middle East respiratory syndrome coronavirus （mERS-CoV） is a betacoronavirus which can cause acute respiratory distress in humans and is associated with a relatively high mortality rate. Since it was first identified in a patient who died in a Jeddah hospital in 2012, the World health Organization has been notified of 1735 laboratory-confirmed cases from 27 countries, including 628 deaths. most cases have occurred in Saudi Arabia. mERS-CoV ancestors may be found in Old World bats of the Vespertilionidae family. After a proposed bat to camel switching event, transmission of mERS-CoV to humans is likely to have been the result of multiple zoonotic transfers from dromedary camels. human-to-human transmission appears to require close contact with infected persons, with outbreaks mainly occurring in hospital environments. Outbreaks have been associated with inadequate infection prevention and control implementation, resulting in recommendations on basic and more advanced infection prevention and control measures by the World health Organization, and issuing of government guidelines based on these recommendations in affected countries including Saudi Arabia. Evolutionary changes in the virus, particularly in the viral spike protein which mediates virus-host cell contact may potentially increase transmission of this virus. Efforts are on-going to identify specific evidence-based therapies or vaccines. The broad-spectrum antiviral nitazoxanide has been shown to have in vitro activity against mERS-CoV. Synthetic peptides and candidate vaccines based on regions of the spike protein have shown promise in rodent and non-human primate models. GLS-5300, a prophylactic DNA-plasmid vaccine encoding S protein, is the first mERS-CoV vaccine to be tested in humans, while monoclonal antibody, m336 has given promising results in animal models and has potential for use in outbreak situations.

Dynamic relaxation processes are fundamental to understand the mechanical and physical properties of metallic glasses. In the current work, mechanical relaxations in a La30Ce30Al15Co25 bulk metallic glass were probed by dynamic mechanical analysis. In contrast to many metallic glasses, La30Ce30Al15Co25 metallic glass shows a pronounced slow β relaxation peak. Physical aging below the glass transition temperature Tgleads to an increase of the apparent activation energy and a decrease of the slow β relaxation magnitude. The correlation between the slow β relaxation and the main α relaxation is discussed.

Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIm To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

TiNi-based shape memory alloys(SmAs)have been used as damping materials to eliminate noise and mechanical ***,their application is limited by low working temperatures and damping *** this work,two novel Ti-Zr-hf-Ni-Co-Cu high entropy shape memory alloys(hESmAs)with different transformation temperatures and damping properties were *** results show that Ti_(25)Zr_(8)hf_(17)Ni_(30)Co_(5)Cu_(15) has superior damping performance arising from martensitic transformation,shape memory effect(thermal cycle at constant load)as well as *** to traditional TiNi-based SmAs,the as-cast hESmAs exhibit a much higher ultrahigh yield strength(∼2 GPa)and storage modulus(∼50 GPa).The high configuration entropy of the hESmAs with high uneven internal stress and severe lattice distortion is revealed as the underlying mechanisms governing distinctive damping *** effects of high configuration entropy and microheterogeneity on the martensitic transforma-tion behavior and damping performance of hESmAs are clarified in this work,which provides a basis for designing alloys with superior damping properties.

Non-equiatomic high entropy bulk metallic glasses were reported recently and show unique mechanical and physical *** mechanical relaxation behavior of Zr_(35)hf_(17.5)Ti_(5.5)Al_(12.5)Co_(7.5)Ni_(12)Cu_(10)high entropy bulk metallic glass was investigated by dynamic mechanical analysis(DmA)and the mechanical spectra could be well described by the quasi-point defects(QPD)*** to typical metallic glasses,the intensity of theβrelaxation of Zr_(35)hf_(17.5)Ti_(5.5)Al_(12.5)Co_(7.5)Ni_(12)Cu_(10)high entropy bulk metallic glass is lower due to the sluggish *** the same time,the correlation factor is higher than that of conventional metallic glasses and this is ascribed to the high configuration *** parallel,physical aging below the glass transition temperature leads to a decrease of atomic mobility,caused by a decrease of the concentration of defects.

Analysis of the anelastic deformation of high-entropy Pd_(20)Pt_(20)Cu_(20)Ni_(20)P_(20) metallic glass under stress relaxation and recovery

The anelastic deformation behavior of Pd_(20)Pt_(20)Cu_(20)Ni_(20)P_(20) high-entropy metallic glass was probed by monitoring the stress relaxation and recovery processes. The stress relaxation under consecutive strain steps can be described by the Kohlrausch-Williams-Watts(KWW) function. In addition, considering a hierarchy of relaxation processes related to the structural heterogeneity, a constitutive model is proposed in order to describe the whole process of stress relaxation and determine the contribution of different time scales. moreover, a crossover from stochastic activation to percolation of flow defects with the ultimate strain can be observed during stress relaxation process. The anelastic recovery process after a strain step is studied as a function of the initial strain level and characterized by means of a direct spectrum analysis. The peaks in the recovery time-spectra revealed the evolution of flow defects in Pd_(20)Pt_(20)Cu_(20)Ni_(20)P_(20) high-entropy metallic glass. The understanding of the atomic free-volume zones effect and the anelastic deformation provides important insight into how atomic structural features affect the deformation behavior of high-entropy metallic glasses, and may provide a new avenue into the improvement of their mechanical properties.

Caroli Disease: A Case Report Observed at the Cocody Teaching hospital of Abidjan (Cote D’Ivoire)

Caroli disease is a rare affection. It’s commonly associated to polycystic kidneys. We report a case of a nine-year-old girl. She was hospitalized at the pediatric unit of the Cocody Teaching hospital for an intermittent fever. During the clinical examination, we found a likely cirrhotic hepatomegaly associated to an edematous-ascitic syndrome. Biological exams showed an infectious syndrome with a hyperleukocytosis at 13.000 per mm3, a hyperneutrophilia at 9.600 per mm3. Serological viral markers of B, C and D hepatitis were negative. We saw an intra-hepatic cystic picture at the liver CT-scan with a vascular lesion at his center called a “dot sign”. The diagnosis of Caroli disease could be evocated.

Reactivation of Tuberculosis during Dual Therapy with Pegylated Interferon and Ribavirin for Chronic hepatitis C about a Case

The infection by the virus of the hepatitis C can be associated with other infectious diseases, including the viral and bacterial infections of which the tuberculosis. The infection to mycobacterium tuberculosis can remain latent during several years and show itself during a state of immunesuppression. The role of the antiviral treatment in the reactivation of the tuberculosis is debated. We bring back an observation of tubercular reactivation during dual therapy with pegylated interferon and ribavirin for a viral chronic hepatitis C. A virologique answer slow was obtained in the 40th week, as well as the reactivation of the tuberculosis. The tuberculosis was of ganglionic localization at our patient. The antituberculous treatment was established while maintaining the treatment. The evolution of the tuberculosis under treatment was favorable but for the VhC, we noted a virologique answer with premature relapse.

Duodenal metastases of malignant melanoma: One Case Report

The duodenal metastases of a malignant melanoma are rare. We report a case at a 86-year-old, hypertense patient, without tumoral history, who was hospitalized in our department for a change of the general state and an anaemia ferriprive without found home (foyer) of display. In the clinical examination, we had found anemia syndrome, nodules under cutaneous about 2 cm in diameter at the level of the thorax, of the belly, the members and of the lower lip partners in a painless left adenopathy axillary about 3 cm in diameter and an absence of méléna in the rectal touch. The gastroscopy had highlighted polypoides ulcéro-bourgeonnantes hurts with a blackish pigmentation by places at the level of the bulb and of the second duodenum. In the histology of biopsie fragments, we noted a fuso-cellular proliferation and epithelioid with deposits of melanin. The positivity of antibodies anti-protein S100 and anti hmB45 in the immunohistochemical study allowed confirming the diagnosis. The gastroduodenal hurts, persons suffering from a liver complaint and lungworts in the scanner testify of a terminal metastatic scattering. The death arose two months later, in a picture of circulatory collapse further to a high digestive bleeding of big abundance.
