S100B protein in the gut:The evidence for enteroglial-sustained intestinal inflammation

Glial cells in the gut represent the morphological and functional equivalent of astrocytes and microglia in the central nervous system （CNS）. In recent years, the role of enteric glial cells （EGCs） has extended from that of simple nutritive support for enteric neurons to that of being pivotal participants in the regulation of inflammatory events in the gut. Similar to the CNS astrocytes, the EGCs physiologically express the SIOOB protein that exerts either trophic or toxic effects depending on its concentration in the extracellular milieu. In the CNS, SIOOB overexpression is responsible for the initiation of a gliotic reaction by the release of pro-inflammatory mediators, which may have a deleterious effect on neighboring cells. SlOOB-mediated pro-inflammatory effects are not limited to the brain： SIOOB overexpression is associated with the onset and maintenance of inflammation in the human gut too. In this review we describe the major features of EGCs and SIOOB protein occurring in intestinal inflammation deriving from such.

Irritable bowel syndrome(IBS) is one of the most common gastrointestinal disorders in Western countries. Despite the high prevalence of this disorders, the therapeutic management of these patients is often unsatisfactory. A number of factors have been suggested to be involved in the pathogenesis of IBS, including impaired motility and sensitivity, increased permeability, changes in the gut microbiome and alterations in the brain-gut axis. Also food seems to play a critical role: the most of IBS patients report the onset or the exacerbation of their symptoms after the meals. Recently, an increasing attention has been paid to the role of food in IBS. In this review we summarize the most recent evidences about the role of diet on IBS symptoms. A diet restricted in fermentable, poorly absorbed carbohydrates and sugar alcohols has beneficial effects on IBS symptoms. More studies are needed to improve our knowledge about the relationship between food and IBS. However, in the foreseeable future, dietary strategies will represent one of the key tools in the therapeutic management of patients with IBS.

Chronic intestinal pseudo-obstruction(CIPO) is a rare disease due to a severe gastrointestinal motility disorder which may mimic,on both clinical and radiological grounds,mechanical *** report a case of a 26-year-old woman who presented to our institution for plain abdominal radiography for referred long-lasting constipation with recurrent episodes of abdominal pain and *** X-ray,performed both in the upright and supine position,an isolated air-fluid level was depicted in the left flank,together with a number of radiological signs suggestive of ***,subphrenic radiolucency could be observed in the upright ***,the intestinal wall of some jejunal loops appeared to be outlined in the right ***,the inferior cardiac border was clearly depicted in the upright *** patient however had no evidence ofperitoneal signs but only hypoactive bowel *** multi-detector computed tomography(MDCT) of the abdomen and pelvis was therefore *** revealed abnormal air-driven distension of the small and large bowel,without evidence of extraluminal *** radiological signs of pneumoperitoneum turned out to be false-positive *** patient was submitted to pan-colonoscopy and to anorectal manometry to rule out Hirshprung's disease,and was finally discharged with a diagnosis of CIPO.

Hypervigilance and symptoms anticipation,visceral hypersensitivity and gastroduodenal sensorimotor abnormalities account for the varied clinical presentation of functional dyspepsia(FD)*** patients recognize meals as the main triggering factor;thus,dietary manipulations often represent the first-line management strategy in this cohort of ***,scarce quality evidence has been produced regarding the relationship between specific foods and/or macronutrients and the onset of FD symptoms,resulting in nonstandardized nutritional *** dietary advises are indeed empirical and often lead to exclusion diets,reinforcing in patients the perception of“being intolerant”to food and self-perpetuating some of the very mechanisms underlying dyspepsia physiopathology(i.e.,hypervigilance and symptom anticipation).Clinicians are often uncertain regarding the contribution of specific foods to dyspepsia physiopathology and dedicated professionals(i.e.,dietitians)are only available in tertiary referral *** in turn,can result in nutritionally unbalanced diets and could even encourage restrictive eating behaviors in severe *** this review,we aim at evaluating the relationship between dietary habits,macronutrients and specific foods in determining FD *** will provide an overview of the evidence-based nutritional approach that should be pursued in these patients,providing clinicians with a valuable tool in standardizing nutritional advises and discouraging patients from engaging into indiscriminate food exclusions.

Motility disorders of the upper gastrointestinal tract encompass a wide range of different diseases. Esophageal achalasia and functional dyspepsia are representative disorders of impaired motility of the esophagus and stomach, respectively. In spite of their variable prevalence, what both diseases have in common is poor knowledge of their etiology and pathophysiology. There is some evidence showing that there is a genetic predisposition towards these diseases, especially for achalasia. Many authors have investigated the possible genes involved, stressing the autoimmune or the neurological hypothesis, but there is very little data available. Similarly, studies supporting a post-infective etiology, based on an altered immune response in susceptible individuals, need to be validated. Further association studies can help to explain this complex picture and find new therapeutic targets. The aim of this review is to summarize current knowledge of genetics in motility disorders of the upper gastrointestinal tract, addressing how genetics contributes to the development of achalasia and functional dyspepsia respectively.

This study aims to improve the accuracy and speed of predictions for thermal comfort and air quality in built environments by creating a coupled framework between computational fluid dynamics(CFD)simulations and deep learning *** coupling approach is showcased by the development of a data-driven turbulence *** new turbulence model is built using a deep learning neural network,whose mapping structure is based on a zero-equation turbulence model for built environment simulations,and is coupled with the CFD software OpenFOAM to create a hybrid *** neural network is a standard shallow multi-layer *** number of hidden layers and nodes per layer was optimized using Bayesan optimization *** framework is trained on an indoor environment case study,as well as tested on an indoor office simulation and an outdoor building array *** show that the deep learning based turbulence model is more robust and faster than traditional two-equation Reynolds average Navier-Stokes(RANS)turbulence models,while maintaining a similar level of *** model also outperforms the standard algebraic zero-equation model due to its superior ability to generalize to various flow *** some challenges,namely the mapping constraint,the limited training dataset size and the source of generation of training data,the hybrid framework demonstrates the viability of the coupling technique and serves as a starting point for future development of more reliable and advanced models.

With the increasing penetration of DC systems into the high voltage and medium voltage power industry,DC voltage is becoming common for distribution/transmission and to supply different typologies of *** allowable extent of AC ripple superimposed to DC,and its effect on insulation ageing,is a long-term discussed *** most harmful phenomenon causing extrinsic ageing acceleration and insulation system premature failure is partial discharges(PD);thus,the risk of incepting PD due to AC ripple could become a primary issue for electrical asset equipment *** this work,the impact of AC sinusoidal ripple on insulation system life and reliability is dealt with,considering both intrinsic and extrinsic ageing but focusing on the latter,that is,the PD *** are performed to assess how the jump voltage(due to AC ripple)and the DC component impact on PD activity in terms of amplitude and repetition *** the first time,the correlation between the magnitude of jump voltage associated with ripple and PD inception is established,shedding a light on the allowable ripple extent which does not impact significantly on ageing and premature insulation *** approach can provide straightforward tools for design specification and ageing inference of insulation systems.

AIMTo evaluate how different levels of adherence to a mediterranean diet (MD) correlate with the onset of functional gastrointestinal disorders. METHODSAs many as 1134 subjects (598 M and 536 F; age range 17-83 years) were prospectively investigated in relation to their dietary habits and the presence of functional gastrointestinal symptoms. Patients with relevant chronic organic disease were excluded from the study. The Mediterranean Diet Quality index for children and adolescents (KIDMED) and the Short Mediterranean Diet Questionnaire were administered. All subjects were grouped into five categories according to their ages: 17-24 years; 25-34; 35-49; 50-64; above 64. RESULTSOn the basis of the Rome III criteria, our population consisted of 719 (63.4%) individuals who did not meet the criteria for any functional disorder and were classified as controls (CNT), 172 (13.3%) patients meeting criteria for prevalent irritable bowel syndrome (IBS), and 243 (23.3%) meeting criteria for prevalent functional dyspepsia (FD). A significantly lower adherence score in IBS (0.57 ± 0.23, P P P P P P P P P CONCLUSIONLow adherence to MD may trigger functional gastrointestinal symptoms, mainly in younger subjects. Moreover, with increasing age, patients tend to adopt dietary regimens closer to MD.
