Impact of propofol and sevoflurane anesthesia on cognition and emotion in gastric cancer patients undergoing radical resection

BACKGROUND Propofol and sevoflurane are commonly used anesthetic agents for maintenance anesthesia during radical resection of gastric ***,there is a debate concerning their differential effects on cognitive function,anxiety,and depression in patients undergoing this *** To compare the effects of propofol and sevoflurane anesthesia on postoperative cognitive function,anxiety,depression,and organ function in patients undergoing radical resection of gastric *** A total of 80 patients were involved in this *** subjects were divided into two groups:Propofol group and sevoflurane *** evaluation scale for cognitive function was the Loewenstein occupational therapy cognitive assessment(LOTCA),and anxiety and depression were assessed with the aid of the self-rating anxiety scale(SAS)and self-rating depression scale(SDS).Hemodynamic indicators,oxidative stress levels,and pulmonary function were also *** The LOTCA score at 1 d after surgery was significantly lower in the propofol group than in the sevoflurane ***,the SAS and SDS scores of the sevoflurane group were significantly lower than those of the propofol *** sevoflurane group showed greater stability in heart rate as well as the mean arterial pressure compared to the propofol ***,the sevoflurane group displayed better pulmonary function and less lung injury than the propofol *** Both propofol and sevoflurane could be utilized as maintenance anesthesia during radical resection of gastric *** anesthesia has a minimal effect on patients'pulmonary function,consequently enhancing their postoperative *** anesthesia causes less impairment on patients'cognitive function and mitigates negative emotions,leading to an improved postoperative mental ***,the selection of anesthetic agents should be based on the individual patient's specific circumstances.

Long-term optical color behavior of a sample of blazars

We report on the optical color variability of a sample of 24 blazars, consisting of nine flat spectrum radio quasars(FSRQs) and 15 BL Lacertae objects(BL Lacs), on timescales of years using simultaneous V-and R-band observations observed by the Kanata telescope at Higashi-Hiroshima *** correlations between color indices V-R and magnitudes reveal that 11 BL Lacs and one FSRQ exhibited significant(i.e., r > 0.2 and P < 0.01) bluer-when-brighter(BWB) trend and two FSRQs followed the redder-when-brighter(RWB) tend, indicating a possibility that the BWB chromatic trend is dominant for BL Lacs and the RWB trend is especially found in FSRQs, which has been presented occasionally in different samples of *** superpositions of the red emission component from the Doppler-boosted relativistic jet and the blue component arising from the accretion disk might be a possible interpretation for the long-term color behaviors.

A survey on model-based reinforcement learning

Reinforcement learning(RL) interacts with the environment to solve sequential decision-making problems via a trial-and-error approach. Errors are always undesirable in real-world applications, even though RL excels at playing complex video games that permit several trial-and-error attempts. To improve sample efficiency and thus reduce errors, model-based reinforcement learning(MBRL) is believed to be a promising direction, as it constructs environment models in which trial-and-errors can occur without incurring actual costs. In this survey, we investigate MBRL with a particular focus on the recent advancements in deep RL. There is a generalization error between the learned model of a non-tabular environment and the actual environment. Consequently, it is crucial to analyze the disparity between policy training in the environment model and that in the actual environment, guiding algorithm design for improved model learning, model utilization, and policy training. In addition, we discuss the recent developments of model-based techniques in other forms of RL, such as offline RL, goal-conditioned RL, multi-agent RL, and meta-RL. Furthermore,we discuss the applicability and benefits of MBRL for real-world tasks. Finally, this survey concludes with a discussion of the promising future development prospects for MBRL. We believe that MBRL has great unrealized potential and benefits in real-world applications, and we hope this survey will encourage additional research on MBRL.

Retinoic acid alleviates rotavirus-induced intestinal damage by regulating redox homeostasis and autophagic flux in piglets

Rotaviruses (RV) are a major cause of severe gastroenteritis, particularly in neonatal piglets. Despite the availability of effective vaccines, the development of antiviral therapies for RV remains an ongoing challenge. Retinoic acid (RA), a metabolite of vitamin A, has been shown to have anti-oxidative and antiviral properties. However, the mechanism by which RA exerts its intestinal-protective and antiviral effects on RV infection is not fully understood. The study investigates the effects of RA supplementation in Duroc × Landrace × Yorkshire (DLY) piglets challenged with RV. Thirty-six DLY piglets were assigned into six treatments, including a control group, RA treatment group with two concentration gradients (5 and 15 mg/d), RV treatment group, and RV treatment group with the addition of different concentration gradients of RA (5 and 15 mg/d). Our study revealed that RV infection led to extensive intestinal architecture damage, which was mitigated by RA treatment at lower concentrations by increasing the villus height and villus height/crypt depth ratio ( P < 0.05), enhancing intestinal stem cell signaling and promoting intestinal barrier functions. In addition, 15 mg/d RA supplementation significantly increased NRF2 and HO-1 protein expression ( P < 0.05) and GSH content ( P < 0.05), indicating that RA supplementation can enhance anti-oxidative signaling and redox homeostasis after RV challenge. Additionally, the research demonstrated that RA exerts a dual impact on the regulation of autophagy, both stimulating the initiation of autophagy and hindering the flow of autophagic flux. Through the modulation of autophagic flux, RA influence the progression of RV infection. These findings provide new insights into the regulation of redox hemostasis and autophagy by RA and its potential therapeutic application in RV infection.

TWO-YEAR OBSERVATION OF A RANDOMIZED TRIAL ON TACROLIMUS-BASED THERAPY WITH WITHDRAWAL OF STEROIDS OR MYCOPHENOLATE MOFETIL AFTER RENAL TRANSPLANTATION

Objective To evaluate the safety and feasibility of steroid or mycophenolate mofetil （MMF） withdrawal from tacrolimus-based immunosuppressant regimen in renal allograft recipients. Methods A cohort of 45 patients following cadaveric renal allograft transplantation were randomly divided into 3 groups based on the regimen of combination of tacrolimus, steroid, and MMF： triple therapy group, steroid withdrawal group, and MMF withdrawal group. During 2 years, survival of patients and allografts, clinical acute rejection, adverse events, hepatic and renal allograft function, and blood lipids were monitored to evaluate the safety and feasibility of steroid or MMF withdrawal after renal transplantation. Results During two-year observation, steroid or MMF was successfully withdrawn from immunosuppressant regimen based on tacrolimus without any clinical acute rejection renal allografts kept excellent function. Some adverse events among groups. Patient and graft survival rates were 100% and all the occurred and there were no significant differences Conclusion Withdrawal of steroid or MMF in low-immunological-risk renal allografts treated with tacrolimus-based immunosuppressant regimen can be achieved with no increased risk of acute rejection.

Enzymatic C1 reduction using hydrogen in cofactor regeneration

Carbon dioxide fixation presents a potential solution for mitigating the greenhouse gas *** carbon dioxide fixation,C1 compound reduction requires a high energy *** calculations suggest that the energy source for cofactor regeneration plays a vital role in the effective enzymatic C1 *** utilizes renewable hydrogen to achieve the regeneration and supply cofactor nicotinamide adenine dinucleotide(NADH),providing a driving force for the reduction reaction to reduce the thermodynamic barrier of the reaction cascade,and making the forward reduction pathway thermodynamically *** on the regeneration of cofactor NADH by hydrogenase,and coupled with formaldehyde dehydrogenase and formolase,a favorable thermodynamic mode of the C1 reduction pathway for reducing formate to dihydroxyacetone(DHA)was designed and *** resulted in accumulation of 373.19μmol·L^(-1) DHA after 2 h,and conversion reaching 7.47%.These results indicate that enzymatic utilization of hydrogen as the electron donor to regenerate NADH is of great significance to the sustainable and green development of bio-manufacturing because of its high economic efficiency,no by-products,and environment-friendly ***,formolase efficiently and selectively fixed the intermediate formaldehyde(FALD)to DHA,thermodynamically pulled formate to efficiently reduce to DHA,and finally stored the low-grade renewable energy into chemical energy with high energy density.

Introduction:Acrodermatitis continua of Hallopeau(ACH)is an uncommon chronic cutaneous *** to date,its treatment is still difficult and frustrating although various topical and systemic agents alone or in different combinations have been tried with variable ***,we report two cases of ACH successfully treated with calcipotriol/betamethasone *** presentation:Two cases of ACH were characterized by thumb involvement alone and showed an excellent response to calcipotriol/betamethasone ointment(1 g:calcipotriol 50µg and betamethasone 0.5 mg).Discussion:Treatment of ACH is challenging for *** systemic therapies and topical drugs have been applied,however,no universally effective treatment regimen has yet been established for long-lasting *** of calcipotriol and betamethasone is more effective in treating psoriasis than the individual components alone,and the current 2 cases with ACH were reported to be treated successfully by the calcipotriol/betamethasone ***:Topical calcipotriol/betamethasone compound is an optional agent for the treatment of ACH with a novel effect.
