GIS as an Efficient Tool to Manage Educational Services and Infrastructure in Kuwait

The State of Kuwait Ministry of Education (MoE) has clearly defined land use standards for the location of public schools, and an inventory of reserved lands for future facilities. Unless, there is a geographical efficient tool to manage and plan the education system in a rapidly developing country such as Kuwait there will be huge deficit in such services. Geographic Information Systems (GIS) was used to fill in this gap and effectively evaluate and analyze their facilities and unoccupied lands to ensure they continue to meet the population and future needs of Kuwaiti students. This paper utilized the GIS to inventory, map, and analyze MoE facilities and unoccupied land reservations with a goal of improved planning and decision making. Unfortunately, the initial spatial analysis of the data showed huge percent of districts that have no schools failing to meet the minimum standard of the MoE including kindergartens, primary schools, intermediate schools for girls, intermediate schools for boys, secondary schools for girls and secondary schools for boys at 72%, 71%, 48%, 43%, 54%, and 55% respectively. Such critical results will enable the decision makers to prioritize the immediate action of relocation the schools or widen the services and accessibility. Moreover, the analysis of the data showed a critical and immediate need to reserve land for five districts where they are heavily populated and lacked reserved land. However, based on long term land use plans, there is an urgent need to relocate some land and reserve others to meet the future urbanization plans and population growth.

Fractional Order Generalized Thermoelastic Infinite Medium with Cylindrical Cavity Subjected to Harmonically Varying Heat

In this work, a mathematical model of an elastic material with cylindrical cavity will be constructed. The governing equations will be taken into the context of the fractional order generalized thermoelasticity theory (Youssef 2010). Laplace transform and direct approach will be used to obtain the solution when the boundary of the cavity is exposed to harmonically heat with constant angular frequency of thermal vibration. The inverse of Laplace transforms will be computed numerically using a method based on Fourier expansion techniques. Some comparisons have been shown in figures to present the effect of the fractional order parameter and the angular frequency of thermal vibration on all the studied felids.

Environmental and Mineralogical Studies of the Sabkhas Soil at Ismailia—Suez Roadbed, Southern of Suez Canal District, Egypt

Eight surface sabkha soils samples were collected from Ismailia—Suez roadbed, southwestern of Suez Canal district. Sedimentological and mineralogical analyses were conducted using grain size;X-ray diffraction and Scanning Electron Microscopy (SEM). Grain size analysis indicates high contents of fine sand and mud as well as presence of salts. X-ray diffraction;Scanning Electron Microscopy (SEM) and EDAX tool clarified that the sabkha soils are enriched by quartz, sulfate minerals (gypsum, anhydrite), carbonate minerals (calcite-dolomite-aragonite), chlorides (halite and bischofite), and clay minerals. The results elucidate that the appearance of sabkha deposits and their distribution in the study area are controlled mainly by the content of water soluble salts through parent materials;ground water table;subsurface structural;and physiographic features for instance surface relief or topography;and human activity.

Seismic Imaging and Reservoir Architecture of Sub-Marine Channel Systems Offshore West Nile Delta of Egypt

Offshore Nile Delta gas reservoirs are dominated by slope-channel systems of Plio-Pleistocene age. High-quality, three-dimensional seismic imaging has significantly helped in defining the geomorphology and architectures of these channels. Integrating seismic, logs and core data from four wells resulted in understanding of different stages of channel development and reservoir quality. The studied reservoirs that are largely controlled by episodes of transgressive-regressive events resulted in deposition of fine grained sediment and shale. Sienna channel complex consists of unconfined channel system with clearly defined development stages. The stages include amalgamated or stacked channels followed by channel abandonment phases and local flooding events. The depositional pattern continued through the Late Pliocene-Pleistocene. SimSat-P1 and SimSat-P2 reservoirs are characterized by isolated sand bodies, most probably relics of fan depositional setting. The depositional scenario that is largely controlled by successive transgression and flooding events resulted in deposition of interbedded, sheet-like, fine grained sediment and shale.

The Effect of Early versus Traditional Hospital Discharge on Activities of Daily Living for Women Undergoing Elective Cesarean Section. An Observational Cohort Study

Background: Cesarean section is one of the most common surgeries performed worldwide and its consequences and complications represent a major public health concern. The policy of enhanced recovery after surgery has been implemented after cesarean section to optimize perioperative care. Enhanced recovery also aims to reduce the length of hospital stay, which might imply substantial healthcare savings. Rising hospital costs are one of the factors in early discharge. Patients & Methods: This was a prospective cohort study. 158 patients met the inclusion criteria of the study, 81 patients were discharged after 48 hours and 77 patients were discharged after 24 hours. All patients received the same postoperative care and instructions. All the study participants received a phone call 1, 3, and 5 days after discharge to objectively assess the activities of daily living through Katz index of independence in activities of daily living. All patients were offered a postpartum visit 1 week after discharge during which the wound was assessed by the Southampton wound assessment scale. Moreover, other maternal complications such as mastitis, puerperal sepsis, or pyelonephritis were assessed and cases who were readmitted were documented. Initiation of successful breastfeeding and neonatal readmission were reported as well. Results: 158 patients met the inclusion/exclusion criteria of the study. There was no significant difference between the two arms of the study regarding baseline characteristics. Katz index of independence showed that the resumption of activities of daily living after CS was similar in both arms of the study on days 1, 3 and 5 after discharge. Time till the passage of flatus and stool after the cesarean section was significantly shorter among the early discharge arm (9.31 VS 14.68, p value < 0.001 & 13.25 VS 24.82, p value < 0.001 respectively). Oral Vinorelbine as Switch Maintenance Therapy versus Best Supportive Care in Patients with Advanced Adenocarcinoma Non-Small Cell Lung Cancer EGFR Wild Type

Background: This study was performed to evaluate the efficacy and safety of switch maintenance therapy with oral vinorelbine in advanced non-small cell lung cancer (NSCLC) with adenocarcinoma limited to epidermal growth factor receptor (EGFR) wild type. Materials and Methods: In this single randomized trial, patients with advanced stage (IIIB and IV) NSCLC with adenocarcinoma EGFR wild-type status, treated with 6 cycles of platinum based chemotherapy. Patients did not show progression after first-line chemotherapy were randomly assigned to receive switch maintenance with vinorelbine (80 mg/m2, day 1, 8) (group I) or the best supportive care until disease progression (group II). Results: The median progression free survival (PFS) was 9.7 months for group I versus 5.7 months for group II with statistically significant difference between both groups [HR = 1.15;95% CI 1.19 to 1.49;P value = 0.002], while the median overall survival (OS) was 13.2 months for group I versus 11.9 months for group II with no statistically significant differences between both groups [HR = 1.24;95% CI 1.05 to 1.46;P value = 0. 3]. The patients who received oral vinorelbine had tolerable toxicity profile. Conclusion: Switch maintenance therapy with oral vinorelbine, though improve PFS, did not improve OS in patients with NSCLC with adenocarcinoma EGFR wild type.

Intraoperative endoscopic retrograde cholangiopancreatography: A useful tool in the hands of the hepatobiliary surgeon

AIM: To evaluate the efficacy of intraoperative endoscopic retrograde cholangio-pancreatography(ERCP) combined with laparoscopic cholecystectomy(LC) forpatients with gall bladder stones(GS) and common bile duct stones(CBDS).METHODS: Patients treated for GS with CBDS were included. LC and intraoperative transcystic cholangiogram(TCC) were performed in most of the cases. Intraoperative ERCP was done for cases with proven ***: Eighty patients who had GS with CBDS were included. LC was successful in all cases. Intraoperative TCC revealed passed CBD stones in 4 cases so intraoperative ERCP was performed only in 76 patients. Intraoperative ERCP showed dilated CBD with stones in 64 cases(84.2%) where removal of stones were successful; passed stones in 6 cases(7.9%); short lower end stricture with small stones present in two cases(2.6%) which were treated by removal of stones with stent insertion; long stricture lower 1/3 CBD in one case(1.3%) which was treated by open hepaticojejunostomy; and one case(1.3%) was proved to be ampullary carcinoma and whipple's operation was scheduled. CONCLUSION: The hepatobiliary surgeon should be trained on ERCP as the third hand to expand his field of therapeutic options.

Free Residual Chlorine Calibration by WaterCAD at El-Nozha Water Network in Alexandria Governorate, Egypt

Most of developing countries suffer from decreasing and poor quality of drinking water which led to emergence of many dangerous diseases. In addition, there isn’t any methodology followed to predict and disinfect drinking water by using advanced software program. Maintaining water quality in the water distribution system has become a prominent issue in the study of the water network. Residual chlorine concentration is the indicator to ensure the quality of water in the water network because it eliminates contaminants in the distribution network beginning of the treatment plant down to the consumer. In collaboration with Alexandria (Egypt) Water Company, samples were taken from El-NOZHA water plant station and El-HADARA water distribution network to know the free residual chlorine. In this paper, WaterCAD software has been used to make hydraulic analysis and calibration of residual chlorine in water distribution network to know the ideal chlorine dose that should be added at the water treatment plant and to know the areas of strength and weakness in the concentration of free residual chlorine in the water distribution network. In addition different scenarios have been found to know the free residual chlorine at the weakness areas after injecting chlorine in some junctions and the impact of a fire case or breaking in the water pipe distribution network on the residual chlorine. Results showed ensuring in the water quality in the distribution network by adding chlorine dose in water less than the existing dose which has been added in the El-NOZHA water treatment plant. It is possible to maintain the percentage of free residual chlorine concentration at different locations without relying on adding chlorine only in water treatment plant by injecting low percentage of chlorine dose in the junctions.

Prophylaxis of Postoperative Acute Endopthalmitis Following Cataract Surgery

Background: Postoperative Endophthalmitis is a rare complication after intraocular surgeries. However, it remains one of the most devastating complications following cataract surgery because of its poor prognosis. We describe the effects of antibiotic prophylaxis on the incidence of postoperative endophthalmitis after cataract surgery, particularly phacoemulsification. Aim of the work: To identify risk factors of acute endophthalmitis and describe the effects of antibiotic prophylaxis on the incidence of postoperative endophthalmitis after phacoemulsification surgery. Methods: A prospective randomized cataract surgery study recruited 300 patients;150 patients who received just intracameral vigamox at the end of surgery without topical antibiotics before surgery and 150 patients who received both intracameral vigamoxas well as topical perioperative vigamox which either just half an hour before surgery or three days preceding surgery. We took a conjunctival swab before and after vigamox administration in order to detect the type of conjunctival flora in the Libyan population as well as the effect of the vigamox on the conjunctival flora by using Phoenix-BD machine. In total we collected 166 specimens (83 patients). Results: In all groups no acute post-operative endophthalmitis was reported. Conclusions: The study was based on intracameral Moxifloxacin at the end of surgery with and without topical antibiotics drops administration. The incidence of postcataract endophthalmitis was not reported in our study after using intracameral antibiotics, as it seems to be effective in preventing endophthalmitis after cataract surgery.
