A Comparative Study of Intrathecal Injection of Bupivacaine Alone or with Fentanyl, Clonidine, and Neostigmine in Lower Abdominal Surgeries

Background: Anesthesiologists are responsible for the development of pain services in the current era. Hence ideal adjuvants that can be used with bupivacaine for stable intraoperative conditions and prolonging the postoperative analgesia with fewer side effects are being investigated. Opioids, despite useful as adjuvants, are associated with undesirable side effects. Aim of the work: The study was done to compare analgesic efficacy and hemodynamic of intrathecal injection of bupivacaine alone or with fentanyl, clonidine, and neostigmine in lower abdominal surgeries, over the first 24 postoperative hours, in a randomized, double-blind, and clinical trial. Methods: 100 Patients were randomized into four equal groups, 25 patients in each group;Group B patients received 2.5 ml of 0.5% hyperbaric bupivacaine and 0.5 ml of normal saline. Group BF patients received 2.5 ml of 0.5% hyperbaric bupivacaine with (25 mics) of fentanyl. Group BC patients received 2.5 ml of 0.5% hyperbaric bupivacaine with 0.5 ml (75 mics) of clonidine. Group BN patients received 2.5 ml of 0.5% hyperbaric bupivacaine with 0.1 ml of neostigmine (50 mics) and 0.4 ml of normal saline. Intrathecal anesthesia was done with a recording of parameters intraoperative and the post-operative period. Each patient was assessed for hemodynamic parameters and effective analgesia in operation, and presence of complications (nausea, vomiting, sedation and pruritus) visual analogue pain score (VAS) postoperatively by a blinded investigator in the post-anesthesia care unit (PACU) and at 1, 2, 3, 4, 8 12, 18 and 24 h postoperatively. Results: The postoperative analgesia is more effective with group BC (the gold standard) than group B, group BF, and group BN. As regard complications during the study in all groups, complications as nausea, and vomiting were mainly with group BN;hypotension was primarily in group BC. Deploying an Interactive GIS System for Facility and Asset Management: Case Study-Ministry of Education, Kuwait

Geographical Information System (GIS) can be considered the core of the Interactive Facilities Management Environment (iFaME) framework in managing different facilities on the building bases. This study aims at building a comprehensive geodatabase for different elements of infrastructure facilities and services on the building bases for a selected number of schools and developing a GIS-based iFaME interactive application to manage different facilities at the school level in Kuwait. The iFaME is a two-dimensional/three-dimensional (2D/3D) desktop application. It is intended to take planners, decision makers, and maintenance experts inside the building to provide an interactive GIS platform to manage, visualize, query, maintain, and update the database related to school assets and facilities. The Autodesk Revit software was used to produce 3D building information models (BIM) for the selected schools. It provides accurate geometrical representation of the school building elements in an integrated data environment. The iFaME application is integrated with the Revit BIM models and the STAR-APIC Elyx 3D software solution. With the development of the iFaME applications, the facility managers in the MoE could establish greater control over the space allocation and management, asset management, emergency planning, and other areas of facility management. The school managers and maintenance engineers are considered among the most beneficiaries from this application. The school managers will monitor the maintenance activities all the time, while the maintenance engineers will use the application as a container for maintenance orders, and on the same time to document what they accomplished on daily basis. The iFaME application will reduce the maintenance cost of school assets and facilities in addition to increase their efficiency.

神经源性吞咽障碍是一种常见的神经系统疾病的并发症,预后差且治疗方法有限。因此,将神经刺激的基础研究转化为临床应用成为当务之急。本文重点介绍咽腔电刺激(pharyngeal electrical stimulation,PES)技术在人类吞咽运动系统中的治疗应用,阐明咽腔电刺激调控吞咽功能的机制,通过大量研究结果证明该刺激能够增加咽部运动皮质的兴奋性,该技术在神经源性吞咽障碍已经完成和正在进行的临床研究也可窥见一斑。尽管目前的研究结果提示咽腔电刺激技术是一种具有广阔临床应用前景的神经调控干预手段,但仍需要进行更多的临床研究,以便充分地探索其潜在用途。

Environmental and Mineralogical Studies of the Sabkhas Soil at Ismailia—Suez Roadbed, Southern of Suez Canal District, Egypt

Eight surface sabkha soils samples were collected from Ismailia—Suez roadbed, southwestern of Suez Canal district. Sedimentological and mineralogical analyses were conducted using grain size;X-ray diffraction and Scanning Electron Microscopy (SEM). Grain size analysis indicates high contents of fine sand and mud as well as presence of salts. X-ray diffraction;Scanning Electron Microscopy (SEM) and EDAX tool clarified that the sabkha soils are enriched by quartz, sulfate minerals (gypsum, anhydrite), carbonate minerals (calcite-dolomite-aragonite), chlorides (halite and bischofite), and clay minerals. The results elucidate that the appearance of sabkha deposits and their distribution in the study area are controlled mainly by the content of water soluble salts through parent materials;ground water table;subsurface structural;and physiographic features for instance surface relief or topography;and human activity.

GIS as an Efficient Tool to Manage Educational Services and Infrastructure in Kuwait

The State of Kuwait Ministry of Education (MoE) has clearly defined land use standards for the location of public schools, and an inventory of reserved lands for future facilities. Unless, there is a geographical efficient tool to manage and plan the education system in a rapidly developing country such as Kuwait there will be huge deficit in such services. Geographic Information Systems (GIS) was used to fill in this gap and effectively evaluate and analyze their facilities and unoccupied lands to ensure they continue to meet the population and future needs of Kuwaiti students. This paper utilized the GIS to inventory, map, and analyze MoE facilities and unoccupied land reservations with a goal of improved planning and decision making. Unfortunately, the initial spatial analysis of the data showed huge percent of districts that have no schools failing to meet the minimum standard of the MoE including kindergartens, primary schools, intermediate schools for girls, intermediate schools for boys, secondary schools for girls and secondary schools for boys at 72%, 71%, 48%, 43%, 54%, and 55% respectively. Such critical results will enable the decision makers to prioritize the immediate action of relocation the schools or widen the services and accessibility. Moreover, the analysis of the data showed a critical and immediate need to reserve land for five districts where they are heavily populated and lacked reserved land. However, based on long term land use plans, there is an urgent need to relocate some land and reserve others to meet the future urbanization plans and population growth.

Transport Properties of AgInSe<sub>2</sub>Crystals

AgInSe2 crystals were grown by Bridgman technique. The crystals were identified structurally by X-ray diffraction technique. Measurements of electrical conductivity and Hall effect were performed in the temperature range (138 K - 434 K) and (220 K - 488 K) for thermoelectric power measurements. From these measurements, many physical parameters were determined. The energy gap was calculated to be 1.24 eV. The conductivity type was found to be n-type. Crystallite size (D) of the obtained AgInSe2 crystals was calculated to be 70 nm. The lattice parameters for the prepared crystals were a = 6.0938 ? and c = 11.7775 ?.

Seismic Imaging and Reservoir Architecture of Sub-Marine Channel Systems Offshore West Nile Delta of Egypt

Offshore Nile Delta gas reservoirs are dominated by slope-channel systems of Plio-Pleistocene age. High-quality, three-dimensional seismic imaging has significantly helped in defining the geomorphology and architectures of these channels. Integrating seismic, logs and core data from four wells resulted in understanding of different stages of channel development and reservoir quality. The studied reservoirs that are largely controlled by episodes of transgressive-regressive events resulted in deposition of fine grained sediment and shale. Sienna channel complex consists of unconfined channel system with clearly defined development stages. The stages include amalgamated or stacked channels followed by channel abandonment phases and local flooding events. The depositional pattern continued through the Late Pliocene-Pleistocene. SimSat-P1 and SimSat-P2 reservoirs are characterized by isolated sand bodies, most probably relics of fan depositional setting. The depositional scenario that is largely controlled by successive transgression and flooding events resulted in deposition of interbedded, sheet-like, fine grained sediment and shale.

The Effect of Early versus Traditional Hospital Discharge on Activities of Daily Living for Women Undergoing Elective Cesarean Section. An Observational Cohort Study

Background: Cesarean section is one of the most common surgeries performed worldwide and its consequences and complications represent a major public health concern. The policy of enhanced recovery after surgery has been implemented after cesarean section to optimize perioperative care. Enhanced recovery also aims to reduce the length of hospital stay, which might imply substantial healthcare savings. Rising hospital costs are one of the factors in early discharge. Patients & Methods: This was a prospective cohort study. 158 patients met the inclusion criteria of the study, 81 patients were discharged after 48 hours and 77 patients were discharged after 24 hours. All patients received the same postoperative care and instructions. All the study participants received a phone call 1, 3, and 5 days after discharge to objectively assess the activities of daily living through Katz index of independence in activities of daily living. All patients were offered a postpartum visit 1 week after discharge during which the wound was assessed by the Southampton wound assessment scale. Moreover, other maternal complications such as mastitis, puerperal sepsis, or pyelonephritis were assessed and cases who were readmitted were documented. Initiation of successful breastfeeding and neonatal readmission were reported as well. Results: 158 patients met the inclusion/exclusion criteria of the study. There was no significant difference between the two arms of the study regarding baseline characteristics. Katz index of independence showed that the resumption of activities of daily living after CS was similar in both arms of the study on days 1, 3 and 5 after discharge. Time till the passage of flatus and stool after the cesarean section was significantly shorter among the early discharge arm (9.31 VS 14.68, p value < 0.001 & 13.25 VS 24.82, p value < 0.001 respectively).
