Background Mycoplasma pneumoniae(***)is a significant contributor to community-acquired pneumonia among *** 1968,when a strain of *** resistant to macrolide antibiotics was initially reported in Japan,macrolide-resistant ***(MRMP)has been documented in many countries worldwide,with varying incidence *** infections lead to a poor response to macrolide antibiotics,frequently resulting in prolonged fever,extended antibiotic treatment,increased hospitalization,intensive care unit admissions,and a significantly higher proportion of patients receiving glucocorticoids or second-line *** 2000,the global incidence of MRMP has gradually increased,especially in East Asia,which has posed a serious challenge to the treatment of *** infections in children and attracted widespread attention from ***,there is still no global consensus on the diagnosis and treatment of MRMP in *** We organized 29 Chinese experts majoring in pediatric pulmonology and epidemiology to write the world’s first consensus on the diagnosis and treatment of pediatric MRMP pneumonia,based on evidence *** evidence searches and reviews were conducted using electronic databases,including PubMed,Embase,Web of Science,CNKI,Medline,and the Cochrane *** used variations in terms for“macrolide-resistant”,“Mycoplasma pneumoniae”,“MP”,“***”,“pneumonia”,“MRMP”,“lower respiratory tract infection”,“Mycoplasma pneumoniae infection”,“children”,and“pediatric”.Results Epidemiology,pathogenesis,clinical manifestations,early identification,laboratory examination,principles of antibiotic use,application of glucocorticoids and intravenous immunoglobulin,and precautions for bronchoscopy are *** and rapid identification of gene mutations associated with MRMP is now available by polymerase chain reaction and fluorescent probe techniques in respiratory *** the resistance rate to mac

Factors influencing further vertebral height loss following percutaneous vertebroplasty in osteoporotic vertebral compression fractures:A 1-year follow-up study

BACKGROUND Osteoporotic vertebral compression fractures(OVCFs)contribute to back pain and functional limitations in older individuals,with percutaneous vertebroplasty(PVP)emerging as a minimally invasive ***,further height loss post-PVP prompts investigation into contributing *** To investigate the factors associated with further height loss following PVP with cement augmentation in OVCF *** A total of 200 OVCF patients who underwent successful PVP between January 2021 and December 2022 were included in this study.“Further height loss”during 1 year of follow-up in OVCF patients with bone edema was defined as a vertical height loss of≥4 *** study population was divided into two groups for analysis:The“No Further Height Loss group(n=179)”and the“Further Height Loss group(n=21).”RESULTS In comparing two distinct groups of patients,significant differences existed in bone mineral density(BMD),vertebral compression degree,prevalence of intravertebral cleft(IVF),type of bone cement used,and cement distribution *** from binary univariate regression analysis revealed that lower BMD,the presence of IVF,cleft distribution of bone cement,and higher vertebral compression degree were all significantly associated with further height ***,the use of mineralized collagen modified-poly(methyl methacrylate)bone cement was associated with a significant reduction in the risk of further height *** multivariate regression analysis,lower BMD and the presence of IVF remained significantly associated with further height *** Further height loss following PVP in OVCF patients is influenced by a complex interplay of factors,especially lower BMD and the presence of *** findings underscore the importance of assessing and managing these factors when addressing height loss following PVP in OVCF patients.

BACKGROUND Intracranial arterial narrowing is a significant factor leading to brief episodes of reduced blood flow to the brain,known as transient ischemic attacks,or fullblown *** atherosclerosis is commonly associated with intracranial arterial narrowing,it is frequently of a non-atherosclerotic nature in younger *** SUMMARY Here,we present the case of a young stroke patient with narrowing of the middle cerebral artery(MCA),characterized as non-atherosclerotic lesions,who experienced an ischemic stroke despite receiving standard drug *** patient underwent digital subtraction angiography(DSA)to assess the entire network of blood vessels in the brain,revealing significant narrowing(approximately 80%)in the M1 segment of the right ***,the patient underwent Drug-Coated Balloon Angioplasty to treat the stenosis in the right MCA's M1 ***-up DSA confirmed the resolution of stenosis in this *** the remaining branches showed satisfactory blood flow,the vessel wall exhibited irregularities.A review of DSA conducted six months later showed no evident stenosis in the right MCA,with a smooth vessel *** The use of drug-coated balloon angioplasty demonstrated favorable outcomes in repairing and reshaping the blood vessel wall in young ***,it may be considered a promising treatment option for similar cases.

BACKGROUND Acute decompensation(AD)of cirrhosis is associated with high short-term mortality,mainly due to the development of acute-on-chronic liver failure(ACLF).Thus,there is a need for biomarkers for early and accurate identification of AD patients with high risk of development of ACLF and *** triggering receptor expressed on myeloid cells-1(sTREM-1)is released from activated innate immune cells and correlated with various inflammatory *** To explore the prognostic value of sTREM-1 in patients with AD of *** A multicenter prospective cohort of 442 patients with cirrhosis hospitalized for AD was divided into a study cohort(n=309)and validation cohort(n=133).Demographic and clinical data were collected,and serum sTREM-1 was measured at *** enrolled patients were followed-up for at least 1 *** In patients with AD and cirrhosis,serum sTREM-1 was an independent prognosis predictor for 1-year survival and correlated with liver,coagulation,cerebral and kidney failure.A new prognostic model of AD(P-AD)incorporating sTREM-1,blood urea nitrogen(BUN),total bilirubin(TBil),international normalized ratio(INR)and hepatic encephalopathy grades was established and performed better than the model for end-stage liver disease(MELD),MELD-sodium(MELD-Na),chronic liver failure-consortium(CLIF-C)ACLF and CLIF-C AD ***,sTREM-1 was increased in ACLF and predicted the development of ACLF during first 28-d *** ACLF risk score incorporating serum sTREM-1,BUN,INR,TBil and aspartate aminotransferase levels was established and significantly superior to MELD,MELD-Na,CLIF-C ACLF,CLIF-C AD and P-AD in predicting risk of ACLF *** Serum sTREM-1 is a promising prognostic biomarker for ACLF development and mortality in patients with AD of cirrhosis.

经过长期的宪法变迁,宪法生态环境条款已经形成了包括人权条款、思想理念条款、国家义务条款、组织保障条款在内的较为完整、系统的规范体系。对这一规范体系的认识必须遵循整体性与关联性的原则,不仅要在整体上充分认识生态环境条款规范内涵的历史发展性,也要充分解读生态环境条款内部各要素的侧重点以及各要素之间的内在逻辑。在公民环境权经由人权条款实质确认的背景下,公民环境权在规范层面构成生态环境条款的最终价值指向,推进生态文明建设和美丽中国国家目标的历史进程也就是公民环境权逐渐实现的过程。由此,推进生态环境条款的实施,根本路径是以宪法生态环境条款为指引构建有利于推进生态文明和美丽中国建设进程的生态环境治理制度体系,特别要注重形成持续凝聚全社会生态文明普遍共识的机制、不断构建绿色政绩制度、完善并强化检察环境公益诉讼保障职能。

软岩地基中新型倒肋式风机基础力学性能有限元分析

目的提出一种新型倒肋式风机基础,解决软岩地基中采用梁板式风机基础带来的挖方量大、模板支设繁琐、施工成本高、周期长等问题。方法以2.5 MW风机为例,采用ABAQUS有限元分析软件,建立相同尺寸的倒肋式与梁板式风机基础模型,对比两种基础在极端工况下的倾斜率、水平位移、破坏特征等,并结合挖方量、模板工程量对两者的经济性进行分析。结果倒肋式基础的水平位移为0.444 mm、倾斜率为0.012 1%,均满足规范要求;倒肋式基础的肋梁受力特征与悬臂梁相似,在基底反力作用下肋梁与墩台连接处承受的剪力及弯矩较大,容易开裂破坏,可通过增加肋梁截面高度改善;倒肋式基础相较于梁板式基础土方挖量可节省54%、模板工程量可节省48%。结论倒肋式基础充分发挥出了倒置肋梁与软岩地基之间齿槽嵌固的优势,相较于梁板式基础不仅具有更优的抗滑移稳定性和受力性能,而且减少了挖方和模板工程量,经济效益显著。
