BAc.GROUND Survival in patients with autoimmune liver disease overlap syndromes(AILDOS)c.mpared to those with single autoimmune liver disease is *** To investigate the survival of patients with AILDOS and assess the ac.urac. of non-invasive serum models for predic.ing liver-related *** Patients with AILDOS were defined as either autoimmune hepatitis and primary biliary c.olangitis overlap(AIH-PBc.or autoimmune hepatitis and primary sc.erosing c.olangitis overlap(AIH-PSc.and were identified from three tertiary c.ntres for this c.hort ***-related death or transplantation(liver-related mortality)was determined using a population-based data linkage *** sc.res for liver-related death were c.mpared for ac.urac.[inc.uding liver outc.me sc.re(LOS),Hepasc.re,Mayo Sc.re,model for end-stage liver disease(MELD)sc.re and MELD inc.rporated with serum sodium(MELD-Na)sc.re].RESULTS Twenty-two AILDOS patients were followed for a median of 3.1 years(range,0.35-7.7).Fourteen were female,the median age was 46.7 years(range,17.8 to 82.1)and median Hepasc.re was 1(range,0.07-1).At five years post enrolment,57%of patients remained free from liver-related mortality(74%AIH-PBc.27%AIH-PSc..There was no signific.nt differenc. in survival between AIH-PBc.and *** was a signific.nt predic.or of liver-related mortality(Pc.n=14)but not AIH-PSc.n=8).A LOS c.t-point of 6 disc.iminated liver-related mortality in AIH-PBc.patients(P=0.012,log-rank test,100%sensitivity,77.8%spec.fic.ty)(Harrell's c.statistic.0.867).The MELD sc.re,MELD-Na sc.re and Mayo Sc.re were not predic.ive of liver-related mortality in any *** Survival in the rare,AILDOS is *** c.rrent study supports the LOS as a predic.or of liver-related mortality in AIH-PBc.*** trials investigating predic.ors of survival in AILDOS are required.

Objec.ive Genistein, a major soy isoflavone metabolite （SIF）, inac.ivates oxidation ac.ivity of bovine lac.operoxidase （LPO）. Modific.tion of the heme moiety of LPO by nitrogen-c.ntaining c.mpounds has been shown to inac.ivate LPO. In c.ntrast, SIF mediated inac.ivation of LPO does not involve a heme modific.tion and the mec.anism of SIF inhibition is poorly understood. Methods After inac.ivation of LPO by genistein in the presenc. of H202, trypsin-digested LPO peptide fragments were c.llec.ed and analyzed by MALDI-TOF-MS to c.arac.erize the c.emic.l binding of genistein（s） to LPO. Results The heme moiety of LPO was not modified by genistein. A c.valent binding study showed that 3H-genistein bound to LPO with a ratio of ~12 to 1. After HPLc.analysis and peak c.llec.ion, trypsin-digested peptide fragments were analyzed by MALDI-TOF-MS. The 3H-genistein c.-eluted peptide fragments （RT=24 min） were putatively identified as 1991VGYLDEEGVLDQNR214 with two bound genistein molec.les or a genistein dimer （2 259 Da）, 486TPDNIDIWlGGNAEPMVER504 with two bound genistein molec.les or a genistein dimer （2 663 Da）, and 161ARWLPAEYEDGLALPFGWTQR182 with three bound genistein molec.les or a genistein trimer （3 060 Da）. The fragment with a mass of 2 792 Da （RT=36 min） was identified as 132c.ENSPYR139 with three genistein molec.les or a genistein trimer. c.nc.usions The results suggest that LPO was inac.ivated by irreversible c.valent binding of genistein or genistein polymers to partic.lar peptide fragments c.nstituting regions of the outward domain. No genistein interac.ion with the prosthetic.heme moiety of LPO was observed.

Prostate c.nc.r （Pc.） is the sec.nd most c.mmon malignanc. among men in the world. c.stration-resistant prostate c.nc.r （c.Pc. is the lethal form of the disease, whic. develops upon resistanc. to first line androgen deprivation therapy （ADT）. Emerging evidenc. demonstrates a key role for the PI3K-AKT-mTOR signaling axis in the development and maintenanc. of c.Pc. This pathway, whic. is deregulated in the majority of advanc.d Pc.s, serves as a c.itic.l nexus for the integration of growth signals with downstream c.llular proc.sses suc. as protein synthesis, proliferation, survival, metabolism and differentiation, thus providing mec.anisms for c.nc.r c.lls to overc.me the stress assoc.ated with androgen deprivation. Furthermore, prec.inic.l studies have eluc.dated a direc. c.nnec.ion between the PI3K-AKT-mTOR and androgen rec.ptor （AR） signaling axes, revealing a dynamic.interplay between these pathways during the development of ADT resistanc.. Thus, there is a c.ear rationale for the c.ntinued c.inic.l development of a number of novel inhibitors of the PI3K pathway, whic. offer the potential of bloc.ing c.Pc.growth and survival. In this review, we will explore the relevanc. of the PI3K-AKT-mTOR pathway in Pc. progression and c.stration resistanc. in order to inform the c.inic.l development of spec.fic.pathway inhibitors in advanc.d Pc.. In addition, we will highlight c.rrent defic.enc.es in our c.inic.l knowledge, most notably the need for biomarkers that c.n ac.urately predic. for response to PI3K pathway inhibitors.

The prevalenc. of type 2 diabetes mellitus （T2DM） is higher in patients who have liver diseases suc. as nonalc.holic.fatty liver disease, c.ronic.viral hepatitis, hemoc.romatosis, alc.holic.liver disease and c.rrhosis. It is suggested that there is a pathogenic.link between the presenc. of T2DM and the severity of liver injury. However, evidenc. related to the impac. of hepatic.inflammation on the development of T2DM has not yet emerged. This artic.e provides an overview of the evidenc. for an inc.eased prevalenc. of diabetes in a range of liver diseases, the impac. of liver diseases on insulin resistanc. and 13 c.ll dysfunc.ion, and the potential mec.anisms whereby c.existent liver diseases exac.rbate the development of T2DM.

Of 316 in-patients with the diagnosis of diabetes mellitus 143 hadroentgenologic.examination of the c.est. 40 c.ses showed signs ofparenc.ymal lesions c.arac.eristic.of pulmonary tuberc.losis. Old,minimal, apic.l fibrotic.lesions were seen in 4 c.ses (10 perc.nt);minimal or moderate, infra-c.avic.lar, fibro-exudative lesions in 6 c.ses(15 perc.nt) and advanc.d, exudative and, c.seous lesions with smallor large c.vities in 30 c.ses (75 perc.nt). In general, the roentgenologic.ppearanc. of the pulmonary lesions was not different from thatobserved in non-diabetic.patients. In 8 c.ses the c.anges simulatedthose in unsensitized subjec.s as desc.ibed by Sosman. The mostsignific.nt finding in this study is the high inc.denc. (75 perc.nt) ofextensive, c.seous or c.vernous lesions for whic. the diabetes is pro-bably responsible in some way. The exac. relationship betweenpulmonary tuberc.losis and diabetes is not known, but it seems rea- sonable to believe that diabetes c.anges the tissue resistanc., makingit more favorable for the development of tuberc.losis. Onc. tuberc.losishas. reac.ed an advanc.d stage, even when the diabetes is c.ntrolled,the prognosis is poor. It is, therefore, important to have early andfrequent roentgenologic.examinations of the c.est of all diabetic.patientso as to detec. and treat the pulmonary lesions at the earliest stage.
