Diet is critical in diabetes management and new nutritional interventions are continuously being tested in randomized controlled trials. However, to make meaningful conclusions about the efficacy of dietary treatments, it is critical to be certain that the participants adhered to the dietary intervention and the dietary changes are valid. The objective of this meta-analysis was to assess the quality of dietary assessment and whether it might impact on study metabolic outcomes. Methods Four databases, MEDLINE, EMBASE, CINAHL and CENTRAL, were searched, from inception until September 2019 for randomized controlled trials of nutritional interventions in people with non-insulin-dependent type 2 diabetes. Trials that measured nutritional intakes in methods and HbA1 c as an outcome were included. Investigators assessed risk of bias and quality of the dietary measurements using the Cochrane Risk of Bias Assessment Tool 2.0 and a redeveloped EURICA tool, respectively. The study was conducted in accordance with the Preferred Reporting in Systematic Reviews and Meta-analyses. PROSPERO registration number: CRD42019146471. Results Of 2552 records retrieved, 23 studies met the inclusion criteria. Nineteen studies aimed to achieve a reduction in HbA1c, and four studies aimed to maintain HbA1c while improving other metabolic/nutritional outcomes. Two studies were rated as ’good’, six as ’medium’, and 15 as ‘poor’ in the quality assessment of the dietary measurement tool. The majority of studies were rated as high risk of bias. Of those studies with medium or high diet quality assessment, six of eight achieved the desired outcome whereas only four of the 15 other studies achieved the desired clinical outcome for HbA1 c. Conclusion The poor quality of dietary assessment in clinical trials manipulating dietary intakes casts uncertainty on the legitimacy of causal mechanisms attributed to dietary interventions. Objective:To correlate the utility of the Fundamentals of Laparoscopic Surgery(FLS)manual skills program with the Objective Structured Assessment of Technical Skills(OSATS)global rating scale in evaluating operative ***:The Asian Urological Surgery Training and Educational Group(AUSTEG)Laparoscopic Upper Tract Surgery Course implemented and validated the FLS program for its usage in laparoscopic surgical ***’basic laparoscopic skills were assessed using three different training models(peg transfer,precision cutting,and intra-corporeal suturing).They also performed live porcine laparoscopic surgery at the same *** surgery skills were assessed by blinded faculty using the OSATS rating ***:From March 2016 to March 2019,a total of 81 certified urologists participated in the course,with a median of 5 years of post-residency *** differences in task time did not reach statistical significance,those with more surgical experience were visibly faster at completing the peg transfer and intra-corporeal suturing FLS ***,they took longer to complete the precision cutting task than participants with less *** OSATS scores correlated weakly with all three FLS tasks(peg transfer time:r=0.331,r^(2)=0.110;precision cutting time:r=0.240,r^(2)=0.058;suturing with intracorporeal knot time:r=0.451,r^(2)=0.203).Conclusion:FLS task parameters did not correlate strongly with OSATS globing rating scale *** FLS task models demonstrated strong validity,it is important to assimilate the inconsistencies when benchmarking technical proficiency against real-life operative competence,as evaluated by FLS and OSATS,respectively.

Current therapy of hilar cholangiocarcinoma

BACKGROUND: Hilar cholangiocarcinoma (HC) is an adeno-carcinoma of the extrahepatic biliary tree arising from the main left or right hepatic ducts or their confluence. This tumor is still considered to be difficult to treat or to cure. DATA SOURCES: We reviewed the medical literature on HC. Relevant and updated information on this tumor was analyzed in a concise and easy-to-read manner. The article is not intended to be a systematic review, but an extensive search was conducted on PubMed and MEDLINE using the keywords 'hilar cholangiocarcinoma' and 'Klatskin tumor' until July 2011. RESULTS: The selection and the timing of management options for patients with HC are determined by the degree of certainty of the diagnosis, the general condition of the patients, the underlying liver function and the stage of the disease. Current treatment of HC can be divided into curative and palliative treatment. For the curative treatment, local excision should only be used on small tumors which are confined to the bile duct wall and Bismuth I papillary carcinoma. Partial hepatectomy should be combined with caudate lobe resection and porta-hepatis lymph node dissection. The results of these major resections can be improved with portal vein embolization, and staging laparoscopy and laparoscopic ultrasound. The role of preoperative biliary drainage is controversial. Autotransplantation for HC gave disappointing results while the Mayo Protocol of chemoradiation for selecting patients with unresectable HC for orthotopic liver transplantation has been widely accepted. Palliative treatment included bypass surgery, endoscopic or percutaneous stenting, photodynamic therapy, intraluminal brachytherapy, and external radiation and systemic therapy. CONCLUSIONS: Adequate surgery with R0 resection should be the main goal of treatment. For patients with unresectable HC, treatment aims to improve the quality and quantity of their survival.

目的觀察年齡對腰脊神經根射頻療效的影響。方法回顧性分析本院2020年1月~2022年12月期間,採用脊神經脈衝射頻治療的68例下肢神經根性疼痛患者的臨床資料,年齡介於42歲~92歲(中位數69.5歲),按年齡中位數分為年輕組與年老組,兩組各34例,分析兩組患者術前、術後1週、術後1個月、術後2個月、術後3個月的視覺模擬評分(VAS)及改良MacNab療效評定標準評價術後3個月療效優良率。結果兩組患者術後各時間點的VAS評分均較術前降低,差異有統計學意義(P0.05);術後1週療效相當,VAS評分差異無統計學意義(P>0.05);術後1個月、術後2個月、術後3個月年老組VAS評分低於年輕組,差異有統計學意義(P<0.05);術後3個月MacNab評定療效優良率年老組為85.2%,顯著高於年輕組的50%(P<0.05)。結論腰脊神經脈衝射頻可有效治療下肢神經根性疼痛,尤其是高齡患者療效確切,持續時間較長,可顯著減輕疼痛、改善生活質量、減少手術風險,具有臨床應用價值。

Hokkien Merchants and the Kian Teik Tong： Economic and Political Influence in Nineteenth- Century Penang and Its Region

This article explores nineteenth-century Penang＇s Hokkkien merchants and their secret society or hui-the Klan Teik Tong diande Tang）--which had a variety of roles and an extensive network. It contextualizes the merchants＇ secret society as a transnational socioeconomic and political organization rather than as an overseas Chinese criminal group in the wider Penang area. By recovering Klan Teik Tong and its network, it can be shown how these merchants secured and mobilized labour, capital, and allies in a way that cut across linguistic, ethnic, class and state boundaries in order to establish control of coolies and the lucrative opium, tin, and rice businesses, in order to exert political influence in the colonial and indigenous milieus of the nineteenth-century Penang region. They established a social contract through their Klan Teik Tong relief activities and initiation rituals, and thus were able to recruit thousands of members who were mainly labourers. With such a substantial social force, the merchants launched organized violence against their rivals to attain dominance in opium revenue farming and tin mining businesses in Penang, Krabi, and Perak. The widespread and strategic location of the Klan Teik Tong in Burma also enabled the same merchants to monopolize the Penang-Burma rice trade. The versatility of the Klan Teik Tong＇s functions allowed them to operate as an alternative political order vis-a-vis the colonial and indigenous powers. This arrangement allowed the Hokkien merchants to gain significant political clout in confronting the Siamese and Dutch authorities.
