexual victimization as an adolescent (AOR = 9.83); pressures

Context: The “ hygiene hypothesis" has implicated sibship as a marker of infection load during early life and suggests that exposure or reexposure to infections can influence the developing immune system. Viral infection has also been implicated in the pathogenesis of multiple sclerosis (MS). Abstract:Objectives: To evaluate whether exposure to infant siblings in early life is associated with the risk of MS, and to explore the possible mechanism for any apparent protective effect, including altered Epstein- Barr virus (EBV) infection patterns. Design, Setting, and Patients: population- based case- control study in Tasmania, Australia, from 1999 to 2001 based on 136 cases of magnetic resonance imaging- confirmed MS and 272 community controls, matched on sex and year of birth. Main Outcome Measure: Risk of MS by duration of contact with younger siblings aged less than 2 years in the first 6 years of life. Results: increasing duration of contact with a younger sibling aged less than 2 years in the first 6 years of life was associated with reduced MS risk (adjusted odds ratios [AORs]: infant- year, 1.00 [reference]; 1 to infant- years, 0.57 [95% confidence interval {Ci}, 0.33- 0.98]; 3 to infant- years, 0.40 [95% Ci, 0.19- 0.92]; ≥ 5 infantyears, 0.12 [95% Ci, 0.02- 0.88]; test for trend, P=.002). A history of exposure to infant siblings was associated with a reduced igG response to EBV among controls. Controls with at least 1 infant- year contact had a reduced risk of infectious mononucleosis and a reduced risk of very high composite EBV igG titers (AOR, 0.33; 95% Ci, 0.11- 0.98) compared with other controls. The inverse association between higher infant contact and MS was independent of EBV igG titer. Conclusion: Higher infant sibling exposure in the first 6 years of life was associated with a reduced risk of MS, possibly by altering childhood infection patterns and related immune responses.
