Background: Surgical site infection (SSI) is a dreaded nightmare for the orthopaedic surgeon. Preoperative skin cleaning with antiseptics has been shown to reduce the microbial burden of the skin and results in reduced incidence of SSI. However, the ideal skin cleaning agent remains to be established. Aim: To compare the efficacy of Povidone-Iodine/Povidone-Iodine (PI-PI) combination with that of Chlorhexidine-Gluconate/Alcohol (CG-A) combination in re-ducing SSI in orthopaedic surgeries. Methods: This was a prospective, randomized, double-blinded, controlled study. Subjects that met the selection criteria and gave consent were randomized into PI-PI group (test group) and the CG-A group (control group). Both the patients and the assessors for SSI were blinded to the group a participant belongs. Blocking was done on the type of surgery to cancel the confounding effect of surgery type on SSI. Standard perioperative protocols were applied to both groups. Assessment for features of SSI was done on the 3rd day, 7th day, 14th day, 6th week and 12th-week postoperative period. The diagnosis of SSI was made based on the Centers for Disease and Control (CDC) guidelines. Results: We recruited 124 patients for this study, 62 males and 54 females. The mean age of the subjects was 37.5 years (SD = 14.7 years). Sixty-two subjects were randomized into each group. There was no significant difference in the distribution of the genders in the study arms. other possible confounders such as duration of hospital stay, use of drains, the surgeon involved and age were evenly distributed in the two groups. eight patients did not complete the study. The overall incidence of SSI in the study was 2.6%. Subjects in the control group had an SSI of 3.4% while those in the PI-PI group had a rate of 1.8%;however, this was not significant, p = 0.579. Conclusion: Both CG-A and PI-PI combinations are equally efficacious as preoperative skin antiseptic in orthopaedic implant surgeries.

Background: Knowledge of the common bacteria that cause surgical site infection (SSI) and their antibiotic sensitivity is mandatory if treatment of surgical infection is to be successful. The threat of the emergence of resistant strains of bacteria is ever-present. Hence, a sensitivity directed therapy is paramount for the successful eradication of organisms with minimal risk of development of antibiotic resistance. Aim: The aim is to identify the common bacteria that cause SSI in orthopaedic implant surgeries in our hospital. Method: This is a prospective longitudinal study that includes all orthopaedic surgeries involving the use of implants within one year. Patients that had major orthopaedic surgeries involving implant were followed up and their wounds inspected for signs of SSI on postoperative days 3, 7, 14, 42 and 90. Wound swab was taken for microscopy, culture and sensitivity analysis from those who had wound infection, based on the CDC guidelines. Results: one-hundred and sixteen patients met our inclusion criteria and were included in the analysis. There were 62 males and 54 females. The mean age of the participant was 39.62 years (SD = 15.02 years). Fracture fixation with plates and screws was the most common implant surgery done. The incidence of SSI was 2.6%, and escherichia coli was the most common isolated pathogen. All the SSIs were superficial incisional type, and the infection was monomicrobial in 67% of cases and polymicrobial in 33%. All of the isolated pathogens were sensitive to Imipenem and Gentamycin. Conclusion: Superficial incisional SSI is the most common type of SSI in this study. escherichia coli is the most frequent pathogen in SSI affecting implant surgeries in our hospital. Gentamycin and Imipenem should be used for the prophylaxis of SSI in our environment.

Zebra mussels em>Dreissena polymorpha em>are a native bivalve from eastern europe. They were first detected in North America in Lake St. Clair in 1988 and were presumably introduced via infested ballast water. Zebra mussels have spread rapidly across the United States, with 31 states reporting infestations as of 2019. Zebra mussels were first detected in South Dakota, USA, in 2015 in Lewis and Clark Lake and McCook Lake, with subsequent infestations occurring in Lake Yankton in 2017, Lakes francis Case and Sharpe in 2019, and Pickerel Lake, Kampeska Lake, and Lake Cochrane in 2020. Chemical composition of the essential oils (eos) is dependent on the plant species and varieties, and within the same variety, from geographical region. Knowledge of the factors that determine the chemical variability and yield for each species are very important in particular for commercially important species, to optimize the conditions that may affect yields and quality of essential oils. In addition to the commercial importance of the variability in yield and composition, the possible changes are also important when the essential oils and volatiles are used as chemotaxonomic tools. Therefore it is important to characterize essential oils native to Tanzania for commercial and therapeutic purpose. In the present study we investigated the variation in the yield and chemical composition of the essential oils isolated from air dried leaves of three eucalyptus species namely eucalyptus saligna, eucalyptus citriodora and eucalyptus camaldulensis collected from two different regions of Tanzania namely Morogoro and Lushoto. The steam-distilled essential oil content of e. saligna, e. citriodora and e. camaldulensis Morogoro were 0.6%(v/w), 1.68%(v/w) and 0.80%(v/w) respectively while from Lushoto are 0.30%, 1.22 and 0.36%(v/w) respectively. Using GC/MS,α-pinene (46.72%) and p-cymene (43.61%) were identified as the most abundant chemical constituents of e. saligna from Morogoro and Lushoto respectively. Citronellal (82.24% and 60.41% respectively) is the main constituents of e. citriodora from Morogoro and Lushoto respectively while p-cymene (52.59% and 64.02%) is the main constituents of e. camaldulensis.

Patients with Chronic kidney disease associated with bone deficiency are observed in *** article reports the finding that kidney cells secrete a bone growth factor that is stimulated by kaempferol from flavones fraction(FF) Rhizoma Drynariae(Gu Sui Bu,GSB). The FF was extracted from GSB by 50%ethanol,and isolated by macroporous resin,silica gel and sephadex LH-20 column *** components were isolated and identified to be naringin (Ⅰ),neoeriocitrin(Ⅱ),Kaempferol(Ⅲ) and naringenin-7-o-β-D-glucoside(Ⅳ).The activity of the components was examined by studying the proliferation and bio-function of opossum Kidney (oK),osteoblast(MC3T3 e1) and human fibroblast(HF) *** was found to be active in promoting oK cell growth,the increase in rates being 1.16-1.3 fold,and 12.6 fold respectively in the presence of *** promoted neither MC3T3 e1 nor HF cell *** cell-conditioned culture medium(oKM) promoted MC3T3 e1 growth 2.98 *** 70nM kaempferol-conditioned oKM,MC3T3e1 cell growth increased to 2.27fold in comparison to control *** oKMs failed to show any significant effect on HF cell *** results of oKM promoted osteoblast proliferation indicate that the kidney cell secretes a bone growth factor(BGF).Kaempferol stimulates kidney cells proliferation and increases secretion of *** data suggest that patients with CKD associated with deficiency of bone may have a lower level of *** may serve to stimulate kidney repair and up-regulate bone formation and may be useful in treating CKD and osteoporosis.

Background: Kidney failure, cardiovascular disease, and early mortality are just a few of the major negative effects of chronic renal disease, a serious global health issue. The considerable financial and public health burden associated with chronic kidney disease can be lessened by raising awareness and adopting better practices for its impact, prevention, and early identification. objective: In this study, individuals with hypertension and diabetes were evaluated for their knowledge of chronic kidney disease, its prevalence, and its risk factors. Method: It was a hospital-based cross-sectional study conducted on adult (>18 years) patients with diabetes mellitus and hypertension. each participant provided written informed consent before having their data collected through interviews, medical information, and blood samples for CKD screening. The CKD epidemiology collaboration (CKD-ePI) equation was used to calculate the glomerular filtration rate (GFR) from serum creatinine, and CKD was determined using the estimated GFR (e-GFR). To find independent CKD factors, multivariate logistic regression was employed, with a p-value of 0.05 being regarded as statistically significant. This was accomplished using SPSS (Statistical Program for Social Sciences) version 22.0, IBM Corp., Armonk, NY. Result: A total of 156 participants took part in the study among which 95 (60.9%) were male, most of the participants 82 (52.6%) were aged between 51 - 70 years (mean 59.42 ± 11.007), 76 (48.7%) were unemployed and 97 (62.2%) were single. overall, the knowledge score of participants on CKD was 65.4% for good knowledge and 34.6% for poor or inadequate knowledge of CKD. More than half of the participants (60%) had chronic kidney disease. Among these, the greatest proportion of CKD patients were those who were hypertensive (88.2%) followed by those who were both hypertensive and diabetic (70.7%). 目的:探索鼻咽癌细胞DNA合成的生物节律为临床制订鼻咽癌时辰化疗方案提供必要的参数。方法:BALB/C裸小鼠15只,置于程控的独立光照系统中(12h光照,12h黑暗)中同步化至少3周。然后双侧腋下接种人鼻咽低分化鳞癌细胞裸小鼠移植瘤CNe2,成瘤后按灯亮后3、9、15、21h(即3、9、15、21HALo)的时间点取瘤块,制成单个细胞悬液固定,染色后流式细胞仪测DNA含量。用SPSS软件系统ANoVA法检验各期细胞在4个时间点差异的显著性,用Cosinor法考察G1、S、G2/M期细胞在24h的分布是否符合余弦函数。结果:G1、S、G2/M期细胞在3、9、15、21HALo的分布随时间变化有显著性差异,G1、G2/M期细胞变化符合余弦节律。结论:移植于裸鼠的人鼻咽癌细胞的DNA合成随昼夜交替呈节律性变化。
