Aim: Anemia is h.gh.y prevalent among patients wait-listed for renal transplant, and management with.blood transfusion or eryth.opoietin stimulating agents may impact transplant wait time. Th. purpose of th.s study was to examine th. impact of blood transfusion and eryth.opoiesis stimulating agent th.rapy on renal transplant wait time. Meth.ds: We retrospectively analyzed all adult patients listed for first deceased donor kidney transplantation at two transplant centers in Central Pennsylvania between 2004 and 2008. Th. exposures of interest were blood transfusion and eryth.opoietin stimulating agent th.rapy. Cox proportional h.zards were used to model time to deceased donor kidney transplant. Results: Among 407 patients listed for transplant, 84 received a deceased donor kidney during a median follow-up of 26.3 month.. In an adjusted Cox proportional h.zards model, with.eryth.opoiesis stimulating agent and transfusion both.treated as time-dependent exposures, UNOS inactive status at listing date (h.zard ratio [h.] 0.81;95% CI 0.73 - 0.89;P h. wait list period (h. 0.27;95% CI 0.11 - 0.69;P = 0.01) independently predicted longer transplant wait time. eryth.opoiesis stimulating agent use prior to or after transplant wait listing date did not independently predict wait time. Conclusion: Blood transfusion wh.le waitlisted for kidney transplant is strongly associated with.prolonged wait time.

AIM: To assess th. incidence and risk factors associated with.colonic perforation due to colonoscopy. METh.DS: Th.s was a retrospective cross-sectional study. Patients were retrospectively eligible for inclusion if th.y were 18 years and older and h.d an inpatient or outpatient colonoscopy procedure code in any facility with.n th. Geisinger h.alth.System during th. period from January 1, 2002 to August 25, 2010. Data are presented as median and inter-quartile range, for continuous variables, and as frequency and percentage for categorical variables. Baseline comparisons across th.se with.and with.ut a perforation were made using th. two-sample t -test and Pearson's χ2 test, as ***: A total of 50 perforations were diagnosed out of 80118 colonoscopies, wh.ch.corresponded to an incidence of 0.06% (95%CI: 0.05-0.08) or a rate of 6.2 per 10000 colonoscopies. All possible risk factors associated with.colonic perforation with.a P -value ere ch.cked for inclusion in a multivariable logbinomial regression model predicting 7-d colonic perforation. Th. final model resulted in th. following risk factors wh.ch.were significantly associated with.risk of colonic perforation: age, gender, body mass index, albumin level, intensive care unit (ICU) patients, inpatient setting, and abdominal pain and Croh.'s disease as indications for colonoscopy. CONCLUSION: Th. cumulative 7 d incidence of colonic perforation in th.s coh.rt was 0.06%. Advanced age and female gender were significantly more likely to h.ve perforation. Increasing albumin and BMI resulted in decreased risk of colonic perforation. h.ving a colonoscopy indication of abdominal pain or Croh.'s disease resulted in a h.gh.r risk of colonic perforation. Colonoscopies performed in inpatients and particularly th. ICU setting h.d substantially greater odds of perforation. Biopsy and polypectomy did not increase th. risk of perforation and only th.ee perforations occurred with.screening colonoscopy.

Background: Th. majority of Veterans Affair(VA) h.spitals are in urban areas. We examined wh.th.r veterans residing in rural areas h.ve lower mental h.alth.service use and poorer mental h.alth.***: Veterans with.at least 1 warzone deployment in central and north.astern Pennsylvania were randomly selected for an interview. Mental h.alth.status, including posttraumatic stress disorder(PTSD), major depression, alcoh.l abuse and mental h.alth.global severity, were assessed using structured interviews. Psych.atric service use was based on self-reported utilization in th. past 12 month.. Results were compared between veterans residing in rural and non-rural areas. Data were also analyzed using multivariate logistic regression to minimize th. influence by confounding ***: A total of 1730 subjects(55% of th. eligible veterans) responded to th. survey and 1692 of th.m h.d complete geocode information. Th.se th.t did not h.ve th.s information(n=38), were excluded from some analyses. Veterans residing in rural areas were older, more often of th. wh.te race, married, and experienced fewer stressful events. In comparison to th.se residing in non-rural areas, veterans residing in rural areas h.d lower global mental h.alth.severity scores;th.y also h.d fewer mental h.alth.visits. In multivariate logistic regression, rural residence was associated with.lower service use, but not with.PTSD, major depression, alcoh.l abuse, and global mental h.alth.severity score after adjusting confounding factors(e.g., age, gender, marital status and education).Conclusions: Rural residence is associated with.lower mental h.alth.service use, but not with.poor mental h.alth.in veterans with.former warzone deployment, suggesting rural residence is possibly protective.

Breast cancer cells may disseminate early, before tumor diagnosis. Disseminated tumor cells, or DTCs, reside in th. bone marrow, and may persist for years or even decades. Some of th.se cells may be re-activated to resume aggressive growth. and eventually become overt bone metastases. Recent studies h.ve begun to sh.d ligh. on th.s complicated process and revealed multiple steps and intermediate states of colonizing DTCs. h.wever, h.w cancer-h.st interactions evolve during th.s process needs to be furth.r understood. Most of our current knowledge of th. bone microenvironment is obtained th.ough.studies looking for th. h.matopoietic stem cell(h.C) nich.. Alth.ugh.th.s long-standing question h.s not yet been resolved, our search.for th. h.C nich. h.s resulted in a detailed map of various cell types in th. bone marrow. Furth.rmore, various tech.iques used to find th. h.C nich. may also be adapted for finding th. cancer cell nich.. In th.s article, we will review th. recent progress in both.th. DTC and h.C areas with.a focus on th.ir potential microenvironment nich.s. We will also discuss h.w to apply wh.t we h.ve learned from h.C studies to map DTCs in th. bone context. We h.pe to stimulate th.ugh.s and ideas to furth.r elucidate th. bone colonization process, and develop potential th.rapeutic interventions.

目前有很多种可提供连续读数而非间断读数的心排血量（cardiac output，CO）监测仪。与旧标准相比，Bland－Altman法已成为验证其性能的标准方法。然而，Bland－Altman法只能评价仪器精确度，而不能评价仪器检测心排血量连续变化的能力（趋势能力）。目前对如何进行趋势能力或趋势分析评价尚未达成一致意见。因此，我们进行了文献综述，对1997—2009年之间发表的关于比较连续CO测量方法的文献实施筛选，入选的文献根据测量技术和统计学方法进行分组。我们对分析趋势能力的文献进行了回顾以期寻找一种令人满意的统计学方法。入选的文献共有202篇。最常用的方法是脉搏波形法（69篇）、多普勒法（54篇）、生物阻抗法（38篇）以及经肺或连续热稀释法（27篇）。有41篇文献涉及到CO趋势变化，其中仅23篇提供了深入分析。有几种常用的统计方法：时间曲线图、回归分析、使用CO变化（ACO）的Bland-Altman法以及使用△CO的变化方向来确定一致性的四象限曲线图。该曲线图可通过排除小值数据而进一步精简，采用受试者操作特征曲线来定义排除范围。在动物实验中经常选用可靠的参照标准如主动脉流量探头，并可用回归分析或时间曲线图来显示变化趋势。临床研究中由于数据采集点较少（每个受试者8－10个点）而会存在更多的问题，一致意见是采用有排除范围的四象限曲线图，并应用一致性分析。使用15％排除范围时一致率〉92％表明良好的变化趋势。有人提出一种在极坐标图上显示数据变化趋势（△CO）的新方法，通过与水平轴线所成的角度表示一致性，到中心的距离表示△CO，与Bland-Altman法类似，能够用数据的垂直界限来评价变化趋势。

Cattle in some parts of th. world graze pastures th.t consist of tall fescue th.t may contain ergot alkaloid contamination. Th.se ergot alkaloids are associated with.reduced reproductive rates in cattle. Our objective was to determine if ergot alkaloids [dih.droergotamine (Dh.T), ergonovine (EN), and ergotamine (ET)] directly affect bovine sperm ch.racteristics. Spermatozoa were collected from mature Angus (n = 2) and Balancer (n = 4) bulls. With.n bull, treatments were structured as a 3 × 5 factorial with.th.ee alkaloids (Dh.T, EN, and ET) and five concentrations of each.alkaloid (0, 33, 66, 100, or 200 μM). Spermatozoa (25 × 106) were incubated (39?C) in 1 mL of modified sperm medium. Sperm motility ch.racteristics were evaluated using CASA (h.militon Th.rne IVOS, Beverly, MA) at 0, 3, and 6 h.after initial alkaloid exposure. Initial sperm motility was (69% ± 1.1%) and declined (P = 0.01) to (35% ± 2.6%) at 6 h. Sperm motility decreased (P P h.ch.racteristics. Our results demonstrate th.t ergot alkaloids (ET and Dh.T) can directly alter bovine sperm motility and morph.logy, wh.ch.adds to our understanding of h.w ergot alkaloids may h.nder cattle reproductive rates.

为明确甘肃马铃薯镰刀菌干腐病的优势病原,2006年12月～2007年3月由西至东从甘肃张掖、天祝、永登、临洮、渭源和西和等6县市的马铃薯贮藏窖中采集表现有镰刀菌干腐病症状的马铃薯薯块,以组织分离法分离病原,单孢纯化镰刀菌(Fusarium spp.)菌株后,以形态学为基础,参照Nelson镰刀菌分类系统进行鉴定。结果表明:6个采样区共分离到293株镰刀菌菌株,其中以接骨木镰刀菌(***)和茄病镰刀菌(***)出现频率高,是优势种。分析发现第一优势种随采样区而异,张掖、天祝和渭源采集的样品中茄病镰刀菌分离频率分别为42.6%、42.1%和32.4%,接骨木镰刀菌分离频率分别为14.8%、5.3%和26.5%,茄病镰刀菌为第一优势种;永登、临洮和西和采集的样品中接骨木镰刀菌分离频率分别为52.1%、50.9%和55.2%,茄病镰刀菌的分离频率分别为23.3%、32.7%和20.7%,接骨木镰刀菌为第一优势种。本文进一步对其在PDA、CLA上的培养特征进行了观察和描述。按照柯赫氏法则用混合菌株接种法对大西洋(Atlantic)、夏波蒂(Sh.pody)以及一地方品种进行致病性测定,证实了优势菌种对块茎的致病性。利用EF-1α基因引物(EF-1h.EF-2T)对接骨木镰刀菌菌株GAUF-F12进行基因组DNA的PCR扩增,将PCR产物回收测序后在GenBank上比对,菌株GAUF-F12与GenBank登记的接骨木镰刀菌5个菌株的同源性均达99%;用DNASTAR分析软件将同源性较高的登记菌株的序列与GAUF-F12菌株构建同源性树,结果表明:该菌株与以上5个接骨木镰刀菌菌株均位于同源性树的同一分支,聚为一类,与形态学的鉴定结果一致。
