Caffeine is the most commonly used medication for treatment of apnea of prematurity. Its effect has been well established in reducing the frequency of apnea, intermittent hypoxemia, and extubation failure in mechanically ventilated preterm infants. Evidence for additional short-term benefits on reducing the incidence of bronchopulmonary dysplasia and patent ductus arteriosus has also been suggested. Controversies existamong various neonatal intensive care units in terms of drug efficacy compared to other methylxanthines, dosage regimen, time of initiation, duration of therapy, drug safety and value of therapeutic drug monitoring. In the current review, we will summarize the available evidence for the best practice in using caffeine therapy in preterm infants.

AIM To investigate the effect of different neonatal risk factors on different language parameters as well as cognitive abilities among Arabic speaking Egyptian children at the age of two to three years of life and to find out which risk factor(s) had the greatest impact on language and cognitive *** This retrospective cohort study was conducted on 103 children with age range of 2-3 years(median age 31 mo).They were 62 males and 41 females who were exposed to different high-risk factors in the perinatal period,with exclusion of metabolic disorders,sepsis/meningitis,congenital anomalies and chromosomal *** studied children were subjected to a protocol of language assessment that included history taking,clinical and neurological examination,audiological evaluation,assessment of language using modified preschool language scale-4,IQ and mental age assessment and assessment of social *** The studied children had a median gestational age of 37 wk,median birth weight of 2.5 *** distribution of the high-risk factors in the affected children were prematurity in 25 children,respiratory distress syndromein 25 children,hypoxic-ischemic encephalopathy in 15 children,hyperbilirubinemia in 10 children,hypoglycemia in 13 children,mixed risk factors in 15 *** results revealed that high-risk neonatal complications were associated with impairment of different language parameters and cognitive abilities(P < 0.05).The presence of prematurity,in relation to other risk factors,increases the risk of language and cognitive delay significantly by 3.9 *** Arabic-speaking children aged 2-3 years who were exposed to high-risk conditions in the perinatal period are likely to exhibit delays in the development of language and impairments in cognitive *** most significant risk factor associated with language and cognitive impairments was prematurity.

Sacral dimples are the most common cutaneous anomaly detected during neonatal spinal examination. Congenital dermal sinus tract, a rare type of spinal dysraphism, occurs along the midline neuraxis from occiput down to the sacral region. It is often diagnosed in the presence of a sacral dimple together with skin signs, local infection, meningitis, abscess, or abnormal neurological examination. We report a case of acute flaccid paralysis with sensory level in a 4 mo old female infant with sacral dimple, diagnosed by magnetic resonance imaging to be a paraspinal subdural abscess. Surgical exploration revealed a congenital dermal sinus tract extending from the subdural abscess down to the sacral dimple and open to the exterior with a minute opening.

Combining Committee-Based Semi-Supervised Learning and Active Learning

Many data mining applications have a large amount of data but labeling data is usually difficult, expensive, or time consuming, as it requires human experts for annotation. Semi-supervised learning addresses this problem by using unlabeled data together with labeled data in the training process. Co-Training is a popular semi-supervised learning algorithm that has the assumptions that each example is represented by multiple sets of features （views） and these views are sufficient for learning and independent given the class. However, these assumptions axe strong and are not satisfied in many real-world domains. In this paper, a single-view variant of Co-Training, called Co-Training by Committee （CoBC） is proposed, in which an ensemble of diverse classifiers is used instead of redundant and independent views. We introduce a new labeling confidence measure for unlabeled examples based on estimating the local accuracy of the committee members on its neighborhood. Then we introduce two new learning algorithms, QBC-then-CoBC and QBC-with-CoBC, which combine the merits of committee-based semi-supervised learning and active learning. The random subspace method is applied on both C4.5 decision trees and 1-nearest neighbor classifiers to construct the diverse ensembles used for semi-supervised learning and active learning. Experiments show that these two combinations can outperform other non committee-based ones.

Objective:To highlight some clinical and epidemiological features of dengue ***: All patients who were admitted to hospitals in Holly Mecca City,Saudi Arabia and were confirmed as dengue fever(DF) or dengue hemorrhagic fever(DHF) were included in this study. The data were collected from patient files and through direct interview with patients or their *** were followed through their hospital *** laboratory investigations were done and diagnosis was confirmed by ***:Most of cases admitted in stable condition(94.37%) and only one case(1.41%) ***-1 and 3 types were the prevalent dengue viruses and cases in age group 16-44 were the most frequent(70.40%).The most common symptoms was fever reported from all cases followed by headache(74.60%),myalgia and anorexia(67.60%),back pain(59.20%) and chills(54.90%).DF represented(60.57%) of the cases while DHF represented(39.43%).About half of cases had underground water tanks for human use,5.60%had over house roof water tanks and 43.70%had both types,16.90% of these tanks were *** 70.00%of cases reported presence of small collection of water nearby houses and 46.80%reported the presence of mosquitoes within their ***:Most dengue fever cases might be endogenous in origin due to prevalence of mosquitoes and their breeding places within the houses and in nearby *** of mosquitoes and their breeding places will contribute to prevention of dengue fever.

Objective:To determine the current seroprevalence of antibodies against hepatitis A virus in selected group of children aged 1-6 years in Northern borders region,Saudi Arabia,and to identify risk factors for ***:A cross-sectional sero-epidemiological study of 950 children who attended 10 randomly selected primary health care centers(5 urban and 5 rural) was *** of all children were subjected to a questionnaire including sociodemographic and housing environmental *** determination of anti-HAV antibodies was carried out by ***:The prevalence of HAV-IgG was 33.8%overall,35.5%among males and 32.0%among females with no statistically significant *** logistic regression analysis revealed that increasing age,rural residence,non Saudi nationality,and non availability of safe municipal water source were the most important independent predictors for HAV seropositivity in the studied ***:There is a clear decrease in hepatitis A prevalence in the studied children particularly in urban areas and indicates that a transition may be underway to intermediate endemicity and possible shift of the risk to the adult age with increased ***,we recommend including Hepatitis A in the schedule of routine childhood vaccinations.

Electrochemical Deposition and Optimization of Thermoelectric Nanostructured Bismuth Telluride Thick Films

Bismuth telluride thick films are suitable for thermoelectric (TE) devices covering large areas and operating at small-to-moderate temperature differences (20 - 200 K). High efficiency and high coefficient of performance (COP) are expected to be achieved by using thick films in some cooling applications. Bismuth telluride thick films fabrication have been achieved with Galvanostatic and Potentionstatic deposition. Stoichiometric bismuth telluride thick film was obtained by Galvanostatic deposition at current density of 3.1 mAcm-2. Bismuth telluride films with average growth rate of 10 μmh-1 and different composition were obtained. Effects of current density and composition of electrolyte in Galvanostatic deposition were studied. The current density affected the film compactness, where films deposited at lower current density were more compact than those deposited at higher current density. The morphology of the films did not depend on the current density, but chemical composition was observed when different composition of electrolyte was used. Effects of distance between electrodes, composition of electrolyte solution, and stirring in Potentionstatic deposition were studied. The shorter the distance between electrodes, the higher the electric field, thus the higher current density was applied and the deposited film was less compact. The current density increased more rapidly with stirring during electrodeposition which leads to less compact film. Through this study, films electrode-posited from solution containing 0.013 M Bi(NO3)3.5 H2O, 0.01 M TeO2 and 1 M HNO3 at 3.1 mA cm-2 for 6 hours without stirring and with interelectrode distance of 4.5 cm were free-standing with average film thickness of 60 μm and optimum film composition of Bi2Te3. The crystallite size of the later films was found to be around 4.3 nm using Scherrer’s equation from XRD patterns. Also, negative Seebeck coefficient for the same samples was revealed with an average value of -82 μV.K-1.

The effect of rare earth oxides （RE=Ce, La, Gd, and Dy） doping of alumina support in NiO/7-A1203 system was investi- gated on its catalytic performance in oxidative dehydrogenation （ODH） of cyclohexane. The physicochemical properties of various samples were followed up through N2 physisorption, temperature programmed reduction （H2-TPR）, X-ray diffraction （XRD）, X-ray photoelectron spectroscopy （XPS） and potentiometric acid-base titration techniques. In the parent NiO/y-A1203 catalyst, Ni species were found to be strongly interacted with alumina surface. Addition of rare earth dopants to ）＇-A1203 in the catalyst system affected the nickel-alumina interaction and resulted in significant modifications in the catalytic performances in the ODH reaction. The results re- vealed the beneficial role of both La203 and Gd2Os doping in enhancing the ODH catalytic activity and selectivity to cyclohexene. H2-TPR and XPS results indicated that majority of Ni species in NiO/La203 modified T-A1203 were more weakly interacted with La203 and alumina whereas both NiO like species and nickel aluminate were present on the surface. Doping with cerium or dyspro- sium increased the nickel-support interaction and led to a decrease in surface nickel concentration. In case of doping with Ce, surface concentration of cerium oxide was higher than those of the other RE oxides; the doped catalyst reached its steady state activity faster than the other catalysts. The acid-base results suggested that RE metals were interacted most likely with acidic surface hydroxyl groups. The degree of nickel-alumina interaction decreased in the following order： LaAI〉GdAI〉CeAI〉DyA1.
