Integrating palliative care in oncologic emergency departments:Challenges and opportunities

Extravascular findings during upper limb computed tomographic angiography focusing on undiagnosed malignancy

BACKGROUND Computer tomography angiography(CTA) has been an established method for diagnostic vascular disease of lower limbs. Recently, the method is widely used for diagnosis of vascular pathologies in the upper limbs too. It also has increased the possibilities of this scans being reviewed by no specially trained *** increases the risk of incidental non vascular findings to be missed or misinterpreted. The study is focusing in the frequency of extravascular incidental finding(EVIF) and highlights the importance for both the reporting radiologist and the referring physician recognizing the frequency of *** To analyse the frequency of EVIF identified on computed angiography(CT) of the upper *** A total of 1383 CT angiographic studies of the peripheral arterial system were performed between August 2015 and August 2017. all upper limb CTAs(n = 79)were retrospectively reviewed for the presence of non-vascular incidental findings within the chest, abdomen/pelvis, musculoskeletal system or head and neck. These EVIFs were subsequently grouped into 3 categories based on clinical significance. EVIFs of immediate clinical relevance were included in category A,findings considered indeterminate but most likely benign were placed in category B, while incidental findings of no clinical significance were included in category *** Complete imaging datasets were available in 74/79(93.7%). Patientdemographics included 39(52.7%) females and 35(47.2%) males with a mean age of 59 ± 19.5 years(range 19-93 years). A total of 153 EVIFs were reported in 52 patients(70.3%). Of these, 44 EVIFs(28.7%) were found in the chest, 83(54.2%) in the abdomen, 14(9.2%) in the musculoskeletal system and 9(5.8%) in the head and neck. Thirteen EVIFs(8.4%) identified in 11 patients were noted to be of immediate clinical significance(Category A), 50 EVIFs(32.3%) were identified in20 patients and were considered indeterminate but most likely benign, while the remaining 91

Introduction: Type 2 Diabetes Mellitus (T2DM) has witnessed a rise in its prevalence worldwide and in the Middle East region. The overall burden associated with the disease is well characterized, but little is known about patient satisfaction in the region. The purpose of the study is to evaluate the quality of life (QoL) and treatment satisfaction of patients T2DM. Methods: The SIMPLIFY study was an observational, cross-sectional, multicenter, regional study that used patient-reported outcomes of T2DM patients in Jordan and Lebanon. Results: Patients were more satisfied when they were treated exclusively with oral medications, mainly metformin alone or combined with either sulfonylurea or dipeptidyl peptidase-4 inhibitor. Targets for glycated hemoglobin (HbA1c) values were better reached in patients treated with oral medications. Occurrence of comorbidities did not seem to be affected by oral or injectable medications or to affect patients’ satisfaction. Data highlighted a suboptimal screening for albuminuria and showed that most patients were overweight or obese and around 30% suffered from hypoglycemia episodes. Conclusion: Data shed the light on the management of T2DM in Jordan and Lebanon and suggested the need for a more comprehensive approach to T2DM management and selection of medications that would support weight control and a lower hypoglycemia incidence.

Synthesis of silver nanoparticles using seeds of Nigella sativa as a capping agent was evaluated in this study. Different concentrations of the aqueous extract of N. sativa with silver nitrate solution were exposed to sunlight;as a force for acceleration of the formulation. Then the silver nanoparticles were characterized by UV-Vis, scanning electron microscope (SEM) and X-ray diffraction (XRD) techniques. Antibacterial activity of the nanoparticles was investigated against Staphylococcus aureus and Escherichia coli by the disc diffusion method. The characterization of nanoparticles was detected by the change in color to yellow-brown which indicated the formulation of silver nanoparticles. Irregular shapes within range of nanoscale were detected using SEM and XRD techniques. The finding suggests that silver nanoparticles may be effectively used as antibacterial agent.

Time Management of Open Lower-Leg Fractures in Morocco

Background: It is recommended that the management of open lower-leg fractures should occur within six hours to avoid complications affecting both prognosis and functionality. This study aimed to evaluate the time taken for each step of management, identify causes of delay, and propose solutions to reduce the rate of complications. Patients and methods: This was a prospective study involving 153 patients treated from January 2011 to December 2012. Standardized data collection forms documented the times taken for each management step from the accident until surgical intervention. Results: For patients admitted directly to our institution, median time to surgical debridement from arrival was 5 hours. Median time for bone fixation was 24 hours. External fixation was used in 119 patients. There were several causes of delay;lack of financial resources was the most common. Certain causes of delay were significantly correlated with time to debridement. However, although 86.7% of patients who developed an infection had a time from injury to debridement greater than six hours, there was no statistically significant association between infection and delay beyond six hours (p = 0.403). Conclusion: By identifying causes of treatment delay, we hope to be able to avoid complications, and therefore reduce the socio- economic impact of these important surgical emergencies.

Synovial sarcoma originating from the kidney is extremely rare. A 13-year-old girl presented with a mild left flank pain of one-week duration, with no associated history of hematuria or any other systemic symptoms. Computed tomography (CT) demonstrated a 6 × 13 × 9 cm mass in the left kidney. No soft tissue or extrarenal masses were identified. The patient received a combined of treatment with doxorubicin and ifosfamide. A radical nephrectomy was performed in the left kidney with no complications. Postoperative pathology revealed post-chemotherapy residue of monophasic spindle cell synovialosarcoma of the left kidney. She received a combined treatment with doxorubicin and ifosfamide in concomittance with external radiation therapy. The patient was re-examined 4 months after surgery. An abdominal and pulmonary CT found no recurrence or metastasis.

Our aim in this project is twofold. Firstly, we aspire to capture as many of the activities that are already present on the site as possible and to build upon them. Secondly, we attempt to bridge the existing gap between the low income east and the higher income west sides of the tracks by designing a campus of activities and spaces which invite the two to mix. The central notion that guides our design is to preserve whether physically or experientially what is there, and then carefully intervene to achieve the desired dynamic quality of space. The set of interventions is drawn into the urban texture by attraction points that anchor the site to its context. Rather than being a negative space of what has been built, these created attraction nodes will connect the structure to the urban fabric. The focus of our intervention is the new library, situated halfway between Wudaokou and Zhichunlu stations, which is to serve both students from the surrounding universities and residents of the site as well as the broader surrounding area.
