Histopathological and Ultrasound Correlation in Women Presenting with Breast Lumps in Yaoundé, Cameroon

Context: Breast cancer is very deadly among women with higher rates in the developing world. Imaging tools such as ultrasound, can be used to differentiate between the types of breast lumps. This study aimed to determine the value of ultrasound as a first-line examination in the diagnosis of breast masses. Methods: This was a retrospective cross-sectional study of women presenting with breast lumps from November 2022 to June 2023 at the Yaounde General Hospital, which lasted seven months, from November 2022 to June 2023. The sampling was exhaustive and consecutive. Association between variables was studied using the x2 test and concordance between ultrasound and histopathological findings was assessed using the Kappa correlation coefficient. Results: 234 women were included in the study. Their mean age was 46.3 ± 11.4 years. Overall, 15 (6.4%) lumps were benign while 219 (93.6%) were malignant. Triple negative (5.6%) was the most recurrent genomic classification. The correlation between the ultrasound and histopathological findings was significant, with an observed concordance rate at 85.1%, kappa = 0.322 and a p-value Conclusion: The performance of ultrasound in differentiating benign and malignant lesion was high. However, the discordant cases highlight the need for a diagnosis system which blends histopathological and radiological findings for an improved management of patients with breast lumps.

Diagnostic Value of the Thoracic Ultrasonography Compared to Conventional Chest X-Rays in Pneumonia for Children between 0 to 15 Years: Case Study in Two Hospitals in Yaoundé

Introduction: The diagnosis of pneumonia is usually made based on clinical manifestations and chest X-ray. The use of ultrasound in detecting pulmonary diseases in general, and especially consolidation syndrome has been demonstrated. The objective of this study was to determine the accuracy of thoracic ultrasound compared to chest X-ray in the diagnosis of infectious pneumonia in children. Methods: Children between 0 to 15 years were included in our study. The lung ultrasound results obtained were compared with those of the chest X-ray used as the reference. Our data were introduced into the EpiInfo 3.5.4 software and analyzed with the EpiInfo 3.5.4 and IBMSPSS Statistics version 20.0 softwares. Microsoft Office Excel 2016 was used to produce Charts. Continuous quantitative variables were presented. Cohen’s Kappa concordance test was applied with confidence interval of 95%. Results: 52 children were enrolled in the study. In imaging, the dominant sign was consolidation syndrome (75.0%) of cases by chest radiography, and in 78.8% of cases by lung ultrasound (p Conclusion: Our study demonstrated that lung echography is a non-ionizing and reliable tool in the diagnosis of childhood’s pneumonia.

Impact of the Covid-19 Pandemic on Severe Childhood Malaria at the University Hospital of Brazzaville

Introduction: Malaria management has been a source of concern for health systems since the advent of the Covid-19 pandemic. Objective: To assess the impact of the Covid-19 pandemic on severe childhood malaria in Brazzaville. Material and Method: A quasi-experimental intervention/non-intervention study was carried out between March and October 2020 in the pediatric departments of the Brazzaville University Hospital. Children aged three months to 15 years hospitalized were the target population. Two groups were formed: the “intervention” group, that of children hospitalized between March and October 2020 and the “control” group that of those hospitalized between January and August 2015. The study variables were epidemiological, clinical, biological and therapeutic. Chi-square and T-Student tests were used. The impact of the intervention was assessed by the absolute risk difference. Results: Of 1392 children hospitalized, 199 (14.6%) had severe malaria with an average age of 6.94 years. Mining Implication in the Geochemical Exploration of Fluvial Sediments of the East Nyambaka Volcanic Zone (Adamawa-Cameroon)

Geochemical characterization in stream sediments from Nyambaka locality was carried out on ten samples collected from the study area watershed. The distribution of trace metals in the sediments showed that some lithophiles (Be, Sc, V, Cr, Sr, Th, U), chalcophiles (Cu, Zn, Ga, As) and siderophiles (Mn, Co, Ni, Mo) have high average contents in the sediments compared to their average concentration in the upper crust. From the factorial analysis of the components, the total variance explained made it possible to represent and capture the main information on the data and to represent them in a graphical frame. Thus, using this graphical frame, the trace elements were divided into four groups opposed to each other. They show an enrichment of stream sediments in group 1 (Cr, Cu, Mn, Be, Ni, Co, Zr, Sr, Ba, Mg) and group 2 (Mo, As, Hg, V, Ag) elements more than group 3 (Ga, Sc, U) and group 4 (Au, Th). Rare earth analysis indicates a high abundance of Lanthanide (28.1 ppm to 42.4 ppm), a high concentration of light rare earth (LREE), and depletion of heavy rare earth (HREE). The variable LREE/HREE (7.24 LREE/ HREE 12.22) and (La/Yb)N (6.70 (La/Yb)N 11.72) ratios suggest an effect of heterogeneous sorting of heavy minerals with variable enrichment of dense and resistant minerals.

Background: The incidence of prostate cancer in Cameroon has been increasing in an alarming rate. The aim of this study is to characterize the form of prostate cancer and associated factors in patients from Cameroon Northern Regions. Methods: All patients with positive prostate biopsy (cancer+) from June 2018 to November 2019 were studied (n = 177). The followings were retrieved: digital rectal examination, standard clinical examinations, laboratory data such as serum prostate-specific antigen (PSA) level, and the Gleason score. Patients self-administered a questionnaire assessing prostate cancer’s risk factors. Results: Patients were mainly from the Far North region (36.72%), and were either farmers or breeders (48.01%). Only prostate adenocarcinoma was present, with predominance of aggressive forms (Gleason score ≥ 7). Significant relationships were observed between Gleason score and 1) patients’ age (P = 0.006), 2) history of urinary tract infections (P = 0.015) and of exposure to agricultural products (P = 0.049), 3) clinical signs (nycturia, pollakiuria, poor acute urine retention, and dysuria) (P = 0.019), 4) prostate weight, and

Antibiotic Resistance Profile of Pathogenic Bacteria Isolated from “Mabokés” Smothered Fish in Brazzaville, Congo

The smothered fish samples were taken from 3 markets. They were grown on different selective and differentiated culture media to target groups of bacteria associated with food poisoning. Isolates were identified on the basis of cellular and colonial morphologies on selective and differentiated culture media, followed by susceptibility testing to certain families of antibiotics, in particular beta-lactams. This study showed that S. aureus and B. cereus had high levels of beta-lactam resistance. However, these strains were sensitive to kanamycin, tobramycin, ciprofloxacin and norfloxacin. The characteristic penicillinase phenotype was dominant in Gram-positive bacteria. Shigella spp, Salmonella spp and E. coli were resistant to beta-lactam antibiotics. Tobramycin and meropenem retained their activity on all strains. Despite the increased rates of resistance observed, vancomycin, kanamycin, tobramycin, gentamicin, ciprofloxacin, and norfloxacin can be used in the treatment of community-acquired infections caused by Gram-positive bacteria while meropenem and tobramycin for Shigella spp, Salmonella spp and E. coli infections.

Background: Group B Streptococcus (GBS) or Streptococcus agalactiae, which asymptomatically colonizes the female genital tract, is one of the leading causes of septicemia, meningitis and pneumonia in neonates. This study was conducted in Bobo Dioulasso, Burkina Faso to determine the prevalence of GBS colonization among pregnant women. Methods: Six hundred and eleven (611) pregnant women were screened for GBS colonization between July and December 2016. Vaginal swab samples were aseptically collected from the subjects after oral informed consent. Standard microbiological methods were used to isolate and identify GBS isolates. The antibiotic susceptibility profile of GBS isolates was assessed using the Kirby-Bauer disk diffusion method. Results: Colonization prevalence was 6.05%. No risk factors associated with the carriage rate was statistically identified. All isolates were susceptible to Amoxicillin, Ampicillin, Cefotaxime, Levofloxacin, Vancomycin and Nitrofurantoin. Resistance to antibiotics was found for erythromycin (35.14%), lincomycin (16.22%) and penicillin G (10.81%). Conclusion: Although a low carriage (6.05%) rate and isolates were susceptible to many antibiotics found in this study, a policy of systematic screening of pregnant women at least in the third trimester must be promoted.

Lumbar Artery Pseudo-Aneurysm after Spinal Trauma and Transpedicular Fixation: A Case Report

Introduction: A case of lumbar artery pseudo-aneurysm after surgery for lumbar vertebra burst fracture is presented. Only two cases secondary to spinal trauma and revealed after surgery have been described in the literature. A review of literature was conducted and a possible relationship with burst fracture or reduction maneuvers was discussed. Case Presentation: Here, we report a post traumatic lumbar burst facture with incomplete motor deficit of lower limbs after a fall. Surgical reduction and stabilization via posterior approach were performed. Post-operatively, a hemorrhage through surgical wound had been noticed. Pseudo-aneurysm of the lumbar artery at L3 level was diagnosed and embolisation was performed. Conclusion: This is a rare case of pseudo-aneurysm occurring in the setting of surgical stabilisation after a trauma. It was revealed by a hemorrhage discovered as a late complication. We should be aware of such potential complication as a delayed post surgery event in order to adapt the care.

Relevance of Chest CT Scan Requests in Two University Teaching Hospitals in a Developing Country

Chest investigation is common in hospital practice. Chest X-ray is readily available and usually the first chest investigation. Thoracic CT scan constitutes an alternative and complimentary chest investigation. It is currently the most efficient investigation for the chest and its contents. Objectives: To evaluate the relevance of chest CT Scan requests in two university teaching hospitals in Cameroon. Material and Methods: We conducted a cross-sectional, retrospective and descriptive study at the Radiology and imaging units of the Yaounde Central Hospital and the Yaounde University Teaching Hospital Centre. Included in our study were files of patients who had a chest CT scan investigation during three years. Results: We had a study population of 323 subjects. The age interval was 23 months to 91 years old. Pulmonologist were the most prescribers with 27.2%. We had 80% conformity of indications with French Society of Radiology (FSR) standards. 50 over of 323 indications were not recommended by the FSR. Conclusion: There is a conformity rate of 80% between indications and the FSR recommendations.
