The diagnostic performance of the JNET classification was equivalent to the Pit pattern classification in each corresponding category. The pooled sensitivity, specificity,and area under the curve(AUC) for each category of the JNET classification were as follows: 0.73(95%CI: 0.55-0.85), 0.99(95%CI: 0.97-1.00), and 0.97(95%CI: 0.95-0.98), respectively, for Type 1;0.88(95%CI: 0.78-0.94), 0.72(95%CI: 0.64-0.79), and 0.84(95%CI: 0.81-0.87), respectively, for Type 2 A;0.56(95%CI: 0.47-0.64), 0.91(95%CI: 0.79-0.96), and 0.72(95%CI: 0.68-0.76), respectively, for Type 2 B;0.51(95%CI: 0.42-0.61), 1.00(95%CI: 1.00-1.00), and 0.90(95%CI: 0.87-0.93), respectively, for Type *** This meta-analysis suggests that the diagnostic efficacy of the JNET classification may be equivalent to that of the Pit pattern classification. Endoscopic ultrasound-guided deep and large biopsy for diagnosis of gastric infiltrating tumors with negative malignant endoscopy biopsies

AIM: To assess the diagnostic yield and safety of adeep and large biopsy technique under the guidance of endoscopic ultrasound(EUS) for diagnosis of gastric infiltrating tumors with negative malignant endoscopy ***: From January 2009 to March 2014, 36 patients in whom gastric infiltrating tumors had been diagnosed by EUS received negative results for malignancy after endoscopic biopsies. The deep and large biopsy technique combined bite-on-bite technique with or without endoscopic mucosal resection(EMR) to obtain submucosal tissue from lesions. EUS was used to select the appropriate biopsy sites. If the lesion protruded into the cavity, EMR was performed for removal of the overlying mucosa and then bite-onbite technique was conducted in the resected area to obtain submucosal tissue. If the lesion appeared to be flat or was difficult to lift by injection, the bite-on-bite technique was directly ***: Twenty-eight of the 36 patients were treated by EMR followed by bite-on-bite technique, while 8 patients only underwent bite-on-bite technique. Histological results showed 23 of the 36 lesions were poorly differentiated adenocarcinomas, 2 diffuse large B cell lymphomas, 4 mucosa-associated lymphoid tissue-type lymphomas, and 7 undiagnosed. The deep and large biopsy technique provided a definitive and conclusive diagnosis in 29(80.6%) of the 36 patients. The 12 gastric linitis plastica and 6 lymphoma patients received chemotherapy and avoided surgery. Minor oozing of blood in 2 mucosal resection wounds was managed by argon plasma coagulation and in 5 cases after deep biopsies by epinephrine(0.001%). 目的比较内镜黏膜下剥离术(ESD)与传统外科手术治疗消化道早癌及癌前病变的有效性、安全性及费效比。方法回顾性分析2010年4月-2013年11月住院接受治疗的65例消化道早癌及癌前病变患者的临床病理资料,根据治疗方法不同分为内科ESD组及外科手术组,比较两种疗法在有效性、安全性及卫生经济方面的差异。结果 ESD组32例与外科手术组33例相比,治愈性切除率(100%vs 100%)、术后生存率(100%vs 100%)、术后复发率(0%vs 0%),差异无显著性,外科手术组虽然整块切除率(100%)高于ESD组(65.6%,P<0.001),但并发症明显增多(39.4%vs 6.3%,P=0.002),平均手术时间明显延长(203.1 m in vs 86.5 m in,P<0.001),住院天数明显增加(22.1 d vs 10.1 d,P<0.001),住院费用明显增多(56 390元vs 14 533元,P<0.001)。结论消化道早癌及癌前病变的治疗,ESD与外科手术疗效相当,但ESD较外科手术住院时间短、费用低、并发症少、恢复快,值得临床首选使用。

Water storage changes in the North China Plain from 2004 to 2019

In the past few decades,the irrational use of water resources has resulted in many issues such as land subsidence in the North China Plain(NCP),hindering its socio-economic *** accurate understanding of water resource changes is important for the allocation of water resources in the *** this study,we employed Gravity Recovery and Climate Experiment(GRACE) satellite data to monitor the total water storage(TWS) change in the NCP during 2004–***,precipitation,and runoff during 2004–2019 were sequentially examined,using the water balance formula(WBF),to retrieve TWS ***,the soil moisture,snow water equivalent,and canopy water storage from the Global Land Data Assimilation System were used to calculate the natural component of the TWS variations.A comparison of these results revealed the drivers of the changes in water *** results showed that:(1) overall TWS retrieved by GRACE decreased substantially as a consequence of human activity,while TWS obtained by WBF fluctuated periodically around zero under the impact of three natural drivers;(2) TWS in the NCP fell at a remarkable rate of-12.39 mm/y from 2004 to 2019,and the natural part of TWS experienced two significant declines,during 2004–2010 and 2013–2016;(3) variations in runoff,precipitation,and evapotranspiration from 2004 to 2019 were not significant,but human activities contributed more to the decreasing TWS than natural *** study provides a reference for water resource management and groundwater exploitation across the NCP under climate change.
