We used reported behavioural data from cisgender men who have sex with men and transgender women(mSm/TGW)in Bangalore,mainly collected from‘hot-spot’locations that attract mSm/TGW,to illustrate a technique to deal with potential issues with the representativeness of this sample.A deterministic dynamic model of HIV transmission was developed,incorporating three subgroups of mSm/TGW,grouped according to their reported predominant sexual role(insertive,receptive or versatile).Using mathematical modelling and data triangulation for‘balancing’numbers of partners and role preferences,we compared three different approaches to determine if our technique could be useful for inferring characteristics of a more‘hidden’insertive mSm subpopulation,and explored their potential importance for the HIV *** for 2009 across all three approaches suggest that HIV prevalence among insertive mSm was likely to be less than half that recorded in the surveys(4.5e6.5%versus 13.1%),but that the relative size of this subgroup was over four times larger(61e69%of all mSm/TGW versus 15%).We infer that the insertive mSm accounted for 10e20%of all prevalent HIV infections among urban males aged *** modelling can be used with data on‘visible’mSm/TGW to provide insights into the characteristics of‘hidden’mSm.A greater understanding of the sexual behaviour of all mSm/TGW is important for effective HIV *** broadly,a hidden subgroup with a lower infectious disease prevalence than more visible subgroups,has the potential to contain more infections,if the hidden subgroup is considerably larger in size.

Larger breast impIants warranted for post-mastectomy reconstruction

Aim: Our goal was to ascertain if there was a role for larger breast implants in breast reconstruction. methods: Patients that underwent mastectomy and implant-based breast reconstruction were identified and reviewed. Results: Of the total specimens, 92 (14.7%) weighed more than 800 g with a mean weight of 1140 g (range 803 to 2177 g). Of the patients with these larger specimens, 45 (48.9%) selected the largest available implants (800 mL implants) for their reconstruction. Conclusion: There are patients undergoing mastectomy and implant-based breast reconstruction who are unable to have reconstruction to their native breast volume because of the current implant-volume restrictions.

LAPAROSCOPIC TREATmENT OF ACUTE CHOLECYSTITIS

Objctive: To assess the current treatment of AC in a single institution in the series, which the best treatment modality for acute cholecystitis (AC) is still under debate, whereas early cholecystectomy is accepted as the optimal timing for ***: From December 1996 to December 2001, 138 (102 women and 36 men) patients underwent laparoscopic cholecystectomy for acute cholecystitis confirmed by histopathological examination. The patients ranged in age from 21 to 85 years of age ( mean age: 45 3 years). Patients were divided into 2 groups (similar in age and ASA classification): group 1 (98 patients) underwent LC within 3 days after the onset of symptoms of acute cholecystitis and group 2 (40 patients) underwent LC after 3 days. Results: Approximately one half of the cases were uncomplicated, 26% were empyema, 13% had gangrenous changes and 7% had hydrops of the gallbladder. Conversion to open cholecystectomy was required in 21 (15 2%) cases. The principal reason for conversion was anatomic uncertainty (14 cases), uncontrolled bleeding (7 cases). The conversion rates in patients who underwent surgery before and after the onset of symptoms were respectively 6∶15. There was no significant difference in operative time (122 0 min in 1 group versus 124 0 min in 2 group) and postoperative stay ( 5 1 days in group 1 vs 6 8 days in group 2). The hepatorenal space was drained in 78 (56 5 %), and the drain is removed in 3rd postoperative days. Twenty six patients (18 8%) had undergone previous abdominal surgery. Thirty seven patients (26 8%) had spillage of bile and/or stones during the procedure. There were no deaths and major complications. Conclusions: LC for acute cholecystitis should be done immediately after the diagnosis is established because delaying surgery allows inflammation to become more intense, thus increasing the technical difficulty of LC. Background Large-river decision-makers are charged with maintaining diverse ecosystem services through unprec-edented social-ecological transformations as climate change and other global stressors *** interconnected,dendritic habitats of rivers,which often demarcate jurisdictional boundaries,generate complex management ***,we explore how the Resist–Accept–Direct(RAD)framework may enhance large-river management by promoting coordinated and deliberate responses to social-ecological trajectories of *** RAD frame-work identifies the full decision space of potential management approaches,wherein managers may resist change to maintain historical conditions,accept change toward different conditions,or direct change to a specified future with novel *** the Upper mississippi River System,managers are facing social-ecological transformations from more frequent and extreme high-water *** illustrate how RAD-informed basin-,reach-,and site-scale decisions could:(1)provide cross-spatial scale framing;(2)open the entire decision space of potential management approaches;and(3)enhance coordinated inter-jurisdictional management in response to the trajectory of the Upper mississippi River *** The RAD framework helps identify plausible long-term trajectories in different reaches(or subbasins)of the river and how the associated social-ecological transformations could be managed by altering site-scale *** reach-scale objectives may reprioritize how,where,and when site conditions could be altered to contribute to the basin goal,given the basin’s plausible trajectories of change(e.g.,by coordinating action across sites to alter habitat connectivity,diversity,and redundancy in the river mosaic).Conclusions When faced with long-term systemic transformations(e.g.,>50 years),the RAD framework helps explicitly consider whether or when the basin vision or goals may no longer be achievable,and direct options may open y

目的员工的工作投入、工作满意度、护理质量和离职意愿是医疗卫生机构绩效的关键指标。本研究旨在调查美国护士工作投入现状及关联因素,分析护士工作投入、工作满意度、感知护理质量和离职意愿之间的关系。方法采用横断面描述性研究设计。2022年3—9月,通过在线方式对美国注册护士进行调查。采用Utrecht工作投入量表(Utrecht Work Engagement Scale)测量护士工作投入状况,收集护士的一般资料,通过问题询问方式调查他们的工作满意度、感知护理质量和离职意愿。结果共有900名护士参与调查。其中,79.2%报告持有专业认证资格,59.4%在工作满意度方面得分较高或非常高,82.2%报告感知护理质量较高或非常高,但有28.4%的人表示在接下来的一年里可能或非常可能离职。护士的工作投入水平受到护士工作满意度、感知护理质量和离职意愿的影响。工作满意度高、感知护理质量高以及离职意愿更低的护士表现出更高水平的工作投入。线性回归分析显示,年龄较大、白种人并获得博士学位的护士与其同行相比,表现出更高水平的工作投入。结论本研究表明,护士的工作投入与其工作满意度、感知护理质量和离职意愿存在关联。护士的工作投入与工作满意度明显关联,护理管理者需要采取积极措施提高护士的工作满意度和留任率。

Pharmacological manipulation of cannabinoid neurotransmission reduces neuroinflammation associated with normal aging

We have previously demonstrated that antagonism of glutamate NmDA receptors or activation of endocannabinoid receptors could reduce experimentally induced neuroinflammation within the hippocampus of young rats. In the current study, we investigated whether pharmacological manipulation of glutamate or endocannabinoid neurotransmission could reduce naturally-occurring neuroinflammation within the hippocampus of aged rats. We investigated whether UCm707, an inhibitor of endocannabinoid re-uptake, WIN- 55,212-2, an endocannabinoid receptor agonist, and URB597, an inhibitor of endocannabinoid catabolism, or memantine, a non-competitive, low-affinity, inhibitor of the open NmDA receptor channel, could reduce the number of mHC II-IR microglia within the hippocampus. All of the drugs, except URB597, reduced the number of reactive microglia, as compared to vehicle treated rats. The current results suggest potential pharmacological approaches that may mitigate the pathological consequences of chronic brain inflammation associated with numerous neurodegenerative diseases.
