Macrophages play an important role in peripheral nerve regeneration,but the specific mechanism of regeneration is still *** preliminary findings indicated that neutrophil peptide 1 is an innate immune peptide closely involved in peripheral nerve ***,the mechanism by which neutrophil peptide 1 enhances nerve regeneration remains *** study was designed to investigate the relationship between neutrophil peptide 1 and macrophages in vivo and in vitro in peripheral nerve crush *** functions of RAW 264.7 cells we re elucidated by Cell Counting Kit-8 assay,flow cytometry,migration assays,phagocytosis assays,immunohistochemistry and enzyme-linked immunosorbent *** debris phagocytosis was observed using the CUBIC(Clear,Unobstructed Brain/Body Imaging Cocktails and Computational analysis)optical clearing technique during Wallerian *** inflammatory factor expression in different polarization states was detected using a protein *** results showed that neutrophil peptide 1 promoted the prolife ration,migration and phagocytosis of macrophages,and CD206 expression on the surfa ce of macrophages,indicating M2 *** axonal debris clearance rate during Wallerian degeneration was enhanced after neutrophil peptide 1 *** peptide 1 also downregulated inflammatory factors interleukin-1α,-6,-12,and tumor necrosis factor-αin invo and in ***,the results suggest that neutrophil peptide 1 activates macrophages and accelerates Wallerian degeneration,which may be one mechanism by which neutrophil peptide 1 enhances peripheral nerve regeneration.

后踝骨折是较为复杂的踝关节骨折,约占踝关节骨折的50%。研究发现,骨折累及后踝通常对预后具有不良影响。近年来,随着CT检查的普及与生物力学研究的开展,后踝骨折中关节内嵌入骨块逐渐受到重视。关节内嵌入骨块可影响后踝骨折复位,容易导致关节面不一致和踝关节不稳,且与远期创伤性关节炎的发生有关。本文对后踝骨折中关节内嵌入骨块的形态学特征、分型与损伤机制、生物力学研究、治疗及预后等研究进行综述,以期为后踝骨折的诊治提供参考。

目的:探讨吸入乙酰半胱氨酸溶液治疗急性鼻窦炎的临床疗效以及对患者睡眠质量的影响。方法:选取2021年4月至2023年4月赤壁市人民医院收治的急性鼻窦炎患者114例作为研究对象,按照随机数字表法分为对照组和观察组,每组57例。对照组采用常规药物治疗,观察组在此基础上给予吸入乙酰半胱氨酸溶液治疗,比较2组临床疗效、鼻腔鼻窦功能、炎症介质水平及睡眠质量。结果:观察组总有效率较对照组高,差异有统计学意义(P<0.05)。观察组症状及视觉模拟评分法(VAS)评分低于对照组,差异有统计学意义(P<0.05)。治疗后观察组SNOT-20、Lund-Kennedy评分及鼻气道阻力(NAR)低于对照组,差异有统计学意义(P<0.05)。治疗后2组TNF-α、CRP、IL-6、IL-8、IL-10明显降低,观察组评分低于对照组,差异有统计学意义(P<0.05)。治疗后2组PSQI、SRSS评分明显降低,观察组低于对照组,差异有统计学意义(P<0.05)。结论:急性鼻窦炎采取乙酰半胱氨酸溶液雾化吸入治疗可明显减轻鼻塞、脓涕等症状,改善鼻腔鼻窦功能,抑制炎症反应,取得良好的治疗效果,利于患者恢复良好睡眠质量,值得推广。

目的探讨可调式外固定牵伸联合关节镜下微骨折治疗距骨软骨损伤(OLT)的效果。方法回顾性分析2017年5月至2022年3月期间首都医科大学附属北京康复医院骨科收治的27例OLT患者资料。男16例,女11例;年龄(32.4±7.2)岁;软骨损伤部位:内侧23例,外侧4例;Hepple分期:Ⅰ期7例,Ⅱ期15例,Ⅲ期5例;病程为(10.6±3.3)个月。所有患者均采用可调式外固定牵伸联合关节镜下微骨折治疗。记录患者术后6、12个月的疼痛视觉模拟评分(VAS)和美国足踝外科协会(AOFAS)踝-后足评分,术后1个月的白细胞介素-1(IL-1)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)等相关炎性因子水平,术后12个月损伤面积及并发症发生情况等。结果本组患者的随访时间为(16.2±6.7)个月。27例患者术前、术后6个月及术后12个月的AOFAS踝-后足评分分别为(61.52±6.75)、(84.15±5.56)、(95.67±4.30)分,疼痛VAS评分分别为(5.88±1.02)、(2.12±0.48)、(0.66±0.36)分,以上项目3个时间点之间两两比较差异均有统计学意义(P<0.05)。27例患者术后1个月IL-1为(32.37±6.64)pg/mL,IL-6为(34.04±7.12)pg/mL,TNF-α为(17.89±4.96)ng/L,均较术前[(96.63±14.80)pg/mL、(102.33±20.42)pg/mL、(54.48±9.33)ng/L]显著降低,差异均有统计学意义(P<0.05)。术后12个月27例患者的损伤面积为(28.66±6.52)mm2,较术前[(128.52±11.32)mm2]显著缩小,差异有统计学意义(P<0.05)。1例患者发生外固定针道感染,2例患者发生下肢深静脉血栓形成。结论可调式外固定牵伸联合关节镜下微骨折治疗OLT可以取得满意的效果,改善患者症状。

Ultrasensitive biosensors based on long-range surface plasmon polariton and dielectric waveguide modes

An ultrasensitive biosensor based on hybrid structure and composed of long-range surface plasmon polariton(LRSPP) and dielectric planar waveguide(PWG) modes is proposed. Both PWG and LRSPP modes have strong resonances to form strong coupling between the two modes, and the two modes can couple to enhance sensitivity of sensors. In the hybrid structure, PWG is composed of cytop–Si–cytop multilayers and the LRSPP configuration is composed of cytop–metal–sensing medium multilayer slabs. The highest imaging sensitivities of 2264 and3619 RIU-1were realized in the proposed sensors based on Au and Al-monolayer graphene, respectively, which are nearly 1.2 and 1.9 times larger than the 1910 RIU-1sensitivity of the conventional LRSPR sensor(LRSPP sensor). Moreover, it is demonstrated that the PWG-coupled LRSPP biosensor is applicable to the sensing medium,with refractive index in the vicinity of 1.34.

目的:比较改良胫骨近端和远端截骨搬移与改良单纯近端截骨搬移治疗Wagner 3/4级糖尿病足的临床疗效。方法:2021年1月至2023年1月收治的40例Wagner 3/4级糖尿病足患者,应用随机数字表法随机纳入观察组(一般治疗+改良胫骨近端和远端截骨搬运)与对照组(一般治疗+改良单纯近端截骨搬移),每组各20例。观察两组患者经相应治疗后的密歇根神经病变筛查仪(MNSI)评分、疼痛视觉模拟量表(VAS)评分、踝肱指数(ABI)、经皮氧分压、足踝部皮温。观察组男12例,女8例;年龄为59~88岁,平均为(72.6±6.4)岁;体重指数(BMI)为22~35 kg/m^(2),平均为(23.4±2.1)kg/m^(2)。术前足背皮肤温度为(27.50±1.08)℃,VAS评分为(7.40±0.36)分,踝肱指数为0.27±0.47,经皮氧分压为(28.4±2.14)mmHg。对照组男11例,女9例;年龄为55~85岁,平均为(76.2±6.9)岁;体重指数为22~36 kg/m^(2),平均为(25.8±1.7)kg/m^(2)。术前足背皮肤温度为(28.10±1.02)℃,VAS评分为(6.80±0.33)分,踝肱指数为0.30±0.64,经皮氧分压为(27.90±2.01)mmHg。两组患者术前基线资料及临床数据差异无统计学意义(P>0.05)。结果:观察组MNSI评分、踝肱指数、经皮氧分压、足踝部皮温高于对照组,VAS评分低于对照组,差异有统计学意义(P<0.05)。结论:改良胫骨近端和远端截骨搬移治疗Wagner 3/4级糖尿病足临床疗效优于改良单纯近端截骨搬移,促进周围血管及周围神经恢复较改良单纯近端截骨搬移快,对于治疗Wagner 3/4级重度糖尿病足患者保肢成功率高,是治疗重度糖尿病足溃疡的理想方法之一。

Clinical Observation of the Hot and Humid Compress Therapy of Traditional Chinese Medicine in the Treatment of Qi Stagnation and Blood Stasis Type of Lumbar Disc Herniation

Background: Although a number of studies have reported that the hot and humid compress from traditional Chinese medicine (TCM) is effective in treating lumbar disc herniation (LDH) with qi stagnation and blood stasis, clinical evidence is limited. Objective: The purpose of this study is to provide high-quality evidence to support the effectiveness of the traditional Chinese hot and humid compress in the treatment of LDH with qi stagnation and blood stasis. Methods: From October 2021 to November 2023, 86 patients with LDH of qi stagnation and blood stasis type were recruited in our hospital and divided into a control (n = 43) and an observation group (n = 43) according to the random number table method. The control group was given routine clinical treatment, and the observation group was treated with the hot and humid compress therapy for two weeks. The visual analogue scale (VAS) score, Japanese Orthopaedic Association (JOA) score, TCM syndrome score, serum interleukin-6 (IL-6), serum interleukin-1β (IL-1β), and tumor necrosis factor-α (TNF-α) were observed and compared between the two groups before and after treatment, and the clinical efficacy of the two groups was evaluated. Results: After treatment, the VAS score, TCM symptom score, and serum IL-6, IL-1β, and TNF-α levels decreased in both groups (P P P P P Conclusions: The hot and humid compress of traditional Chinese medicine can effectively relieve pain, restore lumbar function, improve TCM syndromes, reduce the level of inflammatory factors, and have a curative effect in treating LDH.
