Recent advances in microvascular autologous breast reconstruction after ablative tumor surgery

Breast cancer is a ubiquitous disease and one of the leading causes of death in women in western societies. With overall increasing survival rates, the number of patients who need post-mastectomy reconstruction is on the rise. Especially since its psychological benefits have been broadly recognized, breast reconstruction has become a key component of breast cancer treatment. Evolving from the early beginnings of breast reconstruction with synthetic implants in the 1960 s, microsurgical tissue transfer is on the way to become the gold standard for post oncology restoration of the breast. Particularly since the advent of perforator based free flap surgery, free tissue transfer has become as safe option for breast reconstruction with low morbidity. The lower abdominal skin and subcutaneous fat tissue typically offer enough volume to create an aesthetically satisfying breast mound. Nowadays, the most commonly used flap from this donor site is the deep inferior epigastric artery perforator flap. If the lower abdomen is not available as a donor site, the gluteal area and thigh provide a number of flaps suitable for breast recon-struction. If the required breast volume is small, and there is enough tissue available on the upper medial thigh, then a transverse upper gracilis flap may be a practicable method to reconstruct the breast. In case of a higher amount of required volume, a gluteal artery perforator flap is the best choice. However, what is crucial in addition to selecting the best flap option for the individual patient is the timing of the operation. In patients with confirmed post-mastectomy radiation therapy, it is advisable to perform microvascular breast reconstruction only in a delayed fashion.

Targeting Plant Ethylene Responses by Controlling Essential Protein-Protein Interactions in the Ethylene Pathway

The gaseous plant hormone ethylene regulates many processes of high agronomic relevance throughout the life span of plants. A central element in ethylene signaling is the endoplasmic reticulum （ER）-Iocalized membrane protein ETHYLENE INSENSITIVE2 （EIN2）. Recent studies indicate that in response to ethylene, the extra-membranous C-terminal end of EIN2 is proteolytically processed and translocated from the ER to the nucleus. Here, we report that the conserved nuclear localization signal （NLS） mediating nuclear import of the EIN2 C-terminus provides an important domain for complex formation with ethylene receptor ETHYLENE RESPONSE1 （ETR1）. EIN2 lacking the NLS domain shows strongly reduced affinity for the receptor. Interaction of EIN2 and ETR1 is also blocked by a synthetic peptide of the NLS motif. The corre- sponding peptide substantially reduces ethylene responses in planta. Our results uncover a novel mecha- nism and type of inhibitor interfering with ethylene signal transduction and ethylene responses in plants. Disruption of essential protein-protein interactions in the ethylene signaling pathway as shown in our study for the EIN2-ETR1 complex has the potential to guide the development of innovative ethylene antagonists for modern agriculture and horticulture.

Synthesis, Crystal Structure and Magnetic Property of Disodium Biscarbonato cobaltate(Ⅱ) Tetrahydrate, Na_2Co(CO_3)_2·4H_2O

The title compound, Na 2Co(CO 3) 2·4H 2O, was synthesized by dropwise addition of 1.0 mL of 1 mol/L Co(CH 3COO) 2 to 50 mL of aqueous solution containing 7.0 g(66.0 mmol) of Na 2CO 3 and 2.5 g(29.8 mmol) of NaHCO 3 under stirring. A rose colored rhombohedral crystal grew by the slow evaporation of the solution of the title compoud at room temperature. The crystal structure was established on the basis of the single crystal X ray diffraction data. Na 2Co(CO 3) 2·4H 2O crystallizes in the monoclinic space group C2/c with a =0.856 6(1) nm, b =0.714 6(1) nm, c = 1 438 7(2) nm, β =97.59(1)° , V =0.873 0(2) nm 3 , D x=2.260 g/cm 3, F (000)=596, M r=296 99, μ (Mo K α)=21.06 cm -1 , Z =4 and the final R(F) =0.032 and wR(F 2) =0.068 on the basis of 947 reflections ( F 2 ≥ 2σ(F 0 2)). The crystal consists of H 2O molecules, Na + ions and 2D 2 ∞[Co(CO 3) 2] 2- layers extending parallel to (001). H 2O molecules and Na + cations are located between the 2D layers. Each Co atom is tetrahedrally coordinated by four O atoms from four CO 2- 3 groups with d (Co\_O)=0.206 5(2) nm, 0.207 6(2) nm and the angle of O\_Co\_O is in the range of 97.7(1)°—128.5(1)°. The CO 2- 3 group bridging two Co atoms via two O atoms deviates from D 3h symmetry with d (C\_O)=0.127 2(3)\_0.129 9(3) nm and the angle of O\_C\_O is in the range of 119.5(2)°\_123.4(2)°. Each Na atom, with two close Na neighbours at 0.338 4(2) nm and 0 346 7(2) nm, respectively, is octahedrally coordinated by six O atoms from four H 2O molecules and two carbonate groups belonging to two layers with d (Na\_O)=0.232 7(2)\_0.241 1(2) nm. Of two crystallographically different H 2O molecules, one forms hydrogen bonds to two CO 3 2- groups of one 2D layer, and the other to those from two different 2D layers. New Insight in Ethylene Signaling： Autokinase Activity of ETR1 Modulates the Interaction of Receptors and EIN2

Ethylene insensitive 2 （EIN2）, an integral membrane protein of the ER network, has been identified as the central regulator of the ethylene signaling pathway. Still, the mechanism by which the ethylene signal is transferred from the receptors to EIN2 has not been solved yet. Here, we show that protein phosphorylation is a key mechanism to control the interaction of EIN2 and the receptors. In vivo and in vitro fluorescence studies reveal that the kinase domain of the receptors is essential for the interaction. Cyanide, an ethylene agonist, which is known to reduce auto-phosphorylation of the ethylene receptor ethylene resistant 1 （ETR1） or a mutation in the kinase domain of ETR1 that prevents autophosphorylation （H353A）, increases the affinity of the receptors for EIN2. On the other hand, mimicking permanent auto-phosphorylation of ETR1 as in the mutant H353E releases the EiN2-ETR1 interaction from the control by the plant hormone. Based on our data, we propose a novel model on the integration of EIN2 in the ethylene signaling cascade.

Intrahepatic endometriosis as differential diagnosis: Case report and literature review

Intrahepatic endometriosis is one of the rarest forms of atypical endometriosis; only eighteen cases have been reported in the English literature. We describe the case of a 32-year-old woman, who presented with persistent, non-cyclical upper right quadrant abdominal pain, a central liver cyst, and no history of endometriosis. Three years previous, she was diagnosed with an intrahepatic cyst. The lesion progressed and two laparoscopic deroofing-operations were performed, yet the diagnosis of intrahepatic endometriosis was never reached. She presented in our clinic with further progress of the cyst as well as obstruction of the intrahepatic biliary system. The magnetic resonance imaging showed a 9.5 cm × 12 cm, lobulated intrahepatic cyst. We performed an ultrasonic pericystectomy. Immunostaining confirmed intrahepatic endometriosis. Only one of the previously described eighteen patients with intrahepatic endometriosis presented with cyclical pain in the upper right abdominal quadrant accompanying menstruation. This lack of a "typical" clinic makes it challenging to diagnose extragonadal endometriosis without histopathology. A previous history of endometriosis was described in only twelve cases, thus the diagnosis of this condition should not be limited to patients with a known history of endometriosis. Six of 18 patients were postmenopausal, demonstrating this condition is not limited to women of reproductive age. A preoperative diagnosis was only reached in seven of the previously described cases, highlighting the importance of preoperative biopsies. Yet due to the potential adverse effects, a transhepatic biopsy must be discussed individually. Although rare, intrahepatic endometriosis should always be considered as a differential diagnosis in women with recurrent hepatic cysts, regardless of age or previous medical history. In such cases, histology is essential and a pericystectomy should be performed as standard of care.

Effects of triggers of senescence and senolysis in murine pancreatic cancer cells

Paddy System with a Hybrid Rice Enhances Cyanobacteria Nostoc and Increases N2 Fixation

Biological nitrogen (N) fixation (BNF) plays a significant role in maintaining soil fertility in paddy field ecosystems. Rice variety influences BNF, but how different rice varieties regulate BNF and associated diazotroph communities has not been quantified. Airtight, field-based 15N2-labelling grow th chamber experiments were used to assess the BNF capac 辻 y of different rice varie ties. In addition, both the 16S rRNA and nifH genes were sequenced to assess the influence of different rice varieties on bacterial and diazotrophic communities in paddy soils. After subjecting a rice-soil system to 74 d of continuous airtight, field-based 15N2 labelling in pots in a growth chamber, the amounts of fixed N were 22.3 and 38.9 kg ha^-1 in inbred japonica (W23) and hybrid indica (IIY) rice cultivars plan ted in the rice-soil systems, respectively, and only 1%—2.5% of the fixed N was allocated to the rice plants and weeds. A greater abundance of diazotrophs was found in the surface soil (0-1 cm) under IIY than under W23. Sequencing of the 16S rRNA gene showed significantly greater abundances of the cyanobacterial genera Nostoc, Anabaena, and Cylindrospermum under IIY than under W23. Sequencing of the nifH gene also showed a significantly greater abundance of Nostoc under IIY than under W23. These results indicate that the hybrid rice cultivar (IIY) promoted BNF to a greater extent than the inbred rice cultivar (W23) and that the increase in BNF might have been due to the enhanced heterocystous cyanobacteria Nostoc.

Metallic debris from metal-on-metal total hip arthroplasty regulates periprosthetic tissues

The era of metal-on-metal(MoM) total hip arthroplasty has left the orthopaedic community with valuable insights and lessons on periprosthetic tissue reactions to metallic debris. Various terms have been used to describe the tissue reactions. Sometimes the nomenclature can be confusing. We present a review of the concepts introduced by Willert and Semlitsch in 1977,along with further developments made in the understanding of periprosthetic tissue reactions to metallic debris. We propose that periprosthetic tissue reactions be thought of as(1) gross(metallosis, necrosis, cyst formation and pseudotumour);(2) histological(macrophage-dominated, lymphocyte-dominated or mixed);and(3) molecular(expression of inflammatory mediators and cytokines such as interleukin-6 and tumor necrosis factor-alpha). Taper corrosion and modularity are discussed, along with future research directions to elucidate the antigen-presenting pathways and materi-al-specific biomarkers which may allow early detection and intervention in a patient with adverse periprosthetic tissue reactions to metal wear debris.
