Video-Assisted Thoracic Surgery for Residual Aneurysm after Total Arch Replacement

Background: Residual aneurysms after graft replacement are rare, but they can be detrimental if they are saccular and large. The etiology of residual aneurysms remains unknown, and their management is controversial. One treatment option is late open surgical conversion;however, postoperative respiratory complications resulting from the dissection of pleural adhesions, which is frequently necessary with this approach, are often unavoidable. Case presentation: Herein, we report a case of open surgical repair of a residual distal aortic arch aneurysm that occurred after total arch replacement and thoracic endovascular aortic repair. Contrast-enhanced magnetic resonance imaging was not possible in this case due to the patient’s severe renal dysfunction;however, contrast-enhanced computed tomography using minimal contrast did not detect remarkable leakage through the graft or stent graft into the aneurysm. Late open surgical conversion using video-assisted thoracic surgery was performed by thoracic surgeons, and the adhesion between the aortic wall and the lung was safely and effectively dissected. Because there was no significant pulsation or evidence of feeding arteries in the aortic wall, the aortic wall was opened carefully. No bleeding or backflow from any branch arteries into the aneurysm was noted, so the aortic wall was ligated with continuous sutures. The patient recovered without experiencing any major complications. Conclusions: This case report demonstrates that video-assisted thoracic surgery is safe and effective for late open conversion in cases of residual aneurysm;furthermore, this case suggests that video-assisted thoracic surgery may be particularly beneficial for the dissection of adhesions between the aortic wall and lung in these cases.

Synthesis of 11C-labeled capsaicin ([11C]-2) was accomplished by O-[11C]methylation with [11C]-methyl iodide to a corresponding catechol precursor (1). In Vivo whole-body distribution of [11C]-2 in rats was investigated by positron emission tomography after intravenous injection. The results showed that [11C]-2 accumulated in the brain and body surface, which was closely associated with the expression of transient receptor potential vanilloid 1.

Serial changes in expression of functionally clustered genes in progression of liver fibrosis in hepatitis C patients

AIM: To investigate the relationship of changes in expression of marker genes in functional categories or molecular networks comprising one functional category or multiple categories in progression of hepatic fibrosis in hepatitis C (HCV) patients. METHODS: Marker genes were initially identified using DNA microarray data from a rat liver fibrosis model. The expression level of each fibrosis associated marker gene was analyzed using reverse transcription-polymerase chain reaction (RT-PCR) in clinical biopsy specimens from HCV-positive patients (n = 61). Analysis of changes in expression patterns and interactions of marker genes in functional categories was used to assess the biological mechanism of fibrosis. RESULTS: The profile data showed several biological changes associated with progression of hepatic fibrosis. Clustered genes in functional categories showed sequential changes in expression. Several sets of clustered genes, including those related to the extracellular matrix (ECM), inflammation, lipid metabolism, steroid metabolism, and some transcription factors important for hepatic biology showed expression changes in the immediate early phase (F1/F2) of fibrosis. Genes associated with aromatic amino acid (AA) metabolism, sulfur-containing AA metabolism and insulin/ Wnt signaling showed expression changes in the middle phase (F2/F3), and some genes related to glucose metabolism showed altered expression in the late phase of fibrosis (F3/F4). Therefore, molecular networks showing serial changes in gene expression are present in liver fibrosis progression in hepatitis C patients. CONCLUSION: Analysis of gene expression profiles from a perspective of functional categories or molecular networks provides an understanding of disease and suggests new diagnostic methods. Selected marker genes have potential utility for biological identification of advanced fibrosis.

Quantitative detection of metastasis-associated 1 mRNA and protein expression in breast cancer

Preparation and Characterization of Maltose-Pendant Polymer/Mica Nanocomposites and Their Application to Oxygen Gas Barrier Films

Maltose-pendant polymer/mica nanocomposites were prepared by a solution intercalation method. For organic composite part, 1) maltose-pendant polymer (homopolymer) and 2) the copolymer of maltose-pendant monomer and a small amount of N,N-Dimethylamino propylacrylamide, methyl chloride quartenary were used. The morphological studies (XRD and FE-SEM) revealed that the hybrid of maltose-pendant polymer was a conventional phase separated composite. On the other hand, the hybrid using the copolymer exhibited exfoliated structure. Both the conventional composite of maltose-pendant polymer and the nanocomposite of copolymer were applied to a coating material for oxygen gas barrier layer on a nylon-6 film, and oxygen transmission rates of the films were evaluated. Maltose-pendant polymer had a good oxygen barrier property under dry condition, and the barrier property under wet condition was improved by the hybridization with mica. In contrast, the barrier property of copolymer was slightly inferior to that of maltosependant polymer. However, under dry condition, it can be seen that the nanocomposite of copolymer improves the barrier property more effectively than the case of conventional composite of maltose-pendant polymer.

Thermal stability of lysozyme adsorbed on biomass charcoal powder (BCP), which was prepared from plant biomass wastes such as dumped adzuki bean, bamboo, and wood by pyrolysis without combustion under nitrogen atmosphere and comminution with a jet mill, was examined. Adsorbing lysozyme on BCP could sufficiently prevent proteins from denaturing and aggregating in an aqueous solution at high temperatures, and enhanced the refolding of thermally denatured proteins by cooling treatment. The remaining activities of lysozyme adsorbed on BCP of adzuki bean exhibited 51% by cooling treatment after the heat treatment at 90?C for 30 min, although that of native lysozyme was almost lost under the same experimental conditions. The thermostabilization effect of BCP on the remaining activity of adsorbed lysozyme was markedly dependent upon the kind of plant biomass wastes.

Parental perceptions and childhood overweight/obesity: A population-based study among school children in Japan

The lifestyle of children is primarily controlled by their parents, and that childhood obesity is related to family variables. The aim of this study was to investigate the relationship between parental perceptions on childhood obesity and their child’s overweight/obesity in Japan. The study subjects were 3168 school children (aged 9 or 10 years) and their parents in the town of Ina, Saitama Prefecture, Japan, between 2002 and 2009. Information regarding parental perceptions about childhood obesity and the lifestyle factors of their children was collected using self-administered questionnaires completed by the participants and their parents. Childhood over-weight was defined according to the International Obesity Task Force cutoffs. Parental perceptions on childhood obesity were significantly associated with their child’s overweight status. Specifically, low perceptions regarding childhood obesity significantly increased the odds ratio (OR) for the child’s overweight status (OR: 1.86, 95% CI: 1.32 - 2.62). However, when the analysis was limited to children with both parents being overweight, there was no statistically significant difference between levels of parental perceptions and the overweight status of their children. Accordingly, the present study suggests that, when both parents are obese, not only is raising parental awareness important, but also encouraging parents to prevent themselves from becoming overweight, leads to improvements in the lifestyle habits of children, which may contribute to the prevention of childhood overweight.

Unexpectedly Good Neurological Outcome after Prolonged Cardiac Arrest

Backgoround: Generally, neurological outcome of patients who have achieved return of spontaneous circulations (ROSC) with abnormal blood tests date and some severe organ complications is extremely poor. Because we experienced a case of good neurological outcome using two different types of extracorporeal membrane oxygenation (ECMO) in spite of prolonged cardiac arrest, we reported this case and reviewed the literature. Case presentation: A 36-year-old male suddenly collapsed at bar after drinking. Initial ECG showed ventricular fibrillation (VF) and subsequently underwent advanced cardiovascular life support. On admission, the ECG still showed VF, but pupillary light reflex was faintly present. Then, veno-arterial extracorporeal membrane oxygenation (VA-ECMO) support was established 54 minutes after collapse. Blood tests showed lactate was 15.8 mmol/l and NH3 of 251 μg/dl. To improve respiratory function after cardiogenic shock, we changed to veno-venous ECMO (VV-ECMO) and removed superabundant fluid using continuous hemodiafiltration. Patient was discharged on day-26 after a full functional recovery. Conclusion: In severe respiratory disorder, it would be more effective to change to VV-ECMO after recovering from cardiogenic shock while removing superabundant fluid using continuous hemodiafiltration. In predicting the neurological outcome of a post cardiac arrest patient, the presence of a pupillary light reflex may be more significant than NH3 and lactate.
