目的探讨中间普雷沃菌引起脊柱感染患者的临床特点及诊治方案。方法总结某院脊柱外科收治的1例中间普雷沃菌引起脊柱感染患者的临床诊疗过程,并检索数据库相关文献进行复习。结果患者男性,50岁,因“腰痛伴双下肢疼痛2个月”入院,取病变组织进行宏基因组二代测序(mNGS)检测示中间普雷沃菌,诊断为中间普雷沃菌脊柱炎,经开放病灶清除、置管冲洗+自体骨移植融合内固定术治疗后给予头孢曲松钠和甲硝唑静脉滴注,奥硝唑冲洗治疗后感染获得控制。共检索出符合条件文献16篇,加上本病例,共纳入17例患者,其中男性7例,女性10例;危险因素主要为糖尿病和使用皮质类固醇病史(占35.3%);最常见的侵犯部位为腰椎(12例)、胸椎(6例)等;病原学培养阳性13例;分子学检测阳性3例,染色镜检阳性1例。17例患者均接受了抗厌氧菌治疗,其中14例联合手术治疗;1例死亡,病死率为5.9%,5例存在部分神经功能损伤,致残率为29.4%。抗厌氧菌治疗联合手术者生存率为92.8%,3例单独抗厌氧菌治疗未行手术者均治愈。结论中间普雷沃菌是一种机会致病菌,多发生于免疫功能低下者,易误诊,建议尽早送检mNGS明确病原体,以争取救治时机,降低病死率。

Although herbal medicines(HMs)are widely used in the prevention and treatment of obesity and obesity-associated disorders,the key constituents exhibiting anti-obesity activity and their molecular mechanisms are poorly ***,we assessed the inhibitory potentials of several HMs against human pancreatic lipase(hPL,a key therapeutic target for human obesity),among which the root-extract of Rhodiola crenulata(ERC)showed the most potent anti-hPL *** this study,we adopted an integrated strategy,involving bioactivity-guided fractionation techniques,chemical profiling,and biochemical assays,to identify the key anti-hPL constituents in *** ERC fractions(retention time=12.5e35 min),obtained using reverse-phase liquid chromatography,showed strong anti-hPL activity,while the major constituents in these bioactive fractions were subsequently identified using liquid chromatography-quadrupole time-of-flight mass spectrometry(LC-Q-TOF-MS/MS).Among the identified ERC constituents,1,2,3,4,6-penta-O-galloyl-β-D-glucopyranose(PGG)and catechin gallate(CG)showed the most potent anti-hPL activity,with pIC50 values of 7.59±0.03 and 7.68±0.23,*** investigations revealed that PGG and CG potently inhibited hPL in a non-competitive manner,with inhibition constant(Ki)values of 0.012 and 0.082 mM,***,our integrative analyses enabled us to efficiently identify and characterize the key anti-obesity constituents in ERC,as well as to elucidate their anti-hPL *** findings provide convincing evidence in support of the anti-obesity and lipid-lowering properties of ERC.

Space scientific exploration is rapidly becoming the primary battlefield for humankind to explore the *** worldwide have launched numerous space exploration *** calibration of the detectors on the ground is a crucial element for space science satellites to obtain observational *** the purpose of providing calibration for various satellite-borne detectors,multiple monochromatic X-rays facilities have been built at the National Institute of Metrology,***(NIM).These facilities mainly pertain to grating diffraction and Bragg diffraction,and the energy range of the produced monochromatic X-rays is 0.218–301 ke *** facilities have a high performance in terms of energy stability,monochromaticity,and flux *** monochromaticity was greater than 3.0%.The energy stability of the facility is 0.02%at 25 ke V over 8 h,and the flux stability was within 1.0%at 25 ke V over 8 *** experiments on the properties of satellite-borne detectors,such as energy linearity,energy resolution,detection efficiency,and temperature response,can be conducted at the *** far,the calibration of two satellites has been completed by the authors,and the work on three other satellites is in *** study will contribute to the advancement of X-ray astronomy the development of Chinese space science.

大孔树脂纯化滇黄精黄酮工艺优化及纯化前后抗氧化活性比较

试验研究滇黄精黄酮的纯化工艺及其抗氧化活性。通过单因素试验结合响应面试验,利用超声辅助溶剂提取法对滇黄精黄酮进行提取工艺优化,通过静态和动态试验,考察树脂种类、粗提液浓度、洗脱剂、洗脱流速对滇黄精黄酮吸附解吸性能的影响,确定最佳纯化工艺条件。采用1, 1-二苯基-2-三硝基苯肼(DPPH)自由基和2, 2-联氮-二(3-乙基-苯并噻唑-6-磺酸)二铵盐(ABTS)自由基法,比较纯化前后滇黄精黄酮的抗氧化性。结果显示,滇黄精总黄酮的最佳提取工艺参数为乙醇浓度79%、提取温度61℃、提取时间2 h、液料比20 mL/g,提取率达到0.29%。AB-8大孔树脂纯化滇黄精黄酮效果最好,最佳条件为将40 g/L粗提液上柱,用70%乙醇以2.0 g/L速度洗脱,此条件下滇黄精黄酮纯度提高至5.31%,是粗提物浓度的11.8倍。滇黄精黄酮纯化后对DPPH自由基和ABTS自由基清除率为82.44%、86.22%,远高于纯化前。研究表明,AB-8大孔树脂适合分离纯化滇黄精黄酮,且滇黄精黄酮具有抗氧化活性。

目的比较飞秒激光辅助微小切口角膜基质透镜植入治疗中高度远视与微小切口角膜基质透镜取出术(small incision lenticule extraction,SMILE)治疗中高度近视术后的角膜上皮重塑情况。设计回顾性比较性病例系列。研究对象2021年2月~2022年9月济南明水眼科医院行飞秒激光辅助下微小切口角膜基质透镜植入术患者(透镜植入组)15例(20眼),行SMILE患者(SMILE组)61例(115眼)。方法分别于术前和术后3~12个月使用频域眼前节OCT(RTVue)的Pachymetry模式扫描角膜各象限的上皮厚度值。主要指标各象限角膜上皮厚度值。结果末次随访时透镜植入组在角膜直径5 mm内上皮变薄更明显,SMILE组在7 mm内上皮增厚更显著。在直径5mm内,组内角膜上皮的变化较术前均具有统计学差异(P<0.05),两组间上皮厚度改变值无统计学差异。结论透镜植入组的上皮厚度较术前变薄,总体呈凹透镜样改变;SMILE组的上皮厚度较术前增厚,总体呈凸透镜样改变。两组上皮重塑程度相似。
