Impact of Post Hepatitis C Chronic Liver Disease and Hepatocellular Carcinoma on Health Related Quality of Life

Background: Post hepatitis C virus chronic liver disease (CLD) is prevalent among the Egyptian population with a bad impact upon their quality of life (QOL). Hepatocellular carcinoma (HCC) is one of the long term and fatal complications of CLD and it also has its negative impact on patient’s quality of life. Aim: To assess impact of CLD and HCC on the quality of life of group of hospitalized elderly patients. methodology: Ninety elderly patients were divided into three groups: 30 elderly with post hepatitis C virus CLD, 30 elderly with HCC and 30 others free of liver disease as control group (Cn), all were recruited from the in-patient ward and the outpatient clinic of the Geriatric Department, Ain-Shams University Hospital. After giving consent, comprehensive geriatric assessment was done with assessment of their quality of life by using the Short Form-36 health survey (SF-36). Investigations including liver enzymes, serum albumin, serum bilirubin and abdominal ultrasound were done. Results: All QOL domains were the highest among control group, followed by HCC group and the least among CLD group. The differences were statistically significant in most subscales and total score [mean of Cn = 81.9 ± 12.4, mean of CLD = 47.5 ± 21.9, mean of HCC = 62.3 ± 16.1;P Cn/CLD ≤ 0.001, P Cn/HCC ≤ 0.001, P CLD/HCC = 0.004]. Albumin was the only biochemical marker correlated positively with total SF score and two subscales (PF and EF) [r = 0.408;P = 0.025]. Conclusion & Recommendation: Our study showed a decrease in the QOL of Egyptian post hepatitis C virus CLD and HCC patients compared with Egyptian population norms. The results showed that CLD were more affected than HCC patients. This had a particularly serious negative impact on their life. The findings indicate a need for updated counseling and educational materials designed to provide adequate information and consistent healthcare service to this patient setting.

This paper represents the interaction of well characterized Lewis base [（Pyr）nCuX]4O2, n = 1 or 2, X = Cl, Br or I, Pyr = pyrrolidine with CO2 as a Lewis acid to produce new series of oxidative coupling and catechol oxidase initiators [（Pyr）nCuX]4（CO3）2. These carbonato derivatives are isolated as stable solids. They are easily soluble in aprotic solvents as CH2Cl2 or PhNO2. Cryoscopic measurements support tetranuclear core structure for all of them. Infrared spectra show differences from their oxo analogous in the carbonato domains but those differences did not distinguish between tridentate bridging carbonato and bidentate one. Rate of oxidation of 2,6-dimethylphenol （DmP） by [（Pyr）CuCl]4（CO3）2, supports coordination number six for Cu（Ⅱ） centers in [（Pyr）CuCl]4（CO3）2. In order to fulfill coordination number six, for n = 1, carbonate will act as tridentate while for n = 2, it will act as bidentate, as shown in Scheme 4. Near infrared spectra indicate a [（3 halo） Cu（Ⅱ） charge transfer] for [（Pyr）nCuX]4（CO3）2, n = 1 or 2, X = Cl or Br. Low molecular absorptivities of the maxima at 825 nm and 730 nm for [（Pyr）nCuI]4（CO3）2, n = 1 or 2 with a minimum of high molecular absorptivities at 600 nm, comparing to X= CI or Br analogous, support a step structure for [（Pyr）,Cul]4（CO3）2, as shown in Scheme 5. Cyclic voltammograms for [（Pyr）nCuX]4（CO3）2; n = 1 or 2, X = CI or Br, are irreversible in characters.

A reliable and rapid pharmacokinetic study of pueraria isoflavones using pueraria reference extractive substance in beagle plasma:Application to study of Yufeng Ningxin Tablets

Relationship between bone mineral density, coronary risk factors, C-reactive protein, and Insulin growth factor in frail elderly

Objective: This study aimed to explore the relation between bone mineral density (BmD), inflammation, coronary risk factors, and Insulin growth factor (IGF) in frail elderly. Design: A casecontrol study. Setting: Ain Shams University Hospital Cairo, Egypt. Participants: Ninety aged 60 years and above;sixty frail (30 males and 30 females;mean age 70.03 ± 8.10) and thirty matched controls (15 males and 15 females;mean age 69.07 ± 5.93). measurements: History, examination, comprehensive geriatric assessment, measurement of BmD, and laboratory investigations including;high sensitivity C-reactive protein (hs-CRP) glycated haemoglobin, IGF, lowdensity lipoprotein (LDL), high-density lipoprotein, total cholesterol, and triglycerides. Results: The frail group had significantly lower levels of triglycerides and LDL, hs-CRP and glycated haemoglobin levels were significantly higher (P mD and IGF levels were significantly lower when compared with the control group (P mD had a significant positive linear correlation with IGF and negative correlation with age. Conclusion: Frailty was associated with low BmD, lower triglycerides & LDL levels, and higher hsCRP levels. A borderline significance was found between IGF levels and frailty. A positive correlation was found between BmD and IGF in frail.

This study investigates if the anti-tumor effect of Pterostilbene in the SKOV3 ovarian cancer(OC)cell line involves inhibition of cell metabolism and tested in this effect involves modulating AmPK and Akt-induced regulation of ***,SKOV3 cells were cultured in the humidified conditions in DmEm media for 24 h with or without increasing concentration of ***,the cells were incubated with Pterostilbene(IC_(50)=50μm)under similar conditions with or without pre-incubation with Dorsomorphin,an AmPK *** a dose-dependent manner,Pterostilbene inhibited SKOV3 cell survival and increased their lysate levels of lactate dehydrogenase(LDH)and single-stranded DNA(ssDNA).When SKOV3 cells were treated with 50μm Pterostilbene,Pterostilbene significantly suppressed cell migration and invasion,reduced lysate levels of lactic acid and the optical density of Oil Red O staining,and increased lysate glucose *** also increased levels of malondialdehyde(mDA),reactive oxygen species(ROS),and induced intrinsic cell apoptosis by upregulating protein levels of Bax and cleaved caspase-3 and reducing protein levels of ***,Pterostilbene reduced mRNA levels of sterol regulatory element-binding protein 1(SREBP-1),fatty acid synthase(FAS),acetyl CoA carboxylase-1(ACC-1),and AmP-activated protein kinase(AmPK).Furthermore,Pterostilbene increased the protein levels of p-AmPK,p-p53,p-raptor,p-TSC-2,but significantly decreased protein levels of p-Akt,p-TSC-2,p-mTOR,p-S6K1,and *** with Dorsomorphin(CC)abolished all the anti-tumorigenesis effects afforded by Pterostilbene and prevented Pterostilbene-induced phosphorylation of Akt,p53,and *** conclusion,the tumorsuppressive effect of Pterostilbene in SKOV3 cells involves the induction of ROS and inhibition of dysregulation cell metabolism mainly due to AmPK-induced Akt-dependent or independent suppression of mTOR.

Spectral Properties of [(Ni0.45Co0.2Zn0.35F2O4)1–x(Sodium Acetylacetonate)x] Nanocomposite

The pure spinel phase of Ni0.45Co0.2Zn0.35F2O4 was prepared by coprecipitation method and ceramic method. The nanocomposite [(NCZF)1–x(Na(***))x] (x = 0%, 20%, 40%, 60%, 80%, 100%) was prepared by mixing two phases, Ni0.45Co0.2Zn0.35F2O4 with coprecipitation method [NCZF] and sodium acetylacetonate [Na(***)]. The TEm photographs of the as prepared ferrite samples by coprecipitation method at different ratios of Na(***) show that the crystalline cubic nanoparticles diameter were ranged from 26 - 36 nm and the particle size of Na(***) is greater than NCZF. The same behavior has also confirmed by XRD pat-terns indicating polycrystalline nature of prepared sample. The comparison between the SEm micrographs for NCZF nanoparticles prepared by a) ceramic method and b) co-precipitation method was done to show the effect of preparation condition on the microstructure. IR analysis confirms the composites formation.

Development of an Arabic version of the National Eye Institute Visual Function Questionnaire as a tool to study eye diseases patients in Egypt

AIm:To develop and test an Arabic version of the National Eye Institute Visual Function Questionnaire-25(NEI-VFQ-25).mETHODS:NEI-VFQ-25 was translated into Arabic according to WHO translation guidelines. We enrolled adult consenting patients with bilateral chronic eye diseases who presented to 14 hospitals across Egypt from October to December 2012, and documented their clinical findings. Psychometric properties were then tested using ***:We recruited 379 patients, whose mean age was(54.5±15)y. Of 46.2% were males, 227 had cataract,31 had glaucoma, 23 had retinal detachment, 37 had diabetic retinopathy, and 61 had miscellaneous visual defects. Non-response rate and the floor and ceiling numbers of the Arabic version(ARB-VFQ-25) were calculated. Internal consistency was high in all subscales(except general health), with Cronbach-α ranging from0.702-0.911. Test-retest reliability was high(intraclass correlation coefficient 0.79).CONCLUSION:RB-VFQ-25 isareliableandvalidtool for assessing visual functions of Arabic speaking patients. However, some questions had high non-response rates and should be substituted by available alternatives. Our results support the importance of including self-reported visual functions as part of routine ophthalmologic examination.

Prevalence of Chronic Kidney Disease in Patients with Cardiovascular Disease

Background: Chronic kidney disease (CKD) is a worldwide public health alarming problem. Although both heart and kidneys are separated by a quite distance within the body and they perform varied functions, there is a close physiological relationship between them. The diseases in the kidneys can trigger a disease in the heart and vice versa. High blood pressure is the most significant risk factor for the development and progression of chronic kidney disease (CKD). Lowering blood pressure is a goal to prevent CKD progress. Chronic abnormalities in cardiac function (e.g., chronic congestive heart failure) causing, chronic kidney disease and anemia appear to act together in a vicious circle in which each condition causes or exacerbates the other progressive chronic kidney disease. Objective: To assess the prevalence of chronic kidney disease in patients with cardiovascular disease at Shebin El-Kom Teaching Hospital and menoufia University Hospital Cardiology Outpatient Clinic, menoufia Governorate, Egypt. methods: This is a cross-sectional study that was conducted in Shebin El-Kom Teaching Hospital Cardiology Outpatient Clinic, menoufia University Cardiology Outpatient Clinic from April 2019 to July 2019. This study included 200 patients with cardiovascular disease or hypertension for more than 6 months. All patients were subjected to detailed history taking, clinical examination, laboratory investigation, echo and abdominal ultrasound. Results: This study included 200 patients with cardiovascular disease or hypertension for more than 6 months, which showed that: 63 (31.5%) were diagnosed as chronic kidney disease, 24 (38%) known to be CKD, 39 (62%) not known diagnosed in our study.
