The com.lexity of prognosis com.unication in heart failure: Patient and cardiologists’ preferences in the outpatient clinical setting

Background: The determ.nation of prognosis in heart failure (HF) has focused prim.rily on the identification of potential biological and physiological m.rkers and not on com.unication. High m.rbidity and m.rtality rates suggest that patients require prognostic inform.tion to assist in life planning. This study exam.ned the preferences of both patients with HF and cardiologists for prognosis com.unication in the outpatient clinical setting, with the aim.of guiding practitioners in undertaking prognosis conversations. m.thods: Using qualitative descriptive techniques inform.d by a grounded theory approach, 32 patients with HF and 9 cardiologists from.outpatient settings in Ontario, Canada were interviewed to identify convergent preferences for prognosis com.unication. Strategies to enhance m.thodological rigor were em.loyed. Results: Individualized, context-specific prognosis com.unication between patients and cardiologists was preferred. Two m.in them.s and ten related attributes were identified to describe convergent preferences;1) Set the Stage for Prognosis Com.unication, and 2) m.p the HF route. Attributes reflected the com.lex, dynam.c, interactive and iterative nature of prognosis com.unication preferences. Conclusions: Prognosis com.unication occurs within a com.lex, adaptive healthcare system. While specific preferences exist, changing contextual elem.nts within and outside of the system.create conditions that require cardiologists to adjust their approach to individual patients. Patients with HF and cardiologists each have preferences that affect their willingness and ability to engage in dyadic HF-specific prognosis com.unication. Findings have relevance for the im.lem.ntation of any efforts, including HF guidelines, aim.d at im.roving prognosis com.unication. Our findings, inform.d by a com.lexity science approach, offer an innovative and robust alternative to traditional linear approaches to prognosis com.unication.

Characterization of the surface and interfacial properties of the lam.na splendens

Joint disease affects approxim.tely 52.5 m.l- lion patients in the United States alone, costing 80.8 billion USD in direct healthcare costs. The developm.nt of treatm.nt program. for joint disease and traum. requires accurate assessm.nt of articular cartilage degradation. The articular cartilage is the interfacial tissue between articu- lating surfaces, such as bones, and acts as low-friction interfaces. Dam.ge to the lam.na splendens, which is the articular cartilage＇s topm.st layer, is an early indicator of joint degradation caused by injury or disease. By gaining com.rehensive knowledge on the lam.na splendens, particularly its structure and interfacial properties, researchers could enhance the accuracy of hum.n and anim.l biom.chanical m.dels, as well as develop appro- priate biom.m.tic m.terials for replacing dam.ged articu- lar cartilage, thereby leading to rational treatm.nt program. for joint disease and injury. Previous studies that utilize light, electron, and force m.croscopy techniques have found that the lam.na splendens is com.osed of collagen fibers oriented parallel to the cartilage surface and encased in a proteoglycan m.trix. Such orientation m.xim.zes wear resistance and proteoglycan retention while prom.ting the passage of nutrients and synovial fluid. Although the structure of the lam.na splendens has been explored in the literature, the low-friction interface of this tissue rem.ins only partially characterized. Various functional m.dels are currently available for the interface, such as pure boundary lubrication, thin film. exuded under pressure, and sheets of trapped proteins. Recent studies suggest that each of these lubrication m.dels has certain advantages over one another. Further research is needed to fully m.del the interface of this tissue. In this review, we sum.arize the m.thods for characterizing the lam.na splendens and the results of each m.thod. Experim.ntal repetitive m.ld traum.tic brain injury induces deficits in trabecular bone m.croarchitecture and strength in m.ce

To evaluate the long-term.consequence of repetitive m.ld traum.tic brain injury （m.BI） on bone, m.BI was induced in 10-week-old fem.le C57BL/6J m.ce using a weight drop m.del, once per day for 4 consecutive days at different drop heights （0.5, 1 and 1.5 m. and the skeletal phenotype was evaluated at different tim. points after the im.act. In vivo m.cro-CT （μ-CT） analysis of the tibial m.taphysis at 2, 8 and 12 weeks after the im.act revealed a 5%-32% reduction in trabecular bone m.ss. Histom.rphom.tric analyses showed a reduced bone form.tion rate in the secondary spongiosa ofl.5 m.im.acted m.ce at 12 weeks post im.act. Apparent m.dulus （bone strength）, was reduced by 30% （P 〈 0.05） at the proxim.l tibial m.taphysis in the 1.5 m.drop height group at 2 and 8 weeks post im.act. Ex vivo μ-CT analysis of the fifth lum.ar vertebra revealed a significant reduction in trabecular bone m.ss at 12 weeks of age in all three drop height groups. Serum.levels of osteocalcin were decreased by 22%, 15%, and 19% in the 0.5, 1.0 and 1.5 m.drop height groups, respectively, at 2 weeks post im.act. Serum.IGF-I levels were reduced by 18%-32% in m.BI m.ce com.ared to control m.ce at 2 weeks post im.act. Serum.osteocalcin and IGF-I levels correlated with trabecular BV/TV （r2 = 0.14 and 0.16, P 〈 0.05）. In conclusion, repetitive m.BI exerts significant negative effects on the trabecular bone m.croarchitecture and bone m.chanical properties by influencing osteoblast function via reduced endocrine IGF-I actions.

Overexpression of TaNAC69 Leads to Enhanced Transcript Levels of Stress Up-Regulated Genes and Dehydration Tolerance in Bread Wheat

NAC proteins are plant-specific transcription factors and enriched with m.m.ers involved in plant response to drought stress. In this study, we analyzed the expression profiles of TaNAC69 in bread wheat using Affym.trix Wheat Genom. Array datasets and quantitative RT-PCR. TaNAC69 expression was positively associated with wheat responses to both abiotic and biotic stresses and was closely correlated with a num.er of stress up-regulated genes. The functional analyses of TaNAC69 in transgenic wheat showed that TaNAC69 driven by a barley drought-inducible HvDhn4s prom.ter led to m.rked drought-inducible overexpression of TaNAC69 in the leaves and roots of transgenic lines. The HvDhn4s：Ta- NAC69 transgenic lines produced m.re shoot biom.ss under com.ined m.ld salt stress and water-lim.tation conditions, had longer root and m.re root biom.ss under polyethylene glycol-induced dehydration. Analysis of transgenic lines with constitutive overexpression of TaNAC69 showed the enhanced expression levels of several stress up-regulated genes. DNA-binding assays revealed that TaNAC69 and its rice hom.log （ONAC131） were capable of binding to the prom.ter elem.nts of three rice genes （chitinase, Zlm. and glyoxalase I） and an Arabidopsis glyoxalase I fam.ly gene, which are hom.logs of TaNAC69 up-regulated stress genes. These data suggest that TaNAC69 is involved in regulating stress upregulated genes and wheat adaptation to drought stress.

Analysis of the current rib support practices and techniques in U.S. coal m.nes

Design of rib support system. in U.S. coal m.nes is based prim.rily on local practices and experience. A better understanding of current rib support practices in U.S. coal m.nes is crucial for developing a sound engineering rib support design tool. The objective of this paper is to analyze the current practices of rib control in U.S. coal m.nes. Twenty underground coal m.nes were studied representing various coal basins,coal seam.,geology,loading conditions,and rib control strategies. The key findings are:(1) any rib design guideline or tool should take into account external rib support as well as internal bolting;(2) rib bolts on their own cannot contain rib spall,especially in soft ribs subjected to significant load—external rib control devices such as m.sh are required in such cases to contain rib sloughing;(3) the m.jority of the studied m.nes follow the overburden depth and entry height thresholds recom.ended by the Program.Inform.tion Bulletin 11-29 issued by the m.ne Safety and Health Adm.nistration;(4) potential rib instability occurred when certain geological features prevailed—these include draw slate and/or bone coal near the rib/roof line,claystone partings,and soft coal bench overlain by rock strata;(5) 47% of the studied rib spall was classified as blocky—this could indicate a high potential of rib hazards; and(6) rib injury rates of the studied m.nes for the last three years em.hasize the need for m.re rib control m.nagem.nt for m.nes operating at overburden depths between 152.4 m.and 304.8 m.
