Usefulness of the clip-flap method of endoscopic submucosal dissection: A randomized controlled trial

AIM To prospectively investigate the efficacy and safety of clipflap assisted endoscopic submucosal dissection(ESD) for gastric *** From May 2015 to October 2016, we enrolled 104 patients with gastric cancer or adenoma scheduled for ESD at Shiga University of Medical Science Hospital. We randomized patients into two subgroups using the minimization method based on location of the tumor(upper, middle or lower third of the stomach), tumor size( 20 mm) and ulcer status: ESD using an endoclip(the clip-flap group) and ESD without an endoclip(the conventional group). Therapeutic efficacy(procedure time) and safety(complication: Gastrointestinal bleeding and perforation) were assessed. RESULTS En bloc resection was performed in all patients. Four patients had delayed bleeding(3.8%) and two had perforation(1.9%). No significant differences in en bloc resection rate(conventional group: 100%, clip flap group: 100%), curative endoscopic resection rate(conventional group: 90.9%, clip flap group: 89.8%, P = 0.85), procedure time(conventional group: 70.8 ± 46.2 min, clip flap group: 74.7 ± 53.3 min, P = 0.69), area of resected specimen(conventional group: 884.6 ± 792.1 mm^2, clip flap group: 1006.4 ± 1004.8 mm^2, P = 0.49), delayed bleeding rate(conventional group: 5.5%, clip flap group: 2.0%, P = 0.49), or perforation rate(conventional group: 1.8%, clip flap group: 2.0%, P = 0.93) were found between the two groups. Lessexperienced endoscopists did not show any differences in procedure time between the two *** For patients with early-stage gastric tumors, the clipflap method has no advantage in efficacy or safety compared with the conventional method.

Optical Sum Frequency Generation Spectroscopy of Cracked Non-Glutinous Rice (Oryza sativa L.) Kernels

In order to study the correlation between the cracking of rice (Oryza sativa L.) kernels and the molecular structure of the amylopectin in them, we attempted optical sum frequency generation (SFG) spectroscopy in the C-H stretching vibration region for normal and cracked japonica non-glutinous rice kernels. The samples were Koshihikari and Nipponbare. In Nipponbare, the width of the SFG spectrum peak at 2915 cm- 1 of the cracked rice kernels was broader than that of the normal ones, while for Koshihikari there was no clear difference. The width of the 2915 cm- 1 peak is suggested to originate from the variety of the higher-order structure of the saccharide chains in amylopectin. Although this is a tentative result, this method is shown to have a potential of serving for preventing the cracking of the rice kernels.

Local advanced rectal cancer perforation in the midst of preoperative chemoradiotherapy:A case report and literature review

Standard chemoradiotherapy(CRT) for local advanced rectal cancer(LARC) rarely induce rectal perforation. Here we report a rare case of rectal perforation in a patient with LARC in the midst of preoperative CRT. A 56-year-old male was conveyed to our hospital exhibiting general malaise. Colonoscopy and imaging tests resulted in a clinical diagnosis of LARC with direct invasion to adjacent organs and regional lymphadenopathy. Preoperative 5-fluorouracil-based CRT was started. At 25 d after the start of CRT, the patient developed a typical fever. Computed tomography revealed rectal perforation, and he underwent emergency sigmoid colostomy. At 12 d after the surgery, the remaining CRT was completed according to the original plan. The histopathological findings after radical operation revealed a wide field of tumor necrosis and fibrosis without lymph node metastasis. We share this case as important evidence for the treatment of LARC perforation in the midst of preoperative CRT.

Potential of proteins and their expression level in marine phytoplankton (<i>Prymnesium parvum</i>) as biomarker of N, P and Fe conditions in aquatic systems

Nitrogen (N), phosphorus (P) and Iron (Fe) are important nutrients for phytoplankton, and they are the key limiting nutrients in many marine systems. In the present study, growth and protein expression of marine phytoplankton Prymnesium parvum under different nitrate, phosphate and iron conditions were investigated in order to evaluate whether proteins and their expression level could be used as biomarkers of N, P, and Fe conditions in aquatic systems. The growth of P. parvum increased with the increase of nitrate, phosphate and iron concentrations in the culture medium. Protein expression levels also differed significantly (p < 0.001) for different nitrate, phosphate and iron conditions in the culture medium. The expression level of an 83 kDa protein at 0 and 5 μM nitrate treatments differed significantly (p < 0.001) from those at 20, 30, 50 and 100 μM nitrate treatments, indicating the expression levels of this protein as a biomarker of N status in the culture medium. A 121 kDa protein was up-regulated at phosphate stress conditions ([P] ≤ 1.0 μM), while this protein was not expressed at phosphate replete conditions ([P] ≥ 5 μM). Therefore, the expression of 121 kDa protein in P. parvum is indicative of phosphate deplete condition in aquatic systems. The expression level of a 42 kDa was significantly higher (p < 0.01) at Fe-stress condition ([Fe] = 0.01 μM) than Fe-replete conditions ([Fe] ≥ 0.1 μM). In addition, a new protein of 103 kDa was only expressed under Fe-deplete condition ([Fe] = 0.01 μM). Therefore, the 42 and 103 kDa proteins can be used as a biomarker of Fe-limitation condition of aquatic systems. However, further studies (two dimensional gel electrophoresis and mass spectrometry) are needed to identify and characterize these proteins in P. parvum.

Hybrid Improved Self-adaptive Differential Evolution and Nelder-Mead Simplex Method for Solving Constrained Real-Parameters

In this paper, a new hybrid algorithm based on exploration power of a new improvement self-adaptive strategy for controlling parameters in DE (differential evolution) algorithm and exploitation capability of Nelder-Mead simplex method is presented (HISADE-NMS). The DE has been used in many practical cases and has demonstrated good convergence properties. It has only a few control parameters as number of particles (NP), scaling factor (F) and crossover control (CR), which are kept fixed throughout the entire evolutionary process. However, these control parameters are very sensitive to the setting of the control parameters based on their experiments. The value of control parameters depends on the characteristics of each objective function, therefore, we have to tune their value in each problem that mean it will take too long time to perform. In the new manner, we present a new version of the DE algorithm for obtaining self-adaptive control parameter settings. Some modifications are imposed on DE to improve its capability and efficiency while being hybridized with Nelder-Mead simplex method. To valid the robustness of new hybrid algorithm, we apply it to solve some examples of structural optimization constraints.
