Micropunch grafting for healing of refractory chronic venous leg ulcers

Background: Chronic venous ulcers are a serious problem for both patients and physicians. The CEAP classification(clinical manifestations(C), etiologic factors(E), anatomic distribution of disease(A), and underlying pathophysiologic findings(P)) for chronic venous disorders(CVD) was developed in 1994. Published papers on CVD use all or part of the CEAP. Micropunch grafts, which are a variation of skin grafts, have been used with great success in plastic surgery for both esthetic and reconstructive purposes. This study aimed to assess the outcomes of micropunch grafting in the treatment of chronic refractory venous ***: Patients with chronic venous ulcers who did not respond to conservative treatment for more than 6months were included in the study. All patients underwent ulcer coverage by micropunch skin grafts using a micrometer. The patients were discharged on the same day as the surgery. Micropunch skin grafts were manually counted per square centimeter intraoperatively and during follow-up using Dermlite DermatoschopeTM II Pro *** satisfaction was assessed by using a ***: Twenty patients underwent the surgery. Most of the patients were middle-aged males. The ulcers were predominantly small-and medium-sized. The mean operation time was 167.00 ± 86.01 min. After 6 months, the survival rate of the planted micrografts was 84%. Three ulcers had graft loss and two patients had an infection at the ulcer site, which was managed conservatively. The patients were followed-up for 14 ***: Micropunch grafting is a useful and convenient method for the treatment of difficult venous ulcers and can be performed on an outpatient basis.

Introduction: Healthcare workers in Mogadishu, Somalia face significant occupational injury risks, particularly needle stick injuries, with 61.1% reporting incidents. This poses a serious threat to their health, leading to infections such as hepatitis B, hepatitis C, and HIV. Despite the high prevalence of injuries, awareness of Post-Exposure Prophylaxis (PEP) accessibility is relatively high, with 84.0% of respondents aware of it. However, there are gaps in knowledge and implementation, as evidenced by variations in availability of PEP. Improving workplace safety measures, providing comprehensive training on injury prevention and PEP protocols, and ensuring consistent availability of PEP in healthcare facilities are crucial steps to safeguard the well-being of healthcare workers in Mogadishu, Somalia. Methods: A cross-sectional study was conducted among hospital workers in Mogadishu, Somalia, focusing on professionals from various healthcare facilities. The study targeted nurses, doctors, laboratory personnel, and pharmacists. Purposive sampling was employed, resulting in a sample size of 383 calculated using Fisher’s sample size formula. Data were collected using coded questionnaires entered into Microsoft Excel 2019 and analyzed with SPSS software to generate frequencies and proportions, presented through frequency tables and pie figures. Results: The study in Mogadishu, Somalia, examined the prevalence of occupational injuries and knowledge of Post-Exposure Prophylaxis (PEP) accessibility among healthcare workers. Findings indicate a high prevalence of injuries, with 61.1% reporting incidents, predominantly needle stick injuries (60.6%). Despite the majority seeking prompt medical attention (72.0%), work-related illnesses affected 53.2% of respondents, notably work-related stress (59.5%). Effective damping of local low frequency oscillations in power systems integrated with bulk PV generation

High penetration of renewable sources into conventional power systems results in reduction of system inertia and noticeable low-frequency oscillations (LFOs) in the rotor speed of synchronous generators. In this paper, we propose effective damping of LFOs by incorporating a supplementary damping controller with a photovoltaic (PV) generating station, where the parameters of this controller are coordinated optimally with those of a power system stabilizer (PSS). The proposed method is applied to damp local electromechanical modes by studying a system comprising a synchronous generator and a PV station connected to an infinite bus. The PV station is modeled following the instructions of the Western electricity Coordinating Council. The problem is modeled as an optimization problem, where the damping ratio of the electromechanical modes is designed as the objective function. Constraints including upper and lower limits of decision parameters and damping ratio of other modes are considered by imposing penalties on the objective function. Different optimization algorithms are used to pursue the optimal design, such as political, improved gray wolves and equilibrium optimizers. The results validate the effectiveness of the proposed controller with PSS in damping local modes of oscillations.

Negative pressure wound therapy： An update

Purpose： To get the maximum benefit of vacuum-assisted closure （VAC） in the management of acute and chronic wounds without abuse or misuse. Methods： Fourty one patients were included in the study. Among them, 9 patients had chronic wounds and the rest 32 patients had acute wounds. In acute wounds, 19 patients had co-morbid conditions. Seven patients had gaped wounds, 4 patients had necrotizing fasciitis and 2 patients had enter- ocutaneous fistula. Results： The hospitalization period varied from 1 to 6 weeks. The follow-up period was up to 6 months. No mortality was recorded during this study. All skin grafts showed complete healing. Necrotizing fas- ciitis was managed with good outcome. Wounds with enterocutaneous fistula have improved. In chronic wounds, good healing and excellent outcome were obtained. Conclusion： The VAC therapy is an essential element for the management of problematic acute and chronic wounds.

A new chaotic image encryption scheme based on permutation and substitution in the Fourier domain is *** Fourier Transform(FRFT)is used before the encryption scheme to get a large degree of *** permutation is achieved by Baker map and the substitution by a key-related-to-plain-image algorithm based on the modified Logistic *** of the Logistic map is developed to increase the space of the encryption key,and hence increase the *** key of the encryption algorithm dependents on the plain image,and thus,the cipher image is sensitive to both the initial key and the plain image to resist known-plaintext and chosen plaintext *** key space is large and hence the algorithm can effectively resist brute-force *** proposed scheme is examined using different performance evaluation metrics and the results prove that the proposed scheme is highly secure,and it can effectively resist different attacks.

Efficacy of octreotide in the prevention of complications after pancreaticoduodenectomy in patients with soft pancreas and non-dilated pancreatic duct: A prospective randomized trial

Background: The efficacy of octreotide to prevent postoperative pancreatic fistula(POPF) of pancreaticoduodenectomy(PD) is still controversial. This study aimed to evaluate the effect of postoperative use of octreotide on the outcomes after ***: This is a prospective randomized controlled trial for postoperative use of octreotide in patients undergoing PD. Patients with soft pancreas and pancreatic duct elayed gastric emptying(P = 0.472), and early postoperative mortality(P = 0.727).Conclusions: Octreotide did not reduce postoperative morbidities, reoperation and mortality rate. Also, it did not affect the incidence of POPF and its clinically relevant variants.

Bitter gourd extract improves glucose homeostasis and lipid profile via enhancing insulin signaling in the liver and skeletal muscles of diabetic rats

Objective:To investigate the modulatory effects of bitter gourd extract on the insulin signaling pathway in the liver and skeletal muscle tissues of diabetic ***:The ethanolic extract of bitter gourd was prepared and its contents of total polyphenols and flavonoids were assayed.A neonatal streptozotocin-induced diabetic rat model was established and the diabetic rats were assigned into different groups and were treated with different doses of bitter gourd extract(100,200,400,or 600 mg/kg)or with glibenclamide(0.1 mg/kg)for 30 *** blood glucose,insulin,and lipid profile were evaluated and the insulin signaling pathway in the liver and skeletal muscle of rats was *** correlations between homeostasis model assessment(HOMA)and the components of insulin signaling pathway were also ***:Different doses of bitter gourd extract significantly ameliorated fasting blood glucose level and HOMA index for insulin ***,bitter gourd extract increased serum insulin and improved disrupted serum lipid *** levels of insulin receptor substrate-1(IRS-1),p-insulin receptorβ(p-IR-β),protein kinase C(PKC),GLUT2,and GLUT4 were improved by treatment with bitter gourd *** best results were obtained with 400 mg/kg dose of the extract,the effect of which was equivalent to that of *** in the bitter gourd treated rats was negatively correlated with p-IR-β,IRS-1 and PKC in hepatic and skeletal *** was also negatively correlated with skeletal muscle ***:Bitter gourd extract improves glucose homeostasis and lipid profile in diabetic rats via enhancement of insulin secretion and ***,bitter gourd can be used as a potential pharmacological agent for the treatment of type 2 diabetes mellitus.
