This study was designed to identify clinical predictors of favorable pathology and biochemical recurrence （BCR） in patients with intermediate-risk prostate cancer （IRPCa）. Between 2006 and 2012, clinicopathological and oncological data from 203 consecutive men undergoing robot-assisted radical prostatectomy （RARP） for IRPCa were reviewed in a single-institutional retrospective study. Favorable pathology was defined as Gleason score 〈6 and organ-confined cancer as detected by surgical pathology. Logistic regression analysis was used to determine predictive variables of favorable pathology, and the Kaplan-Meier and multivariate Cox regression model were used to estimate BCR-free survival after RARP. Overall, 38 patients （18.7%） had favorable pathology after RARP. Lower quartile prostate-specific antigen density （PSAD） was associated with favorable pathology compared to the highest quartile PSAD after adjusting for preoperative PSA, clinical stage and biopsy Gleason score （odds ratio, 5.42; 95% confidence interval, 1.01-28.97; P = 0.048）. During a median 37.8 （interquartile range, 24.6-60.2） months of follow-up, 66 patients experienced BCR. There were significant differences with regard to BCR free survival by PSAD quartiles （log rank, P = 0.003）. Using a multivariable Cox proportion hazard model, PSAD was found to be an independent predictor of BCR in patients with IRPCa after RARP （hazard ratio, 4.641; 95% confidence interval, 1.109-19.417; P = 0.036）. The incorporation of the PSAD into risk assessments might provide additional prognostic information and identify some patients in whom active surveillance would be appropriate in patients with IRPCa.

Informational entropy is often identified as physical entropy. This is surprising because the two quantities are differently defined and furthermore the former is a subjective quantity while the latter is an objective one. We describe the problems and then present a possible view that reconciles the two entropies. Informational entropy of a system is interpreted as physical entropy of a whole composed of both the system and “memories” containing information about the system.

Treatment of osteoarthritis(OA)by administration of corticosteroids is a commonly used method in clinics using anti-inflammatory *** administration or intra-articular injection of corticosteroids can reduce the pain and progress of cartilage degeneration,but they are usually insufficient to show local and long-term anti-inflammatory effects because of their fast clearance in the *** this study,we suggest an injectable anti-OA drug depot system for sustained drug release that provides long-term effective therapeutic *** poly(organophosphazene),which has temperature-dependent nanoparticle forming and sol-gel transition behaviors when dissolved in aqueous solution,was synthesized for triamcinolone acetonide(TCA)*** hydrophobic parts of the polymer can interact with hydrophobic parts of the TCA,the TCA was encapsulated into the self-assembled polymeric *** TCA-encapsulated polymeric nanoparticles(TePNs)were well dispersed in an aqueous solution below room temperature so that they can be easily injected as a sol state into an intra-articular ***,the TePNs solution transforms immediately to a viscose 3D hydrogel like a synovial fluid in the intra-articular region via the conducted body *** in vitro TCA release study showed sustained TCA release for six ***-time injection of the TePN hydrogel system in an early stage of OA-induced rat model showed a great inhibition effect against further OA *** OA-induced knees completely recovered as a healthy cartilage without any abnormal symptoms.

Nitric oxide(NO)has been shown to promote revascularization and nerve regeneration after peripheral nerve ***,in vivo application of NO remains challenging due to the lack of stable carrier materials capable of storing large amounts of NO molecules and releasing them on a clinically meaningful time ***,a silica nanoparticle system capable of reversible NO storage and release at a controlled and sustained rate was *** this study,NO-releasing silica nanoparticles(NO-SNs)were delivered to the peripheral nerves in rats after acute crush injury,mixed with natural hydrogel,to ensure the effective application of NO to the *** using a polymer dye and immunohistochemical staining for the detection of CD34(a marker for revascularization)results showed that NO-releasing silica nanoparticles increased revascularization at the crush site of the sciatic *** sciatic functional index revealed that there was a significant improvement in sciatic nerve function in NO-treated *** and anatomical analyses showed that the number of myelinated axons in the crushed sciatic nerve and wet muscle weight excised from NO-treated rats were ***,muscle function recovery was improved in rats treated with *** together,our results suggest that NO delivered to the injured sciatic nerve triggers enhanced revascularization at the lesion in the early phase after crushing injury,thereby promoting axonal regeneration and improving functional recovery.

Upgrading and upstaging of low-risk prostate cancer among Korean patients： a multicenter study

Only 54% of prostate cancer cases in Korea are localized compared with 82% of cases in the US. Furthermore, half of Korean patients are upgraded after radical prostatectomy （41.6%-50.6%）. We investigated the risk factors for upgrading and/or upstaging of low-risk prostate cancer after radical prostatectomy. We retrospectively reviewed the medical records of 1159 patients who underwent radical prostatectomy at five hospitals in Honam Province. Preoperative data on standard clinicopathological parameters were collected. The radical prostatectomy specimens were graded and staged and we defined a ＂worsening prognosis＂ as a Gleason score ≥ 7 or upstaging to ≥ pT3. Multivariate logistic regression models were used to assess factors associated with postoperative pathological upstaging. Among the 1159 patients, 324 were classified into the clinically low-risk group, and 154 （47.5%） patients were either upgraded or upstaged. The multivariable analysis revealed that the preoperative serum prostate-specific antigen level （odds ratio [OR], 1.131; 95% confidence interval [CI], 1.007-1.271; P = 0.037）, percent positive biopsy core （OR： 1.018; 95% CI： 1.002-1.035; P = 0.032）, and small prostate volume （≤30 ml）（OR： 2.280; 95% CI： 1.351-3.848; P = 0.002） were predictive of a worsening prognosis. Overall, 47.5% of patients with low-risk disease were upstaged postoperatively. The current risk stratification criteria may be too relaxed for our study cohort.

The phototropins phot1 and phot2 are plant blue-light receptors that mediate phototropism, chloroplast movements, stomatal opening, leaf expansion, the rapid Inhibition of hypocotyl growth in etiolated seedlings, and possibly solar tracking by leaves in those species in which It occurs. The phototroplns are plasma membrane-associated hydrophilic proteins with two chromophore domains （designated LOV1 and LOV2 for their resemblance to domains In other signaling proteins that detect light, oxygen, or voltage） in their Nterminal half and a classic serine/threonlne kinase domain in their C-terminal half. Both chromophore domains bind flavin mononucleotide （FMN） and both undergo light-activated formation of a covalent bond between a nearby cystelne and the C（4a） carbon of the FMN to form the signaling state. LOV2-cystelnyl adduct formation leads to the release downstream of a tightly bound amphlpathlc α-helix, a step required for activation of the klnase function. This cysteinyl adduct then slowly decays over a matter of seconds or minutes to return the photoreceptor chromophore modules to their ground state. Functional LOV2 is required for light-activated phosphorylation and for various blue-light responses mediated by the phototroplns. The function of LOV1 is still unknown, although It may serve to modulate the signal generated by LOV2. The LOV domain Is an ancient chromophore module found In a wide range of otherwise unrelated proteins In fungi and prokaryotes, the latter Including cyanobacterla, eubacterla, and archaea. Further general reviews on the phototropins are those by Celaya and Liscum （2005） and Christie and Briggs （2005）.

Silica thin films synthesized sol–gel process are proposed as flexible encapsulation materials.A sol–gel process provides a dense and stable amorphous silica structure,yielding an extremely high elastic deformation limit of 4.9%and extremely low water vapor transmission rate(WVTR)of 2.90×10^(−4)g/(m^(2)∙day)at 60℃and relative humidity of 85%.The WVTR is not degraded by cyclic bending deformations for the bending radius corresponding to a tensile strain of 3.3%in the silica encapsulation film,implying that the silica thin film is robust against the formation of pinhole-type defects by cyclic bending *** organic solar cells encapsulated with the silica films operate without degradation in power conversion efficiency for 50,000 bending cycles for a bending radius of 6 mm.

With the development of wireless communications and positioning technologies, tracking the positions of moving objects has increased necessary. This paper proposes a Cache-Conscious TPR -Link tree called CTPR Link tree which store in main memory. To satisfy continuous movement, the QRMBR definition is modified. The compression leads to the reduction of the tree height, which improves the cache behavior of the index and reduces the memory access time. In order to achieve high concurrency control, optimistic dynamic versioning and sibling-link scheme is presented, which not only enable read-only transactions not to fail with latch-free but also reduce cache misses during index updates.

To investigate the characteristic radiologic findings of cryptogenic multifocal ulcerous stenosing enteritis (CMUSE) which can be differentiated from other similar bowel disease and to assess their clinical behavior. METHODSTwenty pathologically and clinically confirmed CMUSE patients (males:females = 8:12; mean age: 40.4 years) between March 2002 and August 2015 from seven academic centers in South Korea were retrospectively reviewed. We evaluated small bowel series (SBS; n = 25), computed tomography (CT) enterography (n = 21), magnetic resonance (MR) enterography (n = 2), and abdominopelvic CT (n = 18) images, focusing on enteric and perienteric manifestations. Any change in radiologic features during follow-up period was recorded. We evaluated clinical data including presenting symptoms, laboratory finding and presence of relapse from electronic medical records. Histopathologic findings were also evaluated. RESULTSThe main symptoms were abdominal pain (n = 12) and anemia (n = 10). All patients showed small bowel strictures (n = 52, mean: 2.6 per patient) on initial CT/MR, located in the ileum (n = 47) or jejunum (n = 5). Strictures showed short-length (mean: 10.44 mm) and circumferential bowel wall thickening (mean: 5.56 mm) with layered enhancement (n = 48) that were also noted on initial SBS (n = 36) with shallow ulcers (n = 10). Some ulcerative lesions or wall thickening progressed into strictures on follow-up SBS/CT, and some strictures revealed recurrent ulceration on follow-up SBS. There were no penetrating disease features like fistula or abscess and no gastrointestinal tract involvement except the small bowel. Nine patients experienced disease recurrence (median relapse-free period: 32 mo) even post-operatively. Histopathologic features of surgically resected specimens were characterized as multiple superficial ulcerations confined to mucosa or submucosa and multiple strictures.
