Wastewater surveillance(WWS)can leverage its wide coverage,population-based sampling,and high monitoring frequency to capture citywide pandemic trends independent of clinical *** we conducted a nine months daily WWS for severe acute respiratory syndrome coronavirus 2(SARSCoV-2)from 12 wastewater treatment plants(WWTPs),covering approximately 80%of the population,to monitor infection dynamics in Hong Kong,*** found that the SARS-CoV-2 virus concentration in wastewater was correlated with the daily number of reported cases and reached two pandemic peaks three days earlier during the study *** addition,two different methods were established to estimate the prevalence/incidence rates from wastewater *** estimated results from wastewater were consistent with findings from two independent citywide clinical surveillance programmes(rapid antigen test(RAT)surveillance and serology surveillance),but higher than the cases number reported by the Centre for Health Protection(CHP)of Hong Kong,***,the effective reproductive number(R_(t))was estimated from wastewater measurements to reflect both citywide and regional transmission *** findings demonstrate that large-scale intensive WWS from WWTPs provides cost-effective and timely public health information,especially when the clinical surveillance is inadequate and *** approach also provides insights into pandemic dynamics at higher spatiotemporal resolutions,facilitating the formulation of effective control policies and targeted resource allocation.

Wastewater surveillance（WWS） can leverage its wide coverage, population-based sampling, and high monitoring frequency to capture citywide pandemic trends independent of clinical surveillance. Here we conducted a nine months daily WWS for severe acute respiratory syndrome coronavirus 2（SARS-CoV-2） from 12 wastewater treatment plants（WWTPs）, covering approximately 80% of the population,to monitor infection dynamics in Hong Kong, China. We found that the SARS-CoV-2 virus concentration in wastewater was correlated with the daily number of reported cases and reached two pandemic peaks three days earlier during the study period. In addition, two different methods were established to estimate the prevalence/incidence rates from wastewater measurements. The estimated results from wastewater were consistent with findings from two independent citywide clinical surveillance programmes(rapid antigen test（RAT） surveillance and serology surveillance), but higher than the cases number reported by the Centre for Health Protection（CHP） of Hong Kong, China. Moreover, the effective reproductive number（R_t） was estimated from wastewater measurements to reflect both citywide and regional transmission dynamics. Our findings demonstrate that large-scale intensive WWS from WWTPs provides cost-effective and timely public health information, especially when the clinical surveillance is inadequate and costly. This approach also provides insights into pandemic dynamics at higher spatiotemporal resolutions, facilitating the formulation of effective control policies and targeted resource allocation.

Comparison between empirical prokinetics, Helicobacter test-and-treat and empirical endoscopy in primary-care patients presenting with dyspepsia: A one-year study

AIM： To investigate the optimal strategy to treat dyspeptic patients in primary care. METHODS： Dyspeptic patients presenting to primary care outpatient clinics were randomly assigned to- （1） empirical endoscopy, （2） H pylori test-and-treat, and （3） empirical prokinetic treatment with cisapride. Early endoscopy was arranged if patients remained symptomatic after 2 wk. Symptom severity, quality-of-life （SF-36） as well as patient preference and satisfaction were assessed. All patients underwent endoscopy by wk 6. Patients were followed up for one year. RESULTS： Two hundred and thirty four patients were recruited （163 female, mean age 49）. 46% were H pylori positive. 26% of H pylori tested and 25% of empirical prokinetic patients showed no improvement at wk 2 follow-up and needed early endoscopy. 15% of patients receiving empirical cisapride responded well to treatment but peptic ulcer was the final diagnosis. Symptom resolution and quality-of-life were similar among the groups. Costs for the three strategies were HK$4343, $1771 and $1750 per patient. 66% of the patients preferred to have early endoscopy. CONCLUSION： The three strategies are equally effective. Empirical prokinetic treatment was the least expensive but peptic ulcers may be missed with this treatment. The H pylori test-and-treat was the most cost-effective option .

目的描述截至2013年5月27日通过中国国家流感样病例哨点监测系统发现的H7N9流感感染者的完整病例系列的特征。设计病例系列研究。研究场所中国大陆地区31个省份的554家哨点医院的门诊和急诊病例通过实验室确诊的H7N9感染病例和流感样病例哨点监测发现的病例间的相互对照来发现感染病例。流感样病例哨点监测系统发现的病例均符合世界卫生组织的流感样病例定义，每周每家监测医院采集10～15例流感样病例的鼻咽拭子用于病毒学检测。我们通过与地、省级和国家级疾病预防控制中心开展的公共卫生调查，获得相关的流行病学数据；通过病历回顾获得临床和实验室数据。主要研究指标病例系列的流行病学、临床和实验室特征结果截至2013年5月27日实验室确诊的130例H7N9感染病例中，5例（4％）是通过流感样病例哨点监测系统首先发现的。5例病例的平均年龄为13岁（2～26岁），均无基础性疾病。5例病例的暴露史、地理位置和发病时间与其他实验室确诊病例均类似。仅有2例病例需要入院治疗，所有5例病例均表现为轻度或中度感染且康复过程顺利。结论我们的发现支持H7N9流感病毒感染存在“临床冰山现象”，强调需要加强关注H7N9病毒感染多样化的临床表现。从公共卫生角度看，本研究为H7N9病毒感染存在相当大比例的轻症病例提供了间接证据。
