BACKGROUND Emergency pancreaticoduodenectomy(EPD)is a rare event for complex periam-pullary *** intraoperative blood loss is correlated with poor post-operative *** SUMMARY Two patients underwent EPD using a no-touch isolation technique,in which all arteries supplying the pancreatic head region were ligated and divided before manipulation of the pancreatic head and *** operative times were 220 and 239 min,and the blood loss was 70 and 270 g,*** patients were discharged on the 14^(th) and 10^(th) postoperative day,***-two patients underwent EPD for the treatment of neoplastic *** mean operative time was 361.6 min,and the mean blood loss was 747.3 *** comp-lication rate was 37.5%.The in-hospital mortality rate was 9.38%.CONCLUSION The no-touch isolation technique is feasible,safe,and effective for reducing intraoperative blood loss in EPD.

Functional Confirmation Using a Medical X-Ray System of a Semiconductor Survey Meter

In recent years, semiconductor survey meters have been developed and are in increasing demand worldwide. This study determined if it is possible to use the X-ray system installed in each medical facility to calculate the time constant of a semiconductor survey meter and confirm the meter’s function. An additional filter was attached to the medical X-ray system to satisfy the standards of N-60 to N-120, more copper plates were added as needed, and the first and second half-value layers were calculated to enable comparisons of the facility’s X-ray system quality with the N-60 to N-120 quality values. Next, we used a medical X-ray system to measure the leakage dose and calculate the time constant of the survey meter. The functionality of the meter was then checked and compared with the energy characteristics of the meter. The experimental results showed that it was possible to use a medical X-ray system to reproduce the N-60 to N-120 radiation quality values and to calculate the time constant from the measured results, assuming actual leakage dosimetry for that radiation quality. We also found that the calibration factor was equivalent to that of the energy characteristics of the survey meter.

BACKGROUND high-grade pancreatic intraepithelial neoplasia(PanIN)exhibits no mass and is not detected by any examination ***,it can be diagnosed by pancreatic juice cytology from indirect *** previous cases were diagnosed based on findings of a focal stricture of the main pancreatic duct(MPD)and caudal MPD dilatation and subsequent pancreatic juice cytology using endoscopic retrograde cholangiopancreatography(ERCP).We experienced a case of high-grade PanIN with an unclear MPD over a 20-mm range,but without caudal MPD dilatation on magnetic resonance cholangiopancreatography(MRCP).CASE SUMMARY A 60-year-old female patient underwent computed tomography for a follow-up of uterine cancer post-excision,which revealed pancreatic *** revealed an unclear MPD of the pancreatic body at a 20-mm length without caudal MPD ***,course observation was *** 24 mo,MRCP revealed an increased caudal MPD caliber and a larger pancreatic *** performed ERCP and detected atypical cells suspected of adenocarcinoma by serial pancreatic juice aspiration cytology *** performed a distal pancreatectomy and obtained a histopathological diagnosis of high-grade *** parenchyma invasion was not observed,and curative resection was *** high-grade Pan-IN may cause MPD narrowing in a long range without caudal MPD dilatation.

Tests with two different sized models were conducted in the Marine Propeller Cavitation Tunnel at the Univer- sity of *** two angles of attack the developed sheet-cloud and bubble cavitation *** pattern was recorded by pure indium specimens and noise was measured with a hydrophone mounted inside the *** broadband SPL was mainly used for correlation of noise with pit counting results from *** case of sheet- cloud cavitation power law relations between velocity and SPL have been found and there existed a linear relation- ship between SPL values and IgPR values(PR-pitting rate).however in case of bubble cavitation such power law relation does not exist between pitting rate and velocity,and neither between SPL and *** was noticed that the different trends of SPL for these two different types of cavitation could be qualitatively linked to their pitting rates by using some SPL values at high center frequencies.

BackgroundWe clarified the predictive factors for changes in the status of medications for lower urinary tract symptoms (LUTS) 2 years after local radiotherapy for nonmetastatic prostate cancer. Materials and methodsWe retrospectively included patients who underwent local external radiotherapy for nonmetastatic prostate cancer in 8 institutions between April 2001 and March 2016. Patients were divided into the medication and no-medication group based on the use of drugs for LUTS before radiotherapy. We defined improvement of LUTS as when the patient did not require medication for LUTS at 24 months after radiotherapy in the medication group and as deterioration when medication was required in the no-medication group. Logistic regression analysis was used to evaluate predictive factors for changes in medication status. ResultsAltogether, 505 patients were divided into a no-medication group (n = 352) and a medication group (n = 153). The number of patients with deterioration and improvement in LUTS was 49 (14%) and 36 (23%), respectively. In the multivariate analysis, the predictive variables for deterioration were the International Prostate Symptom Score (>= 8; odds ratio [OR], 2.21; p = 0.014) and the biopsy Gleason score (he no-medication group, whereas those for improvement were age (he quality of life score (= 50%; OR, 2.530; p = 0.027) in the medication group. ConclusionsThese predictive factors for changes in the status of medications for LUTS at 2 years after external radiotherapy may help determine the definitive therapy for nonmetastatic prostate cancer.

In-situ analysis for SiC bulk single crystal growth was reported using vertical X-ray diffractometer system. A furnace for SiC sublimation growth combined with the XRD system which possessed three kinds of functions including topography, rocking curve measurement and crystal growth rate monitoring was developed. These functions could contribute as a powerful tool finding the optimum growth condition by dynamic observation in the crucible. In this study, the in-situ X-ray topographs succeeded to capture dynamic elongation of defects and dislocation generated in the SiC growing crystals. The in-situ rocking curve measurement reviled appearance of mosaic structure in the SiC crystal grown with high growth rate. The in-situ growth rate monitoring also succeeded very precisely using the direct X-ray beam absorption. On the base of findings and facts obtained by the in-situ observations, the importance for the SiC growth was discussed.

The friction characteristics of a shock absorber are very complex because the reciprocating motion is not always *** this study,a device was developed and used to analyze the dynamic friction characteristics under various reciprocating sliding conditions to determine the sliding materials and hydraulic oils that improve the shock absorber *** study describes the influence of hydraulic oil additive on the fine reciprocating friction characteristics of steel and copper *** oils were prepared by blending a paraffinic mineral oil with zinc dithiophosphate(ZnDTP)and polyhydric alcohol ester as *** results show that the dynamic frictional characteristics vary mainly depending on the additive concentration.A specific additive formulation induces a unique amplitude-dependent friction *** addition,the influence of different additives on the lubrication mechanism is investigated based on the instrumental analysis of the friction surface.

highly precise (σ ~1 mm) temporal deformation measurements are taken across the Xianshuihe fault from two pairs of continuous GPS stations straddling the fault. Baseline vector changes of the two pairs of stations show clearly the difference in deformation behavior between the Qianning and Daofu segments of the fault: the former deforms steadily, and the latter deforms with a strong transient component. The transient deformation across the Daofu segment is possibly related to its irregular geometry, where the fault splits into two branches, that is, the east and west branches. An attempt is made to interpret the baseline vector changes using a kinematic fault model composed of a brittle layer in the upper crust, a ductile layer in the lower crust, and a transition zone in between. The slip in the transition zone of the south segment of the Xianshuihe fault is steady. The slips in the transition zones of the north and Daofu segments of the Xianshuihe fault, however, are not steady, and the average slip rates there are higher than that of the south segment. The difference in deformation behavior is probably associated with the rheological properties of the fault interface, suggesting that the overall fault strength of the south segment is greater than those of the north and Daofu segments, corresponding to longer earthquake recurrence time.

In this study, the inhomogeneous dealloying phenomenon during the liquid metal dealloying(LMD) was investigated using FeNi+Mg and(FeCo)Ni+Mg systems. For the FeNi+Mg system, the inhomogeneous dealloying and wetting of Mg melt occurred along triple junction(TJ) and grain boundary(GB).Temperature increase enhances the inhomogeneous dealloying kinetics and leads to the formation of the plate-shaped abnormal ligaments at the GB region. The energy banlance between a GB energy(γGB)and solid-liquid interface energies(γsl) is the key factor governing the inhomogeneous dealloying and wetting. Particularly, the low-energy twin boundaries were unaffected by the inhomogeneous ***, precursor microstructure is an important factor determining the final morphology of dealloyed material as well as its physical properties. In the case of the(FeCo)Niprecursor, all TJ and GB were stable against the preferred penetration of Mg melt from 600 ℃ to 800 ℃. It was concluded that a minor addition of alloying elements(VorCr) changes GB characteristics as well as γslof the precursor ***, this significantly influences dealloying mechanisms and final morphology of the dealloyed material. The current findings demonstrate the importance of GB engineering in the precursor materials for the technological application of liquid metal dealloying for the synthesis of advanced structural and functional materials.
