Schwannomas are rare tumors derived from the cells of Schwann that form the neural sheath. When located in the gastrointestinal tract, they constitute together with leiomyoma, leiomyoblastoma, and leiomyosarcoma, the gastrointestinal stromal tumors (GIST). Peripheral nerve sheath tumors represent 2-6% GIST with most common location, the stomach and the small intestine. Schwannomas of the colon and rectum are extremely rare and radical excision with wide margins is mandatory, due to their tendency to recur locally and become malignant, if left untreated. In the present study, we report a rare case of a sigmoid schwannoma, which was successfully treated in our department and reviewed the literature.

A structued stormwater infiltration system was developed and constructed at a university campus and monitoring of storm events was performed during a one- year operation period. The flow and pollutant mass balances were analyzed and the overall efficiency of the system was assessed. While significant positive correla- tions were observed among rainfall, runoff and discharge volume （R2= 0.93-0.99; p 〈 0.05）, there was no sig- nificant correlations existed between rainfall, runoff, discharge volume and pollutant load. The system was more effective in reducing the runoff volume by more than 50% for small storm events but the difference between the runoff and discharge volume was significant even with rainfall greater than 10ram. Results showed that the pollutant reduction rates were higher compared to the runoff volume reduction. Average pollutant reduction rates were in the range of 72% to 90% with coefficient of variation between 0.10 and 0.46. comparable with runoff reduction, the system was more effective in reducing the pollutant load for small storm events, in the range of 80% to 100% for rainfall between 0 and 10mm; while 65% to 80% for rainfall between l0 and 20mm. Among the pollutant parameters, particulate matters was highly reduced by the system achieving only a maximum of 25% discharge load even after the entire runoff was completely discharged. The findings have proven the capability of the system as a tool in stormwater manage- ment achieving both flow reduction and water quality improvement.

AIM:To determine if longer battery life improves capsule endoscopy(cE) completion ***:A retrospective study was performed at a tertiary,university-affiliated hospital in Vancouver,*** who underwent cE with either PillcamTM SB2 or SB2 U between 01/2010 and 12/2013 were considered for ***2 and SB2 U share identical physical dimensions but differ in their battery lives(8 h vs 12 h).Exclusion criteria included history of gastric or small bowel surgery,endoscopic placement of cE,interrupted view of major landmarks due to technical difficulty or significant amount of debris,and repeat cE using same *** demographics,comorbidities,medications,baseline bowel habits,and previous surgeries were *** of major landmarks in cE were recorded,and used to calculate gastric transit time,small bowel transit time,and total recording time.A complete cE study was defined as visualization of *** times and completion rates were ***:Four hundred and eight patients,including 208(51.0%) males,were included for *** mean age was 55.5 ± 19.3 *** most common indication for cE was gastrointestinal bleeding(n =254,62.3%),followed by inflammatory bowel disease(n =86,21.1%).There was no difference in gastric transit times(group difference 0.90,95%cI:0.72-1.13,P =0.352) and small bowel transit times(group difference 1.07,95%cI:0.95-1.19,P =0.261) between SB2 U and SB2,but total recording time was about 14% longer in the SB2 U group(95%cI:10%-18%,P corresponding trend toward higher completion rate(88.2% vs 93.2%,OR =1.78,95%cI 0.88-3.63,P =0.111).There was no statistically significant difference in the rates of positive findings(OR =0.98,95%cI:0.64-1.51,P =0.918).cONcLUSION:Extending the operating time of cE may be a simple method to improve completion rate although it does not affect the rate of positive findings.

Laser ablation inductively coupled plasma mass spectrometry （LA-IcP-MS） has been widely adopted for the direct multi-elemental analysis with high sensitivity. Especially analysis of fine ceramics by LA-IcP-MS without time-consuming sample decomposition process has been one of the most expecting application field. Small additive elements in MgO powders were analyzed by LA-IcP-MS. For precise and accurate analysis influence of carrier gas （Ar or He） was presented by the signal intensities of IcP-MS, relative standard deviation of signal intensities and ablated particle size distribution. Ablated particles were collected and analyzed by scanning electron microscopy （SEM） to investigate the particle size distribution, and the ablated sample surface was examined by camscope. In He gas atmosphere, the signal was more stable than in Ar gas atmosphere. The signal intensity was higher in Ar than in He. Examination of ablated particles and sample surface reveals that more particles were generated in Ar atmosphere and the distribution of particle size was larger.

During the first welding process in manufacturing a precise seam container, the steel material must first undergo a full annealing treatment. Multilayer welding operations are conducted and then applied with refining treatment to obtain the goal by hardening. The influence of thermal refining on mechanical properties of annealed SAE 4130 by multilayer GTAW （gas tungsten arc welding） was discussed. The AW （annealing＋welding） and AWST （annealing＋welding＋solution＋tempering） occurred with a minimum hardness value at GGHAZ （grain growth heat affected zone） due to coarse grain growth~ the hardness occurred with sudden drops between the multilayer welding, but was still larger than the minimum value at HAZ （heat affected zone）. The welded joint efficiency of the AWST was 89.9%, with the elongation reduced to 77.2% of AST （annealing＋solution＋tempering）. The cross section of the tensile samples both appeared with a uniform dimple-shaped structure, and however the necking for AST was greater than that for AWST. The impact value for AWST was 166.5% of AST. Moreover, from microstructure observations, it was found that AWST had a greater tendency to ductile failure than AST. These results can be inferred., the process of steady pressure and the initial position of the precision tube breakage HAZ. However, when stress occurs instantly, HAZ can absorb more energy, therefore the initial damage does not occur.

Transcatheter aortic valve implantation （TAVR） has emerged as an alternative, rapidly evolving treatment option for patients with se- vere aortic stenosis and high surgical risk. Stroke is a devastating complication being confined mainly in the periprocedural and 30-day pe- riod following TAVR, with a lower and relatively constant frequency thereafter. Early stroke is mainly due to debris embolization during the procedure, whereas later events are associated with patient specific factors. Despite the fact that the rate of clinical stroke has been constantly decreasing compared to initial TAVR experience, modern neuro-imaging with MRI suggests that new ischemic lesions post-TAVR are almost universal. The impact of the latter is largely unknown. However, they seem to correlate with a reduction in neurocognitive function. Because TAVR is set to expand its indication to lower surgical-risk patients, stroke prophylaxis during and after TAVR becomes of paramount importance. Based on clinical and pathophysiological evidence, three lines of research are actively employed towards this direction： improvement in valve and delivery system technology with an aim to reduce manipulations and contact with the calcified aortic arch and native valve, antithrombotic therapy, and embolic protection devices. careful patient selection, design of the procedure, and tailored antithrombotic strategies respecting the bleeding risks of this fragile population constitute the main defense against stroke following TAVR.

Sixteen crossbred buck goats （Kiko x Spanish; BW = 32.8 kg） and wether sheep （Dorset x Suffolk; BW = 39.9 kg） were used to determine the effect of preslaughter diet and feed deprivation time （FDT） on physiological responses and microbial loads on skin and carcasses. Experimental animals were fed either a concentrate （cD） or a hay diet （HD） for 4 d and then deprived of feed for either 12-h or 24-h before slaughter. Blood samples were collected for plasma cortisol and blood metabolite analyses. Longisimus muscle （LM） pH was measured. Skin and carcass swabs were obtained to assess microbial loads. Plasma creatine kinase activity （863.9 and 571.7 ± 95.21 IU） and non-esterified fatty acid concentrations （1,056.1 and 589.8 _＋ 105.01 mEq/L） were different （P 〈 0.05） between sheep and goats. Species and diet treatments had significant effects on the ultimate pH of LM. Pre-holding total coliform （Tcc） and aerobic plate counts （APc） of skin were significantly different between species. Goats had lower （P 〈 0.05） Tcc （2.1 vs. 3.0 IogTo cFU/cm2） and APc （8.2 vs. 8.5 Iog10 cFU/cm2） counts in the skin compared to sheep. Preslaughter skin E. coil counts and Tcc were different （P 〈 0.05） between species. Goats had lower （P 〈 0.05） counts of E. coil （2.2 vs. 2.9 log10 cFU/cm2） and Tcc （2.3 vs. 3.0 log10 cFU/cm2） in the skin compared with those in sheep. Diet, species, and FDT had no effect （P 〉 0.05） on E. coli and Tcc in carcass swab samples. The APc of carcass swab samples were only affected （P 〈 0.05） by the FDT. The results indicated that preslaughter dietary management had no significant changes on hormone and blood metabolite concentrations and sheep might be more prone for fecal contamination than goats in the holding pens at abattoir.

The current healthcare system in Hong Kong is experiencing pressure due to constrained resources,with dramatic increases in inpatient services queue lengths,dissatisfaction with the working environment,unacceptable workforce arrangements and high turnover rate of hospital *** maintain the robustness of the healthcare system and a sustainable inpatients flow,the Food and Health Bureau launched a public-private partnership programme to utilise the resources of the public and private *** research investigates the potential for extension of the programme and further enhancing the sustainability of the long-term inpatient services under a mixed public-private healthcare policy via system dynamic *** findings show that an increase of human resources in public hospitals does not substantially improve inpatient flow rate ***,the results from the system dynamic approach provide insights into the expansion of the service areas of the programme and suggest increasing the number of referrals to private hospitals.
