Background An elevated serum lipid is one of the major risk factors for coronary heart disease （CHD）. Physicians＇ awareness contributes to successful adoption of practice guidelines. Influence of ambulance use on early reperfusion therapies for acute myocardial infarction

Background Ambulance use expedites the definitive treatment of acute myocardial infarction （AMI）. The aim of this study was to evaluate the effect of ambulance use on the administration of early reperfusion therapies for patients with AMI in Beijing, China. Methods data were prospectively collected from 498 patients with ST-elevation myocardial infarction （STEMI） who were admitted within 12 hours of symptom onset to 19 hospitals in Beijing between November 1,2005 and December 31, 2006. The baseline characteristics of and the initial management of the ambulance users and the non-ambulance users were compared. Results Only 186 （37.3%） patients used an ambulance as transportation to the hospital. Ambulance users were, on average, older and at relatively higher risk on presentation than the non-ambulance users. After adjustment for patient and hospital characteristics, ambulance use was associated with a greater early reperfusion rate, mainly because of a greater incidence of primary percutaneous coronary intervention. In addition, ambulance users had a significantly shorter median door-to-balloon （120 compared with 145 minutes, P 〈0.001） and symptom onset-to-balloon （223 compared with 300 minutes, P 〈0.001） time than non-ambulance users. Conclusions Ambulances are underused by AMI patients in Beijing. Ambulance use may lead to more frequent and faster receipt of early reperfusion therapies. New public health strategies should be developed to facilitate an increased use of ambulances by AMI patients.

Spin effect on redox acceleration and regioselectivity in Fe-catalyzed alkyne hydrosilylation

Iron catalysts are ideal transition metal catalysts because of the Earths abundant, cheap, biocompatible features of iron salts. Iron catalysts often have unique open-shell structures that easily undergo spin crossover in chemical transformations, a feature rarely found in noble metal catalysts. Unfortunately, little is known currently about how the open-shell structure and spin crossover affect the reactivity and selectivity of iron catalysts, which makes the development of iron catalysts a low efficient trial-and-error program. In this paper, a combination of experiments and theoretical calculations revealed that the iron-catalyzed hydrosilylation of alkynes is typical spin-crossover catalysis. Deep insight into the electronic structures of a set of well-defined open-shell active formal Fe(0) catalysts revealed that the spin-delocalization between the iron center and the 1,10-phenanthroline ligand effectively regulates the iron center's spin and oxidation state to meet the opposite electrostatic requirements of oxidative addition and reductive elimination, respectively,and the spin crossover is essential for this electron transfer process. The triplet transition state was essential for achieving high regioselectivity through tuning the nonbonding interactions. These findings provide an important reference for understanding the effect of catalyst spin state on reaction. It is inspiring for the development of iron catalysts and other Earth-abundant metal catalysts, especially from the point of view of ligand development.

Single-source-precursor synthesis and air-plasma ablation behavior of(Ti,Zr,Hf)C/SiC ceramic nanocomposites at 2200℃

Dense monolithic(Ti,Zr,Hf)C/SiC ceramic nanocomposites with four different molar ratios of metallic elements in the(Ti,Zr,Hf)C phase(i.e.,Ti:Zr:Hf=1:1:1,2:3:5,2:3:3,and 1:2:1)were prepared upon pyrolysis of novel(Ti,Zr,Hf)-containing single-source precursors(SSPs),followed by spark plasma sintering(SPS).A thorough characterization was conducted to elucidate the synthesis of the SSPs,polymer-to-ceramic transformation,chemical/phase compositions,and microstructure of the SiTiZrHfC-based *** results revealed the feasibility of synthesizing nanocomposites with high(Ti,Zr,Hf)C contents using the SSP *** nanocomposites were characterized by a unique microstructure with in situ generated(Ti,Zr,Hf)C@C core-shell nanoparticles homogeneously mixed withβ-*** ablation behavior of the nanocomposites was evaluated on an air-plasma device for 60 ***,the nanocomposites exhibited excellent ablation resistance,and the lowest linear ablation rate reached−0.58μm/s at 2200°***,the ablation resistance can be dramatically improved by precisely tailoring the atomic ratios of metal elements within the(Ti,Zr,Hf)C phase via the molecular design of the *** formation of a multiple-oxide layer with both a high-meltingpoint phase((Ti,Zr,Hf)O_(2))and low-melting-point phases((Zr,Hf)TiO4)and glassy SiO_(2),as well as their structure,played a critical role in the enhanced ablation *** uniform distribution of the high-melting-point(Ti,Zr,Hf)O_(2)nano/microparticles throughout the glassy SiO_(2)matrix significantly enhanced the viscosity and stability of the oxide layer by the pinning effect,offering superior protection against the ingress of oxygen atoms and excellent resistance to mechanical erosion.

Public knowledge of heart attack symptoms in Beijing residents

Background Definitive treatment for heart attack is early reperfusion with either angioplasty or thrombolytic therapy, and the benefit is strictly time-dependent. Patient outcomes are improved with either therapy when initiated as soon as possible. Recognition of heart attack symptoms is logically tied to taking action to receive prompt emergency care. Inadequate knowledge of heart attack symptoms may prolong delay. The purpose of this study was to document knowledge about heart attack symptoms in Beijing residents and to identify the characteristics associated with increased knowledge of heart attack. Methods A structured survey was conducted in 18 communities in Beijing from March 1 through June 10 in 2006. Addresses and participants were selected randomly following a stratification. The survey was designed to collect knowledge of heart attack symptoms from sampled adults in each community. Results A total of 4627 respondents completed the questionnaires correctly, and 50.29% of them were female. Totally 64.15% of the respondents reported chest pain or discomfort （common symptoms） as a symptom of heart attack; 75.38% reported at least one of the following eight symptoms as a symptom of heart attack： back pain, shortness of breath, arm pain or numbness, nausea or vomiting, neck, jaw or shoulder pain, epigastric pain, sweating, weakness （less common symptoms）; 20.36% correctly reported four or more heart attack symptoms, only 7.4% knew all the correct heart attack symptoms, and 28.94% knew about reperfusion therapy for heart attack; 31.7% reported to call 120 or 999 while having a heart attack themselves; however 89.6% reported to call 120 or 999 when someone else is suffering from a heart attack. Very old persons and those with health insurance coverage, high education level, high household income, longer living in Beijing and previous experience with heart disease had greater knowledge of heart attack symptoms.
